Silent complexity: a rare combination of polysplenia, cleft palate, retroaortic renal vein, and dorsal pancreatic agenesis.

Polysplenia syndrome is a rare congenital condition within the heterotaxy spectrum, characterized by multiple splenic nodules and abnormal visceral arrangement. Its presentation varies widely and may include cardiovascular, gastrointestinal, and vascular anomalies. We report an asymptomatic child incidentally diagnosed with polysplenia associated with a cleft palate and a retroaortic right renal vein, an unusual combination not commonly described. Imaging revealed multiple spleens, left-sided interrupted inferior vena cava with azygos continuation, and a retroaortic renal vein without hemodyna

First bone marrow transplantation for mucopolysaccharidosis type I in Vietnam: a case report.

Mucopolysaccharidosis type I (MPS-I) is an inherited lysosomal storage disorder characterized by alpha-L-iduronidase deficiency, leading to progressive multi-organ damage and fatal complications. Early treatment is crucial to improve outcomes, particularly in patients with severe phenotypes. We present the first case of successful allogeneic bone marrow transplantation (BMT) for MPS-I in Vietnam. A 10-month-old girl experienced coarse facial features, congenital dermal melanocytosis spots, kyphoscoliosis developmental delay. Laboratory analyses revealed reduced alpha-L-iduronidase activity and

Adaptability of minimally invasive thoracoscopic repair for congenital diaphragmatic hernia: a neonatal case report from a low-income country.

BACKGROUND: Congenital diaphragmatic hernia (CDH) is a rare but potentially life-threatening congenital anomaly characterized by herniation of abdominal organs into the thoracic cavity, resulting in respiratory compromise and pulmonary hypoplasia. CASE PRESENTATION: A 2-day-old female neonate weighing 2.5 kg presented with severe respiratory distress, cyanosis, and a scaphoid abdomen. Chest radiography and computed tomography revealed a large left-sided Bochdalek hernia containing the stomach, spleen, left kidney, and bowel loops, causing compression of the left lung and mediastinal shift. Rare causes of acute abdomen in a paediatric surgery department: a 5-years review.

AIMS: Acute abdomen is a common indication to surgery in paediatric emergency departments. Appendicitis is the most frequent diagnosis, followed by bowel obstruction and female genital conditions. Some rare aetiologies may lead to challenges in surgical management. Aim of this study is to analyse the distribution of rare causes of acute abdomen in patients undergoing emergency surgery in the past five years at our centre. METHODS: Records of patients undergone urgent abdominal surgery were retrospectively reviewed from 2015 to 2019. Information relative to age, sex, underlying pathology, and

[Syphilis and pregnancy].

Syphilis is a bacterial infection caused by the species Treponema pallidum subsp. pallidum, transmitted both sexually and vertically. The disease progresses in three distinct stages, with a high risk of contagion in the early stages. The infection has seen a resurgence in several countries around the world and is particularly concerning during pregnancy.Indeed, from the 16th week of gestation, the treponema can cross the placenta and infect the fetus and it can cause placental and/or fetal lesions. Maternal-fetal transmission is more common in the early stages of the disease. The consequences

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Prevalence of Spinal Meningeal Diverticula in Autosomal Dominant Polycystic Kidney Disease.

BACKGROUND AND PURPOSE: Patients with autosomal dominant polycystic kidney disease (ADPKD) develop cysts in the kidneys, liver, spleen, pancreas, prostate, and arachnoid spaces. In addition, spinal meningeal diverticula have been reported. To determine whether spinal meningeal diverticula are associated with ADPKD, we compared their prevalence in subjects with ADPKD with a control cohort without ADPKD. MATERIALS AND METHODS: Subjects with ADPKD and age- and sex-matched controls without ADPKD undergoing abdominal MRI from the midthorax to the pelvis from 2003 to 2023 were retrospectively evalu

Marginal Indication for Thoracoscopic Surgery for Neonatal Bochdalek Hernia: "Anchor-Shaped Closure" Technique for the Patient's Own Residual Diaphragm Using a Loop Needle Device.

INTRODUCTION: Surgical procedures to avoid using artificial materials require ongoing discussion. We herein report a case of thoracoscopic repair for congenital diaphragmatic hernia (CDH) via anchor-shaped closure with the patient's own residual diaphragm using a loop needle device. PATIENT AND SURGICAL TECHNIQUE: A 2-day-old boy prenatally diagnosed with CDH underwent thoracoscopic repair after his respiratory and circulatory conditions had stabilized. The defect was a typical Bochdalek CDH, approximately 2.5 × 4 cm. The herniated organs of the thoracic cavity were the stomach, small intesti

Non-invasive screening for liver fibrosis by acoustic radiation force impulse in patients with ciliopathies.

Primary cilia are antenna-like structures on the surface of epithelial cells involved in multiple signaling pathways. Their malfunction can cause a heterogenous group of diseases called ciliopathies with a broad spectrum of organ involvements, including liver fibrosis. The aim of this exploratory study was to evaluate elastography measurement via ultrasound based acoustic radiation force impulse imaging (ARFI) as a screening tool for liver fibrosis in ciliopathies. In a prospective cohort of 51 patients with ciliopathies (aged between 2 months and 66 years) from the NEOCYST registry stiffness

Evaluating Animal Models for Improved Hemodialysis Vascular Access: A Focus on Arteriovenous Fistula.

Hemodialysis (HD) is the primary form of renal replacement therapy for renal failure, and autologous arteriovenous fistula (AVF) is the preferred vascular access for long-term hemodialysis. However, AVFs are prone to stenosis during use, leading to fistula dysfunction and failure to meet the high blood flow rates required for dialysis, thereby diminishing the quality of life of patients. Re-interventions following stenosis often fail to address the underlying issues. Therefore, exploring the pathophysiological mechanisms, diagnostic and therapeutic approaches, and preventive measures for AVF s

The Natural History of Gastroesophageal Varices in Children With Portal Hypertension.

BACKGROUND & AIMS: In children with portal hypertension, the natural history of gastroesophageal varices remains unclear. We report our experience. METHODS: From 1990 to 2021, 1586 children with portal hypertension underwent ≥1 diagnostic upper gastrointestinal endoscopies before any treatment, including 590 with ≥2 endoscopies. High-risk varices (HRV) were defined as grade 3 esophageal varices as well as grade 2 esophageal varices with red color signs or gastroesophageal varices type 1 or both. RESULTS: Worsening of the endoscopic pattern occurred in 58% of children over a mean 4-year inter

A Novel Combined Surgical and Interventional Radiology Vascular Reconstruction, With Stenting of Meso-Rex Bypass, to Successfully Manage Recurrent Portal Venous Flow Obstruction Post Segmental Liver Transplant in a Pediatric Patient.

BACKGROUND: Portal vein complications can be difficult to manage in the context of challenging anatomy and porto-systemic collaterals. METHODS: This case illustrates a child with biliary atresia and associated laterality (situs inversus, polysplenia) features, who underwent segmental liver transplantation with portal venous conduit at 4 years of age for end-stage liver disease due to failed Kasai portoenterostomy. He subsequently developed portal venous conduit thrombosis requiring early surgical and interventional procedures. This was thought to be due to angulation of the conduit versus ven

A case of fatal infective endocarditis in uncorrected tetralogy of Fallot with persistent left superior vena cava.

Infectious endocarditis is characterized by the inflammation of the endocardium, and individuals with congenital heart disease (CHD) are at a higher risk for developing infective endocarditis (IE), with a predicted risk 100 times that of the general population. We present a rare case of fatal infectious endocarditis in a 39-year-old female with uncorrected Tetralogy of Fallot (TOF) with persistent left superior vena cava (PLSVC). Recurrent dental infections and multiple skin infections may have contributed to the development of IE, and the patient's prolonged survival without surgical interven

Splenic Lymphatic Malformations: Clinical Characteristics of 35 Patients and Literature Review.

BACKGROUND: Primary lesions of the spleen are very rare, and splenic lymphatic malformations (SLMs) account for 0.007% of all splenic lesions; thus, SLMs are often ignored in clinical practice. The aim of this study was to review the clinical and imaging features of 35 patients with SLMs, classify them on imaging and provide a reference for treatment. METHODS: We reviewed the records of patients with SLMs who entered the medical system at our institution from December 2008 to March 2023. The clinical characteristics and imaging features of these patients were collected to classify SLMs. RESU

Comprehensive comparison of fetal right and left atrial isomerism: cardiac and extracardiac malformations, diagnostic accuracy, and survival outcomes.

BACKGROUND: Right atrial isomerism (RAI) and left atrial isomerism (LAI) are subtypes of heterotaxy syndrome with distinct cardiovascular and visceral anomalies. Accurate prenatal differentiation is essential for risk stratification and clinical decision-making. However, most existing data are derived from postnatal or mixed-age cohorts. This study uniquely focuses on prenatally diagnosed cases with postnatal confirmation and longitudinal outcome analysis. METHODS: In this retrospective cohort study, fetuses prenatally diagnosed with heterotaxy syndrome between 2016 and 2022 were reviewed and
