Cardiac POCUS in Undifferentiated Shock Revealing Aortic Dissection.

Acute aortic dissection is a medical emergency requiring prompt diagnosis and intervention. Mortality in untreated Stanford type A dissection increases approximately 2.6% per hour during the first 24 h and may reach 47%-55% within 24-48 h. In critically ill patients, point-of-care ultrasound has emerged as a valuable bedside tool, particularly when conventional imaging is delayed or contraindicated. We describe the case of a 43-year-old man with Marfan syndrome, prior ascending aortic replacement with a Dacron graft (without aortic valve replacement), intellectual disability, psychiatric disor

Cancer Risk in Marfan Syndrome: A Swedish Population-Based Cohort Study.

Marfan syndrome is an autosomal dominant connective tissue disorder caused by pathogenic variants in the fibrillin-1-encoding gene. The cancer risk in Marfan syndrome is not fully understood, but recent reports have suggested an increased risk in adults. This study assessed cancer risk in Marfan syndrome using population-based data from the Swedish National registers. We performed a register-based nationwide matched cohort study of 1544 individuals with Marfan syndrome, born 1930-2017. Each individual was matched to 50 comparisons by birth year, sex and birth county. Cancer risk was estimated

The aorta in Marfan syndrome: from molecular mechanisms to mechanobiological dysfunction.

Marfan syndrome (MFS) is an autosomal dominant connective tissue disorder that affects cardiovascular, musculoskeletal, and ocular tissues, with premature death often resulting from dissection of the thoracic aorta. MFS results from pathogenic variants in FBN1, which encodes fibrillin-1, a glycoprotein that promotes elastic fibre organization and stability and contributes to smooth muscle cell mechano-sensing of extracellular matrix. It has been 35 years since the discovery that FBN1 variants cause MFS, yet understanding links between a variant and thoracic aortic disease remains incomplete, a

Perivascular adipose tissue modulates aortic reactivity in Marfan syndrome mice in a sex- and region-specific manner: Redox-dependent anticontractile effects in the female ascending aorta.

Marfan syndrome (MFS), caused by fibrillin-1 (FBN1) mutations, leads to thoracic aortic aneurysm formation through mechanisms in which redox imbalance plays an important role. Perivascular adipose tissue (PVAT) modulates vascular tone, but its role in aortic reactivity in MFS is unknown. To address this, we examined thoracic aortic reactivity using wire myography in ascending and descending aortic segments from 2 to 3-month-old male and female wild-type and Fbn1C1041G/+ mice with or without PVAT. PVAT-mediated modulation of α1-adrenergic phenylephrine-induced contractions was preserved in the

Cardiovascular and Clinical Manifestations of Marfan Syndrome and Other Inherited Connective Tissue Disorders with Coexisting Genetic Variants.

Marfan syndrome (MS), Loeys-Dietz syndrome (LDS), Beals-Hecht syndrome (BHS), Ehlers-Danlos syndrome (EDS), and individuals with undifferentiated connective tissue disease (UCTD) exhibit phenotypic overlap, suggesting a likelihood of genotypic coexistence. Our objective was to evaluate genetic variants (GVs), encoding 174 genes related to aortopathies, cardiomyopathies, arrhythmias, structural heart disease, and hypercholesterolemia, and their relationship to clinical and cardiovascular damage in these syndromes. This was a prospective study in Mexican patients with MS, LDS, EDS, BHS, and UCTD

A case report and literature review of pediatric multifocal melorheostosis in a unilateral limb.

INTRODUCTION AND IMPORTANCE: Melorheostosis (Léri-Joanny syndrome) is a rare, nonhereditary, congenital sclerotic bone dysplasia with an estimated prevalence of 0.9 per million. Its enigmatic etiology and distinctive "melting wax" radiological appearance warrant detailed case reporting to enhance understanding of diagnosis and management, particularly in pediatric populations. CASE PRESENTATION: A 9-year-old male presented with incidental right upper limb bone abnormalities discovered post-trauma. Asymptomatic without pain/swelling, he had no relevant family history. Laboratory tests (CBC, CR

Aortic asprosin overexpression does not ameliorate disease pathophysiology in a murine model of Marfan syndrome.

Marfan syndrome (MFS) is a connective tissue disorder caused by FBN1 mutations, characterised by aortic aneurysms leading to life-threatening dissections. Asprosin is the C-terminal propeptide of fibrillin-1 that was shown to function as a glucogenic hormone with critical implications in vascular pathology. However, its role in MFS remains unknown. Here, we investigate the effect of asprosin overexpression in vascular smooth muscle cells (VSMCs) and in the Marfan mouse model mgR/mgR via endothelial-targeted transduction by adeno-associated viruses (AAVs). In both human and murine VSMCs, aspros

The Efficacy of Antihypertensive Drugs and miR-632 Inhibition on Parietal Remodeling in a Model of Marfan Thoracic Aortic Aneurysm.

BACKGROUND: Marfan syndrome (MFS) is a connective tissue disorder caused by FBN1 mutations, leading to elastic fiber disarray and early thoracic aortic aneurysm (TAA) formation. Currently, pharmacological treatments lack specificity and only delay progression. We previously reported a specific TGFβ-driven miR-632 upregulation in MFS TAA tissues and blood causing smooth muscle cell dedifferentiation and aortic wall degeneration. This study evaluated the effects of three conventional antihypertensive drugs (β-blocker, ACE inhibitor, and sartan) on parietal remodeling, comparing them with a miR-6

Stachydrine alleviates thoracic aortic aneurysm via regulating lactate dehydrogenase A in Marfan syndrome mice.

BACKGROUND: Marfan syndrome (MFS) is a genetic disorder predominantly caused by mutations in the FBN1 gene, leading to thoracic aortic aneurysm (TAA) and a lack of effective pharmacological treatments. However, the role of metabolic alterations in this process remains poorly understood. PURPOSE: We aim to investigate the regulatory roles of metabolites in MFS. METHODS: Fbn1C1039G/+ mice were utilized as a TAA mouse model of MFS. Untargeted metabolomic screening of aortic tissues was conducted to identify metabolites in Fbn1C1039G/+ mice compared to controls. Echocardiographic and EVG stainin

Neuraxial anesthesia in obstetric patients with rare spinal pathologies: a scoping review.

BACKGROUND: Neuraxial anesthesia remains the primary approach for obstetric patients, emphasizing the need for a successful blockade. However, many anesthetists view spinal pathologies as contraindications for regional anesthesia. The existing literature on the use of neuraxial anesthesia in obstetric patients with rare spinal pathologies is largely limited to case reports and case series. The goal of this investigation is to evaluate the success rates and complications associated with neuraxial anesthesia in these obstetric patients. METHODS: We searched the databases: PubMed, Embase, Scopus

Could the Phenotypic Outcomes of Genetic Variability in Cells Operating in Mechanically Dynamic Environments be Influenced by a Disrupted "Cell-ECM" Relationship? Using Cystic Fibrosis and Marfan Syndrome as an Example.

Diseases due to mutations in essential molecules can involve tissues functioning in very different environments, with some in mechanically active environments. Diseases arising from mutations in a single molecule, such as the CFTR in cystic fibrosis exhibit varied clinical phenotypes. The lung cells expressing mutations in CFTR are functioning in the mechanically active environment of the lung, but these mutations may also play an adverse role in the cardiovascular system. Similarly, Marfan syndrome arises from mutations in an extracellular matrix (ECM) molecule, fibrillin-1 and this molecule

Case Report: Diagnostic odyssey in rare diseases: when genetic variants are misinterpreted.

INTRODUCTION: The diagnostic odyssey in rare diseases often involves the misinterpretation of genetic data, particularly when multidisciplinary approaches are lacking. This study illustrates the critical process of interpreting variants from next-generation sequencing (NGS) through a real-life case of a child misdiagnosed with Marfan Syndrome (MFS). The misdiagnosis was maintained for over seven years despite repeated clinical evaluations by different specialists. An initial clinical suspicion of MFS due to joint hypermobility at 3 years of age became a definitive diagnosis after an external l

Genetic Bone Diseases: A Scoping Review of Pathology, Symptoms, Diagnosis, Treatment, and New Horizons.

Genetic bone diseases are a rare group of afflictions suffered by the general population. However, their rarity should not diminish research efforts to help patients understand and treat their diseases. This review summarizes the pathology, symptoms, diagnosis, and treatment insight into six well-known genetic bone diseases. Only six bone diseases are included due to the relatively low prevalence of them as whole limiting our scope to ensure accurate information and attention is provided for each disease individually. A literature search of PubMed is conducted, including studies published with

Pediatric Extremity Vascular Malformations: Diagnosis, Referral, and Limb Management from a Pediatric Orthopedic Perspective.

Extremity vascular malformations in children and adolescents are congenital vascular developmental abnormalities that often present to pediatric orthopedic surgeons with pain, swelling, restricted motion, contracture, gait disturbance, limb asymmetry, and growth-related deformity rather than with an obvious vascular phenotype. The orthopedic importance of these lesions lies less in surface appearance than in their potential to affect muscle balance, joint integrity, osseous development, and peri-procedural safety. This review translates contemporary vascular anomaly classification and multidis

HSPB7 Deficiency Drives Aortic Aneurysm Progression in Marfan Syndrome Through Vascular Smooth Muscle Cell Phenotypic Switching.

Marfan syndrome (Mfs) arises from mutations in FBN1, predisposing individuals to thoracic aortic aneurysm (TAA) through abnormal smooth muscle cell behaviour. The role of HSPB7 in Mfs-related TAA remains poorly understood. By integrating multi-omics analysis with hiPSC-derived VSMCs from patients with Mfs, we elucidated the molecular landscape of Mfs-associated TAA. Analysis revealed immune cell infiltration, a reduced proportion of VSMCs, and altered intercellular communication. Pathway analyses indicated changes in cell adhesion, extracellular matrix (ECM) remodelling, and immune signalling,

Nuclear SUN2 coordinates endothelial cell-matrix interactions to regulate blood vessel homeostasis and barrier function.

Vascular endothelial cells respond to environmental forces to remodel vessels during development and to achieve homeostasis, and mis-regulated responses lead to vascular dysfunction and disease. The nucleus participates in force transduction to cell-matrix junctions via the Linker of Nucleoskeleton and Cytoskeleton (LINC) complex that resides in the nuclear envelope, but how these forces are regulated and relayed is incompletely understood. We found that the LINC complex protein SUN2 is required for proper endothelial cell-matrix interactions that occur far from the nucleus and affect angiogen

Lymphatic therapies open the valve in Marfan syndrome.

Myxomatous degeneration of the mitral valve (MDMV) is a common cardiovascular manifestation of Marfan syndrome (MFS), yet the role of lymphatic vessels in the disease progression remains unknown. In this Commentary, we discuss the study by Tan, Kume, and colleagues, which identifies defective lymphangiogenesis as a previously unrecognized driver of MDMV. Their work demonstrates that impaired lymphatic development and drainage promote valve inflammation through reduced ZFP36-mediated antiinflammatory signaling, whereas restoration of lymphatic function or pharmacological activation of ZFP36 wit

Long-term pulmonary function outcomes in early onset scoliosis: A critical literature review.

Early-onset scoliosis (EOS) presents a unique clinical challenge due to its occurrence during critical phases of thoracic and pulmonary development. While growth-sparing strategies aim to preserve thoracic height and facilitate lung growth, their impact on long-term pulmonary function remains uncertain. This review synthesizes current evidence on long-term pulmonary outcomes in EOS, with emphasis on treatment modality, etiology, and prognostic factors. A structured narrative review was conducted using a systematic search of PubMed/MEDLINE, Scopus, and Embase, identifying 194 studies, of which
