Clinical manifestations, genetic characteristics and laboratory markers of patients with epidermolysis bullosa: a single tertiary referral center experience.

BACKGROUND: Epidermolysis bullosa (EB) is a group of genetically and clinically heterogeneous diseases that presents as blisters, erosions, crusts, and scars. MATERIALS AND METHOD: In this retrospective study, we report the clinical manifestations, genetic characteristics, and laboratory markers of patients with EB. RESULTS: Fifteen patients with EB were included in the study. The most common type was dystrophic EB (DEB) (n = 10), followed by EB simplex (EBS) (n = 3), junctional EB (JEB) (n = 1), and Kindler EB (n = 1). The disease onset was from birth in 13 patients. The most common cutaneo

Vascular Patterning in Morphea Mimicking a Capillary Malformation: Case Report and Review of the Literature.

Early, inflammatory morphea can rarely mimic an acquired capillary malformation (CM), and emerging evidence suggests that endothelial injury may precede fibrosis in morphea, with vascular abnormalities driving immune recruitment and fibroblast activation. We present a pediatric case of inflammatory linear morphea initially misdiagnosed as an acquired CM. Dynamic optical coherence tomography (OCT) demonstrated a dense, superficial vascular network, resembling that seen in genetically confirmed capillary malformations. This case highlights the key clinical and morphologic features of morphea, wi

Extensive Aplasia Cutis Congenita with Calvarial Defect and Superior Sagittal Sinus Exposure in a Term Neonate: A Case Report of Successful Healing with Conservative Management.

BACKGROUND: Aplasia cutis congenita (ACC) is a rare congenital disorder characterized by localized absence of skin, most commonly affecting the scalp. Extensive lesions involving the calvarium and dura, particularly when the superior sagittal sinus is exposed, are uncommon and may be complicated by hemorrhage, infection, thrombosis, neurological injury, and mortality. Management may be conservative or surgical and must be individualized according to defect size and depth, the structures exposed, the clinical condition of the neonate, and the resources and expertise available. Case Report: SAPHO syndrome mimicking osseous relapse after haploidentical allogeneic hematopoietic stem cell transplantation for acute myeloid leukemia.

BACKGROUND: New bone pain and destructive skeletal lesions after allogeneic hematopoietic stem cell transplantation (allo-HSCT) raise concern for malignant relapse or infection. Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is an uncommon inflammatory disorder that may produce overlapping imaging findings. CASE PRESENTATION: A 19-year-old man with acute myeloid leukemia underwent haploidentical allo-HSCT in second complete remission. Thirteen months later, while measurable residual disease remained undetectable, he developed severe thoracolumbar pain and extensive i

FAP adipogenesis drives early spinal muscle loss in dysferlinopathy and reveals a therapeutic target.

Dysferlinopathy (Limb Girdle Muscular Dystrophy R2/LGMD2B) causes progressive weakness of limb-girdle muscles. Although gait abnormalities are common, the contribution of spinal muscles to disease symptoms remains poorly understood. Using cross-sectional MRI in patients and longitudinal analyses in a dysferlinopathic mouse model, we identify early-onset, progressive adipogenic degeneration of spinal muscles as a previously unrecognized hallmark of dysferlinopathy. Multiple spinal muscles, including the iliocostalis, longissimus, and spinalis, are affected, with degeneration preceding pathology

The Metabolic-Dermatologic Axis: GLP-1/GIP Agonists as Emerging Immunometabolic Therapies for Obesity-Associated Skin Disease.

Obesity is increasingly recognized as a systemic determinant of dermatologic disease rather than merely a comorbidity accompanying skin disorders. Excess adiposity promotes insulin resistance, adipose-tissue dysfunction, chronic low-grade inflammation, altered immune responses, vascular abnormalities, mechanical stress, and impaired tissue repair, thereby influencing the development and severity of inflammatory and metabolic dermatoses. Psoriasis, hidradenitis suppurativa (HS), psoriatic arthritis (PsA), acanthosis nigricans, intertriginous disease, and impaired wound healing exemplify conditi

Cardiac phenotype in children with X-linked ichthyosis (STS-sEDD): findings from a prospective cohort.

UNLABELLED: X-linked ichthyosis is caused by pathogenic variants or deletions involving STS at Xp22.31. Because some deletions include contiguous genes and cardiac rhythm abnormalities have been reported, we characterized the cardiac phenotype of children with genetically confirmed steroid sulfatase-related syndromic epidermal differentiation disorder (STS-sEDD). This prospective, observational, single-centre study included 14 boys aged 2-16 years. Assessment comprised clinical history and examination, 12-lead electrocardiography, echocardiography, exercise testing when age-appropriate, and 5-

PRNP D178N-129MM prion disease: Creutzfeldt-Jakob disease-like cortical ribboning without prominent early insomnia.

BACKGROUND: The PRNP p.Asp178Asn (D178N) mutation on a methionine-bearing codon-129 allele is classically associated with fatal familial insomnia (FFI), but some carriers develop a Creutzfeldt-Jakob disease (CJD)-like phenotype. CASE PRESENTATION AND FINDINGS: We report a 59-year-old woman with two months of rapidly progressive cognitive-language decline, caregiver-observed nocturnal sitting-up and sleep talking, weight loss, extensive cortical diffusion-weighted imaging (DWI) ribboning, positive cerebrospinal fluid 14-3-3, and positive multisite skin real-time quaking-induced conversion. Scleroderma-Like Skin Changes and Multifocal Muscle Thickening in a Patient with Diffuse Large B-Cell Lymphoma: A Case Report.

BACKGROUND: Paraneoplastic rheumatic manifestations may precede the diagnosis of an occult malignancy and closely mimic primary autoimmune connective-tissue diseases. Scleroderma-like manifestations have rarely been reported in association with lymphoid malignancies, including diffuse large B-cell lymphoma (DLBCL). CASE PRESENTATION: A 47-year-old woman presented with progressive skin thickening, difficulty walking, reduced appetite, and weight loss. She had been diagnosed with scleroderma by an outside physician in early 2024 and had been treated with methotrexate 15 mg once weekly. Physical

[Atypical cutaneous manifestations of B19 parvovirus infection in children].

The most well-known skin manifestation of human parvovirus B19 infection is erythema infectiosum, also known as fifth disease. In atypical cases, particularly among young adults, purpuric-petechial glove-and-sock syndrome (PPGSS) is observed, which is a condition characterized by a symmetrical distribution in the acral areas. Its onset is attributed to the active viremia phase; therefore, patients exhibiting symptoms are contagious, which is of particular importance in the case of immunocompromised individuals and pregnant women. In our report, we present three cases: in two patients, the skin

Challenging Genetic Diagnosis in Siblings With Primordial Dwarfism, Muscle Wasting, Osteogenesis Imperfecta, Craniofacial Dysmorphism, and Ectodermal Anomalies.

Three siblings of consanguineous Pakistani parents have primordial dwarfism with microcephaly, failure to thrive, delayed developmental milestones, thin and lean physique, dysplastic limb joints, ectodermal anomalies, and craniofacial anomalies including dolichocephaly, micrognathia, and irregular dentition. Variable findings are muscular dystrophy and osteogenesis imperfecta. We hypothesized a novel syndrome and launched a disease gene search. At the gene locus identified by single-nucleotide polymorphism-based linkage mapping, genome sequencing detected a very rare intergenic variant g.13276

Development and Content Validation of the Epidermolysis Bullosa Oral Health Assessment Form (EB-OHAF).

OBJECTIVES: Epidermolysis Bullosa (EB) is a rare skin disorder with significant phenotypic variations according to the primary type and subtype of the condition. As a rare disease with uncommon oral features and high phenotypic variability, identifying all characteristics is important to define a patient-specific treatment strategy and monitor the impact of different strategies on the oral outcomes. This project aims to develop and validate an EB-specific oral health assessment tool that helps clinicians to identify every specific oral feature that can be presented in patients with different t

Case Report: dupilumab for severe pruritus in pediatric recessive dystrophic epidermolysis bullosa-a longitudinal clinical assessment and literature review.

Recessive dystrophic epidermolysis bullosa (RDEB) results from biallelic COL7A1 variants and causes trauma-induced blistering, scarring, and progressive extracutaneous complications. Severe pruritus may intensify skin injury through an itch-scratch cycle. We report a 7-year-old, 25-kg girl with infantile-onset generalized blistering, severe pruritus, mucosal and nail involvement, scarring, and digital contractures. Histopathology showed a subepidermal blister. Sequencing identified heterozygous COL7A1 c.5605G>C (p.Gly1869Arg) and c.5532+5G>A, inherited from different parents and therefore occu

Xeroderma Pigmentosum Monozygotic Twins: Same Mutation Divergent Clinical Outcomes.

Xeroderma pigmentosum (XP) is a rare monogenic disorder characterized by UV sensitivity and skin cancer predisposition, caused by mutations in the NER pathway or the gene encoding DNA polymerase eta (Pol η), which acts in translesion synthesis (TLS). Although XP is a monogenic disorder, differences in clinical severity may occur even between monozygotic twins. Here, we present monozygotic twins (M3 and M6) with the XP variant (XP-V) displaying differences in clinical severity despite sharing similar environments and habits. Using whole genome sequencing (WGS), we evaluated their mutational bur

Invisible Challenges of a Visible Disease: A Qualitative Study on Disease-related Impairment, Participation Barriers and Care Experiences in Adults with Ichthyosis.

Ichthyoses are rare genodermatoses characterized by scaling, hyperkeratosis, and barrier dysfunction, often accompanied by hypohidrosis and pruritus. Beyond visible symptoms, ichthyosis imposes substantial psychosocial, financial, and time-related challenges. This qualitative study explored patient-perceived impairments, disability, and experiences with medical and social care. Semi-structured interviews were conducted with 18 adults (13 women, 5 men; aged 18-67 years) with various ichthyosis subtypes, recruited from two specialized dermatology clinics in Germany and the patient organization I

Primary erythromelalgia associated with an SCN9A variant can mimic systemic lupus erythematosus in skin manifestations: a case report.

Primary erythromelalgia (PEM) is a rare autosomal dominant disorder characterized by episodic erythema and burning pain of the extremities, typically triggered by exercise or fatigue. PEM usually begins in childhood and is caused by gain-of-function variants in SCN9A, which encodes the voltage-gated sodium channel Nav1.7. The disease concept of PEM, however, is largely unrecognized among rheumatologists. Here, we report a 20-year-old woman who had experienced recurrent burning pain and redness of the extremities since around age 10. She also exhibited livedo-like rash, leukopenia, and hypocomp
