The association between collagenopathies and the risk of spontaneous preterm birth: A systematic review with selective meta-analysis.

INTRODUCTION: Spontaneous preterm birth (sPTB) remains a leading cause of neonatal morbidity and mortality. Inherited collagenopathies, including Ehlers-Danlos syndrome (EDS) and Marfan syndrome (MFS), may predispose to sPTB through impaired connective tissue integrity; however, the available evidence has not been systematically synthesized according to collagenopathy subtype. We aimed to evaluate the association between inherited collagenopathies and sPTB through a systematic review, undertaking quantitative synthesis only where clinically appropriate and methodologically comparable. Marfan Syndrome versus Loeys-Dietz Syndrome after Aortic Surgery: A Systematic Review and Meta-analysis of Reconstructed Time-to-Event Data.

OBJECTIVE: Marfan syndrome (MFS) and Loeys-Dietz syndrome (LDS) are hereditary connective tissue diseases, both characterized by aortic root and vessel involvement with an increased risk of aneurysms and dissections. Despite advances in surgical techniques and improved postoperative follow-up, the impact of surgery on long-term outcomes in these patient groups is still unknown. METHODS: We performed a systematic review and a meta-analysis to compare the postoperative long-term events. The main outcome was long-term survival. Additional outcomes were freedom from dissection and reoperation. Af

Computational multiscale analysis of growth and remodeling in the ascending aorta of the Marfan mouse.

Marfan syndrome, caused primarily by mutations in the fibrillin-1 (FBN1) gene, is associated with progressive dilation of the proximal aorta, with dissection and rupture representing the leading causes of early mortality. A characteristic feature of Marfan-associated aneurysm is fragmentation of elastic fibers; however, the extent to which elastin loss alone can account for the observed geometric and mechanical changes remains unclear. In this study, we use a multiscale computational growth and remodeling framework to determine which geometric and mechanical features of the Marfan aorta can be

Early onset scoliosis: A comprehensive review of aetiology, diagnosis and classification.

Early-onset scoliosis (EOS) encompasses spinal deformities that develop before ten years of age, occurring during the critical period of thoracic and pulmonary growth. Beyond spinal curvature, EOS represents a developmental disorder in which structural imbalance compromises thoracic expansion and lung maturation, predisposing to thoracic insufficiency syndrome (TIS). The historical evolution of the term-from infantile and juvenile scoliosis to the comprehensive framework of EOS-reflects a shift from age-based categorization to one integrating growth dynamics and physiological impact. The Class

Loss of function variants in ADAMTS6: a new connective tissue disorder with heart defect, aortic aneurysm and neurodevelopmental features.

PURPOSE: Marfan syndrome, Loeys-Dietz syndrome, and heritable thoracic aortic aneurysms and dissections (hTAAD) are autosomal dominant connective-tissue disorders with overlapping features and considerable genetic variability. Many cases arise from pathogenic variants affecting extracellular matrix (ECM) components or TGFβ signaling, yet a substantial proportion of hTAAD cases remain genetically unexplained. This study aimed to identify new genetic contributors to aortic disease and to clarify their molecular and clinical impact. METHODS: Exome and genome sequencing of a French cohort with sy

Selective Endovascular Aortic Repair in Connective Tissue Disease Patients Within a Multidisciplinary Program Achieves Comparable Outcomes in High-Risk Patients.

INTRODUCTION: Endovascular aortic repair (ENDO) in connective tissue disease (CTD) patients has historically been discouraged in societal guidelines because of durability concerns. However, high-volume aortic centers increasingly employ ENDO strategies in carefully selected high-risk scenarios such as rupture, dissection-related pathology, or hostile anatomy. Herein, we describe temporal ENDO adoption at our multidisciplinary CTD aortic program. METHODS: Ninety-two CTD patients undergoing aortic intervention at a single high-volume aortic center between 2005 and 2023 were analyzed. Patients w

Ventricular Arrhythmias in Marfan Syndrome: Prevalence and Clinical Correlates in a Multicenter Cohort.

BACKGROUND: Ventricular arrhythmias are increasingly recognized in Marfan syndrome, but associated clinical and structural features remain incompletely defined. METHODS: We performed a retrospective multicenter cohort study of 783 adults with confirmed Marfan syndrome at 3 Mayo Clinic sites. Clinical, arrhythmic, echocardiographic, cardiac magnetic resonance, surgical, and implantable cardioverter defibrillator data were obtained by manual chart review. Associations with sustained ventricular tachycardia (VT) or ventricular fibrillation (VF) were evaluated using univariable logistic regressio

Allopurinol for Marfan Aortopathy: Pharmacogenomic Safety and Sex-Stratified Trial Design Warrant Consideration.

A recent review by Egea et al. presents a compelling case for adding allopurinol to standard beta-blocker/ARB therapy in Marfan syndrome (MFS) aortopathy, drawing on consistent preclinical evidence and the drug's recent European Medicines Agency orphan-drug designation. This letter identifies two gaps that are genuinely absent from that review and could materially affect the safety and interpretability of any future allopurinol trial in MFS. First, the review discusses allopurinol hypersensitivity syndrome, toxic epidermal necrolysis, and Stevens-Johnson syndrome only as background toxicology,

FBN1 Variant of Uncertain Significance in a Patient With a Marfan Spectrum Phenotype: A Case Report.

Marfan syndrome (MFS) is an autosomal dominant multisystemic connective tissue disorder caused by pathogenic variants in the FBN1 gene and characterized by cardiovascular, ocular, and musculoskeletal manifestations. Ocular findings commonly include strabismus, refractive error, ectopia lentis, glaucoma, and retinal detachment. We report the case of a 14-year-old male patient with a variant of uncertain significance (VUS) in the FBN1 gene and a phenotype compatible with the Marfan spectrum. Clinical data were obtained from ophthalmic examination, ocular imaging, echocardiographic findings, and

Monolithic tantalum pentoxide athermal microrings with intrinsic Q factors exceeding 5 × 106 without SiO2 cladding.

Tantalum pentoxide (Ta2O5), as a silicon-photonic-compatible material platform, has garnered significant attention for high-performance wafer-scale integrated photonics due to its exceptional properties: a broad transparency window spanning from 0.28 µm to 8 µm, a moderate refractive index of 2.05 at 1550 nm, an impressive nonlinear refractive index of 1.0 × 10-14 cm2/W, and thick-film deposition with low residual stress at room-temperature. Despite these advantages, achieving low-loss fabrication of monolithic microrings on the Ta2O5 platform remains challenging due to its inherent hardness a

A Pathology-Responsive Hydrogel with On-Demand Ion Release for Microenvironment Remodeling and Diabetic Wound Regeneration.

The healing of diabetic wounds is often impeded by severe bacterial infections, persistent inflammation, excessive accumulation of reactive oxygen species (ROS), and abnormal matrix degradation. Although conventional free silver nanoparticles (AgNPs) possess broad-spectrum antimicrobial activity, their inherent tendency to aggregate and inactivate, as well as its high cytotoxicity limits their clinical application. Moreover, a single bactericidal strategy is insufficient to completely reverse the complex pathogenic microenvironment of the wound. To address these challenges, this study develope

Delayed diagnostic imaging but stable treatment initiation for kidney cancer during the COVID-19 pandemic: a Hungarian cohort study.

BACKGROUND/OBJECTIVES: The COVID-19 pandemic influenced cancer care worldwide, delaying diagnosis and treatment. We compared waiting times - time to initial diagnostic imaging, time to treatment, time to histopathological diagnosis, and length of hospital stay - of kidney cancer patients between pre-COVID-19 and COVID-19 periods at a large regional Hungarian clinical center. We also aimed to identify factors predicting prolonged waiting times. METHODS: Data from 400 adult kidney cancer patients (all histologically renal cell carcinoma) at the University of Pécs Urology Clinic were analyzed re

Myeloperoxidase aggravates thoracic aortic aneurysm formation in Marfan disease.

AIMS: Patients suffering from Marfan syndrome (MFS), the most prevalent inherited connective tissue disorder, face premature mortality due to dissection and rupture of thoracic aortic aneurysms. Here, we questioned whether myeloperoxidase (MPO), a leucocyte-derived enzyme with high affinity to the vessel wall, contributes to aortic remodelling in MFS. METHODS AND RESULTS: Plasma MPO levels were evaluated in MFS patients and healthy controls. Thoracic aortic aneurysm formation was determined in heterozygous transgenic Fbn1C1041G/+ (MFS) mice, MPO-deficient MFS mice (MFSxMpo-/-), and MFS mice t
