Neural speech encoding in fetal alcohol spectrum disorder: an exploratory study using frequency-following responses.

BACKGROUND: Fetal alcohol spectrum disorder (FASD) is associated with neurodevelopmental impairments, including listening difficulties not always explained by peripheral hearing loss, suggesting alterations at the level of neural auditory processing. The frequency-following response (FFR) provides an objective measure of neural speech encoding and may offer insight into auditory function in this population. METHODS: Twenty-five normal-hearing participants were included: 11 individuals with FASD and 14 controls. Speech-evoked FFRs were recorded using a 160 ms /da/ stimulus at 80 dB SPL with a

Building Toward Better Housing for Canadians With Fetal Alcohol Spectrum Disorder.

BACKGROUND: Individuals with disabilities have a right to live independently, be included within their communities, and have adequate standards of living and social protections. However, individuals with fetal alcohol spectrum disorder (FASD) experience elevated obstacles to obtaining and maintaining housing. In addition to brain-based differences that can create barriers to successful navigation of daily demands, many individuals with FASD experience mental health concerns, substance use, or trauma, and the experience of being houseless is, in itself, traumatic. These factors often compound,

Prenatal Alcohol Exposure as a Potential Contributor to Hydranencephaly: Clinical and Neurodevelopmental Perspectives.

Hydranencephaly is traditionally attributed to prenatal vascular disruption; however, chronic prenatal alcohol exposure may contribute through neurotoxicity, placental dysfunction, oxidative stress, and vascular injury. Given the biological plausibility of this association, alcohol abstinence during pregnancy remains essential, and further studies are required to clarify its role in hydranencephaly.

Increasing Access to FASD-Informed Care: Bringing an Adapted Intervention to Mental Health Providers.

BACKGROUND: An estimated 1 in 4 children seeking mental health care have an undiagnosed fetal alcohol spectrum disorder (FASD). Few mental health providers receive training or feel competent to serve this population. Evidence-based interventions (EBIs) for FASD have low uptake across mental health settings. OBJECTIVE: This study aimed to systematically adapt an EBI to increase access to care for youth with FASD in mental healthcare settings, resulting in the innovative Families Moving Forward (FMF) Connect Pro program. METHOD: A multidisciplinary planning group employed the theoretically-dri

Analysis of fetal MRI data reveals no effect on liver maturation following prenatal alcohol exposure.

OBJECTIVE: Prenatal alcohol exposure (PAE) of the fetus may result in physiognomic alterations and/or impediment of cognitive development, summarized as fetal alcohol spectrum disorders (FASDs). Although alcohol is metabolized in the liver, the effects of PAE on the fetal liver are unknown. MATERIALS AND METHODS: For this study, n = 21 prenatal MRIs of fetuses with PAE and n = 45 MRIs of fetuses without PAE were included. To assess macroscopic organ development, liver volumetry was conducted and compared to an established prenatal estimation formula. A texture-based radiomics analysis based o

Maternal gut and vaginal microbiota, alcohol use during pregnancy, and infant fetal alcohol spectrum disorder diagnosis in a South African cohort.

BACKGROUND: The detrimental impact of alcohol consumption on the gut microbiome is well-established. However, less is known about how alcohol exposure during pregnancy affects the maternal gut and vaginal microbiota, or how these microbial changes relate to subsequent infant diagnosis of fetal alcohol spectrum disorder (FASD). We therefore investigated associations between self-reported alcohol use during pregnancy, infant FASD diagnosis, and maternal gut and vaginal microbiota. METHODS: Fecal samples (n = 207) and vaginal swabs (n = 28) from pregnant participants recruited through antenatal

Early adversity, lasting impact: Characterizing cortical morphology in youth with prenatal alcohol exposure.

BACKGROUND: Prenatal alcohol exposure (PAE) occurs in ~10% of pregnancies in North America and can alter brain development. Prior research on brain morphology in children with PAE shows mixed findings for cortical thickness, and only a few studies have investigated more complex morphological features, showing decreased gyrification with PAE. Here, we use four anatomical measures to provide a comprehensive characterization of cortical morphology in youth with PAE compared to unexposed youth. METHODS: T1-weighted Magnetic Resonance Imaging (MRI) was used to examine cortical morphology metrics i

Assessment of GABAA α1, α4, and α5 receptor subunit protein expression in dorsal and ventral hippocampus of adult male and female Long-Evans rats after moderate prenatal alcohol exposure.

Fetal Alcohol Spectrum Disorders (FASD) describe a range of neurobiological and behavioral changes following prenatal alcohol exposure. Previous work has shown that moderate prenatal alcohol exposure (mPAE) can produce alterations to GABAergic neurotransmission in the hippocampal formation (HPF) as well as morphological alterations in GABAergic interneuron expression in adult rats. In the present study, we tested the hypothesis that mPAE differentially alters GABAA subunit protein expression in dorsal and ventral HPF of naïve adult male and female Long-Evans control and mPAE rats. We focused o

Estimating the lifetime costs associated with maternal alcohol consumption during pregnancy.

OBJECTIVES: Maternal alcohol consumption during pregnancy is a major, yet preventable, risk factor for fetal alcohol spectrum disorders (FASD), which can lead to lifelong neurodevelopmental, educational, and social challenges. The economic burden of FASD in Germany remains poorly quantified. In this study, we aimed to estimate the direct and indirect lifetime costs associated with FASD in Germany. STUDY DESIGN: Static cohort-based cost-of-illness study. METHODS: We conducted a cost-of-illness analysis from the perspectives of the German healthcare (e.g., psychiatric care) and social (e.g., a

Nailfold Capillary Morphology as a Potential Biomarker of Cardiovascular or Metabolic Risk in Children Aged 3-17 Years Prenatally Exposed to Alcohol.

BACKGROUND: Emerging evidence indicates that individuals with prenatal alcohol exposure (PAE) may be at risk for cardiometabolic dysfunction. Nailfold capillaroscopy (NFC) is a noninvasive method for visualizing microvascular dysfunction, which often precedes clinical disease. In this study, we examined whether children and adolescents with confirmed PAE differed in nailfold capillary morphology from unexposed peers. METHODS: We used a cross-sectional design to compare nailfold capillary morphology in children and adolescents aged 3-17 years with confirmed PAE versus no or minimal PAE, enroll

Fetal Alcohol Spectrum Disorder Prevalence Studies in School Settings: A Global Systematic Review of Methods, Measures, Strengths and Limitations.

ISSUES: Fetal alcohol spectrum disorder (FASD) is a prevalent yet widely underdiagnosed neurodevelopmental condition resulting from prenatal alcohol exposure (PAE). FASD has profound lifelong implications for affected individuals, their caregivers and society. Active case ascertainment is the gold standard for estimating FASD prevalence, but methodological variability limits comparability across studies. This review aims to systematically evaluate the methodologies and reporting practices of school-based FASD prevalence studies, with the goal of identifying best practices and informing standar

Validity of an Online Cognitive Screening Tool for FASD: Testing the Equivalence of BRAIN-online to Neuropsychological Assessment Measures.

BACKGROUND: The diagnostic process for fetal alcohol spectrum disorders (FASD) requires investments in both time and cost. The Brief Assessment of Individual Neurobehavior, online version (BRAIN-online), is a screening tool designed to identify cognitive and behavioral symptoms associated with FASD. BRAIN-online is effective in distinguishing individuals with histories of prenatal alcohol exposure (PAE) and FASD; however, criterion validity of this screening tool has not been established. The goal of this study was to evaluate performance on BRAIN-online against gold-standard methods of measur

Collaborative knowledge mobilisation to increase awareness and use of new SNOMED clinical terms for fetal alcohol spectrum disorder in England.

INTRODUCTION: Fetal alcohol spectrum disorder (FASD) affects an estimated 1.8-3.6% of UK school children and up to 27% of children in care. FASD remains vastly under-diagnosed and under-recorded in UK electronic patient records (EPRs), leading to significant challenges for individuals with the condition. In 2024, we developed new SNOMED clinical terms aligned to UK national guidelines and quality standards for FASD. AIMS: Through collaborative knowledge mobilisation (KM), we aimed to: i) raise awareness of the importance of utilising these new clinical terms; ii) identify barriers to FASD cod

Applying the theoretical domains framework to identify determinants of alcohol consumption during pregnancy: A cross-sectional study among women of childbearing age in Southwest Germany.

BackgroundPrenatal alcohol exposure (PAE) is a major teratogen associated with fetal alcohol spectrum disorders, yet alcohol use during pregnancy remains common in Germany. Previous interventions focusing primarily on education have shown limited effectiveness. The Theoretical Domains Framework (TDF) offers a comprehensive approach to identify psychosocial determinants of behavior, but its application to alcohol use in pregnancy has been limited.ObjectivesTo identify cognitive, emotional, social, and contextual determinants of alcohol consumption during pregnancy using a TDF-based framework, i

Prenatal Alcohol Exposure: Impact on Neuroimmune Function in the Stress Response and Pain.

Fetal alcohol spectrum disorder (FASD) encompasses a broad range of central nervous system (CNS)-related disabilities representing a mild-to-severe continuum of neurodevelopmental disorders that include mood and sensory function. The prevalence of FASD is estimated at ~5% in school-aged children who are prone to develop inappropriate responses to stressful stimuli leading to higher rates of anxiety and low touch tolerance. Low touch tolerance has been self-reported in 33.6% of FASD-affected individuals. The FASD manifestations of sensory abnormalities like pathological light-touch hypersensiti

Increased Prevalence of Rare Copy Number Variants in Australian Children With Fetal Alcohol Spectrum Disorder: Experience in a State-Wide Diagnostic Service.

BACKGROUND: Prenatal alcohol exposure (PAE) can result in fetal alcohol spectrum disorder (FASD), a permanent, heterogeneous condition characterized by severe neurodevelopmental impairments with or without microcephaly, characteristic facial dysmorphology, and congenital anomalies. Clinical and pre-clinical studies suggest that genetic factors, in the mother or fetus, influence the phenotypic outcomes following PAE. The objectives of this study were to evaluate the prevalence and types of DNA copy number variants (CNVs) in children (0-18 years) diagnosed with FASD and to identify genes known t

Prenatal Alcohol Exposure and Dysmorphic Features in Adults at Midlife.

BACKGROUND: Dysmorphology examinations have established that fetal alcohol spectrum disorders (FASD) are associated with characteristic physical features from infancy through adolescence. As individuals mature into young adulthood and beyond, the aging process may attenuate characteristic features over time. The purpose of this study was to evaluate FASD-related dysmorphology at midlife. METHODS: Subjects (N = 192) were recruited from two longitudinal cohorts and from the general community (Mage = 39.5 years, SDage = 8.1 years). A majority (n = 132, 68.8%) had an FASD or well-documented prena
