Clonorchis sinensis-infected hepatocellular carcinoma exhibits distinct tumor microenvironment and molecular features.

OBJECTIVES: Clonorchis sinensis (Cs)-infected hepatocellular carcinoma (HCC) patients have a poorer prognosis than non-Cs-infected HCCs. However, the molecular mechanisms of Cs-infected HCC remain unclear. To address this, this study aims to uncover the tumor microenvironment and molecular features that may contribute to these poor outcomes. METHODS: The research involved bulk RNA sequencing of paired tumor and adjacent tissue samples from 10 Cs + HCC and 10 Cs - HCC patients. Differentially expressed genes were identified, followed by enrichment analyses to reveal functional changes. Surviva

Diagnostic potentials of whole and fragmented major egg protein for human schistosomiasis.

Heat shock proteins are known to be antigenic due to their high conservation. A 40 kDa heat shock protein, considered a major egg protein, was identified as a potential candidate antigen for diagnosing schistosomiasis. Consequently, this study aims to evaluate the diagnostic potential of SjMEP-40 for detecting human schistosomiasis. SjMEP-40 was fragmented into three distinct portions-SjMEP-40 N (N-terminal), SjMEP-40 M (middle), and SjMEP-40C (C-terminal)-to determine which segment provides the best diagnostic performance compared to the whole antigen (SjMEP-40 W) for both IgG and IgM detecti

Prevention and improved management of serious neurological adverse events during praziquantel-based mass drug administration in a Taenia solium endemic area: Experiences from Madagascar.

Mass drug administration (MDA) programs involving praziquantel are used in public health programs to control diseases such as schistosomiasis, taeniasis caused by Taenia solium, opisthorchiasis and clonorchiasis. Praziquantel is a systemically distributed anthelmintic drug also used to treat neurocysticercosis (NCC) caused by the larval stages of T. solium in the central nervous system. The doses of praziquantel used in MDA are low compared to those used for the treatment of NCC, but in people with latent NCC (without symptoms or signs), there is a potential risk of neurological adverse events

Clonorchis sinensis and Cholangiocarcinoma.

Clonorchiasis is a parasitic disease caused by Clonorchis sinensis, a trematode that inhabits the intrahepatic bile ducts of humans and mammals. C. sinensis is one of common food-borne trematodes, prevalent in East Asia including Korea. The International Agency for Research on Cancer reclassified C. sinensis as the Group 1 biological carcinogen of human cholangiocarcinoma (CCA). Evidence supporting the carcinogenicity of C. sinensis includes epidemiological studies showing increased prevalence and odds ratio (OR) of CCA in clonorchiasis patients, the development of CCA in experimental animals,

Formation of non-cholesterol gallstones in populations within clonorchiasis endemic regions is closely related to Clonorchis sinensis infection.

BACKGROUND: Numerous risk factors are linked to gallbladder stone disease (GBSD). Nonetheless, the relationship between Clonorchis sinensis (C. sinensis) and this condition remains to be clarified. METHODS: The antibody against C. sinensis in serum and the glucose, triglyceride, and cholesterol levels were investigated in 220 patients with GBSD and 251 controls. Bile components were analysed in patients with gallbladder polyps (GP, n = 18), gallstones (GS, n = 265), and GS combined with C. sinensis infection (GSI, n = 243). Additionally, the gallbladder ejection fraction (%E), residual gallbl

Research of the clinical features, risk factors, and surgical diagnosis of intramural stones in patients with gallbladder stones.

Crystals or stones within the gallbladder wall in patients with gallbladder stones (GBS) have been occasionally reported, but their clinical features and aetiology remain unclear. This retrospective study analysed 323 consecutive patients with GBS who underwent rigid choledochoscopic gallbladder-preserving cholecystolithotomy to determine the detection rate, clinical features, and potential risk factors of gallbladder intramural stones (IS). IS were found in 24.1% (78/323) of patients, characterised by distinct cholangioscopic findings, including stone shadows, yellow floating bands, or a comb

Clonorchis sinensis extracellular vesicles associated with Csi-let-7a-5p activate pro-inflammatory macrophages to induce biliary injury.

During Clonorchis sinensis (C. sinensis) infection, pro-inflammatory macrophages (M1 macrophages) are highly activated, yet their role in the disease remains poorly understood. Previous studies have demonstrated that extracellular vesicles from C. sinensis (CsEVs) can activate these macrophages, and inhibiting a specific miRNA (Csi-let-7a-5p) in CsEVs (InCsEVs) can reduce this activation. In the present study, liver macrophages in mice were removed using clodronate liposomes (Clodlip). Subsequently, different types of bone marrow-derived macrophages (BMDMs) were adoptively transferred into the

Establishing a predictive model for liver fluke infection on the basis of early changes in laboratory indicators: a retrospective study.

BACKGROUND: Hepatic clonorchiasis is one of the most prevalent foodborne parasitic diseases in China and is often overlooked because the initial symptoms are not obvious. In this study, a multivariate model for the early prediction of disease onset using laboratory test data from liver-fluke-infected patients was developed and validated. METHODS: Laboratory data from 147 liver-fluke-infected patients and 151 healthy control subjects were collected. Univariate logistic regression, Spearman correlation analysis, and collinearity diagnosis were used to screen for independent factors. A multivari

Identification of Clonorchis sinensis in bronchoalveolar lavage fluid and peripheral blood using next-generation sequencing in a patient with septic shock: a case report and literature review.

We present a case report of a 59-year-old male diagnosed with pancreatic cancer with multiple abdominal metastases, in whom metagenomic next-generation sequencing unexpectedly identified Clonorchis sinensis genomic sequences in both bronchoalveolar lavage fluid and peripheral blood specimens. Subsequent examination further confirmed the presence of viable C. sinensis eggs in the stool samples. The findings underscore the prognostic significance of enhanced diagnostic protocols for parasitic infections in oncological cases. This investigation demonstrates the clinical utility of molecular diagn

Determinants of Clonorchis sinensis infection and subsequent treatment: a qualitative study in Guangxi, China.

Clonorchiasis is caused by the ingestion of raw freshwater fish containing the metacercariae of Clonorchis sinensis. High burden is exerted by clonorchiasis in East Asia, especially in China. Treatment is compulsory to control the morbidity, while mass drug administration and selective drug administration are recommended in high endemic areas. Few qualitative studies have been implemented to explore the knowledge, attitudes, and practices (KAP) towards clonorchiasis as well as the treatment. In 2024, a qualitative cross-sectional study was conducted in two clonorchiasis-endemic villages of Bin

Hepatic ferroptosis induced by Clonorchis sinensis exacerbates liver fibrosis.

Clonorchis sinensis (C. sinensis) is a food-borne zoonotic parasite link to liver fibrosis and cholangiocarcinoma. Limited understanding of its mechanisms in causing liver fibrosis has impeded therapeutic advances for C. sinensis-induced liver lesions. Ferroptosis, a novel form of cell death involving iron overload and lipid peroxidation, exacerbates liver fibrosis; however, its role in C. sinensis infection remains unexplored. In this study, ferroptosis were detected in C. sinensis-infected C57BL/6 mice as well as in AML12 cells stimulated by C. sinensis excretory/secretory products (ESPs). 1

Clonorchiasis in China: Geospatial modeling of the population infected and at risk, based on national surveillance.

OBJECTIVES: Clonorchiasis is highly endemic in China. The unavailability of fine-scale distribution of population with infection and at risk hinders the control. METHODS: This study established Bayesian geostatistical models to estimate age- and gender-specific prevalence of Clonorchis sinensis infection at high spatial resolution (5 × 5 km2), based on the surveillance data in China between 2016 and 2021, together with socioeconomic, environmental and behavioral determinants. The population at risk and under infection, as well as chemotherapy need were then estimated. RESULTS: In 2020, popul

Absolute quantitative lipidomics of Clonorchis sinensis-infected rats reveals key alterations in metabolic pathways.

Clonorchis sinensis (C. sinensis) infection is a significant public health concern due to its association with chronic hepatobiliary diseases. However, the mechanisms underlying C. sinensis infection and its effects on host metabolism remain poorly understood. In this study, serum samples were collected from rats at 4 and 8 weeks post-infection (wpi), and lipid metabolites were analyzed using absolute quantitative lipidomics with UHPLC-MS/MS. Multivariate analyses were conducted to identify differentially expressed lipids, and pathway analysis was performed utilizing the MetaboAnalyst and KEGG

GalNAc-siRNA conjugates mediate the silencing of the parasite-derived lncRNA: A novel therapeutic approach for liver fluke-induced liver fibrosis.

Clonorchis sinensis, as a liver fluke, can cause chronic liver damage, liver fibrosis, and even cholangiocarcinoma (CCA). However, there is a lack of effective therapeutic approaches for the treatment of clonorchiasis. Thus, we aimed to investigate the role of long noncoding RNAs (lncRNAs) in liver fibrosis induced by C. sinensis and evaluate the therapeutic potential of GalNAc-modified siRNAs targeting these lncRNAs. In this study, we first isolated Clonorchis sinensis extracellular vesicles (CsEVs) and identified the fibrosis-related long noncoding RNA lnc28240.1 in the CsEVs. In vitro and i

Clonorchis sinensis infection remodels chromatin accessibility in hepatocellular carcinoma.

BACKGROUND: Hepatocellular carcinoma (HCC) is a major global health concern, accounting for a significant proportion of liver cancer cases and related deaths. Clonorchis sinensis (C. sinensis) infection, a recognized carcinogen, has been implicated in the progression of liver diseases, including HCC. However, the precise epigenetic mechanisms underlying C. sinensis-associated HCC remain to be elucidated. METHODS: To investigate the role of chromatin accessibility in C. sinensis-related HCC progression, we performed an assay for transposase-accessible chromatin with high-throughput sequencing

Crafting an innovative one health-aligned machine learning framework for neglected tropical diseases elimination.

INTRODUCTION: Strategic investment in new interventions is crucial for controlling and eliminating NTDs. However, selecting the optimal intervention combination is challenging, especially with limited resources and urgent disease transmission. Hybrid models offer a flexible framework for simulating intervention scenarios, but their use in NTDs control remains underexplored. OBJECTIVES: To propose a novel One Health-driven framework to guide and accelerate the elimination of NTDs. METHODS: The proposed simulation framework consists of three parts: the disease dynamics model (the white box), t

Multi-omics analysis of lactate metabolism gene regulation in Clonorchis sinensis-associated hepatocellular carcinoma.

BACKGROUND: Although recent research has highlighted lactylation, a post-translational modification driven by elevated lactate levels, as a critical regulator of key cellular pathways in hepatocellular carcinoma (HCC), its contribution to the poor prognosis of Clonorchis sinensis (Cs)-infected HCC remains poorly understood. METHODS: We first identified the significant upregulation of the lactate metabolism enzyme LDH in Cs-infected HCC patients through clinical retrospective analysis. We then conducted a multi-omics analysis (RNA-Seq, ATAC-Seq, WGBS-Seq, oxWGBS-Seq, and ChIP-Seq) to examine t
