Clonorchis sinensis-driven hepatocarcinogenesis via E2F1-CD24 transcriptional axis: mechanistic and therapeutic implications.

BACKGROUND: Hepatocellular carcinoma (HCC) remains a global health burden, with disproportionately high mortality in China's Guangxi region, where endemic Clonorchis sinensis (C. sinensis) infection coincides with elevated HCC incidence. Preliminary single-cell sequencing revealed marked overexpression of cluster of differentiation 24 (CD24) in HCC tissues, suggesting its potential pathological role. This study aims to elucidate the oncogenic mechanisms of C. sinensis excretory-secretory products (CsESPs) and their link to CD24-mediated HCC progression. METHODS: We employed an integrated clin

Assessing trends in clonorchiasis incidence via prescription data analysis in South Korea.

BACKGROUND: While most parasitic diseases are being eradicated, clonorchiasis remains endemic and continues to pose a significant public health burden in South Korea. The prevalence has been monitored through nationwide or endemic area surveys and sentinel surveillance systems, but surveys based on stool examinations are resource-intensive and unable to determine incidence rates and trends unless conducted nationwide. Indeed, the nation-wide surveys have not been conducted since 2012 and endemic area surveys and sentinel surveillance systems also have limitations. METHODS: We estimated the nu

Ivermectin Identified Using a High-Throughput Screening System Exhibits Anti-Clonorchis sinensis Activity in Rats.

Background: Clonorchiasis, caused by the parasite Clonorchis sinensis, remains a public health concern in East Asian countries. Methods: In this study, high-throughput screening was used to analyze 320 compounds for potential inhibitory activity against Clonorchis sinensis. To ensure the selection of high-confidence hits, a stringent inhibition threshold of 80% was applied, leading to the identification of three active compounds: moxifloxacin, hexachlorophene, and ivermectin (IVM). Ivermectin emerged as a hit compound and was assessed for its anti-C. sinensis efficacy. Results: Ivermectin demo

Current knowledge on the prevalence and detection techniques of Clonorchis sinensis in China.

BACKGROUND: Clonorchis sinensis is one of the most significant zoonotic food-borne parasites, particularly prevalent in China. The adult form of C. sinensis typically inhabits in the hepatic and biliary tracts of various mammals, including humans, and is considered a major contributor to the incidence of cholangiocarcinoma. The life cycle of C. sinensis is complex, understanding its prevalence among different host species is essential for developing prevention and management measures. Furthermore, the clinical manifestations of clonorchiasis are often overlooked or misdiagnosed. Therefore, the

Clonorchis sinensis dopamine transporter (CsDAT) facilitates dopamine uptake.

Clonorchis sinensis is a liver fluke that causes clonorchiasis, a significant public health concern in East Asia, closely associated with hepatobiliary diseases. Dopamine is an essential neurotransmitter involved in neuromuscular signaling, and its uptake by trematodes may contribute to parasite physiology and survival. This study aimed to characterize the dopamine transporter CsDAT in C. sinensis by synthesizing cDNA from adult worms and expressing it in Xenopus laevis oocytes; subsequently, uptake assays were conducted using radiolabeled dopamine. Functional assays confirmed that CsDAT media

A Data-Driven Mathematical Model for Clonorchiasis with Seasonality.

Clonorchiasis is a foodborne disease caused by parasites and transmitted to humans through intermediate hosts. Clonorchis sinensis parasitizes in the bile ducts of human liver and causes organ lesions. The cercariae and metacercaria of Clonorchis sinensis have seasonal variations and may be affected by high water temperature in summer. We formulate a partial differential equations (PDE) model which incorporates seasonality, spatial heterogeneity and the extrinsic incubation period (EIP) of the parasite. We present the basic reproduction number R 0 and discuss the global dynamics of the model.

Impact of Clonorchis sinensis infection on long-term survival after curative resection for hepatocellular carcinoma: A multicenter cohort study.

BACKGROUND: Hepatocellular carcinoma (HCC) prognosis is poor in East Asia. The impact of Clonorchis sinensis (C.sinensis) infection, a known carcinogen for cholangiocarcinoma, on HCC prognosis after curative resection in co-endemic regions is unclear. This study aimed to evaluate the independent association of C.sinensis infection with overall survival (OS) and recurrence-free survival (RFS) after curative HCC resection. METHODS: This retrospective, multicenter cohort study included 1386 patients undergoing R0 hepatectomy for HCC (2011-2021) in Guangxi, China (312 C.sinensis-positive, 1074 C.

Spatiotemporal epidemiological characteristics of human clonorchiasis in China.

This study explores the spatiotemporal distribution of Clonorchis sinensis infection rates across China using multiple spatial analysis methods, including spatial autocorrelation, hot spot analysis, kernel density estimation, and standard deviation ellipse analysis. By systematically reviewing historical literature and incorporating data from three national parasitic disease surveys, we identified significant clustering patterns and high-risk regions associated with the infection. The results show that infection rates were predominantly concentrated along major river systems, including the Don

Transcriptomic profiling of Kupffer cells exposed to Clonorchis sinensis extracellular vesicles: Unraveling the role of CSF3 in hepatic fibrosis.

Clonorchiasis, caused by Clonorchis sinensis, is a significant public health issue in China and East Asia. However, understanding its pathological mechanisms underlying this disease remains limited. Here, we isolated and characterized C. sinensis extracellular vesicles (CsEVs) and evaluated their uptake by Kupffer cells (KCs) in vitro in immortalized KC (ImKC) line and in vivo in a model of C. sinensis infection, investigating genes with altered gene expression after treatment with CsEVs and during progression to liver fibrosis. CsEVs were successfully taken up by KCs to regulate gene expressi

Clonorchiasis misdiagnosed as hepatic space-occupying lesion: a case report.

Clonorchis sinensis, a zoonotic parasitic infection, is contracted by consuming raw freshwater fish harboring infective larvae of the parasite. C. sinensis infection usually cause damage to the liver and biliary tract system. Chronic infection with C. sinensis can lead to long-term liver and bile duct damage, including hepatic fibrosis, and may increase the risk of developing cholangiocarcinoma (CCA). Nevertheless, a significant portion of infected individuals exhibit either asymptomatic or nonspecific, thereby contributing to a high rate of misdiagnosis. CA19-9 is a tumor-associated antigen t

A Novel Clonorchis sinensis Mitogenome: Elucidating Multiregional Strain Phylogeny and Revising the Digenean Mitochondrial Genome Tree.

Clonorchis sinensis, a parasitic liver fluke, is the primary aetiological agent of clonorchiasis, a disease predominantly characterized by liver-related clinical manifestations. Currently, research on the complete mitochondrial (mt) genome of local C. sinensis populations remains inadequate. Thus, in this study, we sequenced and annotated the mt genome of fish-borne C. sinensis (Cs-c2) from Changchun, Jilin Province, China, a strain not previously described. This mt genome is 14,136 bp in length and harbours 12 protein-coding genes (PCGs), 22 transfer RNAs (tRNAs), 2 ribosomal RNAs (rRNAs), an

Induction of TRIM22 during Clonorchis sinensis infection triggers intracellular ROS accumulation by suppressing the mTOR-mediated Nrf2 signaling pathway.

BACKGROUND: Clonorchis sinensis infection triggers various hepatobiliary complications, including epithelial hyperplasia, chronic inflammation, periductal fibrosis, and cholangiocarcinogenesis through direct contact with worms and their excretory-secretory products (ESPs). We previously profiled differential transcriptome expression from three-dimensional cholangiocyte (H69 cells) spheroids constitutively and repetitively exposed to ESPs using microarrays and RNA-seq analysis. TRIM22 was upregulated in response to ESP exposure. OBJECTIVE: This study aimed to elucidate the pathophysiological m

Alterations in the gut microbiota of alcoholic cirrhosis patients infected with Clonorchis sinensis in the Pearl River Delta region of China.

Clonorchiasis, a foodborne parasitic disease caused by Clonorchis sinensis (C. sinensis), is prevalent in certain regions of Asia and can result in severe hepatobiliary complications, including cholangiocarcinoma, peribiliary fibrosis, and hepatic fibrosis. In certain regions of China, the concurrent consumption of raw freshwater fish and alcohol, which are components of the local dietary culture, has contributed to the high prevalence of this disease. Infected individuals often endure the dual burden of clonorchiasis and alcoholic liver disease (ALD). While both C. sinensis infection and alco

Parasitic Gallbladder Infection: Case Study of Biliary Clonorchiasis in a Non-Endemic Region.

BACKGROUND Clonorchis sinensis is a common liver fluke that typically inhabits the bile ducts, with infections primarily reported in endemic regions such as Northeast China, Korea, Taiwan, and Vietnam. Diagnosing parasitic infections of the biliary system can be challenging, particularly because their clinical presentation often overlaps with that of more common conditions such as acute or chronic cholecystitis. In regions where such parasites are not endemic, these infections are often overlooked, potentially leading to delayed or inappropriate treatment. Increased awareness is therefore esse

A case of hypovascular hepatic lesion with suspected clonorchiasis.

Clonorchiasis, caused by Clonorchis sinensis, is a parasitic liver disease endemic in East Asia, primarily transmitted by consuming raw or undercooked freshwater fish. While clonorchiasis is often asymptomatic, chronic infection may lead to serious complications, including cholangiocarcinoma. In Japan, the prevalence has markedly declined, yet sporadic cases persist, especially in regions with traditional dietary practices. We report a rare suspected case of hepatic clonorchiasis in a Japanese woman in her 70s. She presented with mild liver dysfunction and no overseas travel history. Routine u
