Characterization of von Hippel Lindau gene variants in an African American cohort.

Autosomal-dominant (AD) pathogenic/likely pathogenic (P/LP) variants in von Hippel-Lindau (VHL) gene cause VHL disease. We characterize VHL variants and disease phenotypes in Black/African American (AA) patients, a demographic that has not been thoroughly studied. Black/AA patients undergoing germline genetic testing at a CLIA-certified commercial laboratory from November 2014 to March 2022 and carrying a P/LP AD VHL gene variant were identified. Personal and family histories were obtained from test requisition forms, and patients were categorized into VHL disease subtypes: type 1, type 2A, ty

VHL gene fragment analysis: large inversion detection in Alu region for clinical applications.

von Hippel-Lindau (VHL) is an autosomal-dominant tumour susceptibility disorder associated with pathogenic germline variants in the VHL gene that put patients at increased risk of developing benign and malignant tumours within various organs. While current CLIA (Clinical Laboratory Improvement Amendments) genetic tests demonstrate a high detection rate of pathogenic VHL variants, clinical manifestations of VHL with atypical germline alteration undetectable by established genetic tests have been reported by Vocke et al, indicating the need for bespoke methods confirming germline alterations. We

Features and management of kidney manifestations of tuberous sclerosis complex: a real-world study in a Brazilian patient cohort.

BACKGROUND: Tuberous sclerosis complex (TSC) is a rare autosomal dominant disorder resulting from pathogenic variants in the TSC1 or TSC2 genes, leading to hyperactivation of the mechanistic target of rapamycin (mTOR) pathway and subsequent development of benign neoplasms in multiple organs. Kidney involvement is frequent, being angiomyolipoma the most common lesion. Angiomyolipoma can occur since childhood and progress throughout life, potentially leading to complications such as hemorrhage and chronic kidney disease (CKD). This study aims to characterize the kidney phenotype in a Brazilian c

Gut microbiome alterations are sex-dependently associated with brain abnormalities in a mouse model of Neurofibromatosis type I.

Neurofibromatosis type 1 (NF1) is a genetic condition presenting with variable symptomatology, however most individuals will demonstrate cognitive and behavioural difficulties, including autism. Using a heterozygous germline knockout mouse model of NF1 (Nf1 +/-), we performed in-depth behavioural evaluations encompassing learning and memory, stereotypy, social interaction, anxiety- and depression-like behaviour. Anatomical and functional studies of the brain and gastrointestinal tract were followed by the first investigation of gut microbiota composition (via full-length 16S rRNA sequencing) i

Acute Seizure Susceptibility and Chronic Vascular Malformation in a Developmental Mouse Model of Sturge-Weber Syndrome.

Sturge-Weber Syndrome (SWS) is a rare neurovascular disorder caused by a somatic mosaic missense point mutation in GNAQ, resulting in a facial capillary malformation (port-wine birthmark) and abnormal blood vessels in the eye and brain. Symptoms include seizures, stroke-like episodes, and glaucoma. Acute seizures induced with low-dose kainate in a transgenic GNAQ R183Q mouse model of SWS assessed seizure susceptibility and the impact of seizures on cerebral vasculature. Mice expressing human GNAQ mutation experienced more severe seizures and greater seizure-induced mortality compared to litter

A two-step temporal data augmentation and supervised learning framework for predicting autism diagnosis at 36 months in patients with tuberous sclerosis complex.

BACKGROUND: Autism spectrum disorder (ASD) affects approximately 25-50% of children with tuberous sclerosis complex (TSC). Early identification of ASD in this high-risk population is crucial for timely intervention but remains challenging due to the heterogeneous clinical presentation and complex interplay of genetic, neurological, and environmental factors. This study aimed to integrate longitudinal diffusion tensor imaging (DTI) metrics with early behavioral features using supervised learning algorithms to predict ASD outcomes at 36 months. METHODS: Data were obtained from the children enro

[Rupture of Intercostal Aneurysm Associated with von Recklinghausen's Disease:Report of a Case].

Vascular lesions, including aneurysms, are rare but well-documented complications of von Recklinghausen's disease. Although aneurysms associated with this condition are often asymptomatic, rupture can lead to life-threatening events such as hemothorax. We report a 63-year-old male with von Recklinghausen's disease who presented with sudden onset of chest pain. Imaging revealed left-side hemothorax, and contrast-enhanced computed tomography( CT) suggested rupture of the left 10th intercostal artery. Given his hemodynamic stability, percutaneous coil embolization was performed. Angiography revea

Decreased attention in 10- and 14-month-olds with neurofibromatosis type 1 and association with later ADHD traits.

BACKGROUND: Identifying precursors to ADHD, which affects up to 5% of children, is crucial for early identification and support. To this end, we used a prospective sample to investigate endogenous attention and activity level in infants with and without an elevated likelihood (EL) for ADHD and investigated associations with ADHD traits at 3-years. EL status was based on a family history of autism and/or ADHD or a diagnosis of neurofibromatosis type 1 (NF1), a genetic condition associated with higher rates of ADHD. METHODS: Infants (n = 26 typical likelihood (TL), n = 70 EL-autism, n = 28 EL-A

Stigmatization related to cutaneous neurofibromas in neurofibromatosis 1: development, validation and severity strata of the cNF-PUSH-D.

BACKGROUND: Cutaneous neurofibromas (cNFs) are benign skin tumours affecting over 95% of individuals with neurofibromatosis type 1 (NF1). Their visible appearance often leads to altered body image, social isolation and discrimination. Although stigma is a well-recognized burden in NF1 and has been identified by the Response Evaluation in Neurofibromatosis and Schwannomatosis consortium as a key outcome subdomain for future cNF clinical trials, no specific tool has been developed to assess stigma in relation to cNFs. OBJECTIVES: To adapt, develop and validate a patient-reported outcome measure

PHACE(S) Syndrome Minus the H: A Case of Possible PHACE(S) Syndrome Without an Infantile Hemangioma.

A 4-month-old, full-term girl was referred for evaluation of a supraumbilical raphe and an anomalous aortic arch. Further work up for PHACE(S) syndrome was pursued, despite the absence of a cutaneous infantile hemangioma (IH), which identified hypoplastic and dysplastic cerebral vasculature. We present this case to raise awareness of possible PHACE(S) syndrome even in children without cutaneous IHs and the importance of recognizing supraumbilical raphe as a sentinel finding.

In Vitro Models of Sturge-Weber Syndrome: Strengths, Limitations, and Future Goals.

Sturge-Weber Syndrome (SWS) is a rare congenital disorder presenting with a vascular malformation in the upper face and brain, causing impaired blood-brain barrier function and perfusion, increased calcium signaling, inflammation, and seizures. All these neuropathologic processes result in injury to the brain. The somatic GNAQ R183Q variant, which accounts for the majority of SWS cases, increases signaling through MAPK, PI3K, mTOR, and inflammatory pathways, primarily in endothelial cells. The discovery of this variant enabled the creation of transgenic and genetic animal and cell culture mode

Bridging the Diagnostic Gap in Neurofibromatosis Type 1: When to Consider NF1 cDNA Sequencing.

Molecular confirmation of neurofibromatosis type 1 (NF1) is essential for accurate diagnosis, reproductive counseling, and lifelong surveillance, yet genomic DNA (gDNA)-based next-generation sequencing (NGS) panels may miss deep intronic or complex structural variants. We report five children meeting the 2021 NF1 clinical diagnostic criteria who had negative DNA-based NGS testing but were subsequently diagnosed using NF1 cDNA-based testing (NF1 cDNA testing), which identified three deep intronic variants, one 5'UTR variant, and one exonic insertion, including three novel variants. Four of the
