Associations of arsenic exposure and folate in maternal leukocyte DNA methylation: a case-control study of mothers with spina-bifida affected children.

BACKGROUND: In Bangladesh, more than a quarter of drinking water tubewells are contaminated with arsenic above the national standard (50 µg/l), while nearly half exceed the World Health Organization guideline (10 µg/l). Among other negative health consequences, arsenic is a suspected environmental risk factor for neural tube defects (NTDs), including spina bifida. Maternal folate status protects against NTDs, though recent evidence suggests arsenic attenuates folate’s protective effects. Arsenic is methylated prior to excretion with methyl groups produced in one-carbon metabolism, for which fo

Mutational spectrum of EDA, EDAR, EDARADD, and WNT10A genes in the largest cohort of Russian patients with hypohidrotic ectodermal dysplasia.

BACKGROUND: Hypohidrotic ectodermal dysplasia (HED) encompasses a group of rare genetic disorders affecting two or more ectodermal derivatives (hair, teeth, nails, certain glands). The condition can be inherited in an X-linked, autosomal dominant, or autosomal recessive manner, with the majority of cases caused by mutations in the EDA, EDAR, EDARADD, and WNT10A genes. This study aimed to evaluate the distribution of pathogenic and likely pathogenic variants in 261 unrelated families affected by HED in the Russian Federation (comprising 455 patients in total) between 2007 and 2024. To achieve t

Isolated Congenital Hyponychia and Anonychia in a Neonate: A Rare Case.

Congenital hyponychia and anonychia are extremely rare nail disorders characterized by the partial or complete absence of nails. These conditions may be inherited or associated with in utero exposure to teratogenic substances. A full-term male neonate underwent routine head-to-toe examination in the labor room. The baby was noted to have both incomplete (hyponychia) and complete (anonychia) absence of fingernails and toenails. No other systemic abnormalities were identified. Isolated congenital hyponychia and anonychia are uncommon findings in neonates. In the absence of syndromic features, th

Heimler Syndrome Caused by Novel PEX6 Variants: Clinical and Genetic Characterization in a Saudi Cohort.

Background: Heimler syndrome (HS) is a rare autosomal recessive disorder representing the mildest end of the peroxisome biogenesis disorder spectrum. It is caused by hypomorphic mutations in peroxisomal assembly genes, most commonly PEX1 and PEX6, and is characterized by sensorineural hearing loss, amelogenesis imperfecta, and retinal dystrophy. Due to phenotypic overlap with other inherited sensory disorders, particularly Usher syndrome, diagnosis of this condition is frequently delayed. Methods: We investigated two unrelated Saudi families presenting with congenital hearing loss and retinal

Revolutionizing regeneration: stem cells transform treatment of hidrotic ectodermal dysplasia (Clouston syndrome).

Clouston syndrome is a rare autosomal dominant form of hidrotic ectodermal dysplasia that presents significant challenges due to its multisystemic manifestations and limited therapeutic options. This review explores the functional role of stem cells in the treatment and management of Clouston syndrome, highlighting advancements in regenerative medicine and stem cell therapy. Stem cell types such as mesenchymal stem cells (MSCs), epidermal stem cells (EpSCs), and induced pluripotent stem cells (iPSCs) have shown promise in regenerating ectodermal derivatives like skin, hair, and nails. Gene-edi

Adams-Oliver Syndrome: A Clinical Diagnosis in the Genomic Era.

Adams-Oliver syndrome (AOS) is a rare congenital condition marked by defects of the scalp and malformations of the distal extremities. Although several causative genes have been identified, including ARHGAP31, DLL4, NOTCH1, and DOCK6, a subset of clinically suspected cases remains genetically unresolved. We report the case of a four-year-old female patient, the firstborn of a third-degree consanguineous marriage, who presented with a febrile seizure. Physical examination revealed aplasia cutis congenita (ACC) over the parietal scalp, low-set ears, and bilateral lower limb hypoplasia with absen

Comparative Anatomy of Polydactyly and the Adult Nail Unit and Its Implications in the Onychofibroblasts/Onychodermis Theory.

Recent studies have proposed the use of the polydactyly nail unit model as a tool for the study of the molecular basis of onychogenesis. However, the histological and immunohistochemical description of this model is still a matter of debate in the literature, and the validity of the concept of the onychodermis used in the histological interpretation of both polydactyly and the adult nail unit has been challenged on the basis of detailed histological studies of adult fingernail units. The aim of this study was to provide new histological and immunohistochemical data on both the adult toenail un

Management of congenital ichthyoses: guidelines of care: Part two: 2024 update.

In 2019, a group of experts published the first European guidelines for the management of congenital ichthyoses after a multidisciplinary expert meeting held in 2016. An update of these guidelines and literature search was planned every 5 years, given the clinical, molecular and therapeutic advances, including the use of biologic therapies. We present here updated guidelines that have been developed by a reorganized multidisciplinary group of international experts after a systematic review of recent literature, discussions and consensus reached at an expert conference held in June 2023. Hypohidrotic Ectodermal Dysplasias: Phenotypic and Genotypic Findings in 32 Cases.

Hypohidrotic ectodermal dysplasias are a genetic condition affecting ectoderm-derived structures such as hair, teeth, nails, and sweat glands, resulting from variations in the EDA, EDAR, EDARADD, and WNT10A genes. This study examined 32 cases from 25 unrelated families from Türkiye, identifying seven novel variants in the EDA, EDAR, and WNT10A genes. The distribution of genetic alterations across the cohort revealed that 44% of the families (11/25) harbored variants in EDA, whereas EDAR and WNT10A variants were identified in 32% (8/25) and 24% (6/25) of families, respectively. Clinical and Genetic Characterization of Hereditary Sensory and Autonomic Neuropathy Type IV in a Consanguineous Population: Identification of Novel NTRK1 Variants and Expansion of Phenotypic Spectrum.

Hereditary sensory and autonomic neuropathy type IV (HSAN4) is a rare neurological disorder characterized by anhidrosis and congenital insensitivity to pain caused by biallelic pathogenic variants in NTRK1. The condition develops because of dorsal root and autonomic ganglion neurodegeneration, which ultimately results in reduced sensation and autonomic neurological dysfunction. We ascertained several neuropathic patients and performed genetic testing using gene panels and exome sequencing (ES). Genetic variants were confirmed by Sanger sequencing. Thirteen families, each with a single affected

Phenotype puzzle: the role of novel LMBRD1 gene variant in Cbl deficiency causing Dyskeratosis Congenita-like clinical manifestations.

Cobalamin (Cbl) metabolism deficiencies are a heterogeneous group (CblA, CblB, CblC, CblD, CblE, CblF, CblG) of autosomal recessive disorders. CblF deficiency occurs due to mutations in LMBRD1 gene, causing variable phenotype, including neurological, haematological, developmental and dermatological defects. Here, we describe a 15-year-old male, presented with clinical features of Dyskeratosis Congenita (DC) such as dystrophic nails, skin discoloration with additional clinical features of uniform reticulate-brown hued hyperpigmentation, developmental delay, mild intellectual disability, mucosit

Interaction between the TBC1D24 TLDc domain and the KIBRA C2 domain is disrupted by two epilepsy-associated TBC1D24 missense variants.

Mutations of human TBC1D24 are associated with deafness, epilepsy, or DOORS syndrome (deafness, onychodystrophy, osteodystrophy, cognitive disability, and seizures). The causal relationships between TBC1D24 variants and the different clinical phenotypes are not understood. Our hypothesis is that phenotypic heterogeneity of missense mutations of TBC1D24 results, in part, from perturbed binding of different protein partners. To discover novel protein partners of TBC1D24, we conducted yeast two-hybrid (Y2H) screen using mouse full-length TBC1D24 as bait. Kidney and brain protein (KIBRA), a scaffo
