This essay examines a prominent claim in abortion ethics: that bodily autonomy alone suffices to justify abortion even if the fetus is granted the moral status of a person. Instead of revisiting Thomson's violinist, it introduces a comparator offered less as a direct analogy to pregnancy than as a stress test of-and possible counterexample to-that claim. It imagines conjoined twin sisters, one temporarily dependent on the other's body, though a safe separation allowing both to survive is expected within months. The supporting twin's condition imposes serious but non-catastrophic demands; immed

OBJECTIVE: To explore whether ultrasound can identify findings suggestive of fetal complex congenital heart disease in highly selected high-risk fetuses during the mid-first trimester.
METHODS: A retrospective analysis was conducted on 23 highly selected high-risk fetuses who related to CHD that underwent ultrasound examinations in our department between March 2015 and March 2025. Standardized scanning included mid-sagittal view, transverse views of the head, upper limbs, abdomen, lower limbs, thorax, and cardiac were obtained. A transverse scan of the cardiac was performed to obtain the colo

BACKGROUND: Conjoined twins represent a rare condition in which drug exposure may be influenced by shared circulation and organ anatomy. For narrow therapeutic index antimicrobials such as amikacin, therapeutic drug monitoring (TDM) may require interpretation at twin-pair level.
METHODS: A retrospective TDM-informed coupled compartmental pharmacokinetic modeling study of 2 conjoined twin pairs was performed. One pair was pygopagus, and both twins (A and B) received amikacin treatment. The other pair was thoracopagus (X and Y) with complex shared cardiothoracic anatomy; amikacin was administer

Conjoined twins are a rare consequence of aberrant monozygotic twinning; one of the varieties is pygopagus twins, which are fused at the sacral region and may involve complex neural and dural sharing. We report a case of female pygopagus conjoined twins with shared spinal cord and dural sharing who underwent successful delayed elective separation at 6 months of age in a resource-limited tertiary care setting. Initial imaging revealed sacral deficiency, dural fusion, and spinal cord involvement without bony fusion. Surgery was postponed to allow nutritional optimization and adequate tissue grow

The functional significance of monozygotic and dizygotic twinning is explored. The specific question of why twinning is maintained in human populations, despite the risks to mothers and infants, will be addressed. Next, summaries of recent and current research will be presented. The topics cover congenital chimerism, parental favoritism toward one twin, parasitic twins, and a twin study of primal world beliefs. In the final section of this essay a variety of informative general interest stories are considered. They include twins' matching lives; the birth of three triplet sets; a pair of Chine

Conjoined twins are rare, occurring in 1 in 250 000 live births, with cephalopagus twins reported in 1 in 3 000 000 live births. In cases of conjoined twins, early diagnosis is crucial for the health of the mother and pregnancy management and counselling. We present a case of cephalo-omphalopagus conjoined twins, with imaging findings, delivery considerations and patient-centred care recommendations. A patient in her 30s, gravida 2 para 0, was transferred to our care after an anatomical ultrasound demonstrated fusion of the fetal heads, chests and abdomens. Due to a complex congenital heart de

This article presents an overview of the life histories of reared-apart twins from the Philippines. One twin always knew he was a twin, but for the other twin the discovery at age 15 came as a shock. This essay is followed by summaries of recent twin research of interest. Topics include twin discordance for multisystem inflammatory syndrome, in vitro fertilization (IVF) twins and possible associations with impaired hearing, the writings of monozygotic (MZ) twins, and an update on MZ twins with Feingold syndrome whom I have covered in a previous issue of this journal. This essay concludes with

Separation of conjoined twins is a rare and complex surgical endeavor with significant anesthetic implications, particularly in omphalopagus twins, given the risk of massive hemorrhage during hepatic transection and hemodynamic instability from cross-circulation. Blood management strategies include the use of tranexamic acid, viscoelastic monitoring, and preparation for rapid transfusion. We report the anesthetic management of the elective surgical separation of three-year-old female omphalopagus conjoined twins with shared hepatic parenchyma. Extensive multidisciplinary planning, simulation-b

Monochorionic diamniotic triplets with a conjoined pair are exceedingly rare and arise from multiple splitting events of a single zygote. This report presents the earliest documented ultrasound diagnosis of MCDA triplets with a conjoined pair, at 8 weeks and 5 days of gestation, in a patient who underwent in-vitro fertilization with a single embryo transfer. High-resolution transvaginal ultrasound revealed a monochorionic diamniotic configuration with two amniotic sacs: one containing a structurally normal embryo and the other containing di-thoracic parapagus conjoined twins. This case highlig

BACKGROUND: Craniopagus twins, conjoined at the calvarium, are the rarest and most complex form among conjoined twins, comprising ~3.4% of all cases. Surgical separation presents a significant challenge due to shared venous structure, brain parenchyma, and brain tissue. Despite advancements in surgical techniques, factors associated with successful separations remain poorly understood.
METHODS: A systematic literature review was conducted to identify all reported craniopagus separation cases from 1995 to 2025, using Ovid MEDLINE and Scopus. A supplemental search of news articles was additiona

BACKGROUND: Conjoined twins are univitelline, monochorionic, and monoamniotic, with imperfect division. The estimated worldwide prevalence is 1.47/100,000 births, with approximately 50% resulting in live births. They are classified according to the most prominent site of fusion. Pygopagus twins are joined at the sacral region and are the third most common variation, representing 6-19% of conjoined twins. Pygopagus twins may have a fused spinal canal and spinal cord, presenting a neurosurgical challenge during separation surgery.
CASE DESCRIPTION: We report the case of conjoined twins diagnose

INTRODUCTION: Conjoined twins present a rare but clinically challenging scenario requiring highly individualized pharmacologic strategies. Anatomical fusion, shared circulatory systems, and organ overlaps complicate drug absorption, distribution, metabolism, and excretion, including the neonatal intensive care unit setting and perioperative care. This review aims to synthesize current evidence and clinical experience regarding pharmacotherapy in CTs, focusing on drug selection, therapeutic drug monitoring and individualized dosing strategies based on anatomical and physiological variations.

We present a novel, dynamic magnetic resonance (MR) genitography protocol developed for pre-surgical assessment of pediatric genitourinary malformations. This protocol has been applied in patients with cloacal malformations, covered cloacal exstrophy, imperforate anus, urogenital sinus, and conjoined twins with a pelvic connection. Traditionally, a combination of imaging modalities has been used as each has its limitations. Our approach combines catheter placement during an endoscopic exam under anesthesia with real-time, intracavitary gadolinium-enhanced magnetic resonance imaging, allowing d

OBJECTIVE: Describe the golden hour and hospital outcomes of conjoined twins METHODS: Retrospective single center study of conjoined twins. Main outcome measures included delivery room characteristics. Secondary outcomes included survival and length of stay.
RESULTS: From 2013-2025, there were 19 sets of conjoined twins (active resuscitation in 13; palliative in 6). Respiratory interventions in the delivery room were frequently needed including continuous positive airway pressure (75%), positive pressure ventilation (67%), and endotracheal intubation (25%). There were two sets with emergency

Parasitic or asymmetric twins are a rare form of conjoined twins. Thoracopagus and omphalopagus are the most common types, whereas mentonian location remains exceptionally rare. We report the case of a male neonate, born to non-consanguineous parents, who presented with a parasitic twin attached to the left mentonian region, without communication with vital structures, and which was surgically excised. However, the autosite presents a complex congenital heart defect. This case illustrates an extremely rare presentation of parasitic twins located in the mentonian region and highlights the impor

Craniopagus twins are conjoined twins fused at the cranium. Separating these twins is a challenging undertaking, often associated with high levels of morbidity and mortality. Inadequate preoperative planning can lead to suboptimal outcomes, with survival rates reported at approximately 60%. A detailed multiparametric radiologic analysis is essential for understanding the complex anatomy involved in presurgical planning before the separation. Stratifying risk profiles and planning a multistaged separation, when appropriate, is vital to improving the chances of a successful separation. Performin

BACKGROUND AND OBJECTIVES: Total vertical craniopagus represent one of the most complex craniofacial malformations and a demanding surgical challenge. Traditional imaging remains static and insufficient to fully anticipate dynamic venous redistribution during staged separation. We implemented an integrated virtual and physical simulation framework in the multistep separation of total vertical craniopagus twins, within an Avatar Patient Approach as an evolution of the Digital Twin Medicine approach focused on controlled physiological decoupling and operative risk reduction.
METHODS: Multimodal

INTRODUCTION: Epigastric heteropagus (EH) is a rare congenital anomaly, and its management is further complicated by the presence of an omphalocele. We present a case of EH attached to a host with a large omphalocele, along with a review of previously reported cases. We believe it will contribute to increased insight and enable meaningful comparisons on the topic.
CASE PRESENTATION: A 15-hour-old, 3.4-kg male neonate was born at 38 weeks of gestation to a 26-year-old mother. At presentation, he was stable, with a partially formed parasitic twin attached to the epigastrium and an anterior abdo
