Prenatal Diagnosis of Neurofibromatosis Type 1: No Specific Prenatal Features Observed in Sporadic Fetal Presentations.

OBJECTIVE: To present the indications for prenatal diagnosis (PND) of neurofibromatosis type 1 (NF1) in a Chinese referral medical center. METHOD: This study encompasses a 7-year experience in PND of NF1. The cohort consisted of women undergoing PND due to parental carrier status, as well as those whose fetuses were diagnosed with copy number variations (CNVs) involving NF1 gene or NF1 variants based on indications unrelated to parental carrier status. RESULTS: A total of 30 cases underwent PND related to NF1. Among these, 17 cases involved a parental carrier, with pathogenic NF1 variants id

Melanoma in an Unlikely Guise: Diagnostic Challenges of a Solid-cystic Popliteal Mass in Neurofibromatosis Type 1.

Malignant melanoma (MM) uncommonly presents as a solid-cystic soft tissue mass, posing a significant diagnostic challenge. We report a rare case in a 63-year-old woman with neurofibromatosis type 1 (NF-1) who presented with a painful, rapidly progressive swelling on the posterior aspect of her right lower thigh. Imaging revealed a large, well-defined solid-cystic lesion suggestive of a soft tissue sarcoma or malignant peripheral nerve sheath tumor. Initial fine-needle aspiration cytology was nondiagnostic. Definitive diagnosis was achieved following wide surgical excision, with histopathologic

Respiratory function in children and young adults affected by neurofibromatosis type 1 treated with selumetinib.

BACKGROUND: Plexiform neurofibromas (PNs) are benign tumors that can cause pain, disfigurement, and disability in patients with neurofibromatosis type 1. Selumetinib, a MEK1/2 inhibitor, is the first drug who showed efficacy in inducing PNs shrinkage, reducing the associated morbidities in pediatric patients. Overall, selumetinib showed a good safety profile. However, multiple adverse effects have been reported both in pediatric and adult patients, including respiratory ones. Serial spirometry is suggested in patients with thoracic neurofibromas to detect changes that may underly PN's modifica

Longitudinal transvaginal neurosonographic evaluation of intracranial lesions in a fetus with tuberous sclerosis complex.

PURPOSE: Tuberous sclerosis complex is often suspected prenatally when multiple cardiac rhabdomyomas are detected; however, fetal intracranial lesions may be difficult to identify. The purpose of this case report is to demonstrate the importance and utility of serial transvaginal neurosonography in fetuses with suspected tuberous sclerosis complex. METHODS: We reviewed the prenatal and postnatal imaging findings and clinical course of this case. RESULTS: The fetus was referred at 31 weeks' gestation because of multiple cardiac tumors and a paternal history of tuberous sclerosis complex. Tran

Understanding MEG Source Profiles in Tuberous Sclerosis.

PURPOSE: Patients with tuberous sclerosis complex (TSC) and medically refractory epilepsy are increasingly referred for epilepsy surgery, but presurgical evaluation is challenging due to multiple tubers and complex seizure onset zones. The aim of this retrospective study was to correlate key characteristics of magnetoencephalography (MEG) sources with intracranial EEG and surgical outcomes in patients with TSC. METHODS: Patients with TSC and MRE who underwent MEG, stereo-EEG (sEEG) monitoring, and have at least 1-year postsurgical follow-up were included. MEG source number, laterality, locati

Comprehensive transcriptomic and proteomic profiling of mTOR-related epilepsy.

OBJECTIVE: Malformations of cortical development (MCDs) are a major cause of drug-resistant epilepsy (DRE) in children. A subset of MCDs, including focal cortical dysplasia type II, tuberous sclerosis complex, and hemimegalencephaly, shares characteristic histopathological features and somatic mutations in genes within the Phosphoinositide 3-kinase (PI3K)-AKT-mechanistic target of rapamycin (mTOR) signaling cascade, and is collectively termed mTORopathies. However, mechanisms driving epileptogenesis and drug-resistant seizures remain poorly understood. METHODS: We performed integrated genomic

Neurofibromatosis Type 1: An Imaging Review of Multisystem Manifestations in Children.

Neurofibromatosis type 1 (NF1) is a common inherited tumor predisposition syndrome with multisystem involvement of the central and peripheral nervous systems, vasculature, skeleton, skin, and internal organs. Many clinically relevant manifestations are internal, age-dependent, progressive, or initially asymptomatic, making imaging central to diagnosis, surveillance, and treatment. This review provides a radiology-centered overview of the major imaging manifestations of NF1 and emphasizes pattern recognition, modality selection, and imaging features that should prompt closer follow-up, function

VPS34 regulation in pulmonary arterial vascular smooth muscle cells in pulmonary hypertension.

UNLABELLED: Pulmonary arterial hypertension (PAH) is characterized by the hyperproliferative phenotype of pulmonary arterial smooth muscle cells (PAVSMCs) and extensive extracellular matrix (ECM) remodeling. Vacuolar protein sorting 34 (VPS34), a Class III Phosphatidylinositol-3 Kinase (PI3K), regulates cell proliferation, but its specific function in pulmonary vascular remodeling remains poorly understood. We aim to determine the role of VPS34 activation and regulation in PAVSMC proliferation, matrix deposition, and the progression of PAH. First, we found a marked decrease in the inhibitory p

Case Report: Co-occurrence of Wilson disease and tuberous sclerosis complex in a Chinese patient.

BACKGROUND: Wilson disease (WD) and tuberous sclerosis complex (TSC) are rare genetic disorders with distinct pathophysiologies. Their co-occurrence has not been previously reported. When atypical clinical features emerge in a patient with an established genetic diagnosis, comprehensive reassessment, including thorough physical examination and expanded genetic testing, becomes critical. CASE PRESENTATION: We describe a 28-year-old Chinese woman with a confirmed diagnosis of WD who developed focal seizures after substantial clinical improvement during copper chelation therapy. Her seizures wer

Beyond the cerebrospinal fluid: Preclinical and clinical evidence on central nervous system penetration and functional activity of oral rapamycin (sirolimus).

Rapamycin, a potent mTORC1 inhibitor, is a leading candidate for treating age-related neurodegenerative disease. Numerous studies demonstrate beneficial effects in a variety of diseases that affect the brain. Yet a persistent belief among researchers is that rapamycin cannot penetrate brain tissue, citing as evidence its absence of detection in cerebrospinal fluid (CSF). This narrative review synthesizes clinical and preclinical evidence challenging this "CSF misconception." In humans, direct tissue sampling in glioblastoma detects rapamycin in brain at mTORC1-inhibitory concentrations, while

Prenatal Use of Sirolimus in Fetal Therapy: Current Evidence, Clinical Applications, and Future Directions.

Prenatal imaging, genomic diagnosis, and pathway-directed pharmacology increasingly permit treatment of selected fetal disorders before birth. This narrative review evaluates maternally administered sirolimus as transplacental fetal therapy, emphasizing cardiac rhabdomyomas associated with tuberous sclerosis complex and severe vascular or lymphatic malformations. Sirolimus inhibits mechanistic target of rapamycin complex-1, a pathway central to cell growth, angiogenesis, and lymphangiogenesis. Published fetal case reports and small series describe regression of symptomatic cardiac rhabdomyomas

MITF-Overexpressing Perivascular Epithelioid Cell Tumor of the Urinary Bladder Lacking TSC1/TSC2 Alterations and TFE3 Rearrangement: A Case Report and Review of the Literature.

Perivascular epithelioid cell tumors (PEComas) are mesenchymal neoplasms characterized by melanocytic and smooth muscle differentiation, commonly associated with TSC1/TSC2 alterations or TFE3 rearrangements. Herein, we report a rare case of urinary bladder PEComa with MITF overexpression and without identifiable TSC1/TSC2 mutations or TFE3 rearrangement. A 50-year-old man underwent partial cystectomy for a bladder mass. Histologically, the tumor was well circumscribed and composed of mixed spindle and epithelioid cells with cytologic atypia and low mitotic activity. Immunohistochemistry reveal

Integrating longitudinal seizure, medication, and quality of life data into epilepsy eare: A randomized crossover study of an Epic-integrated dashboard.

OBJECTIVE: Epilepsy treatment decisions require clinicians and patients to synthesize longitudinal seizure diaries, anti-seizure medication (ASM) history, quality of life (QOL), and clinical data, which are often distributed across multiple sources. We evaluated whether an electronic health record (EHR)-integrated Epilepsy Dashboard improved patient/caregiver and clinician short-term outcomes compared with standard clinical practice. METHODS: We conducted a prospective randomized crossover study in outpatient adult and pediatric epilepsy clinics at the University of California, San Francisco.

Efficacy and safety of MEK inhibitors for NF1-associated symptomatic, inoperable plexiform neurofibromas: A systematic review and meta-analysis.

Plexiform neurofibromas arise in up to half of people with neurofibromatosis type 1 (NF1), causing pain, disfigurement and functional loss. Loss of neurofibromin releases RAS-MAPK signaling, making MEK1/2 a rational target. Two agents are now approved in the United States, the adult indication only since 2025. We evaluated the efficacy and safety of MEK inhibitor monotherapy in NF1-associated symptomatic, inoperable plexiform neurofibroma across children and adults. We searched six databases and registries from inception to 10 July 2026, reporting per PRISMA 2020. The primary outcome was objec

Ataxia-Telangiectasia, DNA Repair Deficits, Immune Dysfunction, and Cancer: A Review of Mechanisms, Clinical Presentations, and Emerging Therapies.

Ataxia-Telangiectasia (A-T) is a rare, autosomal recessive disorder caused by biallelic pathogenic variants in the ATM gene. Loss of ataxia-telangiectasia mutated (ATM) protein function impairs DNA damage responses, leading to progressive neurodegeneration, genomic instability, immunodeficiency, and an increased risk of malignancy. Some A-T patients develop cancers at a young age and display extreme sensitivity to ionizing radiation and certain chemotherapeutics. Therefore, standard treatments often cause severe, life-threatening toxicities in these patients. This review discusses the dual rol

Optic glioma in children: clinical presentation, diagnosis, and management.

INTRODUCTION: Optic gliomas are slow-growing astrocytic tumors that frequently occur in children. The general treatment approach for optic gliomas should be based on tumor location, patient age, and NF1 status. Optic gliomas respond well to chemotherapy; it is generally preferred over radiotherapy, especially for children under five years and those with NF1. Chemotherapy primarily results in stabilization of tumor volume, and this effect is often temporary. In some cases, a second line treatment may be required. Radiotherapy may be an option for treating patients with sporadic optic gliomas. T
