Stanford Type B Aortic Dissection in a Twin Pregnancy With Marfan Syndrome: A Rare Obstetric Emergency-A Case Report.

The physiologic changes of pregnancy make pregnant women with Marfan syndrome (MFS) more vulnerable to complications of aortic disease. Significant hemodynamic changes in the third trimester and postpartum may explain the increased susceptibility to aortic complications during this time frame. The rate of cardiovascular complications in MFS is 2%-6%. We presented a case of Type B aortic dissection in a pregnant patient with MFS in the third trimester and normal evaluation at the initial prenatal assessment. This case report highlights the importance of comprehensive evaluation during pregnancy

Divergent Paths: A Survey of Cardiologist and Obstetrician Decision-Making in High-Risk Pregnancies with Cardiovascular Disease.

BACKGROUND: During pregnancy, there is a physiologic increase in heart rate and blood flow rate along with an increase in plasma volume. Although these changes are physiological, pregnant women with congenital cyanotic heart disease are known to have an increased risk of maternal heart failure, premature birth, intrauterine growth retardation, increased number of cesarean sections, and even maternal death. According to the World Health Organization, cardiovascular diseases (CVD) are the most common cause of indirect maternal deaths. The aim of this study was to assess how well obstetricians an

Impact of β-Blockers on the Risk of Low-Birth-Weight Infants in Women with Long QT Syndrome or Marfan Syndrome: A Single-Center Retrospective Study from 2008 to 2022 in a Tertiary Care Center.

INTRODUCTION: β-Blockers are an essential treatment for cardiovascular disease, the incidence of which is rising among women of childbearing age. Several large cohort and registry studies have reported an association between β-blocker exposure in pregnancy and reduced birth weight, but most have included women with hypertension or structural heart disease. Our study's primary objective was to evaluate the impact of these medications on the birth weight of infants born to mothers with morphologically normal hearts, including women with long QT syndrome (LQTS) or Marfan syndrome (MFS). Secondary

An unusual segmental retinal venous abnormality detected during retinopathy of prematurity screening in an infant with a PAX2 variant.

PURPOSE: Congenital abnormalities of retinal venous development are rare and may be difficult to distinguish from prematurity-related vascular changes when detected during retinopathy of prematurity screening. In premature infants, atypical posterior pole vascular findings should not be automatically attributed to retinopathy of prematurity alone. The purpose of this report is to describe an unusual segmental retinal venous abnormality detected during retinopathy of prematurity screening in a premature infant with a PAX2 variant and renal dysplasia. METHODS: We report a premature male infant

Two Cases of Successful Kidney Transplantation From a 39-Year-Old Male Deceased Donor With Marfan Syndrome: A Case Series.

BACKGROUND Marfan syndrome is a multisystemic, autosomal dominant connective tissue disorder caused by mutations in the fibrillin-1 gene. It is associated with life-threatening complications, especially aortic aneurysms and dissections, and has a global prevalence of 1 in 3000-5000 individuals. Although kidney transplantation is the standard treatment for end-stage renal disease (ESRD), deceased kidney donors with Marfan syndrome remain uncommon due to concerns about connective tissue fragility. Emerging reports, however, indicate that kidneys from such donors may be safely used. This report d

Differentiating the Clinical and Variant Spectrum of Hardikar Syndrome From Other MED12 -Related Developmental Disorders.

The rare X-linked female-restricted Hardikar syndrome (HDKR, OMIM # 301068) is characterized by multiple congenital anomalies including orofacial clefts, gastrointestinal, genitourinary, and cardiac anomalies, but cognitive and neurobehavioral development is rarely impaired. HDKR is caused by heterozygous frameshift, splice or nonsense variants in the MED12 gene. Besides HDKR, MED12 pathogenic variants cause a broad spectrum of developmental disorders, collectively referred to as MED12-related disorders, including Opitz-Kaveggia syndrome or FG syndrome type 1 (OKS, OMIM #305450), Lujan-Fryns s

Refinement of Connective Tissue Disorder Diagnosis From Marfan to Loeys-Dietz Syndrome Type 4-End of a 30-Year Diagnostic Odyssey.

Characterization of the molecular basis of connective tissue disorders causing aortopathy is important for clinical management. We report a 4-generation family with autosomal dominant inheritance of a connective tissue disorder causing aortic dilatation and/or dissection. Linkage analysis performed in the 1990s had suggested Marfan syndrome, but the molecular basis proved elusive over two decades. Reanalysis of genome sequence data using software to detect copy number variation (CNV) identified a 19 kb deletion including the final exon of TGFB2, revealing Loeys-Dietz syndrome type 4 as the act

Diagnosis in children with Marfan syndrome: red flags for early identification.

BACKGROUND: Marfan syndrome (MFS) is a multisystemic disorder caused by pathogenic (P) variants in the fibrillin-1 gene (FBN1), associated with life-threatening cardiovascular complications. Early diagnosis enables preventive interventions, yet recognition in children can be challenging. OBJECTIVE: To identify common diagnostic traits in children with MFS that may assist general paediatricians in the recognition of children with MFS and lead to early detection of the disease. STUDY DESIGN: This retrospective study included 129 children who underwent genetic testing due to a suspicion of MFS.

Arch-First Technique for Extensive Thoracic Aortic Replacement: A Case Series of Patients With Complex Thoracic Aortic Disease.

BACKGROUND Given its associations with high morbidity and mortality, type A aortic dissection requires urgent surgical intervention. Patients with Marfan syndrome present additional complexity due to underlying connective tissue abnormalities that warrant multiple thoracic aortic operations. The "arch-first" technique was developed to allow single-stage open extensive replacement of the thoracic aorta while maintaining continuous antegrade cerebral perfusion, thereby minimizing brain ischemia. In the current endovascular era, the frozen elephant trunk - a hybrid technique combining a conventio

Impact of Severe Uncorrected Pectus Excavatum on Outcomes After Aortic Surgery in Marfan Syndrome.

BACKGROUND: The prognostic impact of uncorrected severe pectus excavatum (PE) in patients with Marfan syndrome undergoing aortic surgery remains unclear. This study evaluated whether severe PE influences clinical outcomes after aortic surgery. METHODS: We retrospectively analyzed 253 patients with Marfan syndrome who underwent aortic surgery between 2005 and 2020. Patients were stratified into a nonsevere PE group (n = 191; including no PE and nonsevere PE) and a severe PE group (n = 62; Haller index >3.5). The primary end point was 10-year mortality. Secondary outcomes included in-hospital m

Aortic displacement and hemodynamics are abnormal in patients with Marfan syndrome: A combined four-dimensional balanced steady-state free precession and four-dimensional flow cardiac magnetic resonance study.

BACKGROUND: Marfan syndrome (MFS) patients with a history of aortic root surgery (ARS) are at increased risk of type B aortic dissection, possibly because the noncompliant graft fails to absorb systolic forces, leading to undampened flow in the proximal descending aorta (pDAo). In this study, we investigated the magnitude and location of abnormal aortic displacement and wall shear stress (WSS) using cardiac magnetic resonance (CMR) imaging. METHODS: We examined 82 MFS patients (32 with ARS, 34 ± 8 years, 36 women) and 45 age- and sex-matched controls, all undergoing four-dimensional high-reso
