Comparison of 2% lidocaine with epinephrine and 4% articaine with epinephrine in local anesthesia of maxillary molars affected by molar incisor hypomineralization: a randomized controlled trial.

The use of local anesthesia is essential for pain control during the treatment of MIH-affected molars. This study aimed to compare injection-related pain and anesthetic efficacy between 2% lidocaine with epinephrine (LE) and 4% articaine with epinephrine (AE) in the MIH-affected maxillary molars of pediatric patients. Twenty-six children aged 6 to 12 years requiring restorative treatment of MIH-affected maxillary molars were enrolled. Participants randomly received one of the two anesthetic solutions (LE or AE) at the first visit; the other solution was administered at the second visit. Inject

Genetic Bases of Arthrogryposis Multiplex Congenita.

Arthrogryposis multiplex congenita (AMC) is characterized by congenital joint contractures in two or more body areas resulting from reduced or absent fetal movements. AMC exhibits marked phenotypic and genetic heterogeneity, as it is a symptom rather than a disease and may be part of a large number of unrelated conditions. Despite advances in genomic approaches and the increasing number of newly identified genes, disease-gene identification was achieved in fewer than half of cases in several reports, including in a French cohort of 367 AMC patients. The most frequent cause of AMC in these repo

Natural history of hypochondroplasia: A retrospective, matched-cohort study using the Clinical Practice Research Datalink (CPRD) Aurum database.

Hypochondroplasia is a rare genetic condition that causes disproportionate short stature. This retrospective, real-world, matched-cohort study used electronic health records data from the Clinical Practice Research Datalink Aurum primary care database, linked to Hospital Episode Statistics and Office of National Statistics mortality records, to assess the natural history of hypochondroplasia in England from 1998 to 2019. A total of 549 participants with hypochondroplasia and 2196 matched controls were included. Mean follow-up time was 7.50 years for participants with hypochondroplasia and 11.6

Neuromuscular junction dysfunction in a subset of Charcot-Marie Tooth and related peripheral neuropathies mouse models.

Charcot-Marie Tooth (CMT) disease is a clinically and genetically heterogeneous inherited peripheral neuropathy for which there is no treatment. CMT patients often present with weakness, fatigue, and muscle atrophy in the distal limbs. Improving function at the neuromuscular junction (NMJ) may improve function in some CMT patients. Using mouse models, we investigated eight CMT subtypes for NMJ phenotypes by morphology and functional deficits assessed by electromyography (EMG). We did not find NMJ abnormalities in mice with mutations in Gjb1Y/Δ2 (CMT1X), or Yars1E196K/E196K (diCMTC). Mice with

Complex Genomic Rearrangement Involving the TBX4 Promoter Manifesting With Variable Expressivity in a Five-Generation Family.

Variants involving TBX4 are associated with ischiocoxopodopatellar syndrome with or without pulmonary arterial hypertension (ICPPS; also known as small patella syndrome), pulmonary arterial hypertension (PAH), and lethal lung developmental disorders. The variability of penetrance and expressivity of TBX4 variants remains a prominent challenge in understanding their genotype-phenotype correlations. We investigated a five-generation family with 12 affected individuals presenting with isolated ICPPS, lung-related manifestations with features of ICPPS, and other milder abnormalities. Whole-genome

Radiologic Phenotyping in Patients with SOX10 Pathogenic Variants: Insights into Neck, Brain, and Temporal Bone Abnormalities.

BACKGROUND AND PURPOSE: The SOX10 gene is essential for developing neural crest-derived tissues, including the temporal bone, peripheral and central nervous systems, and melanocytes. Pathogenic SOX10 variants are linked to a broad clinical spectrum, including Waardenburg syndrome (WS) in isolation, Kallmann syndrome (KS), as well as an entity characterized by the combination of peripheral demyelinating neuropathy, central dysmyelinating leukodystrophy, Waardenburg syndrome, and Hirschsprung disease (PCWH). This study aims to characterize the full spectrum of abnormalities in the brain, tempora

Case Report: Sustained Cognitive Improvement and Reversible Brain Hypoperfusion in the Context of Posterior Fossa Arachnoid Cyst Shunting Procedure in KBG Syndrome.

KBG syndrome is a genetic syndrome characterized by developmental delay, mild autism spectrum disorder (ASD), variable cognitive disorders and distinctive craniofacial and skeletal features. Posterior fossa arachnoid cysts (PFACs) have been reported in this condition as in other syndromic forms of ASD, but their impact remains uncertain. Here, we report a ten-year follow-up of a patient with KBG syndrome and PFAC shunted for progressive cyst enlargement and his clinical symptoms, who showed important cognitive and behavior improvement post-drainage. Preoperative neuropsychological assessment r

Assessment of airway characteristics in Pierre Robin sequence using computed tomography-based 3-dimensional reconstruction and numerical simulation.

BACKGROUND: Pierre Robin sequence (PRS) syndrome has clinical features of small mandible, tongue drooping, and respiratory obstruction. This study compared the 3-dimensional (3D) reconstruction of the upper airway anatomy based on computed tomography (CT) data between PRS and normal control (NC) children, and analyzed the dynamical changes and airflow field using computer fluid dynamics (CFD) between the two populations. METHODS: A retrospective study was conducted on 66 PRS and 23 NC children, and all CT data were obtained and used for reconstruction and numerical simulation. 10 anatomical l

Autologous cricoid graft reconstruction of complete transglottic laryngeal atresia: Case report.

INTRODUCTION: Improved survival in congenital high airway obstruction syndrome (CHAOS) has shifted management toward definitive airway reconstruction and functional airway independence. However, severe cartilaginous laryngeal atresia may distort normal laryngotracheal anatomy and limit conventional reconstructive approaches. We describe an anatomy-specific reconstruction using obstructing dysplastic cricoid cartilage as an autologous posterior graft. CLINICAL FINDINGS: A male infant with CHAOS underwent EXIT-to-tracheostomy at 32 weeks' gestation. At age three, airway evaluation demonstrated

Endoscopic endonasal management of sellar/suprasellar arachnoid cysts: tailored surgical strategies and systematic review.

INTRODUCTION: Sellar and suprasellar arachnoid cysts(SACs)are rare lesions that may cause headaches, visual impairment, and endocrine dysfunction. While endoscopic endonasal approaches(EEA)are increasingly preferred, optimal surgical strategy-simple fenestration versus wide cyst-subarachnoid space(SAS)communication-remains debated. This study reports one of the largest single-center series of SACs treated via EEA for SACs and integrates these findings with an updated systematic review of the literature. METHODS: A retrospective review was conducted on 6178 endoscopic endonasal surgeries perfo

Horizontal gaze palsy with progressive scoliosis in siblings with opposing spinal curves and heart positions: is there a relationships and/or connection to etiology?

PURPOSE: Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare neurodevelopmental disorder marked by the absence of horizontal eye movement and gradual progressive scoliosis during childhood. Here, we report two biological siblings that presented with a shared diagnosis; however, the curves are in opposite directions and one presents with dextrocardia. METHODS: Two siblings presented with acute scoliosis and conjugate absence of horizontal gaze. The diagnosis was confirmed based on characteristic clinical presentation, congenital absence of horizontal gaze, and supportive radiogr

A Child with Three Legs or Conjoint Parasitic Twin?

AIM: Rachiphagus is a rare congenital anomaly in which conjoined twins are fused at the midline of the vertebral column region. When one twin is malformed, the condition is referred to as a parasitic twin. The term "parasitic twin" also encompasses cases involving extra limbs or limb-like structures. Despite ongoing research, the underlying causes of this condition remain unknown. MATERIAL AND METHODS: This is a systematic review of the literature with a case report. A literature search was done in English language in PubMed and Semantic-Scholar from 1952 to 2023. All articles and cases with

Update on Congenital Cranial Dysinnervation Disorders (CCDDs).

Congenital cranial dysinnervation disorders (CCDDs) are a group of rare, nonprogressive conditions characterized by abnormal development of the cranial motor nerves and variable ocular motility deficits, ptosis, incomitant strabismus, and facial palsy. Advances in genetics and neuroimaging have revealed that these disorders result from defects in neuronal differentiation or axon guidance of the cranial motor neurons. Duane retraction syndrome, the most common CCDD, results from the absence of the abducens nerve and innervation of the lateral rectus by oculomotor nerve axons; causative genes in

[National Protocol for the Diagnosis and Management of Stickler syndrome: Summary for the attending physician].

Stickler syndrome, first described by Günnar Stickler in the United States in 1965 as "progressive hereditary arthro-ophthalmopathy", is classified by ophthalmologists as a rare hereditary vitreoretinopathy of connective tissue. Thanks to advances in molecular genetics, different subtypes of the syndrome are now recognized. Its prevalence at birth has been estimated to be between 1 in 7500 and 1 in 10,000. Stickler syndrome is a rare genetic disorder, specifically a multi-system collagenopathy. It is characterized by ocular abnormalities such as severe congenital myopia, cataracts, and retinal

In Vitro Fertilization and VACTERL Birth Defects.

IMPORTANCE: The VACTERL (vertebral defects, anal atresia, cardiac defects, tracheo-esophageal fistula, renal defects, and limb defects) pattern of birth defects is a complex congenital condition with largely unknown causes. Studies suggest a link between in vitro fertilization (IVF) and birth defects, including VACTERL, but it remains unknown whether IVF has a unique association with VACTERL. OBJECTIVE: To characterize the association between IVF and VACTERL. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included 1 555 936 live births in 4 US States (Massachusetts, New York, North Car
