Allopurinol for Marfan Aortopathy: Pharmacogenomic Safety and Sex-Stratified Trial Design Warrant Consideration.

A recent review by Egea et al. presents a compelling case for adding allopurinol to standard beta-blocker/ARB therapy in Marfan syndrome (MFS) aortopathy, drawing on consistent preclinical evidence and the drug's recent European Medicines Agency orphan-drug designation. This letter identifies two gaps that are genuinely absent from that review and could materially affect the safety and interpretability of any future allopurinol trial in MFS. First, the review discusses allopurinol hypersensitivity syndrome, toxic epidermal necrolysis, and Stevens-Johnson syndrome only as background toxicology,

FBN1 Variant of Uncertain Significance in a Patient With a Marfan Spectrum Phenotype: A Case Report.

Marfan syndrome (MFS) is an autosomal dominant multisystemic connective tissue disorder caused by pathogenic variants in the FBN1 gene and characterized by cardiovascular, ocular, and musculoskeletal manifestations. Ocular findings commonly include strabismus, refractive error, ectopia lentis, glaucoma, and retinal detachment. We report the case of a 14-year-old male patient with a variant of uncertain significance (VUS) in the FBN1 gene and a phenotype compatible with the Marfan spectrum. Clinical data were obtained from ophthalmic examination, ocular imaging, echocardiographic findings, and

Clinical and molecular genetic description of a Marfan-like syndrome in a Golden Retriever puppy.

BACKGROUND: Marfan syndrome (MFS) is a genetic fibrillin disorder causing cardiac, ocular, and musculoskeletal pathology; it is not well reported on in dogs, with minimal descriptions of antemortem diagnosis. HYPOTHESIS/OBJECTIVES: Use whole-genome sequencing (WGS) to diagnose a dog with suspected MFS to guide prognostication and potential treatment. ANIMALS: A client-owned, 10-month-old Golden Retriever presented with signs of possible connective tissue disorder. Blood and DNA samples from a non-affected German Shepherd-Beagle mix and 160 unrelated Golden Retrievers were used as controls.

Monolithic tantalum pentoxide athermal microrings with intrinsic Q factors exceeding 5 × 106 without SiO2 cladding.

Tantalum pentoxide (Ta2O5), as a silicon-photonic-compatible material platform, has garnered significant attention for high-performance wafer-scale integrated photonics due to its exceptional properties: a broad transparency window spanning from 0.28 µm to 8 µm, a moderate refractive index of 2.05 at 1550 nm, an impressive nonlinear refractive index of 1.0 × 10-14 cm2/W, and thick-film deposition with low residual stress at room-temperature. Despite these advantages, achieving low-loss fabrication of monolithic microrings on the Ta2O5 platform remains challenging due to its inherent hardness a

A Pathology-Responsive Hydrogel with On-Demand Ion Release for Microenvironment Remodeling and Diabetic Wound Regeneration.

The healing of diabetic wounds is often impeded by severe bacterial infections, persistent inflammation, excessive accumulation of reactive oxygen species (ROS), and abnormal matrix degradation. Although conventional free silver nanoparticles (AgNPs) possess broad-spectrum antimicrobial activity, their inherent tendency to aggregate and inactivate, as well as its high cytotoxicity limits their clinical application. Moreover, a single bactericidal strategy is insufficient to completely reverse the complex pathogenic microenvironment of the wound. To address these challenges, this study develope

Delayed diagnostic imaging but stable treatment initiation for kidney cancer during the COVID-19 pandemic: a Hungarian cohort study.

BACKGROUND/OBJECTIVES: The COVID-19 pandemic influenced cancer care worldwide, delaying diagnosis and treatment. We compared waiting times - time to initial diagnostic imaging, time to treatment, time to histopathological diagnosis, and length of hospital stay - of kidney cancer patients between pre-COVID-19 and COVID-19 periods at a large regional Hungarian clinical center. We also aimed to identify factors predicting prolonged waiting times. METHODS: Data from 400 adult kidney cancer patients (all histologically renal cell carcinoma) at the University of Pécs Urology Clinic were analyzed re

Growth Trajectories in Ascending Thoracic Aortic Dilation: Classification and Implications for the Effectiveness of Real-World Imaging Surveillance.

BACKGROUND: Ascending aortic dilation is monitored with serial imaging, yet event rates are low, with type A dissections often occurring at nonsurgical sizes. We aimed to characterize ascending aortic growth trajectories in a real-world population to understand their clinical consequences and better inform surveillance strategies. METHODS: We conducted a retrospective, single-center study of adults with ≥2 thoracic computed tomography angiography/magnetic resonance angiography examinations. Midascending diameters from clinical reports were used to analyze real-world surveillance effectiveness

Type B Aortic Dissection in Patients Aged 30 Years or Younger: A Retrospective Multicentre Study.

OBJECTIVES: Type B aortic dissection (TBAD) is rare in patients aged ≤30 years, and data on this population remain limited. This study aimed to characterize the underlying aetiology, and outcomes of invasively treated young patients with TBAD, with a focus on the impact of heritable thoracic aortic disease (HTAD). METHODS: A retrospective multicentre analysis was conducted across 19 international aortic centres, including 139 patients aged ≤30 years with TBAD. Patients with chronic TBAD (n = 42) were excluded for the final analysis. The cohort was classified as uncomplicated (n = 86) or compl

Myeloperoxidase aggravates thoracic aortic aneurysm formation in Marfan disease.

AIMS: Patients suffering from Marfan syndrome (MFS), the most prevalent inherited connective tissue disorder, face premature mortality due to dissection and rupture of thoracic aortic aneurysms. Here, we questioned whether myeloperoxidase (MPO), a leucocyte-derived enzyme with high affinity to the vessel wall, contributes to aortic remodelling in MFS. METHODS AND RESULTS: Plasma MPO levels were evaluated in MFS patients and healthy controls. Thoracic aortic aneurysm formation was determined in heterozygous transgenic Fbn1C1041G/+ (MFS) mice, MPO-deficient MFS mice (MFSxMpo-/-), and MFS mice t

Marfan Syndrome Associated With Intellectual Disability and Behavioral Anomalies: Further Evidence for the Effect of Compound Heterozygous Variants in FBN1 on Phenotypic Severity.

Marfan syndrome (MFS) is a rare connective tissue disorder characterized by involvement of the cardiovascular, ocular, and musculoskeletal systems. Pathogenic variants in FBN1 cause most of the MFS cases; however, intellectual disability (ID) is rarely observed. A non-consanguineous Pakistani family with four affected individuals was recruited. Physical examinations, echocardiography, and doppler ultrasound were performed as part of the clinical assessment. Exome sequencing was conducted on the index patient, and Sanger sequencing was performed for the entire family. ID was the primary symptom

Pulmonary Vascular Abnormalities and Spontaneous Pneumothorax in Loeys-Dietz Syndrome.

Loeys-Dietz syndrome (LDS) is a rare autosomal dominant disorder whose clinical phenotype overlaps with that of Marfan syndrome (MFS) and vascular Ehlers-Danlos syndrome (vEDS), including aortic and/or arterial aneurysms, skeletal abnormalities, and spontaneous pneumothorax. Although the pathological features of MFS- and vEDS-associated pulmonary lesions have been described, the pulmonary pathology of LDS remains virtually unknown. Herein, we report the detailed histopathological features of LDS-associated pulmonary vascular abnormalities and pneumothorax. A 32-year-old Japanese woman underwen

Type A aortic dissection during pregnancy and postpartum: Experience in 60 patients over 25 years.

OBJECTIVE: We seek to evaluate our experience with the management of type A aortic dissection (TAAD) during pregnancy or in the postpartum period over 25 years. METHODS: From 1998 to 2023, our team managed 60 pregnant women (mean age, 31.4 ± 5.0 years) who sustained TAAD at mean of 30.3 ± 8.5 gestational weeks (GWs), including 27 in the third trimester (45.0%) and 13 during the postpartum period (21.7%). Marfan syndrome (MFS) was seen in 39 patients (65.0%) with 13 (21.7%) having a positive family history. Before operation, aortic root diameter averaged 53.6 ± 17.1 mm, which was significantly

Pharmacotherapy Risks in Rare Genetic Diseases: Cross-Referencing ACMG Secondary Findings v3.2 List With Clinical Databases.

Clinical genomics and pharmacogenomics have largely remained separate fields, though some genetic variants have overlapping disease risk and drug implications. However, the extent of this overlap is not well studied. To explore this gap, we cross-referenced genes from the American College of Medical Genetics Secondary Findings v3.2 list with genomic databases and drug labeling to identify gene-phenotype pairs with overlapping clinical genomics and pharmacogenomic implications. We searched GeneReviews and PharmGKB (now called ClinPGx) for each gene-phenotype pair and reviewed the FDALabel datab

FGF12 induces aberrant mechanosignaling in aortic smooth muscle cells during thoracic aortic aneurysm formation in Marfan syndrome mice.

Marfan syndrome (MFS), caused by mutations in the FBN1 gene, predisposes individuals to thoracic aortic aneurysm (TAA), a life-threatening complication. Recent studies have suggested that dysregulated mechanosignaling in aortic smooth muscle cells (SMCs) plays a pivotal role in TAA pathogenesis in MFS. However, the key molecular drivers remain largely undefined. Here we identify fibroblast growth factor 12 (FGF12) as a novel mediator of aberrant mechanosignaling in aortic SMCs during TAA formation in MFS. FGF12 is markedly upregulated in aortic SMCs of thoracic aneurysmal aortas from Fbn1C1039

Thoracic Endovascular Aortic Repair for Type B Aortic Dissections in Patients With Marfan Syndrome.

BACKGROUND: Marfan syndrome is associated with progressive aortic pathology, often necessitating complex surgical interventions. Thoracic endovascular aortic repair (TEVAR) has emerged as a palliative and adjunct therapy for managing type B aortic dissections, though its long-term durability in heritable thoracic aortic disease remains uncertain. CASE SERIES: We report 7 patients with Marfan syndrome undergoing TEVAR for type B aortic dissections. TEVAR in these patients restored true lumen flow, stabilized the aorta, and facilitated staged or hybrid repair strategies. All procedures were gui
