Nutritional strategies to manage postural orthostatic tachycardia syndrome (POTS) and its comorbidities: A narrative review.

BackgroundPostural orthostatic tachycardia syndrome (POTS) is a heterogeneous disorder of the autonomic nervous system characterized by orthostatic intolerance and involvement of multiple organ systems. It commonly coexists with conditions such as hypermobile Ehlers-Danlos syndrome (hEDS), mast cell activation syndrome (MCAS), and a spectrum of gastrointestinal (GI) disorders. These overlapping conditions frequently manifest with shared GI symptoms such as nausea, bloating, abdominal pain, early satiety, and altered bowel habits that can substantially compromise nutritional status and quality

Pharmacotherapeutic management of chronic pain in joint hypermobility and Ehlers-Danlos syndromes: a clinical perspective.

INTRODUCTION: Joint hypermobility-related pain is an increasingly recognized musculoskeletal pain characterized by mixed pain mechanisms, frequent chronicity, and an incompletely defined natural history. Joint hypermobility-related pain may occur in monogenic Ehlers-Danlos syndromes, but it is more frequently encountered in people with hypermobility spectrum disorders and hypermobile Ehlers-Danlos syndrome, conditions that remain molecularly unresolved. AREAS COVERED: This article places joint hypermobility-related pain in its historical and conceptual context, summarizes the mechanisms thoug

Anesthesia Clinical Practice Guidelines for Ehlers-Danlos Syndrome: A Modified Delphi Survey.

Ehlers-Danlos syndrome (EDS) is a family of inherited connective tissue disorders distinguished by joint hyperextensibility, skin hyperelasticity, and increased tissue friability. No standardized anesthesia guidelines currently exist for this population, despite facing increased perioperative risks. To address this gap in literature, a three-round modified Delphi survey was conducted with 15 experts in anesthesia and EDS. Seven preliminary guidelines, developed from a comprehensive literature review, were evaluated and refined across three rounds. Consensus was achieved for all seven guideline

Lingual Traction and the Lateral Decubitus Position Facilitate Awake Fiberoptic Oral Intubation in a Patient With Ehlers-Danlos Syndrome (Hypermobile Type) and Craniocervical Fixation: A Case Report.

BACKGROUND: Awake fiberoptic intubation (AFOI) remains the preferred technique for anticipated difficult airway management. However, severe restriction of mouth opening, fixed craniocervical anatomy, and sedation-induced hypopharyngeal collapse may substantially impair endoscopic visualization and increase procedural difficulty. Simple mechanical maneuvers that improve upper airway patency may facilitate successful intubation in these highly challenging patients. CASE PRESENTATION: We report the airway management of a 24-year-old woman with Ehlers-Danlos-hypermobile type (EDS-HT) syndrome, pr

Multi-Omic Integration of the miRNome and Transcriptome in Dermal Fibroblasts from Hypermobile Ehlers-Danlos Syndrome and Hypermobility Spectrum Disorders Identifies Convergent Candidate miRNA-mRNA Regulatory Networks Linking Extracellular Matrix Remodeling and Inflammation.

Hypermobile Ehlers-Danlos syndrome (hEDS) and hypermobility spectrum disorders (HSD) are clinically defined connective tissue conditions lacking molecular biomarkers. We tested whether their transcriptomic convergence extends to miRNA-mediated post-transcriptional regulation. miRNome sequencing was performed in cultured dermal fibroblasts from 12 individuals with hEDS, 12 with HSD, and 24 controls. Twenty-five differentially expressed miRNAs (DE-miRNAs) were assessed by qPCR in pooled discovery material and an independent pooled cohort. No miRNA met differential-expression threshold in the dir

Contemporary Management of Subclavian and Axillary Artery Aneurysms in Connective Tissue Disease: Navigating Therapeutic Uncertainty in a Rare Vascular Disease Without Dedicated Guidelines.

BACKGROUND: Subclavian artery aneurysms (SAAs) represent one of the rarest forms of peripheral arterial aneurysmal disease. Despite their low incidence, management in patients with underlying connective tissue disorders poses specific challenges related to intrinsic vessel fragility. Because of their rarity, the available literature is largely confined to case reports and small, heterogeneous retrospective series, and management remains largely experience-based rather than evidence-based. Explicit guidance from the Society for Vascular Surgery and the European Society for Vascular Surgery is l

A trial of exosome rich stem cell secretome as a potential treatment for Hypermobile Ehlers-Danlos Syndrome.

INTRODUCTION: Hypermobile Type Ehlers-Danlos Syndrome (hEDS) accounts for 80%-90% of all Ehlers-Danlos cases. hEDS pathophysiology is uncertain. There is no diagnostic test. Symptoms include joint hypermobility, intractable and debilitating joint pain, chronic fatigue, brain fog, irritable bowel symptoms and mast cell activation syndrome. Treatment options are limited and ineffective. None are disease-modifying. This pilot study evaluated the safety and potential for efficacy of a novel biologic investigational product (IP) comprising an extracellular vesicle enriched secretome from human bone

Fracture Prevalence Among Those With Hypermobility Spectrum Disorder/Hypermobile Ehlers-Danlos Syndrome.

Previous series have questioned bone fragility in those with hypermobility spectrum disorders (HSD) or hypermobile Ehlers-Danlos syndrome (hEDS). These conclusions have been extrapolated to infants and children often in the setting of suspected child abuse. Few studies address this in non-ambulatory children, leaving medical professionals and the courts alike to debate the reported observations. One hundred patients with HSD/hEDS were recruited from a large hypermobility clinic. Each was required to have a guardian/parent present to attest to the fracture history for the entire age of the indi

The association between collagenopathies and the risk of spontaneous preterm birth: A systematic review with selective meta-analysis.

INTRODUCTION: Spontaneous preterm birth (sPTB) remains a leading cause of neonatal morbidity and mortality. Inherited collagenopathies, including Ehlers-Danlos syndrome (EDS) and Marfan syndrome (MFS), may predispose to sPTB through impaired connective tissue integrity; however, the available evidence has not been systematically synthesized according to collagenopathy subtype. We aimed to evaluate the association between inherited collagenopathies and sPTB through a systematic review, undertaking quantitative synthesis only where clinically appropriate and methodologically comparable. Inducible Laryngeal Obstruction in Hypermobile Patients.

OBJECTIVE: To determine whether patients with hypermobile Ehlers-Danlos syndrome (hEDS) or hypermobility spectrum disorders (HSD) and inducible laryngeal obstruction (ILO) have more voice, cough, swallowing, GI issues, and MC activation than patients without ILO. METHODS: This retrospective study of 2162 patients meeting 2017 diagnostic criteria for hEDS (n = 507) or HSD (n = 1655) who were seen at the EDS Clinic in Mayo Clinic Florida from November 2019 to June 2025 compared patients who screened positive for ILO to those who screened negative. Patients were examined for self-reported sympto

Acupuncture-Induced Pneumothorax Following Thoracic Acupuncture in a Patient With Ehlers-Danlos Syndrome: A Case Report.

Acupuncture is widely used as an adjunctive therapy for chronic pain and headache disorders and is generally considered a safe procedure when performed by trained practitioners. However, pneumothorax remains one of its most serious, albeit uncommon, complications. Patients with connective tissue disorders, such as Ehlers-Danlos syndrome (EDS), may theoretically be more susceptible to procedure-related complications because of underlying tissue fragility, although reports describing acupuncture-induced pneumothorax in this population are exceptionally rare. We present the case of a young woman

MOLECULAR AND CLINICAL FEATURES OF KYPHOSCOLIOTIC EHLERS-DANLOS SYNDROME DUE TO LYSYL HYDROXYLASE 1 DEFICIENCY.

Kyphoscoliotic Ehlers-Danlos syndrome (kEDS) is caused by biallelic pathogenic variants in either PLOD1, encoding the enzyme lysyl hydroxylase 1 (LH1), which is crucial for collagen lysine hydroxylation and crosslink formation or in FKBP14, encoding the collagen chaperone FKBP22. This study aims to define the clinical and molecular spectrum of kEDS-PLOD1, phenotypically compare it with kEDS-FKBP14 and explore the consequences of pathogenic PLOD1 variants on collagen biosynthesis. We studied the clinical features of 93 individuals with biallelic PLOD1 variants, including 13 unreported patients.

Novel treatment of celiac trunk and proximal splenic artery aneurysms in vascular Ehlers-Danlos patient with combined Onyx and coil embolization.

Vascular Ehlers-Danlos syndrome is associated with severe arterial fragility and life-threatening visceral complications. We report a 30-year-old woman with genetically confirmed vascular Ehlers-Danlos syndrome who presented with abdominal pain, nausea, and emesis. Persistent symptoms prompted serial computed tomography angiography (CTA), which revealed progressive celiac trunk and splenic artery dissections with multiple pseudoaneurysms. Given interval pseudoaneurysm growth and ongoing pain, she underwent joint endovascular repair by interventional radiology and vascular surgery via open left

Bilateral anterior tibial artery pseudoaneurysms triggered by minor blunt trauma as the initial presentation of vascular Ehlers-Danlos syndrome with a whole-exon deletion.

Bilateral lower-limb arterial lesions after minor trauma are rare and suggestive of vascular Ehlers-Danlos syndrome (vEDS). A 40-year-old man developed bilateral anterior tibial artery pseudoaneurysms after being kicked. The right limb, complicated by compartment syndrome, was treated by anterior tibial artery ligation with aneurysm incision and hematoma evacuation; the left, by coil embolization. COL3A1 sequencing was negative, but multiplex ligation-dependent probe amplification revealed a monoallelic whole-exon deletion, confirming vEDS. Hybrid management is safe in vEDS, and copy-number an

Selective Endovascular Aortic Repair in Connective Tissue Disease Patients Within a Multidisciplinary Program Achieves Comparable Outcomes in High-Risk Patients.

INTRODUCTION: Endovascular aortic repair (ENDO) in connective tissue disease (CTD) patients has historically been discouraged in societal guidelines because of durability concerns. However, high-volume aortic centers increasingly employ ENDO strategies in carefully selected high-risk scenarios such as rupture, dissection-related pathology, or hostile anatomy. Herein, we describe temporal ENDO adoption at our multidisciplinary CTD aortic program. METHODS: Ninety-two CTD patients undergoing aortic intervention at a single high-volume aortic center between 2005 and 2023 were analyzed. Patients w
