BACKGROUND: Gelatinous drop-like dystrophy is a rare autosomal recessive disorder caused by pathogenic variants in the TACSTD2 gene and characterized by subepithelial amyloid deposits in the cornea. Alterations in FOXC2 are seen in lymphedema-distichiasis syndrome, in which patients classically exhibit lymphedema of the extremities and a double row of eyelashes. This report links alterations in the FOXC2 gene to a corneal phenotype that is histologically identical to that seen in gelatinous drop-like dystrophy. CASE PRESENTATION: A 14-year-old male with congenital distichiasis presented with c

The circling rat is an autosomal recessive mutant (homozygous ci2/ci2) that displays lateralized circling behaviour, locomotor hyperactivity, ataxia and stereotypic head-movement. These abnormal behaviours occur in phases or bursts either spontaneously or in response to stress. Heterozygous (ci2/+) littermates display normal spontaneous behaviours. We have previously found that ci2/ci2 rats of both genders have a lower tissue content of dopamine in the striatum ipsilateral to the preferred direction of rotation, indicating that the rats turn away from the brain hemisphere with higher striatal

OBJECTIVE: An undetectable postoperative serum cortisol has been regarded as a definition of cure in Cushing's disease. However, we noted disease recurrence amongst patients with Cushing's disease despite undetectable postoperative cortisol levels, and this led us to audit our data. We have also previously assessed surgical outcome for acromegaly and microprolactinoma for a single surgeon. The aims of this study were two-fold: (i) to investigate the treatment and surgical outcome of patients with Cushing's disease. In particular, we wished to compare the data with outcome for other pituitary t

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

Heterozygous pathogenic variants in AXIN2 are associated with oligodontia-colorectal cancer syndrome (ODCRCS), a disorder characterized by oligodontia, colorectal cancer, and in some cases, sparse hair and eyebrows. We have identified four individuals with one of two de novo , heterozygous variants (NM_004655.4:c.196G>A, p.(Glu66Lys) and c.199G>A, p.(Gly67Arg)) in AXIN2 whose presentations expand the phenotype of AXIN2-related disorders. In addition to ODCRCS features, these individuals have global developmental delay, microcephaly, and limb, ophthalmologic, and renal abnormalities. Structural

Primary cilia, hair-like projections on the surface of various cell types, play crucial roles in sensing and regulating environmental cues within the liver, particularly among cholangiocytes. These structures detect changes in bile composition, flow, and other biochemical signals, integrating this information to modulate cellular processes. Dysfunction in cholangiocyte cilia-whether due to structural abnormalities or genetic mutations-has been linked to an array of cholangiopathies and ciliopathies. These include conditions such as biliary atresia, cholangiocarcinoma, primary sclerosing cholan

Since the 2010 classification of ichthyoses, our understanding of hereditary epidermal differentiation disorders (EDDs) has markedly increased, allowing for consideration of new therapeutic targets based on disease pathogenesis. A new gene- and protein product function-based classification focuses on shared mechanisms of disease pathogenesis, with the possibility that grouped disorders may respond similarly to new therapeutics. These EDDs have been subdivided into syndromic (sEDD), nonsyndromic with features limited to skin and appendages, and predominantly palmoplantar skin involvement (nonsy

Netherton syndrome (NS) is a rare, autosomal recessive genodermatosis resulting from mutations in the SPINK5 gene, which encodes the LEKTI (Lympho-Epithelial Kazal-type-related inhibitor) protein. This deficiency leads to dysregulated epidermal protease activity, primarily of kallikrein-related peptidases (KLKs), causing severe skin barrier defects, abnormal desquamation, and a complex immune dysregulation involving the TH2 and TH17 pathways. Clinically, NS is characterised by a triad of ichthyosiform erythroderma (often evolving from congenital ichthyosiform erythroderma to ichthyosis lineari

Hydrocephalus, a condition caused by fluid buildup in the brain, is now recognized as a possible complication of primary ciliary dyskinesia (PCD)-a rare genetic disorder that affects the movement of tiny, hair-like structures called motile cilia. This review brings together current knowledge about the disease's causes, symptoms, and treatments. Specific gene mutations like MCIDAS, DNAH5, CCNO, and IFT46 are key factors, as they disrupt normal cilia movement and fluid flow. Other structural problems, such as narrowed brain passages or overproduction of brain fluid, can worsen the condition. Pat

People with amputations using limb prosthetics commonly develop dermatologic complications, such as impaired skin integrity and folliculitis, at the residuum-prosthesis interface (RPI). Dermatologic complications are the primary cause of dissatisfaction with prosthetics, hindering rehabilitation and leading to prosthesis abandonment. This has a deleterious impact on health-related quality of life (HRQOL). Positive outcomes have been associated with laser hair reduction (LHR) for those with traumatic lower limb amputations; however, this has not been studied in those with upper limb amputations

Clouston syndrome is a rare autosomal dominant form of hidrotic ectodermal dysplasia that presents significant challenges due to its multisystemic manifestations and limited therapeutic options. This review explores the functional role of stem cells in the treatment and management of Clouston syndrome, highlighting advancements in regenerative medicine and stem cell therapy. Stem cell types such as mesenchymal stem cells (MSCs), epidermal stem cells (EpSCs), and induced pluripotent stem cells (iPSCs) have shown promise in regenerating ectodermal derivatives like skin, hair, and nails. Complete androgen insensitivity syndrome (CAIS) is a rare X-linked recessive disorder of sex development (DSD) caused by androgen receptor (AR) gene mutation and present with female phenotypes with male chromosomal karyotype. Primitive bipotent gonads in CAIS differentiate into testes producing androgens and antimüllerian hormone (AMH). However, androgens cannot stimulate embryonic wolffian ducts into male internal reproductive organs owing to AR defect and hormone resistance, while AMH induces the regression of müllerian ducts with the absence of uterus, fallopian tubes, and upper third of th

Although several inherited ocular disorders have been extensively studied in horses, few reports of equine ectopia lentis exist and no genetic investigations have been reported. Ectopia lentis in humans and other species is reported to be caused by trauma, genetic variants, and systemic diseases. The most commonly reported genetic causes are dominant alleles in FBN1. Here we examined a 3-day old Oldenburg x Thoroughbred colt due to concerns over bilateral ocular anomalies and hypothesized that either a recessively inherited allele or a dominant de novo allele was the genetic cause. BACKGROUND: Patients with Gorlin (basal cell naevus) syndrome (GS) have numerous phenotypic abnormalities due to overactivity of the hedgehog (HH) signalling pathway, most commonly caused by a heritable mutation in PTCH1, which encodes a major inhibitor of this pathway. Oral HH inhibitors (HHi) can reverse some of the manifestations, most prominent of which is the development of numerous cutaneous basal cell carcinomas (BCCs). In order to improve the benefit-risk ratio, we developed a gel containing a small cyclopamine-derived molecule that can be applied topically in expectation that this mod

In 2019, a group of experts published the first European guidelines for the management of congenital ichthyoses after a multidisciplinary expert meeting held in 2016. An update of these guidelines and literature search was planned every 5 years, given the clinical, molecular and therapeutic advances, including the use of biologic therapies. We present here updated guidelines that have been developed by a reorganized multidisciplinary group of international experts after a systematic review of recent literature, discussions and consensus reached at an expert conference held in June 2023. Excessive hair growth is a common and distressing complaint in women. It is imperative to differentiate excessive hair growth from hirsutism with possible other signs of virilization. Hirsutism is commonly attributed to polycystic ovary syndrome (PCOS). Hirsutism with rapid onset or progression, potentially combined with other signs of virilisation or secondary amenorrhea, should prompt evaluation of other underlying causes. In this clinical lesson, we present three cases of hirsutism to illustrate red flags to consider rare diagnoses. Diagnoses include non-classical congenital adrenal hyperpl

BACKGROUND: Hirsutism, defined as excessive terminal hair growth in females in a male pattern, is a significant clinical manifestation of increased androgen. Unani scholars such as Hippocrates, Haly Abbas, Rhazes, and Avicenna linked hirsutism with menstrual irregularities, specifically Ihtibās al-Tamth (Amenorrhea). Modern medicine also associates hirsutism with hyperandrogenism, a hallmark of polycystic ovarian syndrome (PCOS), among other conditions.
OBJECTIVE: This study aims to explore the understanding of hirsutism from both Unani classical literature and modern medical perspectives, pa

Metformin, a synthetic biguanide, is widely used to manage type 2 diabetes, and is commonly prescribed in polycystic ovary syndrome (PCOS) to address insulin resistance and associated metabolic and reproductive disturbances. PCOS is characterised by hormonal imbalances such as hyperandrogenism and anovulation, metabolic abnormalities including insulin resistance and increased cardiometabolic risk, and higher rates of pregnancy complications. However, the role of metformin in the multifaceted nature of PCOS remains debated. This review synthesises the mechanisms of action of metformin and its e

BACKGROUND: Hirsutism (excessive and unwanted hair growth) is a distressing and relatively common endocrine problem in women that may prove difficult to manage. Cyproterone acetate (CPA), an antiandrogen, is frequently used to treat hirsutism, usually in combination with ethinylestradiol. This is an update of a Cochrane review first published in 2003.
OBJECTIVES: To assess the benefits and harms of cyproterone acetate (CPA) alone, or in combination with ethinylestradiol, and other medication in reducing hair growth and improving the endocrine profile in women with hirsutism secondary to ovari

Congenital heart diseases, including hypoplastic left heart syndrome (HLHS), frequently lead to malnutrition, which can result in worse outcomes in critical and perioperative situations. This study presents a case report based on clinical and nutritional data extracted from the electronic medical record of a 1-year and 10-month-old male infant diagnosed with HLHS, hospitalized in a tertiary cardiology hospital while awaiting a heart transplant. The length of hospitalization while waiting for the organ was approximately 13 months. Upon hospital admission, the child presented with severe malnutr
