Testosterone and DHEA-S Secreting Adrenal Cortical Adenoma in a Postmenopausal Woman: A Rare Functional Incidentaloma.

Postmenopausal hyperandrogenism is uncommon and should prompt evaluation for an underlying androgen-secreting source. Although ovarian causes are more frequent, adrenal tumors remain an important consideration and may occasionally be malignant. We report a 53-year-old postmenopausal woman who presented with progressive hirsutism, scalp hair loss, and mild clitoromegaly. Laboratory evaluation showed markedly elevated total testosterone and dehydroepiandrosterone sulfate levels with suppressed gonadotropins. Pelvic ultrasonography showed no ovarian mass. CT demonstrated a 3.5 × 2.4 cm left adren

An Infant With Hyponatremia, Hyperkalemia, and Faltering Growth: A Case Report.

Infants presenting with hyponatremia and hyperkalemia routinely engage clinician concern for congenital adrenal hyperplasia (CAH), a set of disorders resulting in difficulty producing glucocorticoids, mineralocorticoids, and sex steroids in the adrenal glands. However, we present the case of an infant with classic CAH-like symptoms, a negative newborn screen, and clinical features inconsistent with the diagnosis of CAH. Ultimately, the patient was diagnosed with pseudohypoaldosteronism (PHA), a condition in which aldosterone is unable to activate its receptor in the kidney. Notably, he also ex

Diagnostic dilemma in a neonate: Pseudo Hypoaldosteronism mimicking congenital adrenal hyperplasia.

UNLABELLED: Pseudo hypoaldosteronism (PHA) and congenital adrenal hyperplasia (CAH) can present with similar biochemical abnormalities in neonates, making early diagnosis challenging. We report a neonate who presented with hyponatremia, hyperkalemia, and metabolic acidosis and was initially managed as CAH with steroid therapy. OUTCOME: Despite treatment, the electrolyte abnormalities persisted, prompting further evaluation. Genetic testing revealed a homozygous pathogenic mutation in SCNN1B, confirming autosomal recessive PHA Type I-B. CONCLUSION: Persistent electrolyte derangements despite

Differential fetal adrenal response in congenital heart disease based on cerebral oxygen delivery.

OBJECTIVES: To evaluate fetal adrenal gland, neocortex, and fetal zone volumes using 2D ultrasound in pregnancies affected by congenital heart disease (CHD). METHODS: This prospective case-control study included 65 fetuses with CHD and 65 gestational age-matched healthy controls. Volumes of the whole adrenal gland (WAGV), fetal zone (FZV), and neocortex (NCV) were calculated using 2D ultrasound and corrected for estimated fetal weight. The CHD group was subdivided into Class A (reduced cerebral oxygen delivery, n=33) and Class B (normal cerebral oxygen delivery, n=32). Corrected volumes (c-WA

Fetus-in-fetu arising from the palate treated with ex-utero intrapartum treatment.

Epignathus, a teratoma arising from the palate or pharynx, is extremely rare and palatal fetus-in-fetu represents an even rarer anomaly. We report a prenatally diagnosed palatal fetus-in-fetu successfully managed with ex utero intrapartum treatment (EXIT). A 4 cm oral mass detected at 23 weeks caused polyhydramnios and gastric shrinkage, suggesting impaired swallowing and potential airway obstruction. Multidisciplinary evaluation determined the need for EXIT to secure the airway under placental circulation. At 34+5 weeks of gestation, caesarean delivery with EXIT was performed; a 14 cm tumour

A 56-Year-Old Male Patient With 21-Hydroxylase Deficiency Presenting With Fatigue: A Case Report.

AIMS/BACKGROUND: Congenital adrenal hyperplasia (CAH) results from 21-hydroxylase deficiency (21-OHD), which is the most frequent form of CAH and often presents with atypical symptoms. Patients with non-classic 21-OHD (NC-21-OHD) are particularly susceptible to diagnostic challenges, including misdiagnosis and underdiagnosis. This study reports a case of NC-21-OHD and underscores the associated challenges in its diagnosis, treatment, and clinical management. CASE PRESENTATION: This study reports a 56-year-old male patient who presented with fatigue lasting over a year. Initial ultrasound reve

Update in Adrenal Steroidogenesis and Congenital Adrenal Hyperplasia.

The adrenal cortex is responsible for the synthesis of glucocorticoids, mineralocorticoids, and adrenal androgens, all derived from cholesterol through a series of enzymatic reactions. This process is known as steroidogenesis. Deficiency of various enzymes required for steroid synthesis causes congenital adrenal hyperplasia (CAH), a group of inherited disorders characterized by impaired cortisol synthesis often accompanied by mineralocorticoid deficiency and abnormalities in androgen synthesis, depending on the specific enzyme defect. The most common type of CAH is 21-hydroxylase deficiency. M

Pheochromocytoma crisis combined with median arcuate ligament syndrome: A rare case report.

RATIONALE: Left ventricular thrombus caused by acute catecholamine cardiomyopathy combined with pheochromocytoma crisis (PC) is extremely rare in clinical practice, and there are no reports of patients with median arcuate ligament syndrome (MALS) at the same time. This report describes the case of a 45-year-old woman admitted to the cardiology department with this rare condition. The patient presented with severe abdominal pain, vomiting, rapid blood pressure (BP) fluctuations, and recurrent hypotensive syncope. We discuss the patient's diagnosis and treatment and share our experience with thi

Androgen production in adrenocortical H295R cells is regulated by thyroid hormone T3 without reciprocal thyroid axis modulation in pediatric CAH.

Thyroid hormones (THs) are critical regulators of human development, cellular differentiation, and metabolism. While their systemic effects are well established, their role in adrenal androgen production remains poorly defined. Moreover, potential feedback regulation of the hypothalamic-pituitary-thyroid (HPT) axis by adrenal androgens has not been thoroughly investigated. This study aimed to clarify the regulatory effects of THs on adrenal androgens and to investigate potential feedback mechanisms in patients with congenital adrenal hyperplasia (CAH). In an in-vitro experiment, treatment with

Preoperative Neutrophil-to-Lymphocyte Ratio is Associated with Early Postoperative Pain and Opioid Consumption after Total Knee Arthroplasty: A Retrospective Study.

BACKGROUND: Improving quality and effectiveness of health care is one of the priorities of health policies. Hospital or physician volume represents a measurable variable with a relevant impact on effectiveness of health care. A recent Italian law, the "spending review", calls for the definition of "qualitative, structural, technological and quantitative standards of hospital care". There is a need for an accurate evaluation of the available scientific evidence in order to identify these standards, including the volume of care above or below which the public and private hospitals may be accredi

Convergent IGF2 overexpression in pheochromocytoma/paraganglioma: insights from Beckwith-Wiedemann syndrome.

Beckwith-Wiedemann syndrome (BWS) is an imprinting disorder characterized by overgrowth and tumor predisposition, caused by dysregulated expression of genes on chromosome 11p15.5. An association between BWS and pheochromocytoma/paraganglioma (PPGL) has been suggested in isolated case reports over the past fifty years, but the molecular basis for this link remains unclear. We identified four patients with BWS who developed metastatic PPGL and investigated IGF2 pathway activation in these tumors and in PPGL across various genotypes. Pan-cancer transcriptomic analysis of The Cancer Genome Atlas (

A Case of Segmental Absence of Intestinal Musculature Associated With a Gastric Diverticulum in an Adult.

A man in his late 50s underwent an abdominal computed tomography (CT) scan as part of a whole-body trauma assessment after a bicycle accident. Incidentally, a mass was detected between the left adrenal gland and the posterior wall of the gastric fundus. Given the differential diagnosis of either an adrenal tumor or a gastric diverticulum, laparoscopic resection was planned. Intraoperatively, the mass was identified as a gastric diverticulum, and a partial gastrectomy was performed. Histopathological examination revealed no abnormalities in the mucosa or the muscularis mucosae. However, the mus

Stress-associated transient enlargement of pre-existing adrenal hyperplasia in 21-hydroxylase deficiency: a case report.

BACKGROUND: The adrenal gland plays a pivotal role in the stress response via the hypothalamic-pituitary-adrenal (HPA) axis and the sympatho-adrenomedullary system (SAMS). Congenital adrenal hyperplasia (CAH) results from pathogenic variants in genes encoding adrenal steroidogenic enzymes. The most common form, 21-hydroxylase deficiency, impairs cortisol biosynthesis, leading to compensatory overproduction of adrenocorticotropic hormone (ACTH) and chronic adrenal hyperplasia. CASE PRESENTATION: A 40-year-old man was referred for incidentally detected bilateral adrenal lesions on abdominal com

Outcomes of Congenital Neuroblastoma With Placental Metastases: A Systematic Review.

Very little is known about congenital neuroblastoma with placental metastases. We reviewed the literature to summarise outcomes and potential risk factors for placental involvement. We performed a systematic review of the PubMed/Ovid/Medline/Embase/Cochrane databases from 1946 to 2024. The initial search identified 61 articles; 16 articles were included in the final analysis. Nineteen cases were identified (9 male, 10 female); 16 were stillborn or dead in the neonatal period. There was an adrenal primary in 89.5%, and these were detected on antenatal ultrasound in 26% of cases. Macroscopic app

Evidence of anti-corticotroph autoantibodies in Down syndrome with isolated adrenocorticotropic hormone deficiency: findings from a single case.

PURPOSE: Down syndrome (DS) is associated with immune dysregulation and a broad spectrum of autoimmune diseases; however, autoimmune involvement of the pituitary gland remains poorly characterized. Isolated adrenocorticotropic hormone deficiency (IAD) is a rare cause of secondary adrenal insufficiency, and its relationship to DS-related autoimmunity has not yet been elucidated. We encountered a single case of DS that was complicated by IAD. We describe the clinical course in detail and present immunological findings suggestive of an autoimmune basis for IAD in the context of DS. METHODS AND R

Endocrine hypertension: the role of imaging in diagnosis and management.

Endocrine hypertension is an uncommon but treatable cause of secondary hypertension. It results from excessive hormone production by the endocrine glands or due to ectopic hormone production. The causes of abnormal hormonal production can be congenital or acquired. Specific syndromes can also predispose to the development of endocrine hypertension. Extensive catecholamine production can occur due to pheochromocytomas and paragangliomas (PPGLs). Excessive aldosterone secretion by the adrenal cortex commonly occurs due to idiopathic (bilateral) adrenal hyperplasia or aldosterone-producing adrena

Carney complex with adrenocorticotropic hormone-independent Cushing syndrome with PRKAR1A gene variant: a case report.

BACKGROUND: Primary pigmented nodular adrenal disease (PPNAD) is an autosomal dominant condition and a rare cause of Cushing syndrome, independent of pituitary origin, and is adrenocorticotropin (ACTH) independent. Carney complex PPNAD (cPPNAD) is associated with inactivating mutations of the regulatory subunit type 1A of the cAMP-dependent protein kinase (PRKAR1A) gene which is found in more than 70% of Carney complex cases. PRKAR1A is a tumor suppressor gene located in the 17q23-24 region of the human chromosome and plays an important role in regulating cellular metabolism, differentiation,
