The fetal fentanyl syndrome: Additional evidence in support of a new human teratogen.

PURPOSE: Prenatal opioid exposure has been inconsistently associated with congenital anomalies. In 2023, fentanyl was proposed as a human teratogen in ten infants with documented exposure and a consistent pattern of abnormalities. Disrupted cholesterol biosynthesis was proposed as the pathogenetic mechanism. METHODS: We evaluated 56 children aged 0 to 4 years with confirmed prenatal fentanyl exposure. We collected data on intrauterine exposures, dysmorphic features, congenital anomalies, neonatal course, feeding, growth and development, cholesterol precursors, and genetic testing. We analyzed

The Role of the Hedgehog Pathway in Alcohol-Induced Birth Defects.

In this chapter, we review the current literature that focuses on the role of Sonic hedgehog (Shh) signaling, a major morphogenic pathway regulating embryonic development, in the pathology of fetal alcohol spectrum disorder (FASD), caused by prenatal alcohol exposure (PAE). Ethanol exposure disrupts Shh signaling during development across a range of species including in mice, chick, and zebrafish. Ethanol appears to interfere with Shh signaling through multiple mechanisms, both direct and indirect, including reduced Shh ligand production, impaired post-translational processing, and inducing ap

Diagnosis across the fetal alcohol spectrum disorders (FASD) continuum.

Fetal Alcohol Spectrum Disorders (FASD) comprise a continuum of neurodevelopmental and physical disabilities resulting from the teratogenic effects of prenatal alcohol exposure. FASD is recognized as the leading cause of preventable developmental disabilities worldwide. Recent U.S. school-based studies estimate its prevalence at 11.3-50.0 per 1000 children (approximately 1-5 %), with rates in certain high-risk populations reaching up to 169 per 1000 (approximately 17 %). Although early diagnosis is associated with improved long-term outcomes, FASD remains frequently underrecognized or misdiagn

The effects of developmental ethanol exposure & postnatal choline supplementation on long-term choline metabolism.

Prenatal alcohol exposure (PAE) can disrupt development, leading to alterations in physical, health, and behavioral outcomes, referred to as fetal alcohol spectrum disorders (FASD). Although alcohol likely impacts fetal development through many mechanisms, PAE could impact metabolism of choline, an essential nutrient that is important for brain development and function. Importantly, both preclinical and clinical studies show that choline supplementation can improve performance on hippocampal-dependent behavioral tasks, even when administered postnatally. However, the mechanisms by which cholin

Large-scale in vivo electrophysiology analysis reveals opposing thalamic and hippocampal excitability after third-trimester-equivalent alcohol exposure in mice.

Acute, binge-like alcohol exposure during the third-trimester-equivalent period (TTAE) produces apoptotic neurodegeneration in brain regions critical for spatial learning and memory, including the anterior thalamic nuclei (ATN), which are necessary for contextual learning and memory. To investigate the neural mechanisms underlying spatial cognition deficits following developmental alcohol exposure, we performed high-density electrophysiological recordings in and around the ATN of behaving mice, sampling 7,490 neurons across multiple, predominantly thalamic depths. To model acute binge-like TTA

Association of maternal alcohol consumption with frequent nocturnal enuresis in offspring.

While the long term adverse effects of maternal alcohol consumption during pregnancy on offspring physical and mental development are well documented, its relationship with elimination function remains understudied. This study aimed to examine the association between maternal alcohol consumption and FNE in children and to assess whether this association differed according to maternal drinking pattern, including alcohol type, drinking frequency, and alcohol intake level. From 1 June 2014 to 30 June 2014, data were collected from 74 251 children aged 3 to 12 years in three urban and four suburba

Coenzyme Q10 Mitigates Anxiety- and Depression-Like Behaviors in a Fetal Alcohol Spectrum Disorder Model by Suppressing Hippocampal Astrocytic Reactivity, Oxidative Stress, and Neuronal Cell Death.

OBJECTIVE: Fetal alcohol spectrum disorder (FASD) results from prenatal alcohol exposure and is associated with long-term neurobehavioral deficits, including anxiety and depression. Ethanol-induced oxidative stress, neuroinflammation, and hippocampal damage are key contributors to these outcomes. Coenzyme Q10 (CoQ10), a mitochondrial antioxidant, may offer neuroprotection against such ethanol-induced damage. This study aimed to investigate the potential therapeutic effects of CoQ10 on anxiety- and depression-like behaviors and hippocampal neurotoxicity in a neonatal rat model of FASD. METHOD:

Organ-Specific Histopathological Effects of Prenatal Alcohol Exposure: A Narrative Review.

The role of alcohol-induced epigenetic modifications that predispose the fetus to metabolic dysregulation, increased susceptibility to future substance use, and long-term behavioral and cognitive impairments has received increasing attention. Fetal alcohol syndrome (FAS) and fetal alcohol spectrum disorders (FASDs) remain underdiagnosed worldwide. As evidence continues to demonstrate that no safe level of alcohol consumption exists during pregnancy, strengthening public awareness, maternal education, and prenatal care services has become increasingly imperative. Prenatal alcohol exposure (PAE)

Does a Biochemical Approach Facilitate the Diagnosis of Prenatal Alcohol Exposure and Fetal Alcohol Spectrum Disorder in Neonatal Period?

Prenatal alcohol exposure (PAE) can cause fetal alcohol spectrum disorder (FASD). The FASD continuum encompasses facial dysmorphism, growth failure, and central nervous system (CNS) abnormalities/dysfunctions. Because some of these features may not be apparent in newborns, detecting PAE in the neonatal period is challenging, while early diagnosis may improve neurodevelopmental outcomes. Maternal self-reported alcohol consumption is limited by recall bias and denial, leading to misdiagnosis. Currently, there is a lack of universally implemented and standardized tools for identifying PAE/FASD in

The NKCC1 Antagonist Bumetanide, Administered After Prenatal Alcohol Exposure, Mitigates Ethanol-Induced Aberrance in Tangential Migration of GABAergic Interneurons in Mouse Medial Prefrontal Cortex.

BACKGROUND: Prenatal ethanol (alcohol) exposure (PreEE) results in abnormal tangential migration of medial ganglionic eminence (MGE)-derived GABAergic interneurons (GINs) in the embryonic mouse medial prefrontal cortex (mPFC); later in life, this is associated with impaired behavioral flexibility commonly seen in fetal alcohol spectrum disorders (FASD). The PreEE-induced abnormal tangential migration involves, in part, ethanol potentiating GABAA receptor-mediated depolarization, the latter due to elevated intracellular chloride sustained primarily by the Na-K-Cl cotransporter NKCC1 in embryoni

Planned Ethanol Consumption in Pregnancy: Impact on Offspring Cardiac Autophagic Pathway via TRPM2 Channels-Implications for Fetal Alcohol Syndrome-Linked Cardiological Disorders.

Fetal alcohol spectrum disorders (FASD) are associated with significant cardiac pathologies. In a compromised uterine environment, maternal stressors drive fetal adaptations that prioritize brain development, potentially at the expense of the heart. This study examined the role of the TRPM2 channel and its interaction with autophagic and stress-signaling pathways in a prenatal alcohol exposure model. Using a rodent model of prenatal ethanol exposure, as described previously, cardiac tissues from male and female offspring were analyzed by H&E staining for histopathology and by immunohistochemis

Prenatal Alcohol Exposure Produces Selective Changes in Neuroimmune Gene Expression Across Brain Regions of Adult Mice.

BACKGROUND: An overwhelming body of evidence suggests neuroimmune dysfunction as a key underlying mechanism of fetal alcohol spectrum disorder (FASD)-associated adverse central nervous system (CNS) outcomes. While few studies have highlighted the lingering effects of prenatal alcohol exposure (PAE) on producing specific immune factors, others suggest a primed neuroimmune state in adulthood, in which a proinflammatory bias is unmasked following subsequent immune activation in later life. However, the PAE-induced neuroimmune landscape in adulthood remains poorly defined. We hypothesized that PAE

Developmental Deficiency of Oligodendrocytes and Myelination in the Central Nervous System of the Fetal Alcohol Syndrome Mouse Model.

BACKGROUND: Fetal alcohol spectrum disorder (FASD) is a severe condition characterized by physical and neurodevelopmental abnormalities in the fetus resulting from prenatal alcohol exposure (PAE) or prenatal and lactation alcohol exposure (PLAE). Behavioral abnormalities associated with white matter damage have been extensively investigated in FASD patients and animal models, which suggested the involvement of myelination in FASD pathology. However, the impact of FASD on oligodendrocytes (OLs) and myelination within the central nervous system (CNS) and the underlying mechanisms are little disc

Early Postnatal Ethanol Exposure Has Long-Term Behavioral Consequences in Female Mice.

Background/Objectives: Fetal alcohol spectrum disorders (FASDs) occur in nearly 5% of children in the United States and have been associated with alterations in neurological functions, neuroanatomical changes, and behavioral deficits encompassing an individual's lifetime. Alterations in myelination have been reported in both rodent models and humans. The cerebellum is a heavily myelinated brain region, and oligodendrocyte and myelination transcripts have been reported to be altered in the cerebellum following early-life ethanol (EtOH) exposure in a mouse model. In this study, we investigated c

Epigenetic pathways linking prenatal alcohol exposure to fetal alcohol spectrum disorder.

Fetal Alcohol Spectrum Disorder (FASD) presents a profound global health burden, yet clinical diagnosis remains severely hindered by phenotypic heterogeneity and the lack of objective biomarkers. Emerging evidence reveals that the pathogenesis of prenatal alcohol exposure (PAE) is heavily driven by fetal programming via epigenetic modifications. In this narrative review, we synthesize data from human cohorts and advanced preclinical models to conceptualize FASD as a dynamic epigenetic trajectory rather than a static teratogenic insult. Ethanol disrupts the epigenome systemically, perturbing on

Impact of Prenatal Alcohol Exposure on the Cerebral Circulation: Potential Implications for Stroke.

Prenatal alcohol exposure (PAE) is recognized as a teratogenic factor that affects neural development, resulting in a range of structural, functional, and cognitive/behavioral abnormalities contributing to the pathogenesis of fetal alcohol spectrum disorder (FASD). FASD is a major preventable cause of developmental delay in humans. There are many molecular and cellular mechanisms by which PAE could contribute to abnormalities seen in individuals with FASD. Understanding these mechanisms will be critical for the development of therapeutic approaches that could benefit not only the developing fe
