Management strategies for acute type A aortic dissection during pregnancy: a single-center retrospective experience.

BACKGROUND: Acute type A aortic dissection (ATAAD) is very rare during pregnancy, it is a devastating condition, and there is no established management guideline. This study shares our center’s observations and management experience to provide a practical reference for developing individualized clinical strategies. METHODS: A total of 12 pregnant patients with acute type A aortic dissection, confirmed by CTA and managed surgically, were included from Union Hospital of Fujian Medical University between January 2014 and September 2024. RESULTS: A total of 12 patients were included, with a mean a

Telomere length in patients with Marfan Syndrome.

BACKGROUND: Marfan syndrome (MFS) is a multisystemic heritable thoracic aortic disease entity characterized by progressive aortic dilatation and life-threatening cardiovascular complications. Chronic inflammation and oxidative stress are increasingly recognized in its pathophysiology, and are important drivers of telomere shortening, a hallmark of biological aging. We hypothesized that adults with MFS have shorter telomere length (TL) compared to healthy controls. METHODS: Relative average leukocyte TL was measured in 59 adults with molecularly confirmed MFS (median age 38 years, 29 females)

Downregulation of S100a8 mitigates fibrosis in skeletal muscles affected by Marfan syndrome.

This study investigated the potential of eccentric training in attenuating the deleterious effects of Marfan syndrome on skeletal muscles. Marfan syndrome and wild-type mice were divided into a trained and a control group; the former performed a downhill running protocol for 8 weeks. Morphological, molecular, and functional analyses were performed. Based on RNA sequencing analyses, the gastrocnemius and plantaris muscles were transfected with S100a8-shRNA (short hairpin RNA) and analyzed after 21 days. The muscle wet weight normalized to body weight and the percentage of myofibers with a highe

Severe elastic fiber fragmentation/loss is correlated with Marfan syndrome in patients with an ascending aortic aneurysm.

CONTEXT AND OBJECTIVES: Aortic wall degeneration was described by a consensus statement published in 2016 by the Society for Cardiovascular Pathology (SCP) and Association for European Cardiovascular Pathology (AECP). The aim of this study was to determine if histopathologic features could predict the presence of genetic syndrome, to assess the prognostic value of histological findings, and to correlate it with clinical data. METHODS: This study included 85 patients operated in the CHRU of Nancy. When available (n=80), all histological sections were reviewed and scored following the SCP/AECP

Endovascular Salvage After False Lumen Misdeployment of Thoracic Endovascular Aortic Repair or the Frozen Elephant Trunk in Aortic Dissection: A Case Report of Two Patients.

Thoracic endovascular aortic repair (TEVAR) and the frozen elephant trunk (FET) are well-established treatment options for aortic dissection (AD). Inadvertent misdeployment of a TEVAR stent graft or FET into the false lumen (FL) can lead to serious complications. We report two cases of FL misdeployment. First, a 68-year-old female with acute Stanford type B AD underwent TEVAR; follow-up computed tomography angiography (CTA) revealed stent-graft misdeployment into the FL. Second, a 44-year-old female with Marfan syndrome, who had undergone supracoronary ascending aortic grafting for type A AD a

The Current State of Endovascular Aortic Repair in Heritable Thoracic Aortic Disease.

Heritable thoracic aortic disease (HTAD) is associated with the development of aortic aneurysms or dissections. These patients often suffer their initial aortic event at a younger age, compared to those without HTADs. Although genetic aortopathy can affect any segment of the aorta, there are characteristic segments for specific HTADs. This population is at increased risk of aortic-related mortality, and early identification and screening of HTAD has been shown to positively impact long-term survival. The standard of aortic care in this population has historically involved open repair. However,

A case report by scientist parents of a paediatric patient diagnosed with Marfan syndrome at 24 weeks of pregnancy.

Marfan syndrome (MFS, OMIM #154700) is an autosomal dominant disorder that presents a challenging diagnosis due to its wide clinical variability. The prenatal diagnosis of the syndrome is rare and often associated with unfavourable prognoses, such as early mortality. This case report, written by scientist parents, aims to document the clinical journey, management, and development of a paediatric patient diagnosed with Marfan syndrome at 24 weeks of gestation. In the immediate postpartum period, the patient did not show any severe warning signs, such as tachypnoea or prostration. The initial fi

Impact of 18F-FDG-PET/CT in Managing a Case of Fungal Prosthetic Valve Endocarditis.

Approximately 0.1% of all prosthetic cardiac valves are affected by fungal endocarditis (bacterial endocarditis being the most common cause), which has a high case fatality rate. Post-treatment clinical improvement and a negative blood culture do not definitively rule out the presence of residual active disease. Among imaging techniques, trans-oesophageal echocardiography has higher sensitivity than transthoracic echocardiography, but has its own limitations. Functional imaging with 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) has shown to have high

Marfan Before Marfan? Iconodiagnostic Analysis of a Possible Marfan Spectrum Phenotype in an Etruscan Sculpture.

Marfan syndrome is a heritable connective tissue disorder characterized by multisystem involvement, particularly affecting the skeleton. We performed an iconodiagnostic evaluation of the Etruscan bronze statue "L'Ombra della Sera" to assess whether its morphological features may reflect a real connective tissue phenotype rather than symbolic elongation. Systemic features were evaluated according to the revised Ghent nosology. The statue exhibits disproportionate longitudinal growth, thoracic deformities, craniofacial characteristics, and pedal findings consistent with a systemic connective tis

PINCH proteins orchestrate vascular mural cell homeostasis through integrated signaling and transcriptional networks.

Vascular mural cells (VMCs) are crucial for vascular stability, and their dysfunction underlies cardiovascular pathologies including atherosclerosis and aortic aneurysms. PINCH proteins are core focal adhesion components mediating integrin signaling, yet their roles in VMC development remain elusive. Here, we generated mice with conditional deletion of both PINCH1 and PINCH2 in Pdgfrb-lineage VMCs, which resulted in perinatal lethality accompanied by severe arterial enlargement, hemorrhage and defective angiogenesis. Mutant VMCs exhibited profound defects in cytoskeletal organization, prolifer

Transcatheter closure of paravalvular leak after Bentall surgery in a Marfan patient: a rare case report.

Paravalvular leak (PVL) following Bentall surgery in patients with Marfan syndrome is exceedingly rare. A 38-year-old Marfan patient underwent Bentall + Sun's procedure for Stanford type A aortic dissection with severe aortic regurgitation. On the third month after surgery, the patient was readmitted due to exertional dyspnea. Echocardiography revealed a paravalvular leak with significant left-to-right shunting, leading to symptoms including exertional dyspnea, hepatomegaly, and heart failure. After adequate preparation, the leak was successfully closed using a symmetric VSD occluder. Post-pro

Novel Generation-Skipping Inheritance Pattern of Marfan Syndrome Due to FBN1 Insertional Translocation: Diagnostic Utility of FISH and Implications for Genetic Counseling.

Marfan syndrome (MFS) is an autosomal dominant connective tissue disorder caused by pathogenic variants in the fibrillin-1 (FBN1) gene on Chromosome 15q21.1. A 3-year-old female presented to the clinic with MFS and a family history of an affected maternal uncle and maternal great-aunt. The proband and the uncle had a positive thoracic aortic aneurysm and dissection (TAAD) panel for MFS revealing an FBN1 deletion. This was confirmed on proband's chromosome microarray; however, the mother was negative for the FBN1 deletion. Fluorescence in situ hybridization (FISH) was used in this case to show

SPP1high fibrogenic macrophages mediate protective fibrotic remodeling and promote vascular stability in hypertension-associated aortic dissection.

BACKGROUND: Hypertension-associated aortic dissection (HTN-AD) features intense immune activation and extracellular matrix disruption, yet the cellular programs that promote aortic-wall stabilization remain unclear. We aimed to identify macrophage states associated with vascular integrity and to determine whether these states could be mechanistically or therapeutically leveraged. METHODS: Single-cell RNA sequencing was performed on thoracic aortas from HTN-AD, Marfan syndrome-associated AD, and donor controls (n = 3 per group). Peripheral and tear-adjacent blood samples were analyzed in patie

Direct Carotid Artery Exposure for Mechanical Thrombectomy in a Patient with Marfan Syndrome and Prior Total Arch Replacement.

Marfan syndrome presents unique challenges for mechanical thrombectomy in acute ischemic stroke due to vascular fragility and complex aortic anatomy, often from prior surgeries. We describe the case of a man in his 50s with Marfan syndrome, total aortic arch replacement, mechanical valve replacement, and extensive chronic dissections, who presented with acute ischemic stroke due to right internal carotid artery occlusion. Conventional transfemoral access failed due to anomalous prosthetic brachiocephalic artery graft anatomy. Consequently, mechanical thrombectomy was performed via direct surgi

Kikuchi Disease With Marfanoid Features in a Background of Fever of Unknown Origin.

Kikuchi-Fujimoto disease (KFD) is a self‑limiting necrotizing lymphadenitis that usually affects young women. It can mimic infection, malignancy, or autoimmune disease, which often leads to diagnostic delays. Marfanoid habitus encompasses a constellation of physical traits associated with Marfan syndrome: tall stature, disproportionate limbs, arachnodactyly, joint hypermobility, and chest wall deformities. Co-existence of KFD and prominent marfanoid features has not previously been reported. Based on a structured PubMed/MEDLINE search from database inception through January 2026 using MeSH ter
