Aortic dissection during the perinatal period in women with Marfan-related disorders: a retrospective cohort study using the Japanese Diagnosis Procedure Combination database.

OBJECTIVE: Women with Marfan-related disorders face an elevated risk of aortic dissection during pregnancy and the postpartum period. This study aimed to investigate the incidence of aortic dissection during the perinatal period in women with Marfan-related disorders. METHODS: This retrospective cohort study analyzed data extracted from the Japanese Diagnosis Procedure Combination, a nationwide administrative claims database. We included women diagnosed with Marfan-related disorders (Marfan syndrome, Loeys-Dietz syndrome, or Ehlers-Danlos syndrome) who delivered between 2010 and 2023. We asse

Cardiovascular and Clinical Manifestations of Marfan Syndrome and Other Inherited Connective Tissue Disorders with Coexisting Genetic Variants.

Marfan syndrome (MS), Loeys-Dietz syndrome (LDS), Beals-Hecht syndrome (BHS), Ehlers-Danlos syndrome (EDS), and individuals with undifferentiated connective tissue disease (UCTD) exhibit phenotypic overlap, suggesting a likelihood of genotypic coexistence. Our objective was to evaluate genetic variants (GVs), encoding 174 genes related to aortopathies, cardiomyopathies, arrhythmias, structural heart disease, and hypercholesterolemia, and their relationship to clinical and cardiovascular damage in these syndromes. This was a prospective study in Mexican patients with MS, LDS, EDS, BHS, and UCTD

Multimodal Wearable and Survey Data Reveal Distinct Physiologic Profiles in Hypermobile-Ehlers Danlos Syndrome for Screening Advancements.

Hypermobile Ehlers-Danlos Syndrome (hEDS) is a genetic connective tissue disorder characterized by hypermobile joints, chronic pain, fatigue, brain fog, orthostatic intolerance, and GI symptoms and dysmotility. Its heterogeneous presentation contributes to poor quality of life, inappropriate interventions, and prolonged diagnostic delays, often up to 10 years. This study primarily aimed to determine if physiological signals captured by a medical-grade wrist wearable could characterize autonomic patterns in hEDS and relate them to symptoms. Individuals with hEDS (n=30) and healthy controls (n=2

A novel deep intronic COL5A1 variant in an Ehlers-Danlos syndrome family: functional characterization by minigene assay.

This study aimed to identify the genetic cause of Ehlers-Danlos syndrome (EDS) in a Chinese family and to evaluate the functional impact of a deep intronic COL5A1 variant using a minigene assay. The genetic analysis was performed using medical whole-exome sequencing, covering 5,398 genes associated with human genetic diseases. A candidate variant was prioritized based on its segregation and potential effect on splicing. To confirm functional effects, wild-type and mutant minigene constructs were created and transfected into HEK293T cells. RT-PCR and sequencing were used to analyze splicing pro

Comorbid Conditions of Ehlers-Danlos Syndromes and Vulvodynia: A Latent Class Analysis.

PURPOSE: Having an Ehlers-Danlos syndrome (EDS) or hypermobility spectrum disorder (HSD) may increase the likelihood of vulvodynia six-fold. Studying vulvodynia in EDS/HSD may help identify causes of vulvodynia and potential treatments. Currently there are no consistently effective treatment methods for vulvodynia. We aim to identify comorbid condition patterns and how they affect a participant's likelihood of screening positive for vulvodynia. DESIGN: Online survey. METHODS: A secondary analysis was conducted using latent class analysis on data from females aged ≥18 years with EDS or HSD (N

Hypermobility and chronic pain in adolescents: diverging functional and neural profiles without sensory differences.

Chronic pain (CP) affects approximately 20% of children, significantly impacting their physical, emotional, and social well-being. Hypermobility disorders (HDs) are prevalent in pediatric populations, including those with CP, and often contribute to impairments in psychological and physical functioning. This study examined differences in sensory, functional, and neural circuitry among CP in youth with and without comorbid HD (CP + HD). Contrary to our a priori hypothesis and previous literature suggesting that youth with CP + HD would exhibit greater suprathreshold pain sensitivity and lower p

The myodural bridge complex: a comprehensive review of morphology, physiology, developmental biology and pathology.

The myodural bridge complex (MDBC) is a phylogenetically conserved composite anatomical structure that anchors the suboccipital musculature and nuchal ligament to the spinal dura mater (SDM) at the cranio-cervical junction, with species-specific morphological adaptations across vertebrates. Physiological functions of the MDBC include hypothesized mediation of cerebrospinal fluid (CSF) circulation dynamics, stabilizing the SDM during head movements to prevent dural folding, and transmitting proprioceptive signals from suboccipital musculature to the central nervous system. Clinical evidence inc

Development and evaluation of a vision pose-tracking based Beighton score tool for generalized joint hypermobility in individuals with suspected Ehlers-Danlos syndromes.

BACKGROUND: Generalized joint hypermobility (GJH) is often challenging to assess, but its presence could suggest a syndromic diagnosis of Ehlers-Danlos Syndromes (EDS). OBJECTIVE: An automated and objective method for estimating joint hypermobility with Beighton score using short video clips is proposed. METHOD: A total of 225 adults (91.8% female, median age 32.0, range 18-64) referred to a specialized EDS clinic were recruited for this study. A video-based method relying on pose-estimation libraries was developed to predict per-joint hypermobility of both elbows, knees, fifth fingers, thum
