Tuberous sclerosis complex (TSC) is a genetic condition with multisystem neurocutaneous signs, including hamartomas, epilepsy, and neuropsychological difficulties. Although sleep disorders are increasingly recognized in TSC, they remain poorly described in adults. Additionally, the influence of clinical features and commonly used medications on sleep is not well understood. This study assessed sleep disorders in adults with TSC and examined contributing clinical and treatment factors. Adults were recruited through the TSC Alliance and the TSC Center of Excellence (CoE) at UTHealth Houston. Sle

INTRODUCTION: Tuberous sclerosis complex (TSC) is a rare autosomal dominant multi-systemic disease, leading to excessive cell proliferation and the formation of hamartomas affecting various organs. International and French national guidelines emphasize the need for regular follow-up by different specialists with regular clinical evaluation, and biological and imaging investigations. This study aims to assess patient follow-up, its cost, and its adherence to the French National follow-up recommendations since the establishment of the day-hospital program tailored for pediatric patients with TSC

BACKGROUND: The PREVeNT trial enrolled 72 infants with tuberous sclerosis complex using the presence of interictal epileptiform discharges (IEDs) on a one-h electroencephalography (EEG) to randomize into a trial of placebo versus early vigabatrin. Here, we ask if early vigabatrin impacts IED presence over the first 3 years of life.
METHODS: Participants underwent an EEG every 6 weeks/first year of life, every 3 months/second year of life and once at age 36 months. EEGs were reviewed by two treatment blinded electrophysiologists; if they disagreed, a third reader adjudicated.
RESULTS: Seven h

BACKGROUND AND PURPOSE: Tuberous sclerosis complex (TSC) is a genetic disorder that affects multiple organs, particularly the brain, and is associated with neurologic impairments such as cognitive deficits and seizures. There are relatively few prior studies investigating brain metabolites in TSC. The aim of this study was to investigate neurochemical alterations in TSC, focusing on cortical/subcortical glioneuronal tubers as well as normal-appearing regions, and comparing them with matched healthy control subjects.
MATERIALS AND METHODS: The study included 19 subjects with TSC (mean, 15.9 [S

Sturge-Weber syndrome (SWS) is a rare neurocutaneous syndrome characterized by facial port-wine birthmarks and associated eye and meningeal angiomatosis. Its neurological manifestations include intellectual disability, seizures, and stroke-like episodes (SLEs), often misdiagnosed as stroke. We report a case of SWS presenting with left-sided weakness. Cranial computerized tomography revealed SWS-related venous anomalies, while digital cerebral angiogram confirmed proliferative angiopathy of the distal middle cerebral artery (MCA) with proximal MCA spasm, resolved by intra-arterial verapamil. Tuberous sclerosis complex (TSC) is a multisystem condition associated with disease-causing variants of either TSC1 or TSC2 genes. Significant kidney involvement in TSC is most often due to development of angiomyolipomas (AMLs) and occurs more frequently in people with TSC2 variants. Kidney cysts are also commonly seen; however, these are usually small and not recognised as problematic. A subset of patients has a severe polycystic kidney disease due to a contiguous gene deletion involving PKD1 and TSC2 on chromosome 16. End-stage kidney disease (ESKD) occurs commonly in patients with TSC2/PKD1

The limited and inconsistent efficacy of existing therapies for tuberous sclerosis complex (TSC) has driven the exploration of novel strategies, including epigenetic regulation. GSK-J4, an inducer of global H3K27me3 accumulation, shows broad anti-tumor activity. However, its therapeutic potential in TSC remains unclear. In the study, we reported that GSK-J4 inhibited cell cycle progression and induced apoptosis in primary Tsc1+/- and Tsc2+/- MEFs. Mechanistically, Tsc1 or Tsc2 deletion reduced global H3K27me3, correlating with increased viability, accelerated cell cycle, and suppressed apoptos

OBJECTIVE: Novel epilepsy treatments for patients with tuberous sclerosis complex (TSC) and focal cortical dysplasia type II (FCDII) are urgently needed. In these patients, mutations in the mechanistic target of rapamycin (mTOR) pathway genes lead to mTOR hyperactivity and focal cortical malformations that frequently cause intractable epilepsy and neurological sequelae. Recent evidence suggests that administration of simufilam, a small molecule thought to modulate the function of filamin A, reduces seizure activity independent of mTOR in a mouse model of FCDII. Here, we tested the hypothesis t

Neurofibromatosis type 1 (NF1), an autosomal dominant genetic disorder resulting from mutations in the NF1 gene, is frequently characterized by the development of plexiform neurofibromas (PNFs). A paradigm shift in the management of PNFs has occurred following the successful development of mitogen-activated protein kinase kinase inhibitors. In light of these advancements, the Expert Consensus for Full Course Management of Plexiform Neurofibroma (2025 Edition) was published by the Group of Neurofibromatosis, Plastic and Reconstructive Surgery Society of Chinese Medical Association in 2025. Infantile hemangiomas are benign vascular tumors that occur exclusively in infants and display a characteristic growth pattern. They usually become noticeable shortly after birth, proliferate over a period of 4-6 months, remain stable in size for a variable duration, and then gradually involute, although residual changes may persist. Diagnosis is typically made clinically, with careful attention to potential complications, functional impairments, and rare associated malformations such as those seen in PHACES and LUMBAR syndromes. A thorough understanding of the different types of hemangiomas,

Neurofibromatosis Type 1 (NF1), an autosomal dominant genetic disorder, is characterized by extensive clinical variability, posing significant challenges for prognosis and patient management. Despite being caused by mutations in a single gene, NF1, the expressivity of the disease ranges from mild cutaneous manifestations to severe, life-threatening complications, including malignant tumors, skeletal deformities, and cognitive impairments. This paper provides a comprehensive review of the genetic determinants underlying this phenotypic heterogeneity. We begin by elucidating the molecular geneti

INTRODUCTION: The mTOR inhibitors sirolimus and everolimus are targeted therapies for tuberous sclerosis complex (TSC). Recent studies suggested their potential efficacy in treating epilepsy in TSC children. However, clinical evidence remains limited, and reports of adverse effects vary widely. This study aims to investigate the efficacy and safety of mTOR inhibitors in reducing epileptic seizures in TSC children.
METHODS: We retrospectively analyzed the clinical data of children with TSC. Children were categorized into groups based on the use of mTOR inhibitors. Seizure frequency and adverse

INTRODUCTION: Hemangioblastomas (HGB) are the most common primary intra-axial tumors in the posterior fossa in adults, with an overall occurrence of 7-10%. They occur sporadically or as part of von Hippel-Lindau (VHL) disease. The role of stereotactic radiosurgery (SRS) as a minimally invasive treatment in larger HGB (>2cc) has not been thoroughly investigated.
METHODS: This multi-center study retrospectively analyzed data from 91 patients with large HGB (>2cc) treated between 1993 and 2023. Patients were stratified into VHL-associated and sporadic groups, with assessments including radiosurg

Neurofibromatosis type 1 (NF1) is an autosomal dominant RASopathy associated with increased risk of early-onset breast cancer, particularly triple-negative breast cancer (TNBC). We report a woman in her early 40s with NF1 who presented for elective cosmetic breast surgery and was incidentally found to have a suspicious right breast lesion on screening mammogram. Biopsy confirmed high-grade ductal carcinoma in situ (DCIS) with invasive ductal carcinoma, immunohistochemically triple-negative. Staging investigations revealed no nodal or distant disease. She underwent bilateral mastectomy with rig

OBJECTIVE: Tuberous sclerosis complex (TSC) is a rare genetic disorder associated with early-onset drug-resistant epilepsy (DRE) secondary to intracranial tubers. Many patients undergo stereo-EEG (sEEG) for seizure onset localization, sometimes requiring multiple sEEGs and subsequent epilepsy surgeries. Our objective was to evaluate the effects of single versus multiple sEEGs on seizure outcomes in pediatric patients with TSC-associated DRE.
METHODS: Retrospective review of 45 pediatric TSC patients who underwent epilepsy surgery at our institution (2/2013-4/2026). Collected data included dem

A female patient in the 70s presented with bilateral flank pain and haematuria. Evaluation confirmed tuberous sclerosis complex (TSC) with cutaneous findings, a giant renal angiomyolipoma (AML) in the patient's left kidney with a bleeding pseudoaneurysm, and a large mass in the right kidney containing distinct AML and high-grade renal cell carcinoma tumours. Situs inversus totalis was noted. Transarterial embolisation controlled the haemorrhage from the left kidney, followed by open partial nephrectomy on the right kidney to address the malignancy while preserving renal function.
