Ingrown Toenail With Hypertrophic Skin Bridging.

Onychocryptosis, or ingrown toenail (IGTN), is a painful condition commonly resulting from improper nail trimming or hypertrophic lateral nail folds. We present a rare variant in which the entire distal nail plate becomes embedded within markedly hypertrophic nail folds, forming a tunnel-like structure. This case was successfully managed with radiofrequency excision followed by chemical matricectomy. This combined approach proved highly effective, offering excellent outcomes with minimal complications.

[Hirsutism: a common problem; when to consider rare causes?].

Excessive hair growth is a common and distressing complaint in women. It is imperative to differentiate excessive hair growth from hirsutism with possible other signs of virilization. Hirsutism is commonly attributed to polycystic ovary syndrome (PCOS). Hirsutism with rapid onset or progression, potentially combined with other signs of virilisation or secondary amenorrhea, should prompt evaluation of other underlying causes. In this clinical lesson, we present three cases of hirsutism to illustrate red flags to consider rare diagnoses. Diagnoses include non-classical congenital adrenal hyperpl

Analysis of Hirsutism as a Symptom of Ihtibās al-Tamth (Amenorrhea) and its Relation to Polycystic Ovary Syndrome.

BACKGROUND: Hirsutism, defined as excessive terminal hair growth in females in a male pattern, is a significant clinical manifestation of increased androgen. Unani scholars such as Hippocrates, Haly Abbas, Rhazes, and Avicenna linked hirsutism with menstrual irregularities, specifically Ihtibās al-Tamth (Amenorrhea). Modern medicine also associates hirsutism with hyperandrogenism, a hallmark of polycystic ovarian syndrome (PCOS), among other conditions. OBJECTIVE: This study aims to explore the understanding of hirsutism from both Unani classical literature and modern medical perspectives, pa

Metformin use in women with polycystic ovary syndrome (PCOS): Opportunities, benefits, and clinical challenges.

Metformin, a synthetic biguanide, is widely used to manage type 2 diabetes, and is commonly prescribed in polycystic ovary syndrome (PCOS) to address insulin resistance and associated metabolic and reproductive disturbances. PCOS is characterised by hormonal imbalances such as hyperandrogenism and anovulation, metabolic abnormalities including insulin resistance and increased cardiometabolic risk, and higher rates of pregnancy complications. However, the role of metformin in the multifaceted nature of PCOS remains debated. This review synthesises the mechanisms of action of metformin and its e

Cyproterone acetate for hirsutism.

BACKGROUND: Hirsutism (excessive and unwanted hair growth) is a distressing and relatively common endocrine problem in women that may prove difficult to manage. Cyproterone acetate (CPA), an antiandrogen, is frequently used to treat hirsutism, usually in combination with ethinylestradiol. This is an update of a Cochrane review first published in 2003. OBJECTIVES: To assess the benefits and harms of cyproterone acetate (CPA) alone, or in combination with ethinylestradiol, and other medication in reducing hair growth and improving the endocrine profile in women with hirsutism secondary to ovari

Nutritional Progression of a Child With Hypoplastic Left Heart Syndrome During the Waiting Period for Heart Transplantation: A Case Report.

Congenital heart diseases, including hypoplastic left heart syndrome (HLHS), frequently lead to malnutrition, which can result in worse outcomes in critical and perioperative situations. This study presents a case report based on clinical and nutritional data extracted from the electronic medical record of a 1-year and 10-month-old male infant diagnosed with HLHS, hospitalized in a tertiary cardiology hospital while awaiting a heart transplant. The length of hospitalization while waiting for the organ was approximately 13 months. Upon hospital admission, the child presented with severe malnutr

Free androgen index and its relation with female pattern hair loss.

Androgenetic alopecia (AGA) is more common in women than in men. This study aims to evaluate various androgenic hormones in women and to assess if free androgen index (FAI) can be used as a marker of hyperandrogenism in women. Forty women with AGA (defined as grade 1, 2, or 3) were taken as subjects, and 40 other healthy women were included as controls in the study. The serum concentrations of testosterone (T) and sex hormone-binding globulin (SHBG) were measured, and FAI was calculated accordingly and compared with the control group. Results revealed that there were significant differences in

Clear Cell Papulosis Associated With Multiple Developmental Abnormalities of the Skin.

Clear cell papulosis of the skin is a rare condition in children characterized by multiple small hypopigmented macules or flat papules, predominantly distributed in the pubic area and over the abdomen, chest, and axillae, along the milk lines, and by mucin- and cytokeratin 7-positive large clear cells within the epidermis. Associations of clear cell papulosis with other diseases or disorders have been rarely described. We present a case of clear cell papulosis associated with a congenital sacral dermoid cyst, nevus of Ota, presumed accessory breast tissue, thin and lightly pigmented hair, cong

Hypohidrotic Ectodermal Dysplasias: Phenotypic and Genotypic Findings in 32 Cases.

Hypohidrotic ectodermal dysplasias are a genetic condition affecting ectoderm-derived structures such as hair, teeth, nails, and sweat glands, resulting from variations in the EDA, EDAR, EDARADD, and WNT10A genes. This study examined 32 cases from 25 unrelated families from Türkiye, identifying seven novel variants in the EDA, EDAR, and WNT10A genes. The distribution of genetic alterations across the cohort revealed that 44% of the families (11/25) harbored variants in EDA, whereas EDAR and WNT10A variants were identified in 32% (8/25) and 24% (6/25) of families, respectively. Clinical evaluat

Mitochondrial Encephalomyopathy, Lactic Acidosis, and Stroke-Like Episodes (MELAS) Patients with m.3243A > G have High Prevalence of Wolff-Parkinson-White Syndrome.

Mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) is one of the most common mitochondrial disorders. Cardiovascular involvement has been reported in up to 30% of MELAS patients with varying clinical presentations from non-specific cardiogenic abnormalities to conduction abnormalities, cardiomyopathies, heart failure, fatal arrhythmias, and cardiac death. Although conduction defects are a known complication, the frequency of Wolff-Parkinson-White (WPW) syndrome among MELAS patients and patients carrying mutations associated with MELAS is uncertain, and their ass

Schinzel-Giedion syndrome: communication, feeding and motor skills in 16 individuals.

Schinzel-Giedion Syndrome (SGS) is a rare neurodevelopmental disorder caused by pathogenic SETBP1 gain-of-function variants. SGS medical features have been well described. Associated skills critical to quality of life have such as communication, feeding, and motor skills are yet to be characterised. Here we used standardised caregiver report tools to characterise these skills as well as the medical features, in 16 children with SGS (median = 5 years, 7 months, range 6 months to 12.5 years). Vineland-3 scores reflected severe impairment in communication, daily living, socialisation and motor sk

Clinical Features and Current Therapeutic Approaches to Monilethrix: A Systematic Review.

Monilethrix is a rare congenital hair disorder characterized by fragile, beaded hair shafts due to mutations in various keratin or desmoglein 4 genes. A systematic review of 24 studies (16 pediatric only, 3 mixed adult and pediatric, 3 adult only, and 2 unspecified) evaluating various treatment modalities identified topical and oral minoxidil as the most effective therapies, while oral retinoids and other treatments showed variable efficacy. Further pediatric research and clinical studies are necessary to assess long-term treatment outcomes and explore novel therapeutic strategies for this cha

A novel LSS variant leading to hypotrichosis with pseudoainhum.

Congenital alopecia/hypotrichosis constitutes a spectrum of rare inherited hair disorders characterized by partial to complete alopecia and possible structural hair shaft defects. The lanosterol synthase (LSS) gene, implicated in cholesterol biosynthesis, has emerged as a novel genetic determinant of congenital hypotrichosis. This article describes a 43-year-old female with congenital lanugo-like scalp hair and absence of eyebrows who later developed painful fibrotic constrictive bands on her fingers, culminating in pseudoainhum, but without manifestations of typical palmoplantar keratoderma (

Co-occurrence of two monogenic diseases within a single family.

Kindler epidermolysis bullosa (KEB), a rare genodermatosis caused by loss-of-function mutations in the FERMT1 gene (encoding kindlin-1), is clinically characterized by congenital blistering, skin atrophy, poikiloderma, photosensitivity, and an increased predisposition to cutaneous squamous cell carcinoma (cSCC). Hearing loss, a common sensory impairment with multifactorial origins, often stems from genetic or environmental factors. Here, we report a unique familial case in which one sibling has KEB complicated by cSCC, and the other has congenital autosomal recessive non-syndromic hearing loss

Clinical and Genetic Characterization of Hereditary Sensory and Autonomic Neuropathy Type IV in a Consanguineous Population: Identification of Novel NTRK1 Variants and Expansion of Phenotypic Spectrum.

Hereditary sensory and autonomic neuropathy type IV (HSAN4) is a rare neurological disorder characterized by anhidrosis and congenital insensitivity to pain caused by biallelic pathogenic variants in NTRK1. The condition develops because of dorsal root and autonomic ganglion neurodegeneration, which ultimately results in reduced sensation and autonomic neurological dysfunction. We ascertained several neuropathic patients and performed genetic testing using gene panels and exome sequencing (ES). Genetic variants were confirmed by Sanger sequencing. Thirteen families, each with a single affected

Variants in KLF4 affecting residue Asp441 cause an autosomal dominant syndromic ichthyosis.

BACKGROUND: Congenital ichthyoses comprise a group of skin scaling diseases with clinical and genetic heterogeneity. They can be subclassified into syndromic and nonsyndromic forms. Syndromic ichthyoses affect organs beyond the skin. An increasing number of causative genes have been identified. OBJECTIVES: To identify the causative gene and the underlying pathogenesis of a previously unreported syndromic ichthyosis. METHODS: Whole-exome sequencing (WES), combined with Sanger sequencing, was used to identify the genetic defect. Protein structural modelling and dual-luciferase reporter assays

Isolated Compound Dislocation of the Pisiform Bone with Associated Neurovascular Injury Following an Assault: A Rare Injury Pattern.

INTRODUCTION: Arteriovenous malformations (AVMs) are rare congenital vascular abnormalities involving direct connections between arteries and veins, bypassing capillaries. While they can occur throughout the body, AVMs in the forearm are uncommon, representing a diagnostic and therapeutic challenge when they produce symptoms, such as neurovascular compressions, such as unspecified swelling or mass, throbbing pain, localized or radiating tingling, and numbness. These lesions often remain asymptomatic until adolescence, when growth spurts and hormonal changes can trigger enlargement and clinical

A Case of Distal Nail Embedding Successfully Treated With a Slit-Tape Strap Taping Method.

Ingrown nails are a common condition, typically affecting the great toe, and are classified into lateral ingrown nails, retronychia, and distal nail embedding (DNE). DNE occurs when the nail plate's distal edge embeds into the nail fold, often following onycholysis or nail avulsion, with hypertrophy of the hyponychium obstructing nail growth. This condition causes nail discoloration, thickening, and pain, requiring treatment to restore proper nail growth. While surgical and conservative treatments are available, conservative approaches such as taping are more accessible but can be hindered by
