Keratosis pilaris (KP) is a highly prevalent, benign disorder of follicular keratinization characterized by rough keratotic papules and variable perifollicular erythema, most commonly affecting the extensor upper arms, thighs, and buttocks. Although medically harmless, KP is frequently associated with cosmetic distress, reduced self-confidence, and persistent dissatisfaction with skin texture and appearance. The fundamental pathological process involves follicular hyperkeratosis with retention keratosis and abnormal desquamation, often occurring in the context of xerosis and epidermal barrier

Rhabdomyosarcoma (RMS), a rare pediatric soft tissue sarcoma, seldom involves the external ear. Congenital ichthyosis vulgaris (IV), a genetic disorder causing hyperkeratosis, has not been previously linked to RMS. We report the first case of embryonal RMS with unique extension in a patient with IV. A 13 year-old male with ichthyosis presented with a 1 year history of a progressive lesion in the right intertragic notch, unresponsive to antibiotics and topical therapies. Imaging revealed a mass infiltrating the parotid gland, external auditory canal, and sternocleidomastoid muscle. Wide surgica

Ichthyosis vulgaris (IV) is an inherited disorder characterized by the scaling of the skin. It is caused by mutations in the filaggrin gene. IV is a reactive skin manifestation that may be associated with malignant hematological disease. Its association with neoplastic diseases such as Hodgkin lymphoma, anaplastic large-cell lymphoma, and mycosis fungoides has been reported. T-cell non-Hodgkin lymphoma (T-NHL) with ichthyosis has been rarely reported in the literature. Here, we report a case of T-cell lymphoma with congenital IV caused by a desmoglein 1 (DSG1) gene mutation associated with hyp

Ichthyosis, a group of genetic disorders of keratinization, often extends beyond the skin to affect mucosal surfaces, including the ocular surface. This case report of a 15-year-old male illustrates the critical interplay between dermatological conditions and ocular health. Initially treated for chronic eczema, the patient presented with recurrent ocular pain, photophobia, and corneal abnormalities including punctate epithelial erosions, punctate deep stromal opacities, and prominent corneal nerves. A skin biopsy revealed orthokeratosis and a normal granular layer, a key finding supporting a d

Hereditary hemochromatosis (HH) is characterized by elevated iron absorption in the body, leading to iron accumulation with subsequent dysfunction and end-organ damage. While the progression of the disease can result in arthralgias, hepatomegaly, cardiomyopathies, and diabetes, over a third of HH patients present with cutaneous manifestations. We present the case of a 56-year-old male with HH who presented to dermatology with a rash and diffuse scaling. The patient exhibited brown plate-like scales clinically consistent with diffuse ichthyosis vulgaris. While ichthyosis has been seen in patien

Steroid sulphatase (STS) cleaves sulphate groups from steroid hormones, and steroid (sulphate) levels correlate with mood and age-related cognitive decline. In animals, STS inhibition or deletion of the associated gene, enhances memory/neuroprotection and alters hippocampal neurochemistry. Little is known about the consequences of constitutive STS deficiency on memory-related processes in humans. We investigated self-reported memory performance (Multifactorial Memory Questionnaire), word-picture recall and recent mood (Kessler Psychological Distress Scale, K10) in adult males with STS deficien

BACKGROUND: Congenital ichthyoses are a rare Mendelian group of disorders affecting the integument with a heterogeneous clinical presentation amongst which scaling is a constant feature. There is scanty epidemiologic data regarding the clinical profile and histologic patterns of inherited ichthyosis from resource-poor countries.
AIMS AND OBJECTIVES: The study was aimed at assessing the clinic-epidemiologic characteristics associated with the different forms of non-syndromic congenital ichthyosis.
MATERIALS AND METHODS: This was a retrospective chart review of ichthyosis patients that present

This case describes a pediatric patient with a history of ichthyosis vulgaris and global anhidrosis who was diagnosed with erythema ab igne (EAI), a rare dermatosis resulting from chronic heat exposure. After developing progressive, reticulated brown patches on his extremities and abdomen, extensive diagnostic investigations were conducted to rule out autoimmune, vascular, and genetic etiologies. Bloodwork was unrevealing and biopsies showed histologic features closely resembling keratosis lichenoides chronica. Ultimately, after discovering the patient had prolonged exposure to a space heater,

Hypertrophic lichen planus (HLP) is an idiopathic inflammatory condition characterized by hyperkeratotic plaques or nodules, typically occurring bilaterally on the wrists, ankles, or lower extremities. This variant of lichen planus is more common among African-American patients and occupies a broad differential with other keratotic skin conditions, some of which are malignant, making recognition and accurate diagnosis essential. We present an unusual case of a 49-year-old African-American woman with four markedly exophytic, horn-like lesions on her shins, ultimately diagnosed as HLP. Her medic

A man in his 30s presented with several months of non-bloody diarrhoea and nausea along with conjunctivitis, diffuse ichthyosis and cellulitis in the setting of progressive neck swelling. He was ultimately diagnosed with nodular sclerosing Hodgkin's lymphoma after undergoing a broad infectious, rheumatological and neoplastic workup. This represents a rare presentation of classic Hodgkin's lymphoma and demonstrates the known alteration of cellular immunity in Hodgkin's lymphoma alongside manifestations of the profound inflammatory state associated with the disease. The patient was initiated on

Primary atopic disorders (PAD) are monogenic disorders caused by pathogenic gene variants encoding proteins that are key for the maintenance of a healthy skin barrier and a well-functioning immune system. Physicians face the challenge to find single, extremely rare PAD patients/families among the millions of individuals with common allergic diseases. We describe case scenarios with signature PAD. We review the literature and deduct specific clinical red flags for PAD detection. They include a positive family history and/or signs of pathological susceptibility to infections, immunodysregulation

BACKGROUND: X-linked reticular pigmentary disorder (XLPDR) is a rare condition characterized by skin hyperpigmentation, ectodermal features, multiorgan inflammation, and recurrent infections. All probands identified to date share the same intronic hemizygous POLA1 hypomorphic variant (NM_001330360.2(POLA1):c.1393-354A > G) on the X chromosome. Previous studies have supported excessive type 1 interferon (IFN) inflammation and natural killer (NK) cell dysfunction in disease pathogenesis. Common null polymorphisms in filaggrin (FLG) gene underlie ichthyosis vulgaris and atopic predisposition.

BACKGROUND: Congenital ichthyosis is a diverse group of keratinization disorders associated with generalized scaling of skin of varying severity. The non-syndromic forms of congenital ichthyosis are further grouped into common ichthyosis (ichthyosis vulgaris and X-linked ichthyosis), autosomal recessive congenital ichthyosis, and keratopathic ichthyosis.
OBJECTIVE: To identify sequence variants involved in different forms of hereditary ichthyoses.
METHODS: We studied eight families with different types of ichthyosis including four families with autosomal recessive congenital ichthyosis and f

Ever since the association between FLG loss-of-function variants and ichthyosis vulgaris and atopic dermatitis disease onset was identified, FLGs function has been under investigation. Intraindividual genomic predisposition, immunological confounders, and environmental interactions complicate the comparison between FLG genotypes and related causal effects. Using CRISPR/Cas9, we generated human FLG-knockout (ΔFLG) N/TERT-2G keratinocytes. FLG deficiency was shown by immunohistochemistry of human epidermal equivalent cultures. Next to (partial) loss of structural proteins (involucrin, hornerin,

North Caucasus has always been a residence of a lot of different authentic ethnic groups speaking different languages and still living their traditional lifestyle. The diversity appeared to be reflected in the accumulation of different mutations causing common inherited disorders. X-linked ichthyosis represents the second most common form of genodermatoses after ichthyosis vulgaris. Eight patients from three unrelated families of different ethnic origin, Kumyk, Turkish Meskhetians, and Ossetian, with X-linked ichthyosis from the North Caucasian Republic of North Ossetia-Alania were examined. BACKGROUND: A genotype study of filaggrin gene loss-of-function mutations in central India can provide valuable insights into the prevalence and association of these mutations with atopic dermatitis (AD) and ichthyosis vulgaris (IV) in the region. The FLG R501X and 2282del4 are both genetic variants in the human gene called filaggrin gene (FLG), which encodes a protein that plays an important role in the formation and maintenance of the skin barrier. In this study, we determined the FLG R501X and 2282del4 variants association with both AD and IV in Central Indian populations.

Coexistence of TGM1 and FLG mutations in a newborn with congenital ichthyosis is not well described in the literature. Early genetic testing and counseling are crucial for accurate diagnosis and appropriate management. Further exploration of associated problems, including hearing loss and developmental delay, is warranted in patients with these mutations.
