Phenotypic Characteristics of a Patient Cohort With Recessive Dystrophic Epidermolysis Bullosa and the Pathogenic Variant c.7485+5G>A in Intron 98 of COL7A1.

Recessive dystrophic epidermolysis bullosa (RDEB) is a rare genetic disorder caused by biallelic pathogenic variants in COL7A1, the gene encoding the alpha-1 chain of type VII collagen (C7). While over 100 pathogenic variants in COL7A1 have been linked to RDEB, recurrent variants or hot spots are rare. A recurrent pathogenic variant, c.7485+5G>A (also known as IVS98+5G>A) in intron 98 of COL7A1, has been noted in patients of Hispanic ethnicity seen in the Epidermolysis Bullosa Clinic at Children's Hospital Colorado. We describe the phenotypic characteristics of 17 patients with this variant an

Porous Silicon Nanoneedles Efficiently Deliver Adenine Base Editor to Correct a Recurrent Pathogenic COL7A1 Variant in Recessive Dystrophic Epidermolysis Bullosa.

Base editing, a CRISPR-based genome editing technology, enables precise correction of single-nucleotide variants, promising resolutive treatment for monogenic genetic disorders like recessive dystrophic epidermolysis bullosa (RDEB). However, the application of base editors in cell manufacturing is hindered by inconsistent efficiency and high costs, contributed by suboptimal delivery methods. Nanoneedles have emerged as an effective delivery approach, enabling highly efficient, non-perturbing gene therapies both in vitro and in vivo. Here we demonstrate that nanoneedle delivery of an adenine ba

Progress in skin gene therapy: From the inside and out.

The skin is the largest organ of the body and forms and serves as the barrier for preventing external material from accessing and damaging internal organs. As the outward interface to the environment, it is accessible for the application of therapeutic agents and cellular and gene therapy represent attractive and promising options to treat severe genetic conditions for which palliation has long been the main stay. However, because of its barrier function, transit across and to the subdermal compartment can be challenging. This commentary examines the current approaches of cell and gene therapi

Successful Use of Upadacitinib in Treating a Child With Epidermolysis Bullosa Pruriginosa: A Case Report and Literature Review.

Epidermolysis bullosa pruriginosa (EBP) is a rare subtype of dystrophic epidermolysis bullosa (DEB) that poses significant management challenges. In this case report, a 12-year-old female with EBP was successfully treated with upadacitinib at a dosage of 15 mg once daily for a period of 12 weeks, after which the frequency was changed to every other day. A review of the literature identified 19 cases successfully treated with a Janus kinase inhibitor (JAKi) suggesting that JAKis are a promising option for EBP patients.

Paw Skin as a Translational Model for Investigating Fibrotic and Inflammatory Wound Healing Defects in Recessive Dystrophic Epidermolysis Bullosa.

Recessive dystrophic epidermolysis bullosa (RDEB) is a severe genetic disease caused by COL7A1 mutations. It leads to skin fragility, chronic inflammation, and impaired wound healing. The condition often results in fibrotic scarring, pseudosyndactyly, and cutaneous squamous cell carcinoma (SCC). However, current animal models fail to fully replicate chronic RDEB wounds. In this study, we used Collagen VII-hypomorphic mice (Col7a1flNeo/flNeo) and created full-thickness wounds on their paw skin, an area prone to fibrosis due to mechanical stress. We analyzed the healing process using histology,

Safe and Efficacious Permanent Removal of Large COL7A1 Exons for Gene Reframing as a Reliable Therapeutic Strategy for Recessive Dystrophic Epidermolysis Bullosa.

Mutations leading to premature termination codons in COL7A1 are commonly associated with severe generalized recessive dystrophic epidermolysis bullosa (RDEB). Previous research, including our own, has indicated that removing mutated COL7A1 exons along with the consequent reframing of COL7A1 may not pose noticeable impact on protein function, offering a potential therapeutic strategy. However, investigations into the long-term in vivo effects of genome editing-mediated removal of mutant exons have only focused on the small exon 80 thus far. Hence, this study focuses on exons 73 and 105 of COL7A

Efficacy and safety of rigosertib in patients with recessive dystrophic epidermolysis bullosa-associated advanced/metastatic cutaneous squamous cell carcinoma.

BACKGROUND: Recessive dystrophic epidermolysis bullosa (RDEB) is an epithelial fragility disease primarily affecting the skin and is caused by variants in the COL7A1 gene. Individuals with RDEB are predisposed to develop highly aggressive cutaneous squamous cell carcinomas (SCCs) which are the most common cause of premature death. There is a lack of effective prevention or treatment options for patients with RDEB-SCC. OBJECTIVES: To evaluate the antitumour activity and safety of the polo-like kinase-1 (PLK1) inhibitor, rigosertib, two investigator-initiated open-label, single-arm phase II stu

Reusing of intraoperative exfoliated finger epidermis in the treatment of hand scar contracture in patients with RDEB.

Patients with recessive dystrophic epidermolysis bullosa (RDEB) frequently develop pseudosyndactyly due to recurrent hand blistering and subsequent scar contracture. Conventional release surgery typically employs allogeneic biological dressings or synthetic materials for wound isolation, yet existing approaches are associated with frequent dressing changes, prolonged healing cycles, and secondary trauma. This study pioneers the clinical application of intraoperatively shed epidermal tissue for fabricating autologous epidermal finger cuffs to cover post-surgical defects, evaluating its therapeu

Natural history study of recessive dystrophic epidermolysis bullosa wounds and patient reported outcomes using mobile application home photography.

Recessive dystrophic epidermolysis bullosa (RDEB) is a rare, blistering genetic disease where wounding and wound pain are the most reported problems by patients. The natural history of RDEB wounds, specifically spontaneous wound closure between chronic open vs. recurrent wounds, has not been prospectively studied, thus limiting the design of clinical trial endpoints. We conducted a prospective observational study in which participants used a mobile application to upload weekly photographs of multiple target wounds for up to 6 months and reported associated wound pain and itch (Photo Cohort, N
