Rice field dermatitis is an occupational health problem in Assam. The afflicted paddy field workers develop exanthema on the exposed parts of their skin mainly legs and arms when they come in contact with water in rice fields. Itching sensations start after 30 minutes to 1 1/2 hours after initial exposure. After 6-12 hours there is intensely pruritic macular eruption and in a few cases secondary skin infection may also develop. The clinical course of the disease is suggestive of cercarial dermatitis. In patients with parasitologically revealed dermatitis caused by cercariae of avian schistosomes (Trichobilhariza szidati) diagnostic indirect immunofluorescence technique (IFAT) was employed for the detection of antibodies. BACKGROUND: An epidemic of cercarial dermatitis caused by Schistosoma spindale cercaria occurred in November 1988 in a district called Chaiya, Surajthani province, in Southern Thailand.
CASE REPORTS: Fifty-eight Thai farmers in Chaiya, Surajthani gave a history of itch following immersion in water while planting rice. The area of rice field was flooded for 10 days before farmers started working. The duration of the disease ranged from 2 days to 1 month. Forty-one patients had the disease for the first time, seventeen had a history of previous manifestations.

On October 28, 1991, an employee of the Division of Public Health (DPH), Delaware Department of Health and Social Services, reported that her son and at least 10 other persons who had recently participated in a high school biology class field trip to Cape Henlopen State Park had contracted pruritic dermatitis. The Delaware Health Monitoring and Program Consultation Section conducted an investigation to confirm the reported presumptive diagnosis of cercarial dermatitis and to assess the extent of this outbreak.
