Quality of life of Hirschsprung disease patients with a stoma.

INTRODUCTION: Hirschsprung disease (HSCR) is a genetic disorder leading to gastrointestinal obstruction due to the absence of ganglion cells in the submucosal and myenteric plexuses. Treatment typically involves a pull-through surgery, sometimes starting with a colostomy. The COVID-19 pandemic has resulted in restrictions on general patient services, i.e., non-COVID-19 patients, at health facilities, leading to reduced hospital visits, including HSCR patients with stomas. This study aimed to determine whether there were differences in the quality of life (QoL) of HSCR patients with stomas befo

Histochemical characterization of muscularis macrophage loss and disruption of a miR-93-5p-AHNAK neuro-immune axis in Hirschsprung's disease.

Hirschsprung's disease (HSCR) is a congenital disorder of the distal intestine characterized by aganglionosis of the enteric nervous system. While defective migration of neural crest-derived precursors is a well-established hallmark, how the intestinal microenvironment contributes to impaired neuronal support remains poorly understood. Here, we combined histochemical analyses of human HSCR colon with functional assays to investigate the role of muscularis macrophages (MMs) and a miR-93-5p-AHNAK pathway in regulating enteric neurons. Using immunohistochemistry and immunofluorescence on ganglion

Malrotation after infancy-can we do better? A 10-year multi-centre retrospective review.

UNLABELLED: Malrotation is classically a neonatal condition, but it can present in older children. This population has a non-specific presentation that can delay diagnosis with fatal consequences. We aimed to identify presentation patterns and efficacy of imaging modalities to inform the management of children with suspected malrotation. Children aged 1-18 who had surgery for malrotation between 2014 and 2024 at four centres in the south of England were retrospectively reviewed. Hospital presentations, radiology, operative findings and outcomes were reviewed. Seventy-five. patients were identi

A patient-derived missense mouse model of Kabuki syndrome 1.

Kabuki syndrome type 1 (KS1) is a rare cause of intellectual disability resulting from heterozygous pathogenic variants in the gene encoding the histone methyltransferase KMT2D. A previously established loss-of-function mouse model of KS1 exhibits key phenotypic features, and therapeutic trials in this mouse model suggest postnatal malleability of neurological symptoms. However, 15-30% of individuals with KS1 carry missense variants. To investigate whether missense variants lead to similar phenotypic presentation in mice, we used CRISPR-Cas9 to introduce the KS1- patient variant R5179, corresp

Case Report: Achilles enthesitis as the presenting clue to occult sacroiliitis in hidradenitis suppurativa.

Hidradenitis suppurativa is increasingly recognized as a systemic inflammatory disease with an established overlap with spondyloarthritis. Axial spondyloarthritis is usually suspected when patients report inflammatory back pain, which remains the entry feature of the ASAS axial spondyloarthritis classification criteria. We report a 31-year-old man with Hurley stage II hidradenitis suppurativa who presented with Achilles enthesitis supported by point-of-care ultrasound but denied any current or previous back pain lasting 3 months or longer, including inflammatory back pain and alternating butto

Is torsion of the undescended testis intrinsically associated with worse salvage? A matched cohort analysis.

BACKGROUND: Testicular torsion in undescended testes (UDT) has poor salvage rates, but it remains unclear if the cryptorchid state itself confers an intrinsic disadvantage. We aimed to compare testicular salvage between UDT and descended torsion while controlling for presentation variables. METHODS: In a single-institutional retrospective cohort (2010-2024), using propensity score matching, 25 UDT torsion cases were matched 1:2 to 129 descended torsion controls on age, ischemia time, and testicular volume. The primary outcome was salvage on post-operative imaging (≥50% volume of contralateral

A CACNA2D2-Related Recessive Form of Cerebellar Abiotrophy in Angus Cattle.

Cerebellar disease in ruminants is often virus-induced and non-genetic, but there are also rare inherited forms of cerebellar hypoplasia and cerebellar abiotrophy (CA). So far, no causal variant has been reported for these conditions in cattle. Two inbred Angus calves suspected of having cerebellar disease were reported in Scotland. The aims of this study were to characterize the clinicopathological phenotype of Angus calves affected by a cerebellar disease, to identify a causal variant assuming autosomal monogenic recessive inheritance and to evaluate its prevalence in Angus populations. Radial Reconstruction With Fibular Free Flap Using CAD-CAM and Virtual Surgical Planning in Pediatric Pseudoarthrosis due to Neurofibromatosis Type 1: A Case Report and Literature Review.

Neurofibromatosis type 1 (NF1) is an autosomal dominant disorder that causes skeletal dysplasia and congenital pseudoarthrosis of the forearm, which is very difficult to repair. The vascularized free fibular flap (FFF) is the standard treatment. CAD-CAM technologies and virtual surgical planning (VSP) offer greater precision and their full integration into primary pediatric reconstruction for NF1 is described. The objective of this case report is to present the simultaneous integration of all these tools in the primary reconstruction of a child with forearm pseudarthrosis due to NF1. We presen

Teratogenic risk of carbamazepine, levetiracetam, and lamotrigine treatment in pregnant women with epilepsy.

OBJECTIVE: Antiseizure medications used during pregnancy may exert teratogenic effects on the developing fetus, with risks varying according to drug type, dosage, and exposure timing. While older agents such as carbamazepine are associated with known risks, newer agents including lamotrigine and levetiracetam have demonstrated more favorable safety profiles. This study aimed to evaluate pregnancy outcomes, major congenital malformation rates, and seizure frequency in pregnant women with epilepsy exposed to carbamazepine, lamotrigine, or levetiracetam as monotherapy or polytherapy, compared wit

Expanding the molecular and clinical spectrum of POLR3A-related charcot-marie-tooth disease.

BACKGROUND: POLR3A encodes the largest subunit of RNA polymerase III (Pol III), which, together with POLR3B, forms the catalytic core essential for transcribing small non-coding RNAs, including tRNAs and 5 S rRNA. Although biallelic POLR3A variants are well-known causes of hypomyelinating leukodystrophy, a recent study has expanded the phenotypic spectrum to include peripheral neuropathy associated with heterozygous de novo variants. METHODS AND RESULTS: We present an additional case supporting an association between POLR3A dysfunction and Charcot-Marie-Tooth (CMT) disease. The patient was di
