Behçet's syndrome incidence and prevalence in Sardinia: implications of a latent class analysis combining administrative and clinical data.

OBJECTIVES: To estimate the epidemiology of Behçet's syndrome (BS) in Sardinia using a combined administrative and clinical data latent class analysis (LCA). METHODS: Cases with a diagnosis of BS were retrieved from 2006-2016 in the Hospital Discharge Records (HDRs), 2006-2016 Rare Diseases Regional Register (RDRR), and 2014-2016 in the Specialist Outpatient Database (SOD). Medical records from regional rheumatology clinics were reviewed and classified by the International Criteria for Behçet's Disease (ICBD) and International Study Group (ISG) criteria. Statistical analysis involved cross-re

Predictors of Acute Exacerbations in COPD: A Systematic Review.

BACKGROUND: COPD is characterized by acute exacerbations (AECOPD), which drive its progression and burden. AECOPD can be caused by various factors. Better prediction of AECOPD provides opportunity for initiating preventive treatment. METHODS: This systematic review provides an evidence-based overview of clinical predictors of moderate, severe AECOPD and relapse of moderate and severe AECOPD (≤28 days of a previous AECOPD). Cohort studies, case-control studies, or (cluster) randomized controlled trials published in English between January 2011-December 2023 in PubMed, CINAHL, Embase, Web of Sc

Combined loss of histone methyltransferases GLP and G9a disrupts neural crest cell fate determination.

Down Syndrome (DS) is a common genetic disorder characterized by an extra copy of chromosome 21, leading to dysregulation of various metabolic pathways. Oxidative stress in DS is associated with neurodevelopmental defects, neuronal dysfunction, and a dementia onset resembling Alzheimer's disease. Additionally, chronic oxidative stress contributes to cardiovascular diseases and certain cancers prevalent in DS individuals. This study investigates the impact of ageing on oxidative stress and liver fibrosis using a DS murine model (Ts2Cje mice). Our results show that DS mice show increased liver o

The transcriptional landscape of developing human trisomy 21 lungs.

RATIONALE: Trisomy 21 (T21), resulting in Down syndrome (DS), is the most prevalent chromosomal abnormality worldwide. While pulmonary disease is a major cause of morbidity and mortality in DS, the ontogeny of pulmonary complications remains poorly understood. We recently demonstrated that T21 lung anomalies, including airway branching and vascular lymphatic -abnormalities, are initiated in utero. Here, we aimed to describe molecular changes at the single-cell level in prenatal T21 lungs. METHODS: Single-cell RNA sequencing was used to generate transcriptomic profiles of individual human lung

CPX-351 in Down syndrome-associated myeloid leukemia: results and prognostic factors from the phase 3 ML-DS 2018 trial.

Myeloid leukemia of Down syndrome (ML-DS) is associated with an excellent prognosis but high treatment-related toxicity and mortality. The Phase 3 Clinical Trial for CPX-351 in ML-DS 2018 aimed to maintain the excellent event-free survival (EFS) achieved in the previous ML-DS 2006 trial while reducing the treatment intensity. Intensity-reduced induction and reinduction therapy with cytarabine and idarubicin with or without etoposide was replaced with CPX-351 (66 U/m2 on 3 days in course 1 and on 2 days in course 2). Risk stratification was based on flow cytometric measurable residual disease (

Molecular risk markers define risk of relapse in myeloid leukemia of Down syndrome beyond measurable residual disease.

Myeloid leukemia of Down syndrome (ML-DS) is a distinct form of pediatric acute myeloid leukemia (AML) that responds to reduced-intensity chemotherapy, as compared with non-DS AML that requires intensive chemotherapy and often stem cell transplant. While most patients with ML-DS have a favorable prognosis, outcomes for those with refractory or relapsed disease are dismal. Children's Oncology Group study AAML1531 introduced the use of measurable residual disease by multiparameter flow cytometry at the end of the first course of induction therapy (EOI-1 MRD) for risk stratification of treatment

Paediatric pulmonary arterial hypertension following congenital heart defect repair: enhanced risk stratification and outcomes in a national cohort.

AIMS: Pulmonary arterial hypertension (PAH) is a common complication of congenital heart disease (CHD). PAH following CHD repair (repaired PAH-CHD) is an emerging population with increased morbidity and mortality. Existing PAH risk scores are unvalidated in young children with this condition. METHODS AND RESULTS: A 20-year cohort from the UK National Paediatric PH Service was studied to describe peri-operative characteristics and outcomes in children with repaired PAH-CHD and to create a tailored risk stratification tool. This tool was externally validated using the national Spanish PH (REHIP

Molecular Stratification of Antiphospholipid Syndrome Through Integrative Analysis of the Whole-Blood RNA Transcriptome.

OBJECTIVE: Antiphospholipid syndrome (APS) is a thromboinflammatory disorder characterized by clinical and mechanistic heterogeneity that complicates early diagnosis and hinders targeted treatment. We aimed to identify distinct molecular endotypes among antiphospholipid antibody (aPL)-positive patients using whole-blood transcriptomics. METHODS: Whole-blood RNA sequencing was performed on 174 aPL-positive patients, including those with primary APS (n = 102), secondary APS (n = 29), and aPL positivity without classifiable APS (n = 43). Unsupervised machine learning and immune cell deconvolutio

Atherosclerotic progression at sites of low shear stress is attenuated by myeloid-PlexinD1 deficiency through suppression of classical macrophage polarization.

BACKGROUND AND AIMS: Atherosclerosis preferentially develops at disturbed flow sites, where macrophage polarization critically determines plaque vulnerability. PlexinD1 may regulate this process by mediating M1 macrophage polarization under low and oscillatory shear stress (OSS). This study aims to investigate the role of macrophage PlexinD1 in OSS-induced atherosclerotic progression. METHODS: Plasma PlexinD1 was quantified in 72 patients with acute coronary syndrome (ACS) stratified by coronary bifurcation lesion involvement. Plaques in carotid bifurcations (exposed to OSS) and those in prox

Difference in Language Profiles of Children With Autism Spectrum Disorder and Down Syndrome Is Not Driven by Non-Verbal Cognition.

BACKGROUND: Autism Spectrum Disorder (ASD) and Down syndrome (DS) are among the most common types of neurodevelopmental conditions that have co-occurring language impairments. Usually, non-verbal IQ has been reported as one of the main predictors of language functioning in children with these conditions. Although language abilities of children with ASD and DS have been described in the previous studies, there is still a lack of direct comparisons of language profiles in the non-verbal IQ-matched groups of children with these disorders, and, therefore, it is largely unexplored whether language

Posterior Fossa Group A Ependymoma in Trisomy 21.

Brain tumors are the most common solid tumors in children. However, the risk of central nervous system tumors appears reduced in individuals with trisomy 21, particularly for tumors of nervous tissue. We present the first reported case of posterior fossa group A (PFA) ependymoma in a patient with trisomy 21. Including this case, only four cases of ependymoma in trisomy 21 have been reported. Similar to medulloblastoma, ependymoma may occur at a markedly reduced frequency in trisomy 21. Investigating the protective role of trisomy 21 in ependymoma development may provide new insights into its p

Frequency and face validity of reported family history of cancer in first-degree relatives and genetic syndromes among children with cancer in Project:EveryChild: A report from the Children's Oncology Group.

BACKGROUND: Taking a family history of cancer (FH) is essential for identifying individuals with heritable cancer predisposition. PROJECT: EveryChild (Children's Oncology Group [COG] trial APEC14B1), the registration and biobanking protocol of the COG, includes suggested questions about FH in first-degree relatives and personal history of genetic syndromes (GS) in pediatric oncology patients. The validity of these items is unclear; therefore, the authors assessed the data quality and face validity of the responses. METHODS: The authors analyzed case report forms regarding FH and GS of 30,157

Leveraging clinical sleep data across multiple pediatric cohorts for insights into neurodevelopment: the Retrospective Analysis of Sleep in Pediatric (RASP) cohorts study.

Sleep disturbances are prominent across neurodevelopmental disorders (NDDs) and may reflect specific abnormalities in brain development and function. Overnight polysomnography (PSG) allows for detailed investigation of sleep architecture, offering a unique window into neurocircuit function. Analysis of existing pediatric PSGs from clinical studies could enhance the availability of sleep studies in pediatric patients with NDDs towards a better understanding of mechanisms underlying abnormal development in NDDs. Here, we introduce and characterize a retrospective collection of 1527 clinical pedi

Bacterial Toxin Exploits Host Membrane Phospholipid as a Receptor for Binding, Entry, and Cytopathogenicity.

Mycoplasma pneumoniae is a leading cause of bacterial community-acquired pneumonia, responsible for severe respiratory and extrapulmonary diseases in children and adults. The Community Acquired Respiratory Distress Syndrome (CARDS) toxin is a key virulence factor that exerts ADP-ribosylating and vacuolating activities on host cells, recapitulating the inflammatory and histopathological damage seen in infected airways. Although the host proteins annexin-A2 (AnxA2) and surfactant protein-A were previously identified as toxin receptors, their absence did not abrogate CARDS toxin activity. Intrigu

Comparable epicardial adipose tissue burden in prediabetes with high cardiometabolic risk and early type 2 diabetes: A need for timely targeted therapies.

BACKGROUND: Epicardial adipose tissue (EAT) independently predicts adverse cardiovascular events. Prediabetes with metabolic syndrome carries high cardiovascular risk, yet EAT in this critical subpopulation versus incident type-2 diabetes (T2D) remains unexplored. METHODS: Coronary CT-angiography records were retrospectively reviewed to compare adults with prediabetes (impaired fasting glucose 110–125 mg/dL and HbA1c < 6.5%) having ≥ 3 of 4 cardiometabolic risk indicators (overweight/obese, hypertension, statin use/hypertriglyceridemia, and statin use/low HDL-cholesterol; n = 44) with drug-naï

APOBEC3C coordinates DDX5 in R-loop resolution and dynamic control of Chk1-mediated stress-responsive circuitry as a prerequisite for gemcitabine resistance in p53-deficient cells.

Genomic instability is a hallmark of cancer, encompassing both sequence and structural alterations that drive tumor evolution and heterogeneity. The APOBEC3 family of deoxycytidine deaminases has emerged as a major source of mutagenic activity in cancers. R-loops are RNA-DNA hybrids and structural barriers that interfere with replication and transcription. Among the APOBEC3 family, APOBEC3C (A3C) is particularly worthy of attention for its upregulation, driving the DNA replication stress tolerance in response to replication stress-inducing drug gemcitabine. However, the molecular mechanisms of
