Innovative Impression Technique for Microstomia Induced by Epidermolysis Bullosa.

A removable partial denture (RPD) is a prosthesis that requires a high degree of precision during impression making. Adequate mouth opening is essential during impression procedures to ensure proper placement and orientation of the impression tray. However, this requirement poses a challenge in patients with microstomia, necessitating modification of conventional impression techniques to complete this crucial step in prosthesis fabrication.  This report describes a modified impression technique for fabricating a conventional RPD in a patient with epidermolysis bullosa and restricted mouth open

Long-Term Efficacy and Safety of Oleogel-S10 (Birch Triterpenes) for Pediatric Patients With Epidermolysis Bullosa.

BACKGROUND/OBJECTIVES: Pediatric patients with epidermolysis bullosa (EB) experience lifelong complications, and wound healing is an important treatment goal. In the phase III EASE study (NCT03068780), Oleogel-S10 accelerated wound healing in EB. This prespecified subgroup analysis evaluated the long-term efficacy and safety of Oleogel-S10 in patients aged < 18 years from EASE. METHODS: EASE comprised a 90-day double-blind phase (DBP) and 24-month open-label phase (OLP). In the DBP, patients were randomized 1:1 to receive Oleogel-S10 or control gel; all patients could enter the OLP and receiv

Tsukamurella pulmonis bacteremia in a young patient with epidermolysis bullosa: A case report.

Tsukamurella species are rare, aerobic, gram-positive actinomycetes that are increasingly recognized as opportunistic pathogens affecting immunocompromised patients, such as those suffering from epidermolysis bullosa. Epidermolysis bullosa is a rare genetic disorder characterized by skin fragility that predisposes patients to chronic wounds and recurrent skin infections. The case of interest in this work was a 22-year-old male with a complex medical history, including the aforementioned disorder. He was admitted to the hospital's emergency department, presenting with fluctuating low-grade feve

Congenital Skin Sloughing in the Setting of Hydrops Fetalis Secondary to Congenital Diffuse Lymphangiectasia Mimicking Epidermolysis Bullosa.

We present a case where the combination of severe hydrops fetalis, lymphangiectasia, and skin desquamation created a clinical picture that masqueraded as epidermolysis bullosa (EB). A neonate presented at birth with severe hydrops fetalis and extensive skin sloughing. Skin biopsy findings were consistent with EB; however, the clinical course and genetic testing were inconsistent with this diagnosis, and the postmortem examination revealed the underlying cause to be severe congenital diffuse lymphangiectasia. It is important for clinicians to consider edema bulla and tissue hypoxia as a cause o

Esophageal Stricture in a 5-Year-Old Child with Epidermolysis Bullosa: Case Report of a Rare Complication at a Very Young Age.

INTRODUCTION: Epidermolysis bullosa (EB) represents a group of rare inherited mechanobullous disorders characterized by extreme skin and mucosal fragility. Dystrophic EB (DEB), caused by mutations in the COL7A1 gene, often leads to cycles of blistering, healing, and scarring with multisystem involvement, including the gastrointestinal tract. Esophageal strictures are not uncommon in recessive DEB (RDEB), appearing after the first decade of life (∼12.6 years). We describe a relatively early complication in a 5-year-old child. CASE PRESENTATION: A 5-year-old girl with genetically confirmed RDEB

An ITGB4 variant modifies the severity of ITGA3-associated interstitial lung disease, nephrotic syndrome, and epidermolysis bullosa (ILNEB).

Biallelic ITGA3 variants cause a rare subtype of junctional epidermolysis bullosa named interstitial lung disease, nephrotic syndrome, and epidermolysis bullosa. Interstitial lung disease, nephrotic syndrome, and epidermolysis bullosa is characterized by extensive clinical heterogeneity, suggesting the existence of modifier genes. We studied a family with 2 siblings affected by interstitial lung disease, nephrotic syndrome, and epidermolysis bullosa due to a homozygous ITGA3 variant. The disease severity was strikingly different in the 2 siblings. Whole-exome sequencing revealed the presence o

Targeting, Mutagenesis, and Functional Testing Strategies for Large Collagens.

Collagens comprise a homogeneous family of cell-surface or extracellular proteins. Many of them are fundamental for health, and consequently, their deficiency or dysregulation occurs in a wide range of diseases, from tissue fragility to neoplastic and fibrosing diseases, to name a few. With the advent of new omics approaches, an increasing number of variants of specific collagens have been discovered in various conditions. Some of these variants lead to a loss of expression, highlighting the need to assess the functions and loss of specific collagens in such conditions, while other variants ma

Cord Blood-Derived Platelet Gel in Wound Healing: From Biological Rationale to Clinical Application.

Chronic and complex wounds are a major burden worldwide, and a considerable number remain unresponsive to traditional therapy. Cord blood-derived platelet gel (CBPG) is a novel allogeneic wound therapy providing a fibrin-based scaffold enriched with neonatal platelet-derived growth factors, including high concentrations of VEGF, HGF and FGF, directly into the wound bed, without the need for blood collection from the patient. This narrative review provides an overview of the biological features of CBPG, discusses clinical evidence across different wound indications, and assesses ethical, regula

Beyond Floristic Quality: A Data-Driven Approach to Assessing Coastal Wetland Condition Using Plants.

Escherichia coli is an enteric bacterium that is capable of growing over a wide range of pH values (pH 5-9) and, incredibly, is able to survive extreme acid stresses including passage through the mammalian stomach where the pH can fall to as low as pH 1-2. To enable such a broad range of acidic pH survival, E. coli possesses four different inducible amino acid decarboxylases that decarboxylate their substrate amino acids in a proton-dependent manner thus raising the internal pH. The decarboxylases include the glutamic acid decarboxylases GadA and GadB, the arginine decarboxylase AdiA, the lysi

Cryptococcus neoformans Meningitis in a Patient With Epidermolysis Bullosa and Rheumatoid Arthritis.

Cryptococcal meningitis (CM) is an opportunistic fungal infection with a high mortality rate commonly associated with those with human immunodeficiency virus (HIV) or acquired immunodeficiency syndrome (AIDS). The epidemiology of CM in HIV-negative individuals is unclear, although it is estimated to be increasing in developed countries. We present a case of Cryptococcus neoformans meningitis in a 59-year-old HIV-negative female taking long-term mycophenolate (MMF) for epidermolysis bullosa acquisita (EBA) and upadacitinib for rheumatoid arthritis (RA). With an aggressive antifungal regimen, th

Disrupted Skin-Nerve Homeostasis in Recessive Dystrophic Epidermolysis Bullosa: a Neurocutaneous Unit Perspective on Pain.

Recessive dystrophic epidermolysis bullosa (RDEB) is a severe inherited disorder characterised by type VII collagen impairment, leading to dermo-epidermal junction (DEJ) instability and chronic skin fragility. Pain is a near-universal and quality-of-life-limiting feature; however, explanations centred solely on tissue injury and inflammation do not fully account for its persistence or treatment resistance. Increasing evidence supports a broader framework in which disrupted skin-nerve homeostasis within the neurocutaneous unit (NCU) contributes to pain generation. This narrative, hypothesis-dri

Impaired skin reinnervation in epidermolysis bullosa due to neurotrophic deficiency.

The skin is densely innervated by peripheral sensory neurons that detect various stimuli through specialized nerve endings in the dermis and epidermis. In recessive dystrophic epidermolysis bullosa (RDEB), repeated skin injury disrupts epidermal nerve fibers, leading to neuropathic pain and reduced thermal sensitivity. Normally, keratinocyte-derived neurotrophic signals guide sensory fiber re-entry into healed epidermis. We hypothesized that impaired neurotrophic support underlies failed reinnervation in RDEB. To investigate the mechanisms behind failed reinnervation, we assessed neurotrophic

Topical gene therapy for severe recessive dystrophic epidermolysis bullosa: clinical outcomes and multidisciplinary management in three patients treated with beremagene geperpavec (B-VEC) in Italy - a case series.

Recessive dystrophic epidermolysis bullosa is a severe inherited blistering disorder characterized by chronic wounds, fibrosis and systemic complications. Beremagene geperpavec (B-VEC), a topical HSV1-based gene therapy delivering full-length COL7A1, has shown efficacy in clinical trials; however, real-world data remain limited. This retrospective, descriptive case series reports on three patients with genetically confirmed recessive dystrophic epidermolysis bullosa - two children and one young adult - treated between 2024 and 2026 at two Italian tertiary centres. All patients had extensive ch

Rigosertib Reverses Hypertrophic Cardiomyopathy in Noonan Syndrome.

BACKGROUND: RASopathies constitute a group of rare genetic disorders caused by mutations in genes along the canonical RAS/mitogen-activated protein kinase (MAPK) signaling pathway, affecting cell growth and differentiation. These syndromes, which include Noonan syndrome (NS), are characterized by developmental delays, distinctive facial dysmorphia, and cardiac defects, notably hypertrophic cardiomyopathy (HCM). Despite their prevalence and impact, therapeutic options for RASopathies remain limited. Rigosertib, a novel dual RAS/MAPK and phosphatidylinositol 3-kinase (PI3K)/AKT pathway inhibitor

Amniotic Membrane Transplantation Preserves Vision in Pediatric Recessive Dystrophic Epidermolysis Bullosa: Case Series.

Background: Recessive dystrophic epidermolysis bullosa (RDEB) is a rare inherited disorder characterized by extreme epithelial fragility and progressive cicatrization, frequently leading to severe ocular surface disease and early visual impairment. Surgical interventions such as ocular surface reconstruction (OSR) in childhood are often delayed because of anesthetic risks and concerns regarding recurrence. Consequently, the effectiveness of OSR, including amniotic membrane transplantation (AMT), and its impact on visual development remain poorly documented. Methods: We report a case series of

Novel insights into diarrhea mechanism from gut-kidney axis perspective: correlations among intestinal mucosal injury, AQP4 water transport disorder and gut microbiota dysbiosis.

In the era of personalised therapies, liquid biopsy is considered an important diagnostic tool in the clinical management of cancer patients. Tissue specimen represents the gold standard for molecular evaluation of specific gene targets alterations that lead cancer patients to benefit of a "tailed therapy" based on molecular features of the tumour. This innovative source of nucleic acids was introduced in clinical setting only for non-small-cell lung cancer (NSCLC) patients to test epidermal grow factor receptor (EGFR) mutations when tissue is not available for a number of reasons (difficult a
