Cochlear Implantation in a Child With Arnold-Chiari Syndrome, Severe Inner Ear Malformations, and Cochlear Nerve Hypoplasia: A Case Report.

BACKGROUND Arnold-Chiari syndrome is a congenital malformation of the posterior cranial fossa associated with neurological and otoneurological abnormalities, including hearing loss of variable severity. Cochlear implantation in patients with complex central nervous system malformations remains challenging due to uncertain auditory outcomes. This study aimed to evaluate the feasibility and early outcomes of cochlear implantation in a child with Arnold-Chiari syndrome and severe bilateral inner ear malformations. CASE REPORT A 1.5-year-old girl with Arnold-Chiari malformation, hydrocephalus, spi

[The role of neuroprotection in treating oculomotor neuropathy in a child following endoscopic cystocisternostomy].

This report presents a clinical case involving a 13-year-old patient diagnosed with an instrumentally confirmed arachnoid cyst located in the right Sylvian fissure (Galassi type III). Following the surgical procedure of endoscopic cystocisternostomy, the patient exhibited signs consistent with oculomotor nerve neuropathy on the right side. Consequently, Cytoflavin (inosine+nicotinamide+riboflavin+succinic acid) was included in the treatment regimen, administered intravenously at a dose of 10.0 g in 200 mL of 0.9% sodium chloride solution daily for 10 days. During the treatment, the patient's c

High-Resolution Ultrasound in the Evaluation of Median and Superficial Radial Nerve Involvement in Neurofibromatosis Type 1.

This presentation highlights the value of high-resolution ultrasound for simultaneously characterizing superficial neurofibromas and detecting associated peripheral nerve involvement in young patients with neurofibromatosis type 1. In a 16-year-old female with hand pain and paresthesia, ultrasound demonstrated a hypervascular superficial neurofibroma together with marked epineural thickening of the median nerve, swelling of its recurrent motor branch, and edema of the superficial radial nerve. These findings provided an anatomic correlate for the patient's symptoms despite normal median and ul

[Newborn with neurofibromatosis type 1 and meningoencephalocele].

In this case report, a newborn with neurofibromatosis type 1 (NF1) and meningoencephalocele is presented. The foetus had inherited the same pathogenic defect in the NF1 gene as the father and was therefore closely monitored prenatally. The infant was born with a large frontonasal meningoencephalocele. This has not been described before and medical experience and knowledge is therefore sparse. In this case, early diagnostic imaging and scheduling of surgical intervention in collaboration with neuro- and plastic surgeons was performed.

Managing Plexiform Neurofibroma and Other Complications of Neurofibromatosis Type 1 in the Adult Neurology Clinic: A Review.

PURPOSE OF REVIEW: Loss-of-function variants in the NF1 gene result in the development of neurofibromatosis type 1 (NF1), a neurogenetic disease and multisystem disorder that includes a predilection for benign and malignant tumors of the central and peripheral nervous systems. Patients with NF1 experience a wide variety of comorbidities, such as chronic pain conditions, learning disabilities, and signs and symptoms that include migraines and NF1-associated plexiform neurofibromas (PNs), which can cause disfigurement and pain. All of these factors can severely affect health-related quality of l

Periconceptional and early-pregnancy exposure to SGLT2 inhibitors and congenital malformations: a systematic review and meta-analysis.

PURPOSE: Sodium-glucose cotransporter-2 (SGLT2) inhibitors are increasingly prescribed to women of reproductive age for type 2 diabetes and cardiorenal protection, and are commonly continued through the periconceptional period until pregnancy is recognized. Human safety data on inadvertent first-trimester exposure remain scarce, creating substantial clinical uncertainty. METHODS: We systematically searched PubMed/MEDLINE, Embase, and CENTRAL from inception to 15 May 2026 for observational cohort studies comparing early-pregnancy or preconception exposure to SGLT2 inhibitors with active antidi

Clinical and genetic analysis of patients with Sjögren-Larsson syndrome in China.

OBJECTIVE: To characterize the clinical features and genetic spectrum of Chinese patients with Sjögren-Larsson syndrome (SLS). METHODS: We retrospectively reviewed genetically confirmed SLS cases managed in the Functional Neurosurgery Department of Beijing Children's Hospital. We also searched Chinese- and English-language databases to identify additional SLS cases for comparative analysis. Structural effects of detected variants on fatty aldehyde dehydrogenase (FALDH) were explored using PDB-derived models and PyMOL. RESULTS: Three SLS patients were identified, including one carrying a nove

Prenatal androgens and female-advantaged cognition: A systematic review and Bayesian meta-analysis in congenital adrenal hyperplasia.

Females with Congenital Adrenal Hyperplasia (CAH) are exposed to elevated levels of androgens during the prenatal period, offering insights into the effects of early hormonal influences on brain and cognition. Although spatial abilities, which typically favor males, have received considerable attention in the context of CAH, this is the first review to systematically examine cognitive domains that generally show a female advantage. By meta-analyzing 43 effect sizes from 11 studies, we assessed whether elevated prenatal androgens are associated with shifts toward male-typical performance across

Hippocampal volume and white matter impairments in patients with congenital adrenal hyperplasia.

CONTEXT: Patients with congenital adrenal hyperplasia (CAH) have lifelong imbalances in adrenal hormones that may result in changes in structure and function of the brain that can impair cognitive performance. The hippocampus might be especially sensitive to cortisol and androgen disturbances but has not been systematically investigated in this patient group. OBJECTIVE: In this study, we assessed hippocampal subfield volumes, mean diffusivity, and resting-state functional connectivity along with memory performance in patients with CAH and healthy controls using structural magnetic resonance i

[Arachnoid web presenting as tetraparesis].

Arachnoid webs are rare intradural lesions which disrupt cerebrospinal fluid flow and may cause spinal cord compression. This is a case report of a previously healthy 19-year-old woman who presented with acute back pain and rapidly progressive tetraparesis. MRI revealed cervical spinal cord ischaemia and an arachnoid web at the thoracic level. Surgical removal led to partial neurological recovery. Early recognition is essential, as timely surgery may prevent irreversible injury.

Late-onset cobalamin C defect with a rare MMACHC c.457 C > T variant: a case report and literature review.

BACKGROUND: Cobalamin C (cblC) defect, also referred to as combined methylmalonic acidemia and homocystinuria, is the most common inborn error of intracellular cobalamin metabolism. Late-onset cblC defect often presents with atypical and heterogeneous manifestations, particularly neuropsychiatric symptoms, which may lead to misdiagnosis and delayed treatment. CASE PRESENTATION: We report a 13-year-old girl with late-onset cblC defect who presented with progressive cognitive impairment, psychiatric and behavioral abnormalities, and motor dysfunction. Because of these atypical manifestations, s

[Branchial cleft anomalies of the neck : Embryology, clinical presentation, and management].

Branchial cleft anomalies are the second most common congenital neck lesion. They present as cysts, sinuses, or fistulas arising from remnants of the pharyngeal apparatus; second arch anomalies predominate. The embryologic origin determines their anatomical relationships to nerves, vessels, and the pharynx and guides diagnosis and surgical planning. Typical findings are cervical swelling, cutaneous discharge, or recurrent infection. Third and fourth branchial anomalies may cause recurrent neck abscesses or acute suppurative thyroiditis, particularly in children. Ultrasonography is the primary

Putative Transcriptomic Endotypes Reveal Neuronal, Immune, and Metabolic Heterogeneity in Temporal Lobe Epilepsy with Hippocampal Sclerosis.

Temporal lobe epilepsy with hippocampal sclerosis (TLE-HS) is a biologically heterogeneous disorder in which neuronal loss, gliosis, neuroinflammation, and metabolic stress may coexist but are unlikely to be uniformly represented across all patients. To investigate this heterogeneity at the molecular level, we analysed publicly available hippocampal transcriptomic data from individuals with TLE-HS. Our analyses identified three transcriptomically defined putative endotypes within this shared neuropathological diagnosis. These states were associated with distinct molecular programs, including a

Epilepsy in paediatric neurofibromatosis type 1: clinical features and associated factors in a single-centere cohort.

UNLABELLED: Neurofibromatosis type 1 (NF1) is associated with an increased risk of epilepsy, but the clinical spectrum and associated factors of epilepsy in paediatric NF1 remain incompletely characterized. We aimed to describe frequency and features of epilepsy in paediatric NF1 and to compare the characteristics of NF1 patients with and without epilepsy. We retrospectively reviewed 253 paediatric NF1 patients followed up between January 2020 and December 2025. Demographic, clinical, electroencephalographic, and radiological data were collected. Categorical comparisons between patients with a

Inherited and early-life vulnerability in functional neurological disorder and functional somatic symptoms: A retrospective cohort and case-control study from a large electronic health record database.

BACKGROUND: Biological factors affecting motor, sensory, and integrative processing may predispose to functional neurological disorder (FND) and functional somatic symptoms. While early-life influences have traditionally been conceptualized within psychological frameworks, the contribution of early-life biological vulnerabilities to symptom expression remains unclear. METHODS: Using TriNetX, a large electronic health records database, we assessed proportions of inherited, congenital, perinatal, and neurodevelopmental diagnoses, capturing a range of early-life conditions. We compared 188,868 i

Development and validation of a prediction model for cryptogenic ischemic stroke risk in migraine patients with right-to-left shunt: A 10-year study.

AimLong-term risk stratification for cryptogenic ischemic stroke (CIS) remains uncertain in patients with migraine and contrast-enhanced transcranial Doppler ultrasound-confirmed right-to-left shunting (RLS). Therefore, we aimed to develop and externally validate a simple baseline score for identifying individuals at low risk.MethodsThis 10-year study included 978 and 237 patients in the development and external validation cohorts, respectively. Internal validation involved a five-fold cross-validation strategy. Multivariable Cox proportional hazards modeling was used to develop the Migraine R

Molecular-genetic mechanisms of cerebral arteriovenous malformations: A systematic review of germline, somatic and experimental evidence.

Cerebral arteriovenous malformations (AVMs) are high-flow vascular anomalies that carry a considerable burden of haemorrhagic morbidity. Work over the past decade has shown that both inherited syndromes and apparently sporadic lesions can be traced to alterations in a handful of signalling pathways, but the relevant evidence sits awkwardly between human genetic studies, animal models, and molecular profiling experiments. To bring together the current evidence on the molecular and genetic underpinnings of cerebral AVM formation, clinical course, and candidate therapeutic targets. We searched ME

Interventricular septal fat-containing mass most consistent with cardiac lipoma in an adult with tuberous sclerosis complex.

Tuberous sclerosis complex (TSC) is a multisystem genetic disorder in which cardiac rhabdomyomas and myocardial fatty foci are recognized cardiac manifestations, whereas discrete cardiac lipoma-like masses are uncommon. We report a 42-year-old woman with genetically and clinically confirmed TSC who underwent contrast-enhanced chest CT for surveillance of pulmonary lymphangioleiomyomatosis. CT demonstrated a well-circumscribed homogeneous macroscopic-fat attenuation mass centered in the mid-to-apical interventricular septum, measuring 2.8 × 2.2 × 2.0 cm with mean attenuation of approximately -9
