Repeated injury can impair transitions between cellular phenotypes, potentially leading to pathological tissue repair. In recessive dystrophic epidermolysis bullosa (RDEB), collagen VII deficiency predisposes to repeated tissue injury, driving inflammation and fibrosis. Although fibroblasts are major effector cells in RDEB fibrosis, their phenotypic and molecular changes underlying disease progression remain elusive. Through cellular and in situ analyses of clinical samples and tissue from an RDEB mouse model, we disclose a phenotypic switch in dermal fibroblasts during disease progression. At

Nail disorders in children are relatively common and anxiety-provoking for families, yet frequently underrecognized in clinical practice. Many nail disorders in children require no treatment or are readably treatable; however, unfamiliarity may lead to unnecessary, inappropriate, or delayed treatment. In this part 1 of a 2-part continuing medical education series we review physiological variants and congenital, hereditary, and infectious nail disorders, and provide diagnostic and management tips for dermatologists managing these conditions.

Recessive dystrophic epidermolysis bullosa (RDEB) is a genodermatosis caused by loss-of-function mutations in the COL7A1 gene, producing a complex and heterogeneous phenotype. Clinical manifestations include skin fragility, local and systemic inflammation, impaired wound healing, dysbiosis, pain, and pruritus, increasing the risk of skin cancer. This work aimed to develop an actionable model of RDEB's molecular mechanisms to identify and prioritize therapeutic targets for drug repurposing. Transcriptomic data of skin biopsies from pediatric RDEB patients and unaffected individuals were used to

BACKGROUND: Epidermolysis bullosa (EB) is a rare inherited blistering disorder characterized by extreme skin fragility. Autosomal recessive dystrophic EB (RDEB) represents one of the most severe subtypes, typically presenting at birth with widespread cutaneous and mucosal involvement. Management during the neonatal period is particularly challenging, and early diagnosis with appropriate supportive care is essential.
CASE DESCRIPTION: We report the case of a full-term female neonate who developed generalized bullous skin lesions and oral mucosal involvement shortly after birth. Infectious caus

Junctional epidermolysis bullosa (JEB) caused by laminin-332 deficiency (JEB-LAM332) is a severe inherited skin disorder characterized by impaired dermal-epidermal adhesion, chronic wounds, and increased cancer susceptibility. The management of JEB-LAM332 is primarily symptomatic, focusing on wound care and general supportive measures; however, many therapeutic needs remain unmet. While gene and stem cell therapy has been effective in JEB-LAM332, it is not yet broadly available owing to its complexity and cost. Translational readthrough-inducing drugs (TRIDs) represent an alternative strategy

BACKGROUND: Recessive dystrophic epidermolysis bullosa (RDEB) is a life-limiting collagen type VII alpha-1 (COL7A1)-related disorder characterized by skin fragility, chronic wounds, and early mortality. Gene therapies aim to restore collagen type VII (C7) expression and improve wound healing, but clinical evidence remains limited.
OBJECTIVE: To synthesize current evidence on the safety and effectiveness of gene therapy approaches for RDEB.
METHODS: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, we searched major databases through 7 March 2025 for

We report a case of epidermolysis bullosa acquisita (EBA) in a 47-year-old man who suffered for more than 10 years. To evaluate the longitudinal immunological changes, we measured serum levels of anti-basement membrane zone (BMZ) IgG antibodies by indirect immunofluorescence and anti-type VII collagen antibodies by enzyme-linked immunosorbent assay throughout the clinical course. In addition, serum concentrations of B-cell activating factor (BAFF), interleukin (IL)-9, and IL-10 were measured. Titers of anti-BMZ antibodies and anti-type VII collagen antibodies fluctuated during the follow-up pe

OBJECTIVES: Epidermolysis Bullosa (EB) is a rare skin disorder with significant phenotypic variations according to the primary type and subtype of the condition. As a rare disease with uncommon oral features and high phenotypic variability, identifying all characteristics is important to define a patient-specific treatment strategy and monitor the impact of different strategies on the oral outcomes. This project aims to develop and validate an EB-specific oral health assessment tool that helps clinicians to identify every specific oral feature that can be presented in patients with different t

Recessive dystrophic epidermolysis bullosa (RDEB) results from biallelic COL7A1 variants and causes trauma-induced blistering, scarring, and progressive extracutaneous complications. Severe pruritus may intensify skin injury through an itch-scratch cycle. We report a 7-year-old, 25-kg girl with infantile-onset generalized blistering, severe pruritus, mucosal and nail involvement, scarring, and digital contractures. Histopathology showed a subepidermal blister. Sequencing identified heterozygous COL7A1 c.5605G>C (p.Gly1869Arg) and c.5532+5G>A, inherited from different parents and therefore occu

Genodermatoses impose substantial psychosocial and functional burdens on affected children and their families, yet structured community-based support models remain underrepresented in the literature. We describe Casa Santa Teresinha, a Brazilian non-profit organization providing multidisciplinary care for children with genodermatoses and present a programmatic evaluation using the RE-AIM framework (Reach, Effectiveness, Adoption, Implementation, and Maintenance). In 2025, the program engaged 103 patients and their families, delivering 4419 service encounters across psychology, social work, phy

OBJECTIVES: Epidermolysis bullosa (EB) is a rare genetic disease characterized by skin and mucosal fragility caused by pathogenic variants in structural protein genes. Malocclusions have been previously described in patients with EB, mainly in case reports, with no cohort studies characterizing dentofacial characteristics across all EB types. This study aims to describe and compare the dentofacial characteristics of patients with EB across all types in a nationally representative cohort.
MATERIAL AND METHODS: A cross-sectional study examined 101 patients with genetically confirmed EB diagnosi

In a dairy herd of Thuringian Forest goats, a male goat kid of around three weeks of age was detected with skin lesions, particularly on the distal limbs, chest and ears. These skin lesions were characterized by alopecia, pus-filled blisters, incrustations and erosions. The goat kid was referred to the clinic for treatment but had to be euthanized despite intensive treatment due to increasing deterioration in its general condition and was subsequently referred to necropsy. Histologic investigation of the altered areas of skin and mucosa revealed a separation of epithelium from the underlying d

Junctional epidermolysis bullosa (JEB) caused by COL17A1 pathogenic variants (JEB-C17) is characterized by skin fragility, chronic wounds, and an increased risk of squamous cell carcinoma. Approximately 20% of cases are caused by nonsense mutations, which induce premature termination codons (PTCs) resulting in truncated protein production and/or trigger mRNA degradation through nonsense-mediated decay (NMD) pathway. Different approaches have been suggested for nonsense mutation suppression including translational readthrough- inducing drugs (TRIDs) therapy, which is extensively examined in her

Pemphigoid diseases (PDs) are antibody-mediated autoimmune blistering diseases caused by loss of tolerance to basement membrane proteins such as type VII collagen (COL7), triggering pathogenic T and B cell responses. The contribution of regulatory T cells (Tregs) to disease resistance remains unclear. Using a COL7 immunization-induced PD mouse model, we show that transient Treg depletion breaks tolerance and induces skin blistering in otherwise PD-resistant H2b mice. Disease occurred predominantly in females and was associated with impaired Treg recovery, early skin inflammation, altered B-cel

BACKGROUND: Cyclin-dependent kinases (CDKs) are involved in basic cellular processes like regulation of cell-cycle progression and transcription. However, recent data also indicate a specific role in terminally differentiated neutrophils by promoting reactive oxygen species (ROS) release, degranulation, neutrophil extracellular trap formation, or apoptosis. Since these mechanisms are implicated in multiple autoimmune diseases, we aimed to delineate the role of CDKs in immune complex (IC)-mediated autoimmune diseases both in vitro and in vivo.
METHODS: We analyzed CDK activity and gene express
