ACAN-related disorder, antenatal presentation and phenotypic variability: a case series.

BACKGROUND: Pathogenic ACAN variants ('aggrecanopathies') are increasingly recognised as a non-syndromic cause of skeletal dysplasias and short stature. Unlike many other aetiologies, ACAN-related disorder is reportedly associated with advanced bone age due to early epiphysial fusion, although presentation can vary. This study aimed to further characterise skeletal findings in paediatric and adult patients. METHODS: A retrospective study of confirmed ACAN-related disorder cases from a tertiary genetics centre in England was performed. Data collected included patient demographics, antenatal ul

Omega-3 fatty acid supplementation improves skeletal muscle mitochondrial function in a model of Barth syndrome.

The composition of mitochondrial membrane lipids is crucial to cellular respiration, as seen in Barth syndrome (BTHS), a rare disease affecting skeletal muscle, heart, and neutrophils. In BTHS, mutations in the tafazzin (TAZ) gene reduce remodeling of the mitochondrial phospholipid cardiolipin, causing mitochondrial dysfunction in skeletal muscle and heart. Here, we investigated effects of altering polyunsaturated fatty acid content in cardiolipin using preclinical models of BTHS. In vitro, the absence of TAZ did not impair omega-3 fatty acid incorporation into cardiolipin and resulted in incr

Clinical and genetic findings in 103 individuals in Norway with basal cell naevus syndrome.

BACKGROUND: Basal cell naevus syndrome (BCNS) is a rare autosomal dominant condition characterized by early onset and multiple basal cell carcinomas (BCCs), skeletal and ocular abnormalities, and extracutaneous tumours and cysts. In most cases, BCNS is caused by a pathogenic variant in the tumour suppressor gene PTCH1. Pathogenic variants in SUFU account for a minority of cases. No clear genotype-phenotype associations between different PTCH1 variants have been established. OBJECTIVES: To identify all individuals with BCNS in Norway, characterize their clinical and genetic profiles, and inves

Imaging findings in pediatric interferonopathies.

Type I interferon (IFN-I) is a proinflammatory cytokine that activates signalling pathways crucial for antiviral defence in innate immunity. The IFN-I system is a self-preserving mechanism that normally responds to exogenous nucleic acids derived from pathogens, but not to endogenous nucleic acids from host cells. However, genetic defects that constitutively activate this pathway can lead to aberrant IFN-I production (the "IFN signature") and sterile inflammation (autoinflammation), giving rise to a group of monogenic disorders collectively termed "autoinflammatory interferonopathies." This em

[Microcephalic osteodysplastic primordial dwarfism type II (MOPD II): clinical case].

Small for gestational age (SGA) refers to the size of an infant at birth, and is defined as a birth weight and/or birth length below the -2.0 SDS for the gestational age. In approximately 10% of cases, SGA is not compensated for in the postnatal period, with the pathogenesis of this condition being attributed to various monogenic syndromes or chromosomal abnormalities. The difficulty in making a pathogenetic diagnosis in this group of patients is due, on the one hand, to the similarity of phenotypic manifestations in the structure of the disease, on the other hand, to the variability of clinic

Retinal astrocytoma and Jeune syndrome relationship from ciliopathy perspective: a case report.

BACKGROUND: Jeune syndrome is an autosomal recessive chondrodysplasia characterized by skeletal deformities and extra-skeletal organ involvement. Retinal astrocytic hamartomas (astrocytomas) are benign glial cell 10 15 Q1 tumors that are generally asymptomatic and diagnosed incidentally. The IFT74 gene is responsible for the formation of IFT proteins, which play a major role in ciliogenesis. CASE PRESENTATION: In this retrospective clinical laboratory observational study, an 18-year-old male with Jeune syndrome and night vision loss is presented. Fundus examination revealed bilateral optic di

Isolated Cutis Marmorata Telangiectatica Congenita in a Full-Term Neonate: A Case Report.

Cutis marmorata telangiectatica congenita (CMTC) is a rare condition present at birth, marked by a persistent, net-like purplish discoloration of the skin caused by abnormal blood vessels. We report a case of a full-term female neonate born following an uneventful pregnancy, who exhibited widespread, nonblanching, marble-like skin changes involving the trunk, limbs, and face. These findings were present at rest, did not resolve with warming, and were more prominent during crying. Physical examination revealed no associated limb asymmetry, neurologic abnormalities, or dysmorphic features. A com

Isogenic hiPSC models of Turner syndrome development reveal shared roles of inactive X and Y in the human cranial neural crest network.

Viable human aneuploidy can be challenging to model in rodents due to syntenic boundaries or primate-specific biology. Human monosomy-X (45,X) causes Turner syndrome (TS), altering craniofacial, skeletal, endocrine, and cardiovascular development, which in contrast remain unaffected in X-monosomic mice. To learn how monosomy-X may impact embryonic development, we turned to 45,X and isogenic euploid human induced pluripotent stem cells (hiPSCs) from male and female mosaic donors. Because the neural crest (NC) is hypothesized to give rise to craniofacial and cardiovascular changes in TS, we asse

Unexpected molecular mechanism of Orc6-based Meier-Gorlin syndrome: insights from a humanized Drosophila model.

Meier-Gorlin syndrome (MGS) is a rare autosomal recessive disorder characterized by microtia, primordial dwarfism, and skeletal abnormalities. Patients with MGS often carry mutations in genes encoding the subunits of the origin recognition complex (ORC), components of the prereplicative complex and replication machinery. ORC6, an essential ORC subunit, plays a critical role in both DNA replication and cytokinesis. Approximately 30% of reported ORC6-related MGS cases exhibit compound heterozygosity for the ORC6 variants c.2T > C (p.Met1Thr) and c.449 + 5G > A. The c.2T > C mutation disrupts the

Hereditary Vitreoretinopathies: Molecular Diagnosis, Clinical Presentation and Management.

Hereditary vitreoretinopathies (HVRs), also known as hereditary vitreoretinal degenerations comprise a heterogeneous group of inherited disorders of the retina and vitreous, collectively and variably characterised by vitreal abnormalities, such as fibrillary condensations, liquefaction or membranes, as well as peripheral retinal abnormalities, vascular changes in some, an increased risk of retinal detachment and early-onset cataract formation. The pathology often involves the vitreoretinal interface in some, while the major underlying abnormality is vascular in others. Recent advances in molec

Long-Term Immune Alterations After Thymectomy in Early Childhood: A Case Series.

The thymus plays a central role in T-lymphocyte maturation and in maintaining immune defense against infections and autoimmunity. We describe three boys (mean age 7.3 years) who underwent total thymectomy during surgical correction of congenital heart disease, two in infancy and one in the neonatal period. Patient A presented (14 years old) with recurrent cervical lymphadenopathy, generalized Epstein-Barr virus (EBV) infection, and hepatic fibrosis. Patient B (two-year-old) developed splenomegaly and had a history of frequent lower respiratory tract infections and pseudohypoaldosteronism type

PIEZO1 loss-of-function compound heterozygous mutations in the rare congenital human disorder Prune Belly Syndrome.

Prune belly syndrome (PBS), also known as Eagle-Barret syndrome, is a rare, multi-system congenital myopathy primarily affecting males. Phenotypically, PBS cases manifest three cardinal pathological features: urinary tract dilation with poorly contractile smooth muscle, wrinkled flaccid ventral abdominal wall with skeletal muscle deficiency, and intra-abdominal undescended testes. Genetically, PBS is poorly understood. After performing whole exome sequencing in PBS patients, we identify one compound heterozygous variant in the PIEZO1 gene. PIEZO1 is a cation-selective channel activated by vari

Human split hand/foot variants are not as functional as wildtype human PRDM1 in the rescue of craniofacial defects.

BACKGROUND: Split hand/foot malformation (SHFM) is a congenital limb disorder presenting with limb anomalies, such as missing, hypoplastic, or fused digits, and often craniofacial defects, including a cleft lip/palate, microdontia, micrognathia, or maxillary hypoplasia. We previously identified three novel variants in the transcription factor, PRDM1, that are associated with SHFM phenotypes. One individual also presented with a high arch palate. Studies in vertebrates indicate that PRDM1 is important for development of the skull; however, prior to our study, human variants in PRDM1 had not bee

Loss-of-function variants in ERF are associated with a Noonan syndrome-like phenotype with or without craniosynostosis.

Pathogenic, largely truncating variants in the ETS2 repressor factor (ERF) gene, encoding a transcriptional regulator negatively controlling RAS-MAPK signaling, have been associated with syndromic craniosynostosis involving various cranial sutures and Chitayat syndrome, an ultrarare condition with respiratory distress, skeletal anomalies, and facial dysmorphism. Recently, a single patient with craniosynostosis and a phenotype resembling Noonan syndrome (NS), the most common disorder among the RASopathies, was reported to carry a de novo loss-of-function variant in ERF. Here, we clinically prof

Biliary, Renal, Neurological, and Skeletal syndrome in a Chinese boy.

Biliary, Renal, Neurological, and Skeletal syndrome (BRENS syndrome) is a very rare ciliopathy caused by variants in the TTC26 (OMIM 617453) gene. There are only a few case reports of BRENS syndrome in the literature. We report here a Chinese case of BRENS syndrome who presented with kidney, neurological, skeletal, and other features. It is the first description of BRENS syndrome without biliary involvement. Gene testing revealed three novel compound heterozygous variants in the TTC26 gene, c.1069 + 5G > A in one allele from the mother and c.511A > G (p.Ile171Val) and c.1099T > C (p.Ser367Pro)

Proteomic profiling of oleamide-mediated polarization in a primary human monocyte-derived tumor-associated macrophages (TAMs) model: a functional analysis.

BACKGROUND: Tumor-associated macrophages (TAMs) play a critical function in the development of tumors and are associated with protumor M2 phenotypes. Shifting TAMs towards antitumor M1 phenotypes holds promise for tumor immunotherapy. Oleamide, a primary fatty acid amide, has emerged as a potent anticancer and immunomodulatory compound. However, the regulatory effects of oleamide on TAM phenotypes remain unclear. METHODS: We used real-time quantitative reverse transcription polymerase chain reaction (qRT-PCR) and enzyme-linked immunosorbent assay (ELISA) techniques to study the influence of o

Skeletal abnormalities in mice with Dnmt3a missense mutations.

Overgrowth and intellectual disability disorders in humans are typified by length/height and/or head circumference ≥ 2 standard deviations above the mean as well as intellectual disability and behavioral comorbidities, including autism and anxiety. Tatton-Brown-Rahman Syndrome is one type of overgrowth and intellectual disability disorder caused by heterozygous missense mutations in the DNA methyltransferase 3A (DNMT3A) gene. Numerous DNMT3A mutations have been identified in Tatton-Brown-Rahman Syndrome patients and may be associated with varying phenotype severities of clinical presentation.

Evidence for therapeutic use of cannabidiol for nail-patella syndrome-induced pain in a real-world pilot study.

Nail-patella syndrome (NPS) is a rare genetic disease characterized by dysplastic nails, patella abnormalities, skeletal malformation, and chronic pain. Although chronic pain in NPS is mainly due to bone and musculoskeletal symptoms, it can also result from neurological dysfunction. Conventional analgesics are often insufficient to relieve NPS-associated chronic pain. Cannabinoids, which act on the serotonergic and/or noradrenergic pain systems, may therefore represent valuable non-psychoactive alternatives for managing pain in these patients. The effectiveness and safety of synthetic cannabid
