Finding hybrid schistosomes in genital schistosomiasis: are we missing their clinical relevance?

Genital schistosomiasis is a parasitic disease that affects both men and women as male and female genital schistosomiasis (MGS and FGS), respectively. Symptoms mimic sexually transmitted infections in men and women, owing to inflammation in the genital organs caused by migrating and trapped parasite ova. Schistosoma haematobium is the main causative organism, with an estimated 75% of infected women experiencing FGS, equating to approximately 56 million women mainly in sub-Saharan Africa. Estimates for MGS are mostly lacking, but numbers may be equivalent. S. haematobium has been found in hybri

Clinical morbidity of single or mixed schistosome species infection in two communities of southern Malawi.

As part of a larger community-based epidemiological study entitled Hybridisation in Uro-Genital Schistosomiasis (HUGS), a parasite infection and clinical morbidity sub-study, implementing portable ultrasonography annually, was undertaken upon 701 participants from two communities in Mangochi and Nsanje Districts, southern Malawi. Our aim was to document the clinical morbidity a year after praziquantel treatment in those with previously proven human and/or zoonotic schistosomiasis, repeated a calendar year later after biannual praziquantel treatment. The median participant age was 12.0 years, w

Dynamic interactions between Schistosoma haematobium, Schistosoma mattheei and Schistosoma mansoni underscore the complex polyparasitism of intestinal schistosomiasis in southern Malawi.

Schistosomiasis is prevalent among school-aged children (SAC) in Mangochi District, Malawi, where both intestinal and urogenital forms are endemic. In 2024, we identified schistosomiasis cases predominantly associated with excretion of Schistosoma haematobium × Schistosoma mattheei ova in the faeces of two individuals from Samama village, Mangochi District. In this expanded cross-sectional study, we characterize the prevalence and species composition of Schistosoma infections among 247 SAC in Samama, using genus- and species-specific molecular diagnostics. We also present follow-up data from t

Public health challenge of hybridization in urogenital schistosomiasis: New insights and one health perspectives from Malawi.

The emergence of hybrid schistosomes resulting from interspecies mating between human and animal Schistosoma species is set to reshape the epidemiology of urogenital schistosomiasis in Malawi. Findings from the hybridization in urogenital schistosomiasis (HUGS) study in Malawi confirm the occurrence and circulation of S. haematobium × S. mattheei in humans, livestock and snails, from exemplar studies in Nsanje and Mangochi districts. Introgressed schistosomes complicate traditional diagnosis with atypical egg morphologies that defy standard microscopic identification, challenge current prevent

Tissue-specific and sex-biased glycoproteomic landscape of Schistosoma mansoni.

Schistosomiasis is a major global health burden, with praziquantel as the sole treatment. Its inability to prevent reinfection highlights the need for new interventions such as vaccines. Protein glycosylation is essential for parasite biology and immune evasion, yet remains poorly characterized in schistosomes. Here, we conduct N- and O-glycoproteomic and intact glycopeptides analyses of adult male and female Schistosoma mansoni, integrating single-cell transcriptomics. We uncover tissue-specific and sex-biased glycosylation patterns, with greater glycan complexity in the parenchyma and gut an

Endothelial cells protect Schistosoma mansoni from hydrogen peroxide-induced death.

INTRODUCTION: Schistosoma mansoni, the causative agent of intestinal schistosomiasis, thrives in the human host, particularly within the vascular system. Understanding the role of endothelial cells during infection is crucial. Currently, schistosomiasis treatment depends solely on praziquantel (PZQ), but emerging evidence suggests decreasing efficacy. This highlights the need for new therapeutic strategies, including agents that modulate the host antioxidant response, such as dapsone. METHODS: Adult S. mansoni worms were harvested from infected mice via portal perfusion. Human umbilical vein

Liver and spleen ultrasonography and elastography are useful for identifying a history of upper gastrointestinal bleeding in patients with schistosomiasis.

BACKGROUND: Schistosoma mansoni infection can lead to periportal fibrosis (PPF), non-cirrhotic (pre-sinusoidal) portal hypertension, development of varices, and gastrointestinal bleeding. In this context, we evaluated hepatic disease morbidity and signs of portal hypertension in patients with schistosomiasis, with and without a history of upper gastrointestinal bleeding (UGIB), using ultrasonography and point shear wave elastography (pSWE) of the liver and spleen. METHODS: An analytical case-control study involving 177 patients with schistosomiasis, with and without a UGIB history, conducted a

IL-6 and TGF-β1 as biomarkers of schistosomiasis-associated pulmonary hypertension in a murine model.

Schistosomiasis can lead to vascular damage resulting in pulmonary arterial hypertension (PAH). Although its pathophysiology remains unclear, cytokine imbalance is known to play a key role. This study aimed to evaluate serum mediators in association with hemodynamic, echocardiographic, and histological parameters in a murine model of Schistosoma mansoni-induced pulmonary hypertension (Sch-PH). Twenty male C57BL/6 mice were randomized into infected group and non-infected control group. Sch-PH was induced by intraperitoneal inoculation of S. mansoni eggs (240 eggs/g body weight), followed by int

The gut microbiome and metabolome associate with Schistosoma mansoni infection and cardiovascular disease risk in Uganda.

Helminth infections are consistently associated with reduced cardiovascular disease (CVD) risk, yet the biological mechanisms underlying this relationship remain unclear. The gut microbiome and metabolome are key regulators of cardiometabolic health and may mediate infection-associated effects on host physiology. Here we show that Schistosoma mansoni infection associates with distinct gut microbial and metabolic profiles linked to CVD risk in people living in Uganda. In a cross-sectional study of 209 individuals living in communities with contrasting S. mansoni endemicity, we profile the gut m

Characterization of Transcriptional, Epigenetic, and Phenotypic Plasticity and Discovery of Biomarkers in Acute and Chronic Murine Schistosomiasis Infection.

Parasites can induce changes in their hosts, favoring the success of the infection and its development at each stage of their life cycle. The host minimizes the effects of the parasite's presence through its defense system, balancing the parasite-host relationship. The intricate parasite-host relationship provides physiological, immunological, and molecular cues that suggest interaction and mutual regulation of the transcriptome and epigenome, promoting phenotypic plasticity and survival in a changing environment. There has been a growing interest in the epigenetic mechanisms of Schistosoma ma

Bleeding rectal polyp as an atypical presentation of intestinal schistosomiasis: A case report from Egypt.

BACKGROUND: Schistosomiasis remains an important public health challenge in Egypt despite decades of control programs. Intestinal involvement is relatively common but usually presents with diffuse mucosal disease, ulcerations, or multiple polyps. A solitary bleeding rectal polyp as the sole manifestation is exceedingly rare and can be mistaken for inflammatory bowel disease or colorectal neoplasia. This report aims to highlight this diagnostic challenge within the Egyptian context. CASE PRESENTATION: A 28-year-old Egyptian male from a rural area presented with a 3-month history of intermitten

Implications of host sex on liver metabolism during Schistosoma mansoni Infection.

Various studies suggest that host gender is a disease-determining factor in schistosomiasis. Men have a higher prevalence in endemic areas and show a greater degree of liver damage in chronic infection with Schistosoma mansoni. It is currently unclear whether this is only due to socioeconomic causes, such as the working environment and behavior. It was recently suggested that molecular biological differences in the infected host could also contribute to the disease. Current studies in model systems suggest that hormonal influences affect liver metabolism during infection with S. mansoni. In pa

Functional characterization of secreted Schistosoma mansoni serpin SmSPI reveals factor XIIa inhibition as evidenced by biochemical and human plasma assays and supported by AI-assisted structural modeling.

Schistosoma mansoni persists in host vasculature for years without inducing thrombosis or excessive inflammation, suggesting active interference with host serine proteases involved in hemostasis and immunity. To explore the mechanisms behind these evasion strategies, we investigated S. mansoni Serine Protease Inhibitor (serpin), SmSPI, previously described as a circulating antigen. Our gene expression profiling revealed SmSPI to be the most highly expressed parasite serpin in intravascular life stages, including adult worms and eggs. This indicates its constant accessibility to host proteolyti

Usefulness of environmental DNA for Biomphalaria glabrata and Schistosoma mansoni surveillance and control of low schistosomiasis endemic areas in Brazil.

Schistosomiasis is a neglected parasitic disease that continues to affect hundreds of millions of people worldwide, particularly in developing countries. In Brazil, chemotherapy combined with access to safe water and improved sanitation has been effective in reducing the number of severe clinical cases. However, despite the current low endemicity, active transmission persists in several regions, where both infected individuals and the intermediate host, Biomphalaria glabrata, remain present. Therefore, more sensitive and scalable methods are needed to detect parasite dispersion and the presenc
