Cannabidiol against Epilepsy: Insights and an Experimental In Silico Approach.

Cannabidiol (CBD), one of the compounds found in cannabis sativa, has drawn a lot of interest in the study and treatment of epilepsy. The antiepileptic qualities of CBD are being investigated for their ability to reduce seizure frequency and intensity in people with rare epilepsies, including West syndrome, Ohtahara's syndrome, Dravet syndrome, Lennox-Gastaut syndrome, and Tuberous Sclerosis. This review attempts to analyze the antiepileptic effects of cannabidiol against voltage-gated calcium channel T-type (CaV), GammaAminobutyric Acid A (GABAA), voltage-gated potassium channel of the Q fami

A rare early-onset bilateral renal cysts, focal seizures in a 1-year-old male with tuberous sclerosis and No mutation identified.

Tuberous Sclerosis Complex (TSC) is a rare genetic disorder characterized by the development of benign tumors in multiple organs. This report presents an unusual case of early-onset renal cystic disease in a 1-year-old male with TSC, despite the absence of detectable mutations in the TSC1 or TSC2 genes. Postnatal imaging revealed bilateral polycystic kidney disease by 2 months of age. The patient presented with secondary hypertension and seizures. Neuroimaging confirmed cortical tubers and a subependymal giant cell astrocytoma (SEGA), while echocardiography identified cardiac rhabdomyomas. Des

Successful Everolimus Therapy for Atrioventricular Block Due to Cardiac Rhabdomyoma Associated With Tuberous Sclerosis Complex.

BACKGROUND: Cardiac rhabdomyoma (CR) associated with the tuberous sclerosis complex can cause atrioventricular (AV) block (AVB) by compressing the AV node. Everolimus, an mTOR inhibitor, is known to shrink these tumors and alleviate obstruction and tachycardia; however, improvement in AVB has not been previously described. CASE SUMMARY: A 25-year-old woman was found to have a fetus with subependymal nodules and multiple CRs. After birth, an electrocardiogram demonstrated a persistent 2:1 AVB with a ventricular rate of 60 to 70 beats/min. Echocardiography revealed CRs, including one near the A

An integrated, scaled approach to resolve TSC2 variants of uncertain significance.

Obtaining a precise genetic tuberous sclerosis diagnosis is a challenge as many missense TSC2 variants are variants of uncertain significance (VUS). VUS in TSC2 have been resolved by one-at-a-time functional assays, but these assays cannot scale to the 3,634 TSC2 missense VUS observed so far. To address this challenge, we used massively parallel sequencing to measure the steady-state abundance of almost 9,000 TSC2 missense variants and developed an mTOR pathway activity assay using genome editing and cell sorting to generate activity scores for 391 missense variants. 1,288 of 8,891 (14.49%) mi

PAX8-positive conventional urothelial carcinomas of the urinary bladder and their distinct molecular profiles - A clinicopathologic study of 101 consecutive cases with next-generation sequencing in 20 cases.

AIMS: PAX8 immunohistochemistry (IHC) is often used to distinguish urothelial carcinomas (UCs) from tumours of renal and Mullerian origin. However, some UCs have been shown to be PAX8-positive. This study investigates the frequency of PAX8-positive conventional UCs of the urinary bladder without subtype morphology and/or divergent differentiation and their molecular profiles. METHODS: One hundred and one consecutive transurethral resections of the urinary bladder from 2019 to 2022 with a diagnosis of conventional urothelial carcinoma (UC) without subtype morphology and/or divergent differenti

Rhabdomyomas of the Mitral Valve: Case Series and Conservative Management Approach.

Rhabdomyomas are relatively rare, cardiac tumors that are predominantly seen in infancy. Involvement of the mitral valve is considered exceptionally uncommon. We retrospectively reviewed three cases of mitral valve rhabdomyomas in patients with tuberous sclerosis complex (TSC) at a single institution, focusing on clinical presentation, imaging, and management. All patients presented with low cardiac output and mitral inflow obstruction, resulting in compromised circulatory dynamics. Echocardiography revealed mobile, hyperechoic masses on the mitral leaflets. Despite moments of hemodynamic conc

mTOR pathway mediates the endoplasmic reticulum stress -apoptosis of CD4+ T cell through inhibiting autophagy flux in sepsis.

BACKGROUND: CD4 + T cells are major reactive subpopulation for cellular and humoral immune responses following sepsis. The apoptosis of CD4 + T cells may contribute to sepsis-induced immunosuppression, and preventing the induction of endoplasmic reticulum stress (ERS) can ameliorate apoptosis of CD4 + T cells in sepsis. The mechanistic target of rapamycin (mTOR) pathway performs an essential regulatory role on ERS-apoptosis of CD4 + T cells. This study aims to elucidate the underlying mechanisms of mTOR regulation of ERS-apoptosis of CD4 + T cells. METHODS: In this study, based on the cecal l

Molecular convergence enables precision medicine for pediatric low grade gliomas.

Pediatric low-grade gliomas are the most common CNS tumors in children, comprising heterogeneous WHO grade 1–2 neoplasms. Despite histologic diversity, these tumors show remarkable molecular convergence with near-universal RAS/MAPK pathway activation, typically driven by single oncogenic alterations. The most frequent molecular alterations include BRAF abnormalities (V600E mutations, KIAA1549-BRAF fusions) and FGFR alterations. Most tumors occur sporadically, though some arise in cancer predisposition syndromes, particularly neurofibromatosis type 1 and tuberous sclerosis complex. Clinical pre

Genkwanin enhances survival of rat random skin flap: promoting autophagic flux by modulation of AMPK/TSC2/mTOR signaling pathway through SIRT1.

Large scale skin defects and skin reconstruction require the use of random skin flaps, and the size and shape of the flaps can be designed according to the wound. However, distal ischemic necrosis of random skin flaps is a major challenge after transplantation. The purpose of this study is to investigate the role of Genkwanin (GE) in ischemic necrosis of random skin flaps. Network pharmacology predicted the potential target SIRT1 and related pathways (including AMPK/TSC2 pathway) of GE. Transcriptomic sequencing further elucidated the mechanism of GE action. On postoperative day 7, skin flap s

Integration of intraoperative ultrasound and depth-electrode electrocorticography for resection guidance in epilepsy surgery: technical workflow and feasibility.

BACKGROUND: Complete resection of the epileptogenic zone (EZ) is the strongest predictor of seizure freedom in drug-resistant epilepsy (DRE). However, even in MRI-positive cases with anatomo-electro-clinical concordance, the EZ may not be clearly delineated, complicating intraoperative decision-making. Intraoperative ultrasound (ioUS) provides real-time anatomical feedback, while depth-electrode intraoperative electrocorticography (iECoG) enables electrophysiological delineation of epileptogenic tissue beyond the cortical surface, sampling deep regions not accessible to subdural electrodes. Clinically Silent Seizures in Neonates with Tuberous Sclerosis: An International Case Series.

INTRODUCTION: In patients with tuberous sclerosis complex (TSC), seizure onset can be as early as in the neonatal period. Recent studies showed that earlier treatment of TSC positively improves epilepsy and neurodevelopmental outcomes. METHODS: This is an international retrospective study on neonates with TSC monitored with long-term video-EEG. RESULTS: Six of ten neonates with a perinatal diagnosis of TSC were found to have electrographic-only seizures within the first 10 days of life on long-term video-EEG. All patients in this series were found to have TSC2 variants and, except for 1 pati
