Cercarial dermatitis caused by bird schistosomes comprises both immediate and late phase cutaneous hypersensitivity reactions.

Avian schistosomes are the primary causative agent of cercarial dermatitis in humans, but despite its worldwide occurrence, little is known of the immune mechanism of this disease. Using a murine model, hosts were exposed to primary (1x) and multiple (4x) infections of Trichobilharzia regenti via the pinna. Penetration of larvae into the skin evoked immediate edema, thickening of the exposure site, and an influx of leukocytes, including neutrophils, macrophages, CD4+ lymphocytes, and mast cells. A large proportion of the latter were in the process of degranulating. After 1x infection, inflamma

[Cercarial dermatitis].

A POTENTIAL PUBLIC HEALTH PROBLEM: Cercarial dermatitis is caused by the infestation of the skin by cercariae of nonhuman schistosomes whose commonest hosts are aquatic birds. Human contamination can occur during swimming in fresh water infested with cercariae and notably ducks. Its geographic distribution is worldwide and is increasingly described in France during the summer months. FROM A CLINICAL POINT OF VIEW: A diffuse eruption composed of prurigenous maculopapules appears within the 24 hours following exposure. Regression is spontaneous within one day to three weeks. PREVENTION IS ADVISA

Swimmer's itch: incidence and risk factors.

Swimmer's itch (cercarial dermatitis) affects people engaged in open-water activities. We report incidence and risk factors for a US lake. Water exposures and swimmer's itch experience were reported daily for riparian household residents and guests at Douglas Lake, Michigan, in July 2000. Incidence of swimmer's itch was 6.8 episodes per 100 water exposure days. Positive risks were (1) exposures in shallow water and in areas with onshore winds and (2) more days of lake use in July. Further epidemiological studies will help public health agencies address this bothersome problem at recreational l

First occurrence of Schistosomatidae infecting Aplexa hypnorum (Gastropoda, Physidae) in France.

Due to the recrudescence of cercarial dermatitis in the world, larval Schistosomatidae have been researched during 20 months in gastropods of stagnant fresh waters in a wetland. Brevifurcate ocellate cercariae have been detected after crushing in a species of Physidae, Aplexa hypnorum, with a prevalence of 6.52%. Up to now, this species had never been found infected by larval trematodes. Lymnaeidae, Planorbidae and other Physidae didn't harbour schistosomatids. Cercariae of A. hypnorum present furcal fin-folds and the length of the tail stem is inferior to that of the body. Only cercariae of H

Prospective study of swimmer's itch incidence and severity.

This epidemiologic study reports incidence, severity, and risk factors of swimmer's itch (cercarial dermatitis). Daily diaries about water exposures and swimmer's itch symptoms were completed by 40 riparian households at Douglas Lake, Michigan, for July 2000. Minutes spent in the water, minutes in shallow water, location, time of day, preventive action, age, and gender were recorded for all residents and guests. Incidence of swimmer's itch was 6.8 episodes per 100 water-exposure days. Probability of an episode increased with more days of water use and at locations with onshore winds. Episode s

Presence of Trichobilharzia szidati in Lymnaea stagnalis and T. franki in Radix auricularia in northeastern France: molecular evidence.

A molecular approach was used to analyse a focus of cercarial dermatitis in northeastern France (Lake Der-Chantecoq), including both cercariae and snails,by sequencing the internal transcribed spacers (ITS1 for ocellate furcocercariae and ITS2 for snails). Lymnaea stagnalis were found infected with the furcocercariae of Trichobilharzia szidati, and T. franki furcocercariae were found in Radix auricularia. The record of these two visceral parasites of birds in northern France confirms strong host-parasite relationships. The use of these standardised markers will be of the highest significance f

Waterborne zoonotic helminthiases.

This review deals with waterborne zoonotic helminths, many of which are opportunistic parasites spreading directly from animals to man or man to animals through water that is either ingested or that contains forms capable of skin penetration. Disease severity ranges from being rapidly fatal to low-grade chronic infections that may be asymptomatic for many years. The most significant zoonotic waterborne helminthic diseases are either snail-mediated, copepod-mediated or transmitted by faecal-contaminated water. Snail-mediated helminthiases described here are caused by digenetic trematodes that u

Failure of an ointment based on IR3535 (ethyl butylacetylaminopropionate) to prevent an outbreak of cercarial dermatitis during swimming races across Lake Annecy, France.

Cercarial dermatitis, a re-emerging disease world-wide, may affect those engaged in water sports. Many of those who swim in the annual cross-lake races at Annecy in France, for example, develop such dermatitis. In an attempt to see whether ethyl butylacetylaminopropionate (IR3535) would protect swimmers from the dermatitis, the exposed skin on the right side of each of 78 participants in the cross-lake races of 2001 was treated with IR3535-based ointment. When contacted by telephone during the week following the races, 64 (82%) of the subjects reported that they had developed cercarial dermati

A study on cercarial dermatitis in Khuzestan province, south western Iran.

BACKGROUND: Cercarial dermatitis' or swimmer's itch' is an itchy inflammatory response to the penetration of the skin by non-human schistosome parasites. In the hot season, (May to September) in Khuzestan province in the south west of Iran, swimming in canals and agriculture activities in swampy areas are common. This survey was made on people from villages north of Ahwaz city in south west Iran, to estimate cercarial dermatitis in this region. METHODS: 2000 people were observed for clinical signs of cercarial dermatitis. Also 2000 Lymnaea gedrosiana snails were collected from agriculture can

Neurotropic behaviour of Trichobilharzia regenti in ducks and mice.

The bird nasal schistosome Trichobilharzia regenti is a new agent of cercarial dermatitis. Cercariae are able to penetrate the skin of birds and mammals including man. The parasite then attacks the central nervous system. The present study has shown that schistosomula avoid penetration of blood capillaries and enter the peripheral nerves of the legs of mice and ducks as early as 1 day post-infection (p.i.) and 1.5 days p.i., respectively. These peripheral nerves are used as a route to the spinal cord. In the specific host (duck) schistosomula were found in the spinal cord from 2 days p.i. unti

Variations in immunofluorescent antibody response against Trichobilharzia and Schistosoma antigens in compatible and incompatible hosts.

Schistosome cercariae are able to penetrate into the skin of various vertebrate hosts. However, in contrast to the compatible host, the infection of incompatible hosts results in the death of parasites at various intervals post-infection. In order to compare the immune responses in both types of host infected with Trichobilharzia regenti, Trichobilharzia szidatior Schistosoma mansoni, antibody responses against various T. regenti, T. szidatiand S. mansonischistosome developmental stages were studied. Investigation of an outbreak of cercarial dermatitis.

We present the investigation of an outbreak of cercarial dermatitis that occurred in a recreational-tourist lake in the Quebec City region (Canada) in the summer of 1999. A case-reporting form was sent to 450 families likely to have activities that would bring them in contact with the lake's water. The snails were characterized and the prevalence of their infestation by schistosomes was investigated. In total, 63 episodes consistent with cercarial dermatitis were reported. Sixty-nine percent of the cases occurred from swimming at the same beach. This location was the one where the only populat

Biology of the schistosome genus Trichobilharzia.

Trichobilharzia is the largest genus within the family Schistosomatidae, covering over 40 species of avian parasites. To clarify the existing confusion in the systematics of the genus, we recommend combining knowledge of life cycles and developmental stages, snail/bird hosts, cytogenetical and molecular data together with morphological criteria for the characterization of particular species. The high specificity of Trichobilharzia for the intermediate host is a likely reflection of the ability to avoid the internal defence of specific snails. The spectrum of final hosts (birds) seems to be muc

[Visceral lesions in mammals and birds exposed to agents of human cercarial dermatitis].

Over the past few years, the cercarial dermatitis has become a new problem of public health, obviously linked to the prolonged stay of migrant birds on our territory. This is a skin affection characterized by pruriginous and papulous eruptions caused by penetration of avian bilharzian larvae under the skin. These larvae are emitted by molluscs, mostly limneids. In aquatic birds, especially in migrating Anatidae, these larvae reach the visceral vessels, become adults in a few weeks, lay eggs, then degenerate. Corresponding miracidia contaminate new limneids. Since 1993, the total number of annu

Bird schistosomes: do they die in mammalian skin?

The cercariae of bird schistosomes, released from the intermediate water snail host, actively penetrate the skin of both birds and mammals. Whereas in birds the infection leads to worm maturation and egg production, in the mammalian hosts skin invasion is accompanied by cercarial dermatitis (swimmer's itch, clam-digger's disease) and the fate of the parasites is not clear. Here, we review bird schistosomes as causative agents of cercarial dermatitis, underline adaptations of bird schistosomes to their life in vertebrate hosts, and discuss potential risks caused by the parasites migrating in hu

Investigations of intermediate host specificity help to elucidate the taxonomic status of Trichobilharzia ocellata (Digenea: Schistosomatidae).

The avian schistosomatid Trichobilharzia ocellata plays an important role as causative agent of cercarial dermatitis of humans in Europe. In order to improve the taxonomic knowledge on this parasite, studies of miracidial chemo-orientation as well as experimental infections of different snail species were conducted using strains of T. ocellata and T. franki. Both schistosomes exhibited a high intermediate host specificity. The miracidia clearly preferred the SCW (snail-conditioned water) of the respective natural intermediate host to SCW of other sympatric snail species. T. ocellata proved to

Prevalence of cercariae from Lymnaea stagnalis snails in a pond system in Southern Germany.

Lymnaea stagnalis snails were collected from 174 individual ponds of an extensive pond system in South Germany; 43,441 snails collected during 1980 2000 were examined for shedding cercariae. The species richness (at least 18 species of cercariae) and the high cercarial prevalence (at least 44.9%) may result from the high abundance and diversity of vertebrate and invertebrate hosts in the area. The mean prevalence (% +/- SEM) of most cercarial species increased from May-June to August-October, e.g. in Diplostomum spathaceum from 4.1% to 18.6%, in all echinostomatids from 5.6% to 18.0%, in Pseud

Survival of bird schistosomes in mammalian lungs.

Bird schistosome cercariae have a low specificity to vertebrate skin and, thus, they are also able to penetrate into mammals. As a consequence, a hypersensitive skin response-cercarial dermatitis-develops. It was thought that the parasites die in the skin soon after penetration. Our results on Trichobilharzia szidati and Bilharziella polonica in the non-specific murine host confirm that some of the penetrating bird schistosomes may fully transform to schistosomula and migrate to the lungs. They persist there for up to 10days post exposure. In a duck, the worms grow and feed rapidly, but in a m
