Case Report: dupilumab for severe pruritus in pediatric recessive dystrophic epidermolysis bullosa-a longitudinal clinical assessment and literature review.

Recessive dystrophic epidermolysis bullosa (RDEB) results from biallelic COL7A1 variants and causes trauma-induced blistering, scarring, and progressive extracutaneous complications. Severe pruritus may intensify skin injury through an itch-scratch cycle. We report a 7-year-old, 25-kg girl with infantile-onset generalized blistering, severe pruritus, mucosal and nail involvement, scarring, and digital contractures. Histopathology showed a subepidermal blister. Sequencing identified heterozygous COL7A1 c.5605G>C (p.Gly1869Arg) and c.5532+5G>A, inherited from different parents and therefore occu

Species-specific outcomes in wasp sting challenges during venom immunotherapy and their respective follow-up.

BACKGROUND: Insect venom allergy is a major cause of life-threatening anaphylaxis, with venom immunotherapy (VIT) being the gold standard for prevention. Sting challenges (SC) are used to assess immune tolerance and VIT efficacy, but factors influencing outcomes remain unclear. This study examined whether the stinging insect species affects SC outcomes during VIT and evaluated outcomes during long-term follow-up. MATERIALS AND METHODS: A retrospective analysis of 1,163 patients undergoing SC during wasp venom VIT was performed. The stinging species was entomologically confirmed. Associations

XPA lactylation is required for UV-induced DNA damage repair.

Xeroderma pigmentosum group A (XPA) serves as a core nucleotide excision repair (NER) factor essential for the repair process triggered by UV irradiation. Lactylation, a newly identified post-translational modification, plays a key role in tumorigenesis and cancer progression. Herein, we discovered that XPA was lactylated in the DNA-binding domain by the acyl-transferase KAT5. In addition, UV irradiation promoted XPA lactylation, thereby increasing the chromatin recruitment of XPA and NER. However, the mutation of XPA to a lactylation-deficient form impeded XPA chromatin recruitment under UV i

Xeroderma Pigmentosum Monozygotic Twins: Same Mutation Divergent Clinical Outcomes.

Xeroderma pigmentosum (XP) is a rare monogenic disorder characterized by UV sensitivity and skin cancer predisposition, caused by mutations in the NER pathway or the gene encoding DNA polymerase eta (Pol η), which acts in translesion synthesis (TLS). Although XP is a monogenic disorder, differences in clinical severity may occur even between monozygotic twins. Here, we present monozygotic twins (M3 and M6) with the XP variant (XP-V) displaying differences in clinical severity despite sharing similar environments and habits. Using whole genome sequencing (WGS), we evaluated their mutational bur

Malar Edema after Hyaluronic Acid Dermal Fillers: Expert Recommendations on Etiology, Prevention, and Management.

Malar edema is a relatively common complication following injection of hyaluronic acid (HA) filler into the infraorbital area. The Complication Assessment and Risk Evaluation (CARE) board, established to develop guidelines on prevention and management of adverse reactions after HA filler use, identified malar edema as a priority complication. This review presents the CARE board recommendations on etiology of malar edema, prevention, and management. Malar edema is a result of HA injection interacting adversely with a delicate lymphatic system situated between the orbicularis retaining ligament

Invisible Challenges of a Visible Disease: A Qualitative Study on Disease-related Impairment, Participation Barriers and Care Experiences in Adults with Ichthyosis.

Ichthyoses are rare genodermatoses characterized by scaling, hyperkeratosis, and barrier dysfunction, often accompanied by hypohidrosis and pruritus. Beyond visible symptoms, ichthyosis imposes substantial psychosocial, financial, and time-related challenges. This qualitative study explored patient-perceived impairments, disability, and experiences with medical and social care. Semi-structured interviews were conducted with 18 adults (13 women, 5 men; aged 18-67 years) with various ichthyosis subtypes, recruited from two specialized dermatology clinics in Germany and the patient organization I

Dentofacial Characteristics in Patients With Epidermolysis Bullosa-A Cross-Sectional Study.

OBJECTIVES: Epidermolysis bullosa (EB) is a rare genetic disease characterized by skin and mucosal fragility caused by pathogenic variants in structural protein genes. Malocclusions have been previously described in patients with EB, mainly in case reports, with no cohort studies characterizing dentofacial characteristics across all EB types. This study aims to describe and compare the dentofacial characteristics of patients with EB across all types in a nationally representative cohort. MATERIAL AND METHODS: A cross-sectional study examined 101 patients with genetically confirmed EB diagnosi

Primary erythromelalgia associated with an SCN9A variant can mimic systemic lupus erythematosus in skin manifestations: a case report.

Primary erythromelalgia (PEM) is a rare autosomal dominant disorder characterized by episodic erythema and burning pain of the extremities, typically triggered by exercise or fatigue. PEM usually begins in childhood and is caused by gain-of-function variants in SCN9A, which encodes the voltage-gated sodium channel Nav1.7. The disease concept of PEM, however, is largely unrecognized among rheumatologists. Here, we report a 20-year-old woman who had experienced recurrent burning pain and redness of the extremities since around age 10. She also exhibited livedo-like rash, leukopenia, and hypocomp

Senotherapeutic potential against xeroderma pigmentosum.

BACKGROUND: Xeroderma pigmentosum (XP) is an inherited photoaging syndrome caused by mutations in genes involved in the nucleotide excision repair (NER) pathway. XP patients exhibit hypersensitivity to ultraviolet (UV) radiation, leading to accelerated skin aging and requiring lifelong sun avoidance. In this study, we aimed to investigate UV-induced cellular senescence in XP melanocytes and explore potential senotherapeutic strategies. METHODS: Melanocytes differentiated from iPSCs derived from an XP-A patient and a healthy individual, respectively, were used to induce senescence. Senescence

Epidermolysis bullosa dystrophica in a goat kid.

In a dairy herd of Thuringian Forest goats, a male goat kid of around three weeks of age was detected with skin lesions, particularly on the distal limbs, chest and ears. These skin lesions were characterized by alopecia, pus-filled blisters, incrustations and erosions. The goat kid was referred to the clinic for treatment but had to be euthanized despite intensive treatment due to increasing deterioration in its general condition and was subsequently referred to necropsy. Histologic investigation of the altered areas of skin and mucosa revealed a separation of epithelium from the underlying d

Fragmentation patterns of human telomeric chromatin in plasma cfDNA.

The specialized chromatin structure at telomeres protects the linear ends of chromosomes from DNA surveillance machineries. Here, we demonstrate that circulating cell-free DNA (cfDNA) from plasma can map chromatin structure at telomeres. We find that the telomeric 6-mer repeats (TTAGGG/CCCTAA) are the most abundant circulating 6-mers in cfDNA. Telomeric sequences in cfDNA contain subnucleosomal footprints distinct from the rest of the genome, arising from specific cleavages in the C-rich strand, and a nucleosome repeat length of 145 bp, markedly shorter than the ~170 bp observed genome-wide. Development and internal validation of a clinical response-estimation model incorporating treatment-course exposure after HMME-PDT in patients with port-wine stain.

BACKGROUND AND OBJECTIVE: Response to hematoporphyrin monomethyl ether photodynamic therapy (HMME-PDT) varies among patients with port-wine stain (PWS), and treatment response is associated with both patient clinical characteristics and lesion properties. This study sought to establish a prediction model to assess the likelihood of essential cure in PWS patients after HMME-PDT. METHODS: This study retrospectively included PWS patients who received HMME-PDT in our department. All patients completed final efficacy assessments with complete data for the formal prediction model, which focused on

A Novel Pathogenic Variant in RTEL1 Causes Dyskeratosis Congenita/Hoyeraal-Hreidarsson Syndrome: A Report of Two Cases.

BACKGROUND: Telomere biology disorders (TBD) are a spectrum of conditions that result from shortened or dysfunctional telomeres. Dyskeratosis congenita (DC) is the prototypic TBD, classically presenting with leukoplakia, dysplastic nails, and reticular skin pigmentation. Immunologic abnormalities are common, and severe infection may be the presenting sign of DC. RTEL1 gene mutations are known to cause DC and are associated with a severe disease phenotype (Hoyeraal-Hreidarsson syndrome). OBSERVATION: In this report, we describe 2 patients with severe immunodeficiency and very early onset infla

Insights into the microcirculation in pediatric chronic spontaneous urticaria: A nailfold videocapillaroscopy study.

BACKGROUND: Chronic spontaneous urticaria (CSU) is increasingly recognized as a systemic inflammatory disease with endothelial involvement extending beyond the skin. Nailfold videocapillaroscopy (NVC) has demonstrated systemic microvascular abnormalities in adults with CSU, but pediatric data are lacking. We investigated nailfold microvascular alterations in children with CSU and their associations with disease activity, disease control, autoreactivity, and inflammatory markers. METHODS: In this cross-sectional study, 41 children with CSU underwent standardized NVC according to EULAR recommen

When the Electromyography (EMG) and the Patient Disagree: Unraveling a Motor Neuron Disease Mimic With Small-Fiber Dysfunction.

Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder characterized by upper and lower motor neuron dysfunction. Electromyography (EMG) is central to its evaluation, but neurogenic abnormalities and fasciculations are not specific to ALS and must be interpreted within the clinical phenotype and disease trajectory. This study reports a 52-year-old man with an approximately eight-year history of intermittent muscle twitching, bilateral upper-limb tremor, muscular tightness and warmth, and asymmetric cutaneous hypersensitivity. An initial electrodiagnostic evaluation rai

Melanosome Clusters and Apoptotic Melanocytes Internalized by Dermal Fibroblasts Impairs Cellular Function.

Skin pigmentation disorders with dermal melanin involvement are therapeutically challenging due to poor responsiveness to conventional treatments and unclear pathogenesis. While melanin in the dermis is typically cleared by macrophages, human dermal fibroblasts (HDFs) can also internalize extracellular melanosome clusters (EMC) and apoptotic melanocytes that have occasionally been observed in the dermis, which may contribute to persistent pigment-related abnormalities. However, the functional impact of phagocytosis for fibroblasts remains unclear. In this study, HDFs were incubated with EMC or
