[Advances in the diagnosis and treatment of capillary malformation-arteriovenous malformation syndrome].

Capillary malformation-arteriovenous malformation (CM-AVM) syndrome is a rare hereditary vascular disorder encountered in clinical practice. Owing to its complex pathogenesis, heterogeneous clinical manifestations, and challenging diagnostic criteria, CM-AVM syndrome is frequently misdiagnosed or underdiagnosed clinically. Accumulating studies have demonstrated that CM-AVM syndrome is predominantly induced by mutations in the RASA1 and EPHB4 genes. Such genetic aberrations activate the Ras/MAPK signaling pathway, thereby driving the onset and progression of the disease. The primary clinical ha

Nail disorders in pediatric patients: Part 1. Clinical overview, diagnosis and management of physiological variants, and congenital, hereditary, and infectious etiologies.

Nail disorders in children are relatively common and anxiety-provoking for families, yet frequently underrecognized in clinical practice. Many nail disorders in children require no treatment or are readably treatable; however, unfamiliarity may lead to unnecessary, inappropriate, or delayed treatment. In this part 1 of a 2-part continuing medical education series we review physiological variants and congenital, hereditary, and infectious nail disorders, and provide diagnostic and management tips for dermatologists managing these conditions.

Anesthesia Clinical Practice Guidelines for Ehlers-Danlos Syndrome: A Modified Delphi Survey.

Ehlers-Danlos syndrome (EDS) is a family of inherited connective tissue disorders distinguished by joint hyperextensibility, skin hyperelasticity, and increased tissue friability. No standardized anesthesia guidelines currently exist for this population, despite facing increased perioperative risks. To address this gap in literature, a three-round modified Delphi survey was conducted with 15 experts in anesthesia and EDS. Seven preliminary guidelines, developed from a comprehensive literature review, were evaluated and refined across three rounds. Consensus was achieved for all seven guideline

Vulvar Mastocytosis in Children and Adolescents: A Case Series.

BACKGROUND: Pediatric cutaneous mastocytosis is a rare condition characterized by clonal mast cell accumulation in the skin, most commonly presenting on the trunk and extremities. Vulvar involvement is exceedingly rare, particularly as an isolated lesion. METHOD: We retrospectively reviewed five pediatric cases of vulvar mastocytosis: one case from a children's hospital and four cases enrolled in an NIH protocol for mastocytosis (NCT00044122). RESULTS: Five girls (aged 16 months to 15 years), initially presented to the NIH or tertiary children's hospital with lesions on the vulva characteriz

Multi-Omic Integration of the miRNome and Transcriptome in Dermal Fibroblasts from Hypermobile Ehlers-Danlos Syndrome and Hypermobility Spectrum Disorders Identifies Convergent Candidate miRNA-mRNA Regulatory Networks Linking Extracellular Matrix Remodeling and Inflammation.

Hypermobile Ehlers-Danlos syndrome (hEDS) and hypermobility spectrum disorders (HSD) are clinically defined connective tissue conditions lacking molecular biomarkers. We tested whether their transcriptomic convergence extends to miRNA-mediated post-transcriptional regulation. miRNome sequencing was performed in cultured dermal fibroblasts from 12 individuals with hEDS, 12 with HSD, and 24 controls. Twenty-five differentially expressed miRNAs (DE-miRNAs) were assessed by qPCR in pooled discovery material and an independent pooled cohort. No miRNA met differential-expression threshold in the dir

Endothelial AHR-APLNR signaling preserves vascular glycocalyx integrity and limits neutrophil-driven skin injury during local MRSA infection.

Methicillin-resistant Staphylococcus aureus (MRSA) is a major cause of skin and soft tissue infection, yet the contribution of the cutaneous vasculature to host defense remains incompletely understood. Here, we identify endothelial aryl hydrocarbon receptor (AHR) signaling as a key regulator of vascular integrity and inflammatory responses during local MRSA skin infection. MRSA challenge reduced AHR expression in cutaneous endothelial cells in both human and murine skin. Endothelial-specific AHR deficiency exacerbated skin injury, increased bacterial burden, enhanced inflammatory cytokine prod

The Adipovascular Unit in Lipedema: A Framework for Regional Adipose Tissue Dysregulation.

Lipedema is a chronic, predominantly female disorder characterized by disproportionate regional expansion of subcutaneous adipose tissue, pain, tenderness, and easy bruising. Reported abnormalities involve adipocytes, stromal/progenitor cells, microvascular and immune compartments, extracellular matrix (ECM), and, variably, lymphatic function, but their causal and temporal relationships remain unresolved. Here, we propose the adipovascular unit (AVU) as a systems-level framework that links these compartments within a spatially organized regenerative niche. The conceptual advance is not the ide

A Mechanistic Map of the Phenotypic Complexity in Recessive Dystrophic Epidermolysis Bullosa Provides Basis for Therapeutic Target Discovery and Drug Repurposing.

Recessive dystrophic epidermolysis bullosa (RDEB) is a genodermatosis caused by loss-of-function mutations in the COL7A1 gene, producing a complex and heterogeneous phenotype. Clinical manifestations include skin fragility, local and systemic inflammation, impaired wound healing, dysbiosis, pain, and pruritus, increasing the risk of skin cancer. This work aimed to develop an actionable model of RDEB's molecular mechanisms to identify and prioritize therapeutic targets for drug repurposing. Transcriptomic data of skin biopsies from pediatric RDEB patients and unaffected individuals were used to

Marine-Derived Extracellular Vesicles: Bioinspired Nanobiomaterials for Skin Regeneration and Dermatological Applications.

Extracellular vesicles (EVs) represent a significant advancement in the study of biological nanocarriers, facilitating intercellular communication by transferring proteins, lipids, and nucleic acids. Although there has been considerable research into mammalian-derived EVs, the exploration of marine-derived EVs is relatively limited, despite their significant potential as bioinspired biomaterials. The marine environment, characterized by extreme physicochemical conditions and significant biodiversity, produces EVs that exhibit distinctive molecular cargo, improved structural stability, and adap

The active immune role of keratinocytes: from passive targets to key hubs in psoriatic inflammation.

Psoriasis is a chronic immune-mediated inflammatory skin disease historically viewed as a disorder of keratinocyte hyperplasia, with subsequent recognition of central immune cell involvement. However, accumulating multi-omics and functional genomic evidence has reshaped this paradigm, positioning keratinocytes not merely as passive targets of immune attack but as active participants within a bidirectional epithelial-immune network that drives inflammatory initiation, maintenance, and amplification. This critical narrative review synthesizes the molecular mechanisms by which keratinocytes integ

An Investigational Mechanism-Informed Therapy for Netherton Syndrome: Introduction to QRX003 and A Pediatric Compassionate-Use Case Report.

QRX003 is an investigational topical lotion under clinical evaluation for Netherton syndrome (NS) and other genodermatoses. Netherton syndrome is a severe genetic ichthyosis caused by a deficiency of LEKTI, a serine protease inhibitor crucial for kallikrein regulation. Hyperactive kallikreins drive premature corneodesmosome cleavage and accelerated desquamation, causing impaired skin barrier integrity, chronic inflammation, and intense chronic pruritus. The active ingredient in QRX003 is DiPalmitoyl HydroxyProline (DPHP), a competitive broad-spectrum serine protease inhibitor that is being eva

Association of Xeroderma Pigmentosum group F (XPF) with epigenetic regulation of T helper cell differentiation in coronary artery disease-associated prostate cancer.

BACKGROUND: Coronary artery disease (CAD) and prostate cancer share common genetic and epigenetic risk factors. Xeroderma Pigmentosum group F (XPF), a DNA repair endonuclease, has been implicated in genomic stability and epigenetic regulation. This study investigated the association between XPF expression and epigenetic changes during T helper (TH) cell differentiation in CAD-associated prostate cancer (CAD-PC). RESEARCH DESIGN AND METHODS: CD4+ T cells isolated from healthy controls and CAD-PC patients were subjected to CRISPR/Cas9-mediated XPF knockout or overexpression. TH cell differentia

Myeloid neoplasm with PDGFRA rearrangement presenting in blast-phase with basophilia.

An adult man, from Southern Africa, presented to a public health centre with symptoms of progressive fatigue. Peripheral blood morphology showed marked basophilia and 21% myeloblasts, confirmed by flow cytometry, suggesting acute myeloid leukaemia with basophilia. Bone marrow biopsy revealed increased mast cells, raising systemic mastocytosis as a differential. However, next-generation sequencing identified a PDGFRA fusion, establishing the diagnosis of blast phase of a myeloid/lymphoid neoplasm with PDGFRA rearrangement. The presence of cells with abnormal basophilic granulation and absence o

Identification of Homing Receptors and Activation-Linked Surface Antigens on Human Mast Cells and Basophils.

Mast cells (MC) and basophils (BA) are key effector cells in atopic disorders. Development, homing, and activation of these cells are regulated by various surface antigens. We examined the expression and cytokine-mediated regulation of functionally relevant surface antigens on human BA and MC. Primary BA were obtained from healthy or allergic donors, or patients with chronic myeloid leukemia (CML). Primary MC were isolated from lung, skin, normal/reactive bone marrow (BM) or BM of patients with mastocytosis. The BA cell line KU812 and the MC lines HMC-1, ROSA, and MCPV-1 were also examined. As

Vascular Patterning in Morphea Mimicking a Capillary Malformation: Case Report and Review of the Literature.

Early, inflammatory morphea can rarely mimic an acquired capillary malformation (CM), and emerging evidence suggests that endothelial injury may precede fibrosis in morphea, with vascular abnormalities driving immune recruitment and fibroblast activation. We present a pediatric case of inflammatory linear morphea initially misdiagnosed as an acquired CM. Dynamic optical coherence tomography (OCT) demonstrated a dense, superficial vascular network, resembling that seen in genetically confirmed capillary malformations. This case highlights the key clinical and morphologic features of morphea, wi
