[MINOCA revealing an anomalous origin of right coronary artery and a pheochromocytoma : A case report with literature review].

Pheochromocytoma is a rare secretory neuroendocrine tumor of the adrenal medulla, which usually presents with Ménard's triad or high blood pressure. Its diagnosis is based on elevated adrenaline and noradrenaline levels. Cross-sectional imaging, such as computed tomography or magnetic resonance imaging highlights the tumor and management consists of excision of this tumor. Cardiovascular complications of pheochromocytoma are multiple, such as rhythm disturbances, hypotension, shock, acute coronary syndromes, transient ischemic attacks and cerebrovascular accidents. Thus, in the face of acute c

Secondary hypertension diagnosis and management among pregnant patients: a review.

Hypertensive disorders are a common complication of pregnancy, with secondary hypertension estimated to impact around 1% of pregnancies. There are numerous causes of secondary hypertension including chronic kidney disease, renal artery stenosis, primary aldosteronism, Cushing syndrome, pheochromocytomas, thyroid disorders, obstructive sleep apnea, coarctation of the aorta, and medication side effects. Identifying the underlying cause of hypertension in pregnancy is critical to determine the appropriate treatment. This review will focus on the pregnancy specific diagnosis and management conside

Case Report: Challenges of an extremely delayed diagnosis of classic congenital adrenal hyperplasia in a completely virilized 46,XX patient.

Classic Congenital Adrenal Hyperplasia (CAH) due to 21-hydroxylase deficiency is typically diagnosed in early life. We report a 46,XX completely virilized 46,XX patient who was diagnosed with classic CAH at the age of 73 years. He was under follow-up for prostate hyperplasia and referred after the finding of giant bilateral adrenal myelolipomas. He presented with hormonal values initially interpreted as suggestive of hypogonadotropic hypogonadism, prompting further biochemical and genetic analysis. Next-generation sequencing identified heterozygous variants in X-linked genes, uncovering a 46,X

Pediatric and adolescent von Hippel-Lindau disease: tumor profiles, genotype-phenotype correlation and comparison with adults.

PURPOSE: Data on pediatric/adolescent von Hippel-Lindau (VHL) disease is sparse, and surveillance/management recommendations rely on expert opinions/extrapolations from adults. We aimed to characterize the childhood/adolescent VHL disease phenotype, compare it with adults, and identify genotype-phenotype correlations. METHODS: Retrospective review of children/adolescents (≤ 19 years) and adults with VHL disease from a single endocrine center (2000-2024). Only neoplasms diagnosed until age 19 years were included in the childhood/adolescent group and compared with the last follow-up of adults.

[A Family case of von Hippel-Lindau syndrome].

Von Hippel-Lindau syndrome (FHL) is a rare autosomal dominant disease that leads to the formation of multiple organ tumor syndrome. The pathology is primarily caused by the inactivation of the VHL gene, which is located on chromosome 3 (3p25/26) and encodes ubiquitin ligase, which destroys hypoxia-induced factor-1α (HIF-1α). The genetic defect leads to the accumulation of HIF-1a protein, activating key carcinogenic pathways, and activated cytokines cause abnormal proliferation of tumor cells and oncogenesis. To date, more than 500 mutations have been registered in VHL. FHL syndrome is characte

Adrenergic Storm with Obstructive Hydrocephalus: Atypical Neurological Presentation of Von Hippel-Lindau Disease with Bilateral Pheochromocytoma in an Adolescent.

Pheochromocytoma is often not the first diagnosis considered for many neurological complaints, especially obstructive hydrocephalus. We report a case of Von Hippel-Lindau disease in an adolescent presenting with obstructive hydrocephalus due to adrenergic storm requiring emergency ventriculoperitoneal shunt. This vignette emphasizes the importance of an integral approach, particularly accurate diagnosis that would lead to targeted pharmacological therapy focusing on alphaa-drenergic receptor blockade, which was followed by total curative excision of pheochromocytoma in a timely fashion.

Recovery of pituitary function in patients with apoplexy immediately after surgical resection of necrotic tumors.

BACKGROUND: Pituitary tumor apoplexy [PTA] and Rathke's Cleft Cyst apoplexy (RCCA) are uncommon disorders causing hypopituitarism. OBJECTIVE: Examine the value of perioperative hormonal alterations in predicting recovery of impaired function in patients with PTA and RCCA. METHODS: Consecutive PTA (N = 97) and RCCA (N = 41) patients who had emergent surgery received dexamethasone until surgery. Perioperative HPA function and prolactin levels were frequently assessed while hydrocortisone therapy was provided to those exhibiting clinical or biochemical evidence [cortisol <10 ug/dL; <276 nmol/L]

Unique Case Report: A Rare Association of 21-Hydroxylase Deficiency with Triple X Karyotype.

Background: Congenital adrenal hyperplasia (CAH) represents a group of autosomal recessive disorders characterized by impaired cortisol synthesis in the adrenal glands. Over 90% of CAH cases result from a deficiency of the enzyme 21-hydroxylase (21OHD). The clinical spectrum of 21OHD ranges from the severe, life-threatening salt-wasting classic form, often presenting with prenatal virilization in females, to the non-classic (milder) form, which lacks glucocorticoid deficiency. Females with the non-classic form may experience symptoms of hyperandrogenism or infertility later in life, while male

Adrenal Insufficiency in Adults: A Review.

IMPORTANCE: Adrenal insufficiency is a syndrome of cortisol deficiency and is categorized as primary, secondary, or glucocorticoid induced. Although primary and secondary adrenal insufficiency are rare, affecting less than 279 per 1 million individuals, glucocorticoid-induced adrenal insufficiency is common. OBSERVATIONS: Primary adrenal insufficiency, which involves deficiency of all adrenocortical hormones, is caused by autoimmune destruction, congenital adrenal hyperplasia, pharmacological inhibition (eg, high doses of azole antifungal therapy), infection (eg, tuberculosis, fungal infectio

[Testicular adrenal rest tumors (TARTs) and adrenogenital syndrome (AGS) - Do not mix up with malignant testicular tumors!].

Testicular adrenal rest tumors and adrenogenital syndrome (AGS) - Do not mix up with malignant testicular tumors! Testicular adrenal residual tumors (TARTs) frequently occur in men with adrenogenital syndrome. Without knowledge of AGS, diagnosis is problematic due to difficult differentiation from other testicular tumors. However, early treatment is crucial for maintaining or regaining fertility, among other aspects. This article provides background knowledge for general practitioners.

PHOX2B: a diagnostic cornerstone in neurocristopathies and neuroblastomas.

Paired-like homeobox 2B (PHOX2B) is a gene essential in the development of the autonomic nervous system. PHOX2B mutations are associated with neurocristopathies-Hirschsprung disease (HSCR) and congenital central hypoventilation syndrome (CCHS)-and peripheral neuroblastic tumours. PHOXB2 plays an important role in the diagnostics of these conditions.Genotyping of a PHOX2B pathogenic variant is required to establish a diagnosis of CCHS. In HSCR patients, PHOX2B immunohistochemical staining has proven to be a valuable tool in identifying this disease. Furthermore, PHOXB2 is a predisposition gene

Prenatal diagnosis of intrathoracic kidney and adrenal gland in left-sided congenital diaphragmatic hernia.

A 29-year-old primigravida at 31 weeks of gestation was referred for intrathoracic kidney (ITK). Ultrasound revealed left kidney intrathoracic placement with an anteriorly positioned ectopic adrenal gland. Magnetic resonance imaging confirmed diaphragmatic interruption and colon herniation. A female neonate, delivered at 37 weeks, underwent successful thoracoscopic repair for a left Bochdalek hernia. Despite compression of the left lung, notably optimistic lung-to-head ratio (LHR) values were observed, correlating with favorable outcomes. This case underscores the rare occurrence of ITK, its a

Evidence of the Role of Inflammation and the Hormonal Environment in the Pathogenesis of Adrenal Myelolipomas in Congenital Adrenal Hyperplasia.

Adrenal myelolipomas (AML) are composed of mature adipose and hematopoietic components. They represent approximately 3 percent of adrenal tumors and are commonly found in patients with congenital adrenal hyperplasia (CAH). CAH provides a unique environment to explore AML pathogenesis. We aimed to evaluate the role of the immune system and hormones that accumulate in poorly controlled CAH in the development of AML. When compared to normal adrenal tissue, CAH-affected adrenal tissue and myelolipomas showed an increased expression of inflammatory cells (CD68, IL2Rbeta), stem cells (CD117) B cells

Giant Bilateral Adrenal Myelolipomas in a Non-Compliant Patient with Congenital Adrenal Hyperplasia.

BACKGROUND 21-hydroxylase deficiency, an essential enzyme for glucocorticoid and mineralocorticoid synthesis, is the cause of congenital adrenal hyperplasia (CAH) in more than 95% of cases. It is an autosomal recessive disorder encoded by the CYP21A2 gene, categorized into classical forms, which encompass the salt-wasting (SW) and simple virilizing (SV) forms, as well as the nonclassical form (NC). The aim of medical treatment is to replace missing glucocorticoids and, if necessary, mineralocorticoids, while also reducing elevated adrenal androgens. CASE REPORT We present the case of a 42-year
