Aplasia Cutis Congenita in Bart's Syndrome: A Case Report and Literature Review.

Aplasia cutis congenita (ACC) is a partial or complete absence of skin layers which can be a part of a syndromic disease. Bart's syndrome is a combination of clinical manifestations including ACC, epidermolysis bullosa, blistering in the oral mucosa, nail dystrophy, or congenital absence of nails. Additional anomalies, such as pyloric atresia, a broad nasal root, hypertelorism, and ear abnormalities, may also be present. We aimed to raise awareness about concomitant anomalies with ACC. A 23-year-old female with gravid 3 para 2 gave birth to a male fetus of 36 weeks and 3 days who had multiple

Epidermolysis bullosa accompanied by long-segment gastric outlet obstruction in a newborn: a report of a rare case.

Epidermolysis bullosa (EB) is a rare congenital disorder caused by mutations affecting structural proteins of the epidermis and dermo-epidermal junction, resulting in extreme skin and mucosal fragility, blistering with minimal trauma, and impaired wound healing. Gastric outlet obstruction (GOO) in newborns arises from disruption of normal gastric emptying and may cause poor feeding, weight loss, severe vomiting, dehydration, and electrolyte imbalance. Long-segment gastroduodenal atresia is a rare subtype of GOO in which the blockage spans a continuous region from the distal stomach into the pr

Bruton tyrosine kinase (Btk) in neutrophils is indispensable for initiating and maintaining skin inflammation in a model of pemphigoid diseases.

BACKGROUND AND PURPOSE: Bruton tyrosine kinase (Btk) is essential for B cell function. Its role in myeloid cells is less understood. Greater insights into Btk significance in myeloid cells are needed to evaluate its potential as a therapeutic target during the effector phase of antibody-induced autoimmune diseases, where inhibiting autoantibody production suppresses tissue inflammation only after a delay. Such a situation can be observed, for example, in acute flares of pemphigoid diseases, a group of autoimmune blistering skin diseases. EXPERIMENTAL APPROACH: We examined the effect of neutro

Case report: Epidermolysis bullosa acquisita following dipeptidyl peptidase-4 inhibitor therapy and complicated by immune thrombocytopenic purpura.

Epidermolysis bullosa acquisita (EBA) is a rare autoimmune blistering disease caused by autoantibodies to type VII collagen. While dipeptidyl peptidase-4 (DPP-4) inhibitors are established triggers for bullous pemphigoid (BP), their association with EBA has not been reported. A 68-year-old Japanese man with type 2 diabetes and chronic renal dysfunction, treated with linagliptin, developed widespread tense blisters with mucosal involvement. Histopathology and immunological studies confirmed EBA. Despite corticosteroids, cyclosporine, pulse therapy, and intravenous immunoglobulin, the disease re

Moxifloxacin Inhibits Neutrophil Responses to Immune Complexes and Ameliorates Skin Inflammation in a Model of Pemphigoid Diseases.

Moxifloxacin, a fluoroquinolone antibiotic with immunomodulatory activity, has not been evaluated for effects on autoantibody-driven inflammation. Pemphigoid diseases are a group of autoimmune blistering skin diseases driven by pathogenic autoantibodies and granulocytes. With current treatments for pemphigoid diseases associated with substantial adverse effects, there is a high medical need for new treatment strategies. We investigated the effect of moxifloxacin on skin inflammation in the antibody transfer mouse model of bullous pemphigoid-like epidermolysis bullosa acquisita. Moxifloxacin at

European S2k guidelines on management of autoimmune blistering diseases in children and adolescents.

INTRODUCTION: Autoimmune blistering diseases (AIBDs) in children and adolescents present a major challenge in diagnosis and management because of their rarity and the presence of other differential diagnoses with blistering. They are potentially serious, and timely diagnosis and optimal management can help prevent morbidity related to both the diseases and their treatments. METHODS: The Autoimmune Blistering Diseases Task Force of the European Academy of Dermatology and Venereology (EADV) has initiated evidence-based, expert-informed clinical guidelines for the diagnosis, treatment and monito

Selective Complement Inhibition in Anti-p200 Pemphigoid: Immune Infiltrate Profiles and Therapeutic Implications Compared to Bullous Pemphigoid.

Anti-p200 pemphigoid is an autoimmune blistering disease (AIBD) caused by autoantibodies against laminin β4 and/or γ1, and clinically resembles bullous pemphigoid (BP) as well as the inflammatory variant of epidermolysis bullosa acquisita (EBA). All three diseases show IgG and/or C3 deposition along the cutaneous basement membrane zone (BMZ). Although complement activation is central to BP and EBA pathogenesis, its role in anti-p200 pemphigoid remains unclear. To investigate this, we analyzed inflammatory infiltrates in lesional and perilesional skin from anti-p200 pemphigoid patients (n = 11)

Laparoscopic management of congenital pyloric Atresia: A case series and surgical perspective.

BACKGROUND: Congenital pyloric atresia (CPA) is an extremely rare neonatal gastric outlet obstruction (approximately 1 in 100,000 live births) that requires prompt surgical correction. It can occur in isolation or in association with junctional epidermolysis bullosa (JEB), and historically it has been managed with open surgery. Laparoscopic repair has only been reported in a few cases, so its safety and efficacy remain unclear. METHODS: We retrospectively reviewed five paediatric patients with Type I CPA (pyloric web) who were treated laparoscopically at three centers between 2020 and 2024. P

The role of antibody class and target on subepidermal blistering disease phenotype and management.

Subepidermal blistering diseases are driven by autoantibodies targeted to the basement membrane zone. These autoantibodies can stem from multiple antibody classes (e.g., IgE, IgG, IgM, IgA) and differing antibody classes and antibody targets can impact disease phenotype and potentially management. Despite significant advances, there remains a paucity of data and funding for further elucidating subepidermal blistering disease pathophysiology and the impact this has on management. This review highlights current understanding of subepidermal blistering disease antibody targets, classes, and the i

Adult-onset PLEC-related congenital myasthenic syndrome-myopathy overlap with upper limb predominant weakness.

Congenital myasthenic syndromes (CMS) are genetically heterogeneous disorders in which precise molecular diagnosis is essential for management. Pathogenic variants in PLEC are a rare cause of autosomal recessive CMS and are frequently associated with epidermolysis bullosa simplex (EBS). We report an adult-onset case with EBS from birth, upper limb–predominant weakness, combined myopathic and myasthenic features, and subtle cardiomyopathy. Initial CMS gene panel testing identified a heterozygous canonical splice-site variant in PLEC. Subsequent trio exome sequencing revealed a second heterozygo

Quality of life in children and adults with epidermolysis bullosa: the QoL-REB explorative study.

BACKGROUND: Epidermolysis bullosa (EB) is a rare inherited disorder characterized by skin and mucosal fragility, with severe implications for physical, psychological, and social well-being. Research on quality of life (QoL) in EB remains limited, particularly in Italy, where systematic patient-reported outcome measures are lacking. To address this gap, Fondazione REB ETS developed a patient-centered QoL questionnaire (QoL-REB) constructed directly by patients and caregivers, with support from clinicians and researchers. METHODS: We conducted a cross-sectional online survey between March and A

Tyrosine kinase signaling pathways as therapeutic targets in autoimmune subepidermal blistering skin diseases (pemphigoid diseases).

Pemphigoid diseases, such as bullous pemphigoid and epidermolysis bullosa acquisita, are severe organ-specific autoimmune diseases characterized by subepidermal skin blistering with increasing incidence in recent years. Although there have been substantial advances in understanding the pathomechanism of these diseases in the last decades, and the first specific therapy targeting the IL-4 and IL-13 pathway (dupilumab) has been approved by the FDA for bullous pemphigoid, further research is needed to eventually improve patient care. The characteristics of pemphigoid diseases include the formatio

Challenges of Ontology-Based Concept Normalization for Deep Phenotyping in Rare Skin Diseases.

Extracting and standardizing phenotypic information from free-text medical reports remains a main challenge in biomedical natural language processing (NLP). Conceptual normalization, which maps textual mentions to standardized vocabularies such as the UMLS or HPO, is a key step for deep phenotyping. Dystrophic epidermolysis bullosa (DEB) is a rare severe and heterogeneous skin disorder causing skin and mucosal fragility that leads to early morbidity and mortality. To better understand the disease's natural history and severity spectrum, it is important to comprehensively identify DEB-related p
