Extracellular Matrix and Fibroblast Activation in Lymphangioleiomyomatosis.

Lymphangioleiomyomatosis (LAM) is a rare lung disease caused by hyperactivation of the mTORC1 (mechanistic/mammalian target of rapamycin 1) growth pathway in a subset of mesenchymal lung cells. Histopathologically, LAM lesions have been described as immature smooth muscle-like cells that are positive for the immature melanocytic marker HMB45/PMEL/gp100 and phosphorylated ribosomal protein S6 (pS6). Advances in single-cell sequencing technology allowed us to group LAM cells according to their expression of cancer stem cell (CSC) genes and identify three clusters: a high CSC-like state (i.e., st

RETINAL RACEMOSE HEMANGIOMA ASSOCIATED WITH INTRACRANIAL VASCULAR MALFORMATION (WYBURN-MASON SYNDROME).

PURPOSE: To describe a case of Wyburn-Mason syndrome in a young child with initially declined treatment leading to delayed intervention. METHODS: Case report. RESULTS: We report a 6-year-old boy with mild periocular trauma after a fall, who was incidentally found to have a retinal arteriovenous malformation associated with a brain arteriovenous malformation, indicative of Wyburn-Mason syndrome. The patient's visual examination was unremarkable except for a visual acuity of 20/150 in the right eye, corrected to 20/60, and 20/30 in the left eye, along with mild proptosis and facial asymmetry.

Zebrafish neurofibromatosis type 1 mutants show disruption of sleep but not of circadian rhythms.

Neurofibromatosis type 1 (NF1) is a common neurodevelopmental disorder that is associated with several developmental and behavioral phenotypes, including reduced and fragmented sleep at night. Here, we describe the role of NF1 in sleep in zebrafish, a diurnal vertebrate whose mechanisms of sleep control are broadly conserved with those of mammals. The zebrafish genome encodes two neurofibromin 1 (nf1) paralogs: nf1a and nf1b, which function in a partially redundant manner. We found that zebrafish with reduced nf1 function exhibit decreased and delayed sleep at night, similar to humans with the

p53 Drives Lung Cancer Regression through a TSC2/TFEB-dependent Senescence Program.

UNLABELLED: Pharmacologic restoration of p53 tumor suppressor function is a conceptually appealing therapeutic strategy for the many deadly cancers with compromised p53 activity, including lung adenocarcinoma. However, the p53 pathway has remained undruggable, partly because of insufficient understanding of how to drive effective therapeutic responses without toxicity. In this study, we use mouse and human models to deconstruct the transcriptional programs and sequelae underlying robust therapeutic responses in lung adenocarcinoma. We show that p53 drives potent tumor regression by direct Tsc2

Pharmacogenomic Synthetic Lethal Screens Reveal Hidden Vulnerabilities and New Therapeutic Approaches for Treatment of NF1-Associated Tumors.

Neurofibromatosis type I (NF1) is a common cancer predisposition syndrome caused by heterozygous loss-of-function mutations in the tumor-suppressor gene NF1. Individuals with NF1 develop benign tumors of the peripheral nervous system (neurofibromas), originating from the Schwann cell (SC) lineage after somatic loss of the wild-type NF1 allele, some of which progress further to malignant peripheral nerve sheath tumors (MPNST). There is only one FDA-approved targeted therapy for symptomatic plexiform neurofibromas and none approved for MPNSTs. The genetic basis of NF1 syndrome makes associated t

Tuberous sclerosis-associated psychosis: a systematic review.

A variety of neuropsychiatric symptoms have been reported in tuberous sclerosis. Although psychosis may occur, little has been written about this topic. This systematic review sought to characterize the nature of tuberous sclerosis-associated psychosis, including with respect to treatment response. A literature search was completed in June 2025 using PubMed and Scopus. Only articles that mentioned psychotic symptoms occurring in tuberous sclerosis in a patient-specific manner were eligible. Twenty-six papers describing 27 individuals were included in the main analyses, all of which are case re

Anti-seizure potential of J4, an equilibrative nucleoside transporter 1 inhibitor, in a mouse model of tuberous sclerosis complex in response to pentylenetetrazol.

Tuberous sclerosis complex (TSC) is a neurodevelopmental disorder with epileptic seizures caused by genetic mutations in either TSC1 or TSC2 gene. Multiple genetic, epigenetic, and environmental factors can affect the phenotypical outcomes of TSC individuals. Accumulating evidence has shown that the seizures occurred in early life may contribute to the epileptogenesis and aggravate the neurological setting and neuropsychiatric symptoms of TSC. Therefore, treatments targeting seizures and/or epileptogenesis have always been the main focus on TSC therapies. Current anti-epileptic drugs and mTOR

Oroxylin A Suppresses Pathological Vascular Smooth Muscle Cell Phenotypic Switching and Neointima Formation Through Hindering TSC2/mTORC1/HIF-1-Dependent Glycolysis.

Neointima formation, characterized by similarities to tumor-like growth, plays a pivotal role in the progression of vascular occlusive diseases. Oroxylin A (OroA), a bioactive flavonoid from Scutellaria roots, shows protective effects against cardiovascular diseases and cancers. This study explores whether OroA inhibits neointima formation caused by vascular injury and investigates the mechanisms involved. Models of carotid artery ligation and guide wire-induced injury were used to study vascular smooth muscle cell (VSMC) phenotypic switching and neointima formation. Glycolytic activity and ox

Resection of intracardiac tumors in infants.

Intracardiac tumors in children are rare, mostly benign, and occasionally related to genetic syndromes (p.e. tuberous sclerosis or Gorlin syndrome). Echocardiography, CT and MRI are the golden standard for diagnosis and surgical removal is indicated when the tumor is symptomatic by hemodynamic interference. We present 4 cases of infants with an intracardiac tumor, all of them needing surgical removal.

Delayed Diagnosis of Tuberous Sclerosis Complex: A Case Report of Abdominal Distension, Pain, and Severe Anemia.

We present a case of a 31-year-old woman with progressive abdominal distension, bilateral loin pain, and severe anemia requiring multiple blood transfusions. She was referred for a computed tomography (CT) scan of bilateral renal masses before potential surgery. The scan revealed large renal masses with fat attenuation, diagnosed as angiomyolipomas. Additionally, suspicious facial lesions raised concerns for a neurocutaneous disorder, prompting further imaging. A non-contrasted CT of the head showed multiple calcified subependymal nodules, leading to a delayed diagnosis of tuberous sclerosis c

Rasd2 Knockout Exaggerates the Hearing Loss Phenotype of Tsc1-Deficient Mice.

Rasd2 was originally identified as a striatum-enriched transcript and has been suggested to regulate several signaling pathways including the mTOR pathway. Transcriptomic analyses revealed that Rasd2 is also enriched in inner ear hair cells, while its function in the inner ear remains elusive. Here we showed that RASD2 is localized in the stereocilia as well as the cytoplasm of the hair cells. To investigate its physiological function, we established Rasd2 knockout mice using the CRISPR/Cas9 technique. Rasd2 knockout does not affect stereocilia morphology, and Rasd2 knockout mice manifest norm

Renal Angiomyolipoma in a Young Woman With Tuberous Sclerosis: A Case Report.

Tuberous sclerosis is a rare autosomal dominant genetic disorder caused by mutations in tumor suppressor genes (TSC1/2), leading to hamartomas in multiple organs. Renal angiomyolipomas are often asymptomatic but can cause mass effects or bleeding if they enlarge, requiring treatment. A 29-year-old woman presented with severe left flank pain, an abdominal mass, and facial papules. Imaging revealed a large, 18.3 × 13 × 11.5 cm mass in the lower pole of the left kidney, which was fat-poor, a smaller lesion on the opposite kidney, pulmonary cysts, and osteoblastic bone lesions, all indicating tube

Multimodal treatment with thiotepa, bevacizumab, teniposide, and tunlametinib in a patient with neurofibromatosis type 1-associated oligodendroglioma: a rare case report.

Neurofibromatosis type 1 (NF1) is a common autosomal dominant disorder associated with central nervous system gliomas, most frequently optic pathway gliomas; however, oligodendrogliomas in the setting of NF1 are exceedingly rare, with no prior documented cases and no established treatment strategies to date. A 53-year-old female with NF1-associated oligodendroglioma experienced multiple recurrences following surgery, radiotherapy, chemotherapy, and targeted therapy. Upon further disease progression, she was treated with a novel combination of thiotepa, bevacizumab, teniposide, and the mitogen-

Epilepsy, neuroinflammation and cannabidiol What do we know thus far?

Epilepsy is a common neurological disorder associated with recurring seizures that in about one-third of individuals are resistant to conventional medications. Neuroinflammation and alterations in the endocannabinoid system are involved in epileptogenesis and represent attractive targets for therapeutic interventions. Randomized placebo-controlled trials have shown that cannabidiol (CBD), one of the main active principles found in the Cannabis plant, significantly reduces seizure frequency in patients with Lennox-Gastaut syndrome, Dravet syndrome, and tuberous sclerosis complex (TSC). The FDA'

Seizure outcomes after VNS therapy in children with drug-resistant epilepsy due to monogenic etiologies versus malformations of cortical development.

OBJECTIVE: Vagus nerve stimulation (VNS) is an established adjunctive therapy for drug-resistant epilepsy (DRE). However, evidence regarding its efficacy in children with structural and nonstructural etiologies of epilepsy remains limited. Herein, the authors aimed to explore the effectiveness of VNS in patients with either etiology. METHODS: In this retrospective single-center study, authors evaluated children (ages 2-18 years) with DRE due to nonlesional monogenic epilepsies (MEs) or malformations of cortical development (MCDs) who had undergone VNS device implantation between 2008 and 2024
