The homozygous founder Psmb8 variant of Nakajo-Nishimura syndrome/proteasome-associated autoinflammatory syndrome causes panniculitis-associated lipoatrophy and a shortened lifespan in mice.

Nakajo-Nishimura syndrome/proteasome-associated autoinflammatory syndrome (NNS/PRAAS) is a hereditary autoinflammatory disease. Clinically, NNS/PRAAS is characterized by periodic fever, skin rash, partial lipo-muscular atrophy, and joint contractures. Among PRAAS, NNS, is genetically characterized by a homozygous founder variant in the proteasome subunit beta type 8 (PSMB8) gene encoding an inducible proteasome component β5i. To establish an in vivo animal model recapitulating NNS/PRAAS, we generated mice harboring this founder variant. In Psmb8G201V/G201V mice, the immature β5i subunit was in

Haemodynamic alterations associated with hepatic vascular malformations in hereditary haemorrhagic telangiectasia: A cohort study.

BACKGROUND & AIMS: Hereditary haemorrhagic telangiectasia (HHT) is a rare autosomal dominant disorder, frequently associated with hepatic vascular malformations (HVMs). Up to 78% of patients develop HVMs, which may lead to haemodynamic alterations, increased cardiac preload, and progression to high-output heart failure (HOHF). The aim was to determine the prevalence of HOHF and assess the relationship between HVM severity, stroke volume, and cardiac index in patients with HHT. A secondary aim was to explore the haemodynamic mechanisms underlying HOHF and evaluate the usefulness of transthoraci

Neuropathological measures of increased tau phosphorylation across the Down syndrome lifespan.

Individuals with Down syndrome (DS) have an increased risk of developing Alzheimer disease (AD), with nearly all individuals exhibiting AD neuropathology, including amyloid beta (Aβ) plaques and neurofibrillary tangles (NFT), by age 40 years. Fluid AD biomarker studies highlight an increase in several phosphorylated tau (p-tau) epitopes in DS. However, neuropathological measures of p-tau epitopes in DS have not been examined. Therefore, our main objective was to characterize p-tau epitope burdens across the DS lifespan at autopsy. We analyzed postmortem brain samples of 98 individuals with lat

Hirschsprung's disease and Mowat-Wilson syndrome: should a pull-through be performed?

BACKGROUND: Hirschsprung's disease (HSCR) is characterized by the absence of ganglion cells. Five percent of cases are associated with syndromic conditions, one of which is Mowat-Wilson syndrome (MWS), with an incidence rate of 50%. HSCR may be the first feature of this syndrome to be diagnosed. MWS is an autosomal dominant genetic disorder caused by a variant in the ZEB2 gene (ZFHX1B). OMIM #235730. It involves severe clinical manifestations such as ocular hypertelorism, intellectual disability, congenital heart defects, epilepsy, and HSCR. The association between MWS and HSCR is regarded as

Full-thickness versus rectal suction biopsy for the diagnosis of Hirschsprung's disease: study protocol for a prospective, randomized open-label, within-patient crossover trail.

BACKGROUND: Hirschsprung disease (HD) is a congenital malformation characterized by the absence of ganglion cells in the bowel wall. The aganglionosis extends from the rectum and proximally into the gastrointestinal tract in various lengths. The diagnosis relies on histological examination of rectal biopsies. Two main biopsy techniques: rectal suction biopsy (RSB) or full-thickness biopsy (FTB) are available. They differ in the degree of invasiveness, postoperative complications and diagnostic accuracy. METHODS: This is a prospective trial including 300 patients aged 0-17 years scheduled for

Hereditary haemorrhagic telangiectasia as a cause of exertional dyspnoea.

Hereditary haemorrhagic telangiectasia (HHT) is an uncommon autosomal dominant vascular disorder, most frequently associated with pathogenic variants in ENG or ACVRL1, resulting in multisystem arteriovenous malformations (AVMs). We report a middle-aged woman with primary antiphospholipid syndrome on chronic enoxaparin therapy who presented with exertional dyspnoea. CT demonstrated bilateral pulmonary AVMs (PAVMs), and clinical evaluation revealed mucocutaneous telangiectasias. Genetic testing identified an ENG mutation, confirming HHT type 1 according to the three Curaçao criteria. Selective m

Cornelia de Lange syndrome in an extremely low birth weight infant: a case report.

Cornelia de Lange syndrome (CdLS) is a rare genetic disorder characterized by distinctive facial features, fetal growth restriction, intellectual disability, and hirsutism. Few reports have described extremely low birth weight (ELBW) infants with CdLS. We report an ELBW infant diagnosed with CdLS. The infant was delivered by emergency cesarean section at 34 weeks and 1 day of gestation, weighing 994 g (-3.7 SD score). A diagnosis of CdLS was suspected based on dysmorphic features, including forehead hirsutism, cleft palate, long philtrum, wide nostrils, syndactyly of toes, femoral hirsutism, a

[Deep vein thrombosis resulting in arterial occlusion].

A patent foramen ovale (PFO) is present in approximately 25-30% of adults and is usually asymptomatic. However, under conditions of elevated right heart pressure, a PFO can enable right-to-left shunting, allowing venous thrombi to enter the arterial circulation: a mechanism known as paradoxical embolism. We present a case of a 51-year-old woman who developed dyspnea, abdominal pain and, a painful swollen leg. Imaging revealed a saddle pulmonary embolism, renal and splenic infarctions and occlusion of the right brachial artery. Echocardiography confirmed the presence of a PFO. Other common site

Advanced neurosurgical intervention of recurrent tethered cord syndrome with split cord malformation type 1: an illustrative case.

PURPOSE: Recurrent tethered cord syndrome (TCS) with split cord malformation (SCM) is a rare presentation with distinct surgical challenges to successful patient outcomes. SCMs are rare and can progress to cause significant neurological deficits including pain, weakness, sensory disturbance, bowel and/or bladder dysfunction. In these circumstances, surgical management of TSC with SCM is indicated. Reoperation for recurrent TCS, particularly in the context of retained or regrown bony septa and prior spinal instrumentation, is technically demanding and carries elevated risk. ILLUSTRATIVE CASE:

Don't Cut the Cord! Sutureless Repair of Gastroschisis-A Case Report.

Gastroschisis is a congenital anomaly that results in the presentation of various portions of the bowel and abdominal contents outside of the body through a defect or opening in the abdominal wall. The absence of a protective membrane around the bowel is a hallmark sign of gastroschisis, in which prior to birth the bowel floats freely in the amniotic fluid. The anomaly is detectable via ultrasound, occurring early in embryologic development, but is not identified prior to 12 weeks' gestation, when the bowel fails to involute to the abdomen. Gastroschisis occurs in 4-4.5/10,000 live births in t

Life expectancy and causes of death in classical laminopathic progeroid syndromes: systematic review with individual-patient data synthesis.

Classical laminopathic progeroid syndromes link nuclear-envelope defects to accelerated aging, but the specific prognosis for each subtype remains unclear. We conducted a PRISMA-guided systematic review and individual-patient data (IPD) synthesis (PROSPERO CRD420251080312; PubMed/Scopus to 16 July 2025). Genetically confirmed IPD constituted the primary cohort; causes of death were harmonized a priori and survival assessed by Kaplan-Meier with log-rank tests. We included 169 studies and two institutional cases, previously illustrated in the literature but not comprehensively characterized, yie

Segmental Pallor and Hair Loss in a Patient With Ross Syndrome: Case Report.

A 30-year-old male sought medical consultation due to sudden anisocoria. Clinical examination confirmed Adie's tonic pupil. Subsequently, the presence of segmental anhidrosis with compensatory hyperhidrosis and areflexia established the diagnosis of Ross syndrome. In a 15-year follow-up, uncommon manifestations developed in the facial territory affected by compensatory hyperhidrosis: unilateral facial pallor and alopecia following a midline-defined pattern. This case emphasizes the importance of recognizing neurocutaneous manifestations in dermatology practice and suggests that Ross syndrome m

Impact of Trisomy 21 on post pull-through enterocolitis and bowel function in children with Hirschsprung disease: A PCPLC analysis.

INTRODUCTION: We evaluated whether Trisomy 21 (T21) affected bowel management use, ability to toilet train for stool, and rates of Hirschsprung-Associated Enterocolitis (HAEC) in children with Hirschsprung disease (HD). We hypothesized that T21 would be associated with increased rates of HAEC, bowel management program (BMP) use, and delayed toilet training for stool. METHODS: Children with HD in the Pediatric Colorectal and Pelvic Learning Consortium (PCPLC) database who underwent primary pull-through were included and stratified by T21 status: HD only versus HD + T21.
