Ataxia-telangiectasia (A-T) is a heterogeneous genetic disorder with a recessive mode of inheritance resulting from biallelic variants in the A-T mutated gene (ATM). Besides ataxia, the disorder involves compromised immunity and an increased risk of malignancies. We recruited a consanguineous Pakistani family with multiple individuals having adolescent-onset ataxia. Phenotyping and clinical testing were completed for the patients. DNA samples from multiple individuals were used for bidirectional exome sequencing at 100X coverage, and data were aligned to the hg19 genome assembly. Sanger sequen

BACKGROUND: Ataxia-telangiectasia (AT) is a rare disorder leading to multisystem involvement due to impaired DNA repair. Recent advances in targeted therapies highlight the importance of early diagnosis. This study aimed to evaluate clinical, radiological, and genetic variability in patients with AT and examine genotype-phenotype associations and diagnostic timing.
METHODS: A retrospective study was conducted on genetically confirmed AT patients followed in four pediatric neurology centers in Türkiye over a 12-year period.
RESULTS: A total of 21 patients with AT were included; the median age

Ataxia-Telangiectasia (A-T) is a multisystem disorder caused by loss of A-T mutated (ATM) protein activity, characterized clinically by immunodeficiency and cerebellar ataxia. ATM is a master regulator of DNA damage responses and loss of ATM function is accompanied by persistent activation of PARP1 leading to depletion of intracellular NAD+ and dysfunction of a series of cellular signalling pathways dependent on NAD+, providing a mechanistic rationale for NAD+ augmentation therapy. We performed a clinical trial of NAD+ augmentation with nicotinamide riboside (NR) over 24 months in A-T patients

Neurofibromatosis type 1 (NF1) is an autosomal dominant disorder that causes skeletal dysplasia and congenital pseudoarthrosis of the forearm, which is very difficult to repair. The vascularized free fibular flap (FFF) is the standard treatment. CAD-CAM technologies and virtual surgical planning (VSP) offer greater precision and their full integration into primary pediatric reconstruction for NF1 is described. The objective of this case report is to present the simultaneous integration of all these tools in the primary reconstruction of a child with forearm pseudarthrosis due to NF1. We presen

Patients with neurofibromatosis type 1 develop Schwann cell tumors called neurofibromas that arise within peripheral nerves, driven by loss of neurofibromin and consequent increased RAS/RAF/MEK signaling. MEK inhibitors achieve partial responses for benign neurofibromas but are limited by toxicity and incomplete efficacy, necessitating alternative approaches. Using the Dhh-Cre;Nf1fl/fl neurofibroma mouse model, we found that genetic ablation of Kras, but not Hras, markedly reduced neurofibroma development, inhibited MAPK activation, and rescued disrupted Remak bundles that are a morphologic ha

Pancreatic cystic lesions (PCLs) may occur in several hereditary syndromes, although their frequency, phenotype, and clinical implications differ considerably across entities. This review summarizes the spectrum of hereditary-associated PCLs, focusing on von Hippel-Lindau syndrome, autosomal dominant polycystic kidney disease, cystic fibrosis, multiple endocrine neoplasia type 1, and other hereditary pancreatic cancer predisposition syndromes. The lesions most commonly encountered are simple cysts, serous cystic neoplasms, and intraductal papillary mucinous neoplasms, whereas cystic pancreatic

NF2 (neurofibromatosis type 2)-related schwannomatosis (NF2-SWN) is a cancer predisposition syndrome characterized by the development of bilateral vestibular (VS) and spinal schwannomas. While benign, these tumors can cause substantial morbidity, and effective pharmacological treatments remain limited. Here, we demonstrate that genetic ablation of focal adhesion kinase (Fak/Ptk2) impairs tumor formation and preserves hearing in a murine model of NF2. Mechanistically, we show that Fak deletion decreases macrophage infiltration, attenuates nucleotide-binding oligomerization domain-containing pro

While vestibular schwannomas (VS) have traditionally been managed with surgery and radiation, systemic targeted therapies have emerged as a noninvasive means of potentially improving tumor control and functional outcomes in this patient population. In this narrative review, we explored the current landscape of systemic therapies that have been considered for the management of sporadic VS and neurofibromatosis type 2 (NF2)-VS or NF2-related schwannomatosis. In NF2-VS patients, cell proliferation has been targeted through receptor tyrosine kinase blockade and inhibition of downstream kinase acti

BACKGROUND: Language impairments impact 72% of individuals with tuberous sclerosis complex (TSC). Nevertheless, evaluations are often delayed until children are 7 years old or older. Earlier prediction of language impairment is needed. Infant vocal behavior is a predictor of language development in typically developing children, but early vocal development had not been studied in TSC until recent research on 38 infants. In this study, the canonical babbling ratio (CBR) and volubility, key indicators of infant vocal behavior, were low. CBR reflects the proportion of syllables that are canonical

BACKGROUND: Ataxia telangiectasia is a rare, multisystem disorder with progressive cerebellar neurodegeneration and no approved treatments. The efficacy of corticosteroids, including erythrocyte encapsulated dexamethasone sodium phosphate (eDSP), which have been studied for two decades in this disease, has not yet been proven in randomised trials. We aimed to investigate the safety and efficacy of eDSP in children aged 6-9 years with ataxia telangiectasia.
METHODS: NEAT was a multicentre, randomised, double-blind, placebo-controlled phase 3 study, conducted at 20 sites across nine countries (

BACKGROUND: Spinal ependymomas (SE) account for 10% of ependymomas and are the most frequent spinal glial tumors. NF2-related Schwannomatosis (NF2-SWN) patients are predisposed to multiple SE in addition to other tumors, schwannomas and meningiomas. Surgery remains the main treatment option, but mays lead to severe morbidity. In many cases, it is the cystic rather than the solid component of the tumor that drives neurological symptoms and ultimately necessitates surgical intervention. Understanding tumor and associated cyst formation is therefore important to develop new therapeutic approaches

BACKGROUND Solitary neurofibromas of the colon are extremely rare and are most often reported in patients with neurofibromatosis type 1 (NF1). Although usually benign, these lesions can rarely undergo malignant transformation, particularly when associated with NF1. Therefore, isolated cases merit careful evaluation and follow-up. CASE REPORT A man in his 70s with a history of benign prostatic hyperplasia presented with recurrent abdominal pain, constipation, and unintentional weight loss of 5 kg over 1 month. Physical examination revealed a firm, non-mobile mass in the right lower quadrant, wi
