Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Cryptorchidism in dogs and cats presented for elective gonadectomy: A descriptive cohort study of 306 animals treated between 2018 and 2023.

The objective of this study was to describe the incidence, breed distribution, clinical findings (anatomic location, number of testicles affected, and side), and surgical management of cryptorchidism in dogs and cats presented for elective gonadectomy. Medical record abstracts from a non-profit, high-volume, high-quality, spay-neuter clinic from 2018 to 2023 were reviewed retrospectively for cases of elective surgical gonadectomy of cryptorchid dogs and cats. During the study period, 5,476 dogs and 11,559 cats were presented to the same facility for elective surgical castration, suggesting a c

Curcumin promotes spermatogenesis in mice with cryptorchidism by regulating testicular protein O-GlcNAcylation.

INTRODUCTION: Curcumin has garnered increasing attention in male reproductive research due to its potential anti-infertility properties. This study aimed to explore the protective effects of curcumin on spermatogenesis impairment in mice with cryptorchidism and to elucidate the underlying mechanisms. METHODS: A total of 56 male Kunming mice aged 10 to 15 days were randomly assigned to different groups, including a control group (BO) and a cryptorchid group with five curcumin treatment groups (CC25, CC50, CC100, CC200, and CC300) receiving varying doses of curcumin (25, 50, 100, 200, and 300 m

Evolutionary relaxation and functional change of INSL3 and RXFP2 may underlie natural cryptorchidism in mammals.

Cryptorchidism is a common congenital abnormality that increases infertility and testicular cancer risk in adulthood. However, a few mammals exhibit naturally undescended testes while maintaining normal reproduction. The mechanisms underlying this natural cryptorchidism remain unclear. Here, we found evolutionary relaxation in INSL3 and RXFP2 of cryptorchid mammals, with the highest dN/dS ratio observed in cetaceans. Cellular experiments demonstrated that cetacean INSL3 downregulated the cAMP-PKA-CREB pathway, thereby reducing gubernacular cell proliferation and contraction. Cetacean INSL3 kno

Modelling adrenal steroid profiles to inform monitoring guidance in congenital adrenal hyperplasia.

BACKGROUND: There is no consensus on how to monitor adrenal androgens in Congenital Adrenal Hyperplasia (CAH). METHODS: Modelling of serum and salivary steroid profiles in healthy participants and patients with CAH randomised to either standard treatment or modified-release hydrocortisone hard capsules (MRHC). FINDINGS: Changes in serum 17-hydroxyprogesterone (17OHP) and androstenedione (A4) paralleled each other in healthy participants (n = 19) and patients with CAH (n = 122). However, healthy participants had similar absolute levels of 17OHP and A4 whereas patients with CAH had proportiona

Laparoscopic single-stage orchidopexy followed by groin exploration: The best two-stage orchidopexy?

AIM: Various laparoscopic approaches have been described for managing intra-abdominal testicles. We aimed to evaluate the outcomes of patients who had laparoscopic single stage vessel sparing (LSSVO) with regards to testicular atrophy, testicular ascent, and with special emphasis on surgical outcomes of a subsequent groin exploration for testicular ascent. METHODS: Retrospective review of patients who had LSSVO at a UK tertiary paediatric surgery centre between 2006 and 2024. We included all patients under 16 years following LSSVO who had complete follow up. We analysed age at operation, late

STARD9 and CDK5RAP2-Novel Candidate Genes for 46,XY Complete Gonadal Dysgenesis.

46,XY gonadal dysgenesis, characterised by absent or defective testicular development in individuals with a 46,XY karyotype, results from disruptions in the genetic programme governing testis determination and differentiation during embryogenesis. While monogenic causes explain approximately 50% of cases, emerging evidence suggests an oligogenic basis in some patients. However, many cases remain without a definitive molecular diagnosis. In this study, we investigated a patient with 46,XY gonadal dysgenesis to explore the underlying genetic aetiology. Whole-exome sequencing in the patient did n

Virilization and Hyperandrogenism in Turner Syndrome without Y Mosaicism: A Case Report and Review of the Literature.

BACKGROUND: Up to 10%-12% of individuals with Turner syndrome (TS) have Y chromosome material (eg, 45,X/46,XY), which can lead to virilization. We present the case of a patient with TS who developed virilization due to ovarian hilus cell (OHC) hyperplasia. CLINICAL CASE: An 11-year-old with TS presented with clitoromegaly, hyperandrogenism, and primary ovarian insufficiency (POI). Thorough workup was unable to identify the source of hyperandrogenism. She underwent laparoscopic bilateral gonadectomy and was diagnosed with OHC hyperplasia on pathologic evaluation. Hyperandrogenism resolved post

Is it possible to separate the testicular and ovarian components of an ovotestis?

UNLABELLED: The presence of an ovotestis is a rare etiology of differences of sex development. Histologically, ovotestis come in two forms: 1) Mixed or 2) Bipolar. OBJECTIVE: We hypothesize that it is surgically impossible to cleanly separate and preserve either the testicular or ovarian component of an ovotestis. STUDY DESIGN: Twenty human gonads with a previous diagnosis of ovotestis were re-sectioned in entirety and re-stained with markers for testicular (SOX9, TSPY, SALL4, DDX4, cP450, AR, α-actin) and ovarian tissue (FOXL2, SALL4, DDX4). Histologic sections were photographed at low powe

Prognostic value of health-related quality of life in patients with acute dyspnoea: a cohort study.

The elbow is a common site of orthopaedic injury in the paediatric population. The number of these injuries continues to rise following increased levels of participation in paediatric recreational and competitive sport. Injuries to the paediatric elbow can be classified as either overuse or acute. Delineating injury patterns to the elbow in children can be challenging, given the cartilaginous composition of the distal humerus and the multiple secondary ossification centres that appear and unite with the epiphysis at defined ages. Pitching in baseball, serving in tennis, spiking in volleyball,

Adaptive mechanisms in pancreatic islets counteract mitochondrial dysfunction in Barth syndrome.

AIMS/HYPOTHESIS: Barth syndrome is a mitochondrial disorder caused by Tafazzin (TAZ) mutations, which impair cardiolipin remodelling and contribute to systemic metabolic alterations. While islet dysfunction has been implicated in Barth syndrome, its underlying mechanisms remain unknown. We aimed to determine how Tafazzin (Taz) deficiency affects mouse pancreatic islet metabolism and hormone secretion, and whether systemic signals, such as circulating factors, modulate these effects in vivo. In vivo and in vitro models were used to separate direct islet effects from systemic influences of Taz d

Inadequate Gonadal Replacement in Patients with Turner Syndrome may Result in Pituitary Volume Enlargement.

OBJECTIVES: Patients with Turner syndrome need hormone replacement therapy for puberty induction. However, it is not known whether inadequate hormone replacement therapy affects the pituitary. MATERIAL AND METHODS: Patients with Turner syndrome (n=35) and healthy control (n=20) (age/gender matched) subjects were included. MRI imaging of the pituitary was used to calculate pituitary volumes. According to the estradiol regimen, patients were divided into two groups; (i) those treated with low-dose conjugated oestrogen (CE, 0.625 mg) and (ii) those treated with combination therapy (ethinyl estra

Anogenital distance in Klinefelter syndrome: A case-control study.

BACKGROUND: Fall in total testosterone (tT) levels in Klinefelter syndrome (KS) may occur at different times of sexual development, although overt hypogonadism generally occurs after puberty. Nevertheless, it is still debated whether patients with KS are exposed to normal androgen levels during fetal life. Anogenital distance (AGD) is an anthropometric measure reflecting androgenization during the fetal period. A shorter AGD in men was associated with infertility, genital anomalies, and testicular cancer. Although other androgen-dependent anthropometric measures such as testicular volume and s

Natural Progression of Pineal Cysts in Relation to Headache: A Prospective Observational Study.

BACKGROUND: Pineal cysts are a frequent, potentially incidental finding in routine imaging studies. Their relationship with headache has been a matter of some controversy. In this prospective observational study, we sought to track the clinical and radiological trajectory of pineal cysts in both patients with and without concurrent headache. METHODS: A cohort study was conducted in patients referred to the Department of Neurosurgery at Karolinska Hospital for evaluation of a pineal cyst ≥ 5 mm between June 2005 and April 2017. Patients were stratified according to headache status and classifi

Symptomatic Pineal Cysts Without Ventricular Enlargement: Diagnosis and Treament.

BACKGROUND: Pineal cysts that are not associated with ventricular enlargement can lead to manifestations of intracranial hypertension, usually paroxysmal. If these are due to local mass effect or narrowing of the entrance to the aqueduct, surgery may be indicated. METHODS: This is a narrative review based on publications retrieved by a search in PubMed. RESULTS: Autopsy studies have revealed pineal cysts in 40% of subjects. Pineal cysts that are not associated with ventricular enlargement can cause intermittent attacks of headache; the second most common symptom is visual disturbance. Patien

Ectopic thyroid tissue in the airway: a case report.

BACKGROUND: Ectopic thyroid tissue (ETT) is a rare congenital anomaly caused by the abnormal embryonic migration of thyroid tissue, leading to its presence outside its usual pretracheal location. This condition can lead to diagnostic challenges, especially when located within the airway, as it mimics other respiratory disorders such as asthma. CASE PRESENTATION: We report the case of a 69-year-old man with endotracheal ETT presenting with severe dyspnea, and the lesion was initially suspected to be malignant. The diagnosis of ETT was confirmed through bronchoscopy and histopathological examin
