Urinary biomarkers and haematuria as indicators of urogenital schistosomiasis: A systematic review and meta-analysis.

BACKGROUND: Schistosomiasis, is a parasitic infection that leads to chronic illness and socioeconomic challenges in tropical regions. Adult Schistosoma (S.) haematobium worms inhabit the bladder's venous plexus and produce eggs that cause tissue inflammation and systemic pathologies, detectable as urinary biomarkers in bodily fluids like urine, blood, and serum. Urinalysis offers a rapid, cost-effective, non-invasive assessment of schistosomiasis-related urinary biomarkers. OBJECTIVES: This systematic review aimed to identify urinary biomarkers indicative of S. haematobium infections and asse

Diagnostic Potential of S. mansoni Egg and Worm Antigens for Urogenital Schistosomiasis in Resource-Limited Settings.

Around 90% of those at risk for schistosomiasis live in Africa, with urogenital schistosomiasis (UGS) prevalent in Sub-Saharan Africa. This study examines Schistosoma mansoni egg and worm antigens as cost-effective diagnostic alternatives, addressing challenges in maintaining S. haematobium in animal models. Sera and urine samples from schistosomiasis endemic and non-endemic areas were analysed against S. mansoni worm (Sm SWA) and egg antigens (Sm SEA) using indirect ELISA to detect S. haematobium specific antibodies. Microscopy was adopted as the diagnostic reference standard. Sensitivity (SS

Phimosis of foreskin uncovering schistosomiasis of the urinary bladder: a case report.

BACKGROUND: Schistosoma haematobium (S. haematobium) is a freshwater parasite endemic to Africa and parts of the Middle East. While snails serve as intermediate hosts, second-stage larvae infest humans. These migrate to bladder tissues, where they cause inflammation, leading to fibrosis, obstruction, and squamous cell carcinoma of the bladder. CASE DESCRIPTION: This report presents a case of schistosomiasis found in a 10-year-old immigrant from Sudan. He initially presented to urology clinic for consideration of circumcision due to phimosis, but thorough patient interview revealed a history o

Mirroring rarity of ectopic adult Schistosoma haematobium worms in a Sudanese patient.

INTRODUCTION AND IMPORTANCE: Schistosomiasis is the second most common endemic disease after malaria. This case highlights the rarity of adult S. haematobium worms in a sub-granuloma area and explores how they may reach this site. CASE PRESENTATION: A 25-year-old man presented with hematuria, dysuria, and urinary incontinence, with no prior history of schistosomiasis. Investigations revealed numerous RBCs, pus cells, and S. haematobium eggs in his urine. An abdominal ultrasound showed a thickened bladder wall with filling defects. Cystoscopy revealed two bladder tumors and sandy patches, wher

Imported Schistosomiasis in Southwestern Europe: Wide Variation of Pure and Hybrid Genotypes Infecting Sub-Saharan Migrants.

Mitochondrial cox1 and nuclear ribosomal ITSs are commonly combined to distinguish Schistosoma species and identify hybrids in endemic countries but very rarely applied to patients diagnosed in Europe despite the increasing arrival of migrants in southwestern Europe. To assess whether those migrants are carriers of pure or hybrid schistosomes, a complete genetic characterization of Schistosoma entering Spain is performed. A total of 759 eggs (from urine + stools) from 58 patients from 8 African countries were individually processed to describe their mito-nuclear signature by cox1 rapid diagnos

Precision mapping of schistosomiasis and soil-transmitted helminthiasis among school-age children: Targeting interventions in western Kenya.

BACKGROUND: Mapping of schistosomiasis (SCH) and soil-transmitted helminthiasis (STH) is a critical step in understanding where at-risk populations live in order to effectively plan and target available resources and to achieve maximum impact on disease burden. A precision mapping protocol was developed and implemented in Kakamega, Bungoma, Trans Nzoia and Vihiga Counties in western Kenya by applying the current World Health Organization (WHO) mapping guideline at a lower administrative level (Ward). METHODS: Cross-sectional surveys were conducted among school-age children (SAC) in 5 primary

Test accuracy of loop-mediated isothermal amplification for schistosomiasis in low endemicity areas: a systematic review and meta-analysis.

BACKGROUND: Schistosomiasis, caused by parasitic flatworms of the genus Schistosoma, remains a significant public health challenge in tropical and subtropical regions, affecting over hundreds of millions of people in these areas. Accurate diagnosis is crucial for effective disease control, particularly in low-endemic areas where traditional methods like microscopy are no longer effective. We aimed to evaluate the diagnostic performance of loop-mediated isothermal amplification (LAMP) for Schistosoma infection. METHODS: Adhering to Preferred reporting items for systematic reviews and meta-anal

Village and age based precision mapping of schistosomiasis and soil-transmitted helminths in Chevakadzi ward of Shamva district in Zimbabwe.

Schistosomiasis is earmarked for elimination as a public health problem, while morbidity control is a priority for soil-transmitted helminthiasis (STH) by 2030. To develop targeted interventions for the two diseases, micro-mapping and an understanding of disease transmission dynamics across different age groups are necessary. This study determined the age-stratified and ward-level prevalence of schistosomiasis and STH in Chevakadzi ward, Shamva district, Mashonaland Central province, Zimbabwe. A community-wide cross-sectional pilot study was conducted in June and July 2023 in all the 14 villag

Multi-contrast machine learning improves schistosomiasis diagnostic performance.

Schistosomiasis currently affects over 250 million people and remains a public health burden despite ongoing global control efforts. Conventional microscopy is a practical tool for diagnosis and screening of Schistosoma haematobium, but identification of eggs requires a skilled microscopist. Here we present a machine learning (ML)-based strategy for automated detection of S. haematobium that combines two imaging contrasts, brightfield (BF) and darkfield (DF), to improve diagnostic performance. We collected BF and DF images of urine samples, many of them containing S. haematobium eggs, during t

From clinical research to public health action: addressing chronic female genital schistosomiasis in non-endemic countries.

We analysed the early results of a screening and management protocol of imported female genital schistosomiasis (FGS) among migrant women from endemic countries. In total we screened 136 women, from which 39 had suspicion of FGS (serology-positive test and clinical signs and symptoms compatible). Median age was of 42.7 years [interquartile range (IQR 35-45)], median time living in the Europe (EU) was of 14 years (IQR 10-19) and all of them were coming from a West African country, mainly Senegal (17 out of 39; 43.6%). All of them had 1 or more clinical genitourinary findings [median 5 (IQR 2-6)

Microscopic detection and genetic characterization of schistosome eggs within cervicovaginal lavage sediments from cases of female genital schistosomiasis.

Control of female genital schistosomiasis (FGS) has gained significant international attention, driven in part, as a newly appreciated underlying aetiological risk factor for HIV, HPV and cervical dysplasia. Whilst diagnosis and clinical staging of FGS typically relies upon colposcopy, alternative methods of incrimination have grown, particularly upon application of PCR diagnostic assays that detect schistosome DNA within tissue biopsy, genital (self-)swab and/or cervicovaginal lavage (CVL). With regard to the latter, we present novel evidence that microscopy alone of CVL sediments can be suff

Novel sequences of schistosomes and their snail intermediate host species in the Democratic Republic of Congo: first phylogenetic analyses.

Malacological surveys were conducted in 2021 in the Kimpese region of Central Kongo Province, west of the Democratic Republic of Congo (DRC). Snail specimens were collected following a standardised protocol, identified using morphological and molecular methods, and tested for schistosome infection using a diagnostic PCR assay. Positive snail samples were sequenced to characterise the infecting schistosome species. Partial mitochondrial cytochrome c oxidase subunit 1 (COX1) gene sequences were used in phylogenetic analyses to explore the evolutionary position of these snail species within the b

The impact of Schistosoma haematobium hybridization on molecular diagnosis of schistosomiasis: A review with emphasis on female genital schistosomiasis.

Female genital schistosomiasis (FGS) is a gynecological manifestation of urinary schistosomiasis in female genitals. FGS is a neglected tropical disease; not only are most patients unaware of the condition, but healthcare workers and policymakers have inadequate knowledge about it. The treatment and control of FGS relies on current guidelines for controlling and eliminating schistosomiasis without rigorous focus on clinical evidence of the presence of FGS. Neglect of FGS has led to the misconception that the disease is sexually transmitted. Diagnosing FGS remains challenging as there is no wid

Mapping of snail intermediate host habitats reveals variability in schistosome and non-schistosome trematode transmission in an endemic setting.

The intermediate snail host of Schistosoma haematobium, the etiological agent of urogenital schistosomiasis, serves as a critical sentinel for tracking the spread of associated disease risks. In addition to S. haematobium, Bulinus spp. snails also transmit S. bovis to cattle as well as several non-schistosome trematodes to cattle and wildlife. Identifying transmission foci of these multi-parasite hosts is critical for targeted and effective One Health intervention. We investigated 467 waterbodies in 86 villages across six districts in northwestern Tanzania. A total of 43,348 Bulinus nasutus we

Genital schistosomiasis in non-endemic settings: a clinical perspective.

Genital schistosomiasis, caused mainly by infection with Schistosoma haematobium flukes, causes a variety of symptoms and significant complications in men and women. With high levels of migration from sub-Saharan Africa to Europe and North America, genital schistosomiasis is likely to be encountered more frequently by clinicians in non-endemic areas. In this article, we review the current knowledge of genital schistosomiasis in non-endemic areas, available guidelines and barriers to clinical care of patients. Future work to address these barriers will likely improve care for patients with this

Preliminary screening of urinary host protein biomarkers for Schistosomiasis haematobium: A proteome profiling study identifying candidate diagnostic targets in school-aged children.

Schistosomiasis is a major public health challenge and a globally neglected tropical disease. Schistosoma haematobium, the causative agent of urogenital schistosomiasis, is endemic in African countries; with school-aged children ages 7-15 years being the most vulnerable population. Current diagnostic methods rely on microscopy to identify parasite eggs in urine; which is labor-intensive, requires specialized skills, and often lacks sensitivity, especially in mild infections. To address these limitations, we explored host disease-related biomarkers as a promising avenue for advancing diagnosis

Benzalkonium Chloride Significantly Improves Environmental DNA Detection from Schistosomiasis Snail Vectors in Freshwater Samples.

Urogenital schistosomiasis, caused by Schistosoma haematobium and transmitted by Bulinus snails, affects approximately 190 million individuals globally and remains a major public health concern. Effective surveillance of snail vectors is critical for disease control, but traditional identification methods are time-intensive and require specialized expertise. Environmental DNA (eDNA) detection using qPCR has emerged as a promising alternative for large-scale vector surveillance. To prevent eDNA degradation, benzalkonium chloride (BAC) has been proposed as a preservative, though its efficacy wit
