PURPOSE: Tuberous sclerosis complex is often suspected prenatally when multiple cardiac rhabdomyomas are detected; however, fetal intracranial lesions may be difficult to identify. The purpose of this case report is to demonstrate the importance and utility of serial transvaginal neurosonography in fetuses with suspected tuberous sclerosis complex.
METHODS: We reviewed the prenatal and postnatal imaging findings and clinical course of this case.
RESULTS: The fetus was referred at 31 weeks' gestation because of multiple cardiac tumors and a paternal history of tuberous sclerosis complex. Tran

PURPOSE: Patients with tuberous sclerosis complex (TSC) and medically refractory epilepsy are increasingly referred for epilepsy surgery, but presurgical evaluation is challenging due to multiple tubers and complex seizure onset zones. The aim of this retrospective study was to correlate key characteristics of magnetoencephalography (MEG) sources with intracranial EEG and surgical outcomes in patients with TSC.
METHODS: Patients with TSC and MRE who underwent MEG, stereo-EEG (sEEG) monitoring, and have at least 1-year postsurgical follow-up were included. MEG source number, laterality, locati

OBJECTIVE: Malformations of cortical development (MCDs) are a major cause of drug-resistant epilepsy (DRE) in children. A subset of MCDs, including focal cortical dysplasia type II, tuberous sclerosis complex, and hemimegalencephaly, shares characteristic histopathological features and somatic mutations in genes within the Phosphoinositide 3-kinase (PI3K)-AKT-mechanistic target of rapamycin (mTOR) signaling cascade, and is collectively termed mTORopathies. However, mechanisms driving epileptogenesis and drug-resistant seizures remain poorly understood.
METHODS: We performed integrated genomic

UNLABELLED: Pulmonary arterial hypertension (PAH) is characterized by the hyperproliferative phenotype of pulmonary arterial smooth muscle cells (PAVSMCs) and extensive extracellular matrix (ECM) remodeling. Vacuolar protein sorting 34 (VPS34), a Class III Phosphatidylinositol-3 Kinase (PI3K), regulates cell proliferation, but its specific function in pulmonary vascular remodeling remains poorly understood. We aim to determine the role of VPS34 activation and regulation in PAVSMC proliferation, matrix deposition, and the progression of PAH. First, we found a marked decrease in the inhibitory p

BACKGROUND: Wilson disease (WD) and tuberous sclerosis complex (TSC) are rare genetic disorders with distinct pathophysiologies. Their co-occurrence has not been previously reported. When atypical clinical features emerge in a patient with an established genetic diagnosis, comprehensive reassessment, including thorough physical examination and expanded genetic testing, becomes critical.
CASE PRESENTATION: We describe a 28-year-old Chinese woman with a confirmed diagnosis of WD who developed focal seizures after substantial clinical improvement during copper chelation therapy. Her seizures wer

Rapamycin, a potent mTORC1 inhibitor, is a leading candidate for treating age-related neurodegenerative disease. Numerous studies demonstrate beneficial effects in a variety of diseases that affect the brain. Yet a persistent belief among researchers is that rapamycin cannot penetrate brain tissue, citing as evidence its absence of detection in cerebrospinal fluid (CSF). This narrative review synthesizes clinical and preclinical evidence challenging this "CSF misconception." In humans, direct tissue sampling in glioblastoma detects rapamycin in brain at mTORC1-inhibitory concentrations, while

Prenatal imaging, genomic diagnosis, and pathway-directed pharmacology increasingly permit treatment of selected fetal disorders before birth. This narrative review evaluates maternally administered sirolimus as transplacental fetal therapy, emphasizing cardiac rhabdomyomas associated with tuberous sclerosis complex and severe vascular or lymphatic malformations. Sirolimus inhibits mechanistic target of rapamycin complex-1, a pathway central to cell growth, angiogenesis, and lymphangiogenesis. Published fetal case reports and small series describe regression of symptomatic cardiac rhabdomyomas

Perivascular epithelioid cell tumors (PEComas) are mesenchymal neoplasms characterized by melanocytic and smooth muscle differentiation, commonly associated with TSC1/TSC2 alterations or TFE3 rearrangements. Herein, we report a rare case of urinary bladder PEComa with MITF overexpression and without identifiable TSC1/TSC2 mutations or TFE3 rearrangement. A 50-year-old man underwent partial cystectomy for a bladder mass. Histologically, the tumor was well circumscribed and composed of mixed spindle and epithelioid cells with cytologic atypia and low mitotic activity. Immunohistochemistry reveal

RATIONALE: Lymphangioleiomyomatosis (LAM) is a rare disease characterized by the proliferation of neoplastic smooth muscle-like cells in women. The role of the immune system in LAM progression is unclear.
OBJECTIVES: Investigate the influence of T cells on TSC2null smooth muscle cell behaviour at cellular and transcriptome levels.
METHODS: Archival LAM and control lung tissues were compared by gene profiling with NanoString analyses where elevated CD8 T cell accumulation was observed in LAM vs. control tissues. Acknowledging the limited translational potential of animal models, a human T cel

OBJECTIVE: Epilepsy treatment decisions require clinicians and patients to synthesize longitudinal seizure diaries, anti-seizure medication (ASM) history, quality of life (QOL), and clinical data, which are often distributed across multiple sources. We evaluated whether an electronic health record (EHR)-integrated Epilepsy Dashboard improved patient/caregiver and clinician short-term outcomes compared with standard clinical practice.
METHODS: We conducted a prospective randomized crossover study in outpatient adult and pediatric epilepsy clinics at the University of California, San Francisco.

BACKGROUND: Angiomyolipoma (AML) is a rare benign mesenchymal tumor, originating from perivascular epithelioid cells (PECs), composed of smooth muscle, fat, and thick-walled blood vessels, most often found in the kidney. Uterine AML is exceptionally rare, with under 40 cases reported worldwide and 3 from India prior to the present case. Unlike renal AMLs, uterine lesions are typically sporadic and not linked to tuberous sclerosis (TS), which, combined with their resemblance to fibroids, makes preoperative diagnosis challenging.
CASE DESCRIPTION: A 49-year-old woman presented with six months o

BACKGROUND: Tuberous Sclerosis Complex (TSC) is caused by inactivating mutations in either the TSC1 or TSC2 genes, leading to activation of the mammalian target of rapamycin complex 1 (mTORC1) and unhindered cell growth and proliferation. The epithelium of TSC renal cysts in both mice and humans is composed of proliferating A-intercalated (A-IC) cells. The exact molecular mechanism of kidney cystogenesis in TSC remains speculative.
HYPOTHESIS: Superfluous cell proliferation driven by mTORC1 activation increases metabolic demand, causing oxidative stress and excess reactive oxygen species. If

Tuberous sclerosis complex (TSC) is a multisystem genetic disorder frequently accompanied by epilepsy and TSC-associated neuropsychiatric disorders. Psychiatric-predominant seizure presentations may, however, delay recognition. This clinical challenge describes a 26-year-old woman with recurrent panic-like episodes characterized by dyspnea, choking sensations, palpitations, and derealization, followed by depressive symptoms. The episodes persisted despite treatment with selective serotonin reuptake inhibitors and benzodiazepines. Further examination revealed hypomelanotic macules, prompting ne

BACKGROUND: PEComas are neoplasms originating from perivascular epithelioid cells. Their incidence is higher in patients with tuberous sclerosis; however, hepatic PEComas are exceptionally rare compared to those in other locations. Diagnostic confirmation relies on histopathological findings and immunohistochemical phenotypes.
STUDY PURPOSE: To evaluate the clinical-radiological presentation, histopathological characteristics, and surgical outcomes of a multicenter cohort of hepatic PEComas.
METHODS: A retrospective, multicenter, nationwide observational study of patients with histologically

Birt-Hogg-Dubé (BHD) and Tuberous Sclerosis (TSC) are inherited cancer syndromes associated with kidney cystogenesis and tumorigenesis and caused by mutations of the folliculin (FLCN) and TSC1/2 genes, respectively. We and others previously showed that Transcription Factors EB (TFEB) and E3 (TFE3) are the main drivers of the kidney phenotypes observed in mouse models of these conditions. These transcription factors are also responsible for the feedback hyperactivation of the mechanistic Target of Rapamycin Complex 1 (mTORC1), a known tumorigenic factor. This raises the question of whether TFEB

BACKGROUND: Tuberous sclerosis complex (TSC) is a rare autosomal dominant neurodevelopmental disorder caused by loss-of-function mutations in TSC1 or TSC2 , resulting in chronic hyperactivation of the mechanistic target of rapamycin (mTOR) pathway. TSC patients experience a wide spectrum of cognitive, behavioral, and psychiatric features collectively termed Tuberous Sclerosis Complex Associated Neuropsychiatric Disorders (TAND). Moreover, these patients develop a clinical trajectory in adulthood with clinical and neuropathological features overlapping those of Alzheimer's disease (AD). Prior n

The nutrient-sensing mechanistic target of rapamycin complex 1 (mTORC1) signaling pathway controls cellular and organismal growth and metabolism, and aberrant activation is linked to human disease, including metabolic disease. Cellular studies have established regulatory mechanisms influencing mTORC1 activation, but the physiological signals that control mTORC1 at the organismal and tissue levels are less well-defined. mTORC1 is dynamically regulated by fasting and feeding in metabolic tissues, with both nutrients and insulin proposed to activate mTORC1 in response to feeding. Here, studies em

History An asymptomatic 45-year-old man with sickle cell disease and coinherited α-thalassemia was referred to the hepatology department for workup of an indeterminate hepatic mass discovered incidentally at screening cardiac MRI. The patient was diagnosed with sickle cell disease in childhood and required several hospitalizations with blood transfusions. During adulthood, disease management was suboptimal, with infrequent transfusion therapy and undertreated chronic hemolytic anemia. There was no history of malignancy, hepatitis B or C infection, alcohol consumption, metabolic syndrome, diabe
