Ketamine anesthesia in dermolytic bullous dermatosis (epidermolysis bullosa).

An unusual hereditary disease entity of the integument, dermolytic bullous dermatosis (epidermolysis bullosa dystrophica), recessive type, is presented. The disease is characterized by the formation of bullae, vesicles, and erosions upon the application of the least trauma. Healing results in scarring and incapacitating deformities which require extensive reconstructive surgery. A technic utilizing ketamine-diazepam "dissociative anesthesia" for several surgical procedures for this condition is described. The technic enabled the patient to undergo 3 complex operations over a 2-year period with

Preponderance of lysosomal bodies in cultured fibroblasts from patients with recessive epidermolysis bullosa dystrophica. An electron microscopic study.

Fibroblasts of skin explants from three normal men and six patients with recessive epidermolysis bullosa dystrophica (EBD-R) were cultured in Dulbecco's medium, grown to confluence, sectioned and studied with the electron microscope. The normal control fibroblasts from the 6th to 13th passage (secretory phase) showed irregular or lobed nuclei with centrally scattered chromatin. They also had prominent RER, Golgi complexes, variously shaped mitochondria and cytoskeletal microfibrils. Their peripheral cytoplasm exhibited many vacuoles and a small number of these were autophagic lysosomes. In add

Epidermolysis bullosa dystrophica polydysplastica. A case of anesthetic management in oral surgery.

Epidermolysis bullosa dystrophica is a rare disease that affects the skin and mucous membranes. Manifest at birth, it is characterized by poor dentition, esophageal strictures, syndactyly, and severe chronic anemia. Our 12-year-old patient required extensive dental treatment which necessitated overcoming problems of anesthesia as well as developing a technique of management that provided maximum safety and a minimum of discomfort. Transmission electron microscopy of sections of the gingiva revealed possible degenerative collagen fibers and an interrupted basement membrance. Anchoring fibrils n

[Ultrastructural and genetic aspects of epidermolysis bullosa albo-papuloidea (author's transl)].

The authors report a case of epidermolysis bullosa dystrophica and albo-papuloidea, which led them to the discovery of eight similar cases in the same family. After analysing the main symptoms, including the histological ones, they underline the ultrastructural and genetic data. This disease can be described as a dermolysis bullosa with missing fibrils, abnormal connective tissue and an intense activity of fibroblasts. As far as we know, there is no genetic relationship with the HLA system in this dominant hereditary trouble, which is believed to be transmitted through different non-allelic ge

Epidermolysis bullosa dystrophica dominans (Pasini)-a primary structural defect of the anchoring fibrils.

In epidermolysis bullosa dystrophica dominans Pasini a structural defect of the anchoring fibrils, a structural protein of the epidermo-dermal junction, has been demonstrated by electron microscopy to be constantly present not only in involved skin but also in intact skin of nonpredilection sites of blister formation. These findings render support to the concept that in dominant disorders an abnormality in a non-enzymic structural protein is more likely than in an enzymic protein.

Ultrastructural studies in epidermolysis bullosa hereditaria. II. Dominant dystrophic type of Cockayne and Touraine.

Ultrastructural examination was performed in 9 biopsy specimens from 4 patients with the Cockayne-Touraine type of epidermolysis bullosa dystrophica dominans. The specimens were taken from: 1. clinically normal skin from the blister-nonpredilected sites (trunk) as well as 2. atrophic, 3. intact, and 4. experimentally frictioned skin regions from the blister-predilected sites (extremities). In the frictioned skin a dermolytic blister formations was observed. Development of anchoring fibrils showed a marked regional difference, the counts of fibrils being significantly lower (40%) in the predile

Ultrastructural studies in epidermolysis bullosa hereditaria. III. Recessive dystrophic types with dermolytic blistering (Hallopeau-Siemens types and inverse type).

Ultrastructural examination was performed in 42 biopsy specimens from 22 patients with the Hallopeau-Siemens types or with the inverse type of epidermolysis bullosa dystrophica recessiva. The patient group consists of 8 cases of the localized Hallopeua-Siemens type, 9 of the generalized Hallopeau-Siemens type and 5 of the inverse type. The origins of the biopsy specimens are involved, intact and experimentally frictioned skin from blister-predilected sites, as well as clinically normal skin from nonpredilection sites. It is confirmed that all the blisters initiate below the basal lamina. Ancho

[Epidermolysis bullosa dystrophica inversa: report on 2 sisters].

Two sister cases of epidermolysis bullosa dystrophica inversa are reported. This type, first described by Gedde-Dahl (1971), is characterized by the inverse site of skin involvement, the intermittent course, frequent traumatic corneal erosions, retarded development of skin atrophy and absence of milia formation. Our present cases had all these characteristics except the corneal involvement. The pedigree of this family favors an autosomal recessive mode of inheritance; no consanguinity is demonstrable in the ancestors over four generations. Electron microscopic examination in one of the patient

Ultrastructural studies in epidermolysis bullosa heriditaria. I. Dominant dystrophic type of Pasini.

Ultrastructural examination was performed in 8 biopsies from 4 patients with the Pasini type of epidermolysis bullosa dystrophica dominans. The biopsies were taken from: 1. clinically normal skin from nonpredilection areas, 2. intact skin from predilection areas, 3. involved skin and 4. experimentally frictioned skin. The main ultrastructural alterations detected are as follows: hypoplasia of anchoring fibrils, split or blister formation between basal lamina and dermis, hernia-like protrusion of basal cells, sub- and intraepidermal deposition of fibrillar bodies, and duplications of basal lami

Epidermolysis bullosa dystrophica-recessive: a possible role of anchoring fibrils in the pathogenesis.

The purpose of this study was to define the ultrastructural defects and pathogenesis of epidermolysis bullosa dystrophica-recessive (EBD-R). The only consistent ultrastructural alteration found in EBD-R was an absence of anchoring fibrils. In many specimens of nonblistered, nontraumatized EBD-R skin, absence of anchoring fibrils was the only ultrastructural abnormality observed. The possibility that lack of anchoring fibrils was a secondary change resulting from previous blistering and scarring was eliminated by our observation that anchoring fibrils were consistently absent in the never previ

Epidermal neoplasms with epidermolysis bullosa dystrophica with the first report of carcinoma with the acquired type.

Carcinoma, usually always squamous cell carcinoma, is one of the most serious complications in epidermolysis bullosa dystrophica. It can occur on the skin, mucous membranes, the esophagus and possibly the upper part of the bronchial tree. We are reporting on four new patients; one, the youngest to be so reported, one with a definite autosomal dominant inheritance and one with a chronic acquired dystrophica epidermolysis bullosa. Most cases have an autosomal recessive inheritance, but the disorder is probably more hetereogeneous in its inheritance than has been reported. Studies of the collagen

[Pathomorphogenesis of blistering in epidermolysis bullosa acquisita and epidermolysis bullosa dystrophica (author's transl)].

Electronmicroscopical examination on skin lesions of epidermolysis bullosa dystrophica recessiva (E.b.d.r.) and epidermolysis bullosa acquisita (E.b.a.) associated with Crohn's disease have demonstrated that blistering occurs between epidermis and dermis beneath the basal lamina. The structural defect concerns the anchoring fibrils only which are missing in the junctional zone of the involved skin. All other junctional structures are intact. Beneath the basal lamina a band-like zone of a moderate electrondense, amorphous material is seen in the skin-lesions of epidermolysis bullosa acquisita,

[Acquired epidermolysis bullosa in Crohn's disease].

A case of epidermolysis bullosa acquisita associated with colitis granulomatosa (Crohn's disease) is reported. Although the cutaneous lesions of this unusual disease resemble in their clinical, histological and ultrastructural features to those of the hereditary epidermolysis bullosa dystrophica, the epidermolysis bullosa acquisita has to be regarded as a separate entity among the blistering epidermolysis because of its late manifestation in adults without evidence of hereditary disorders. The deposits of immunoglobulins in the involved skin of the acral areas demonstrated fluorescentmicroscop
