Intellectual disability (ID) affects approximately 1%-3% of the population and spans diverse clinical presentations with marked genetic heterogeneity, especially in consanguineous populations where autosomal-recessive ID is common. Despite advances in diagnostic methods, ~50% of individuals with ID remain without a molecular diagnosis. Genome sequencing (GS) can detect variant classes poorly captured by other methodologies, including deep intronic splice changes and structural variants. We performed short-read GS on 38 Iranian autosomal recessive intellectual disability (ARID) families that re

Escalating geopolitical instability and the persistent risk of radiological or nuclear incidents highlight an urgent need for resilient global radiation emergency preparedness. In such crises, rapid and reliable biological dose assessment is indispensable for triage, therapeutic stratification, and long-term health surveillance. While the dicentric chromosome assay remains the gold standard for biodosimetry, its labour-intensive nature, limited throughput, and variability in implementation constrain large-scale responsiveness. Emerging approaches, including DNA-DSB repair foci analysis and G0-

Familial glucocorticoid deficiency type 2 (FGD2) is a rare autosomal recessive disorder caused by pathogenic variants in the MRAP gene, typically characterized by isolated cortisol deficiency with markedly elevated ACTH levels. We report a term male neonate born to consanguineous parents who presented with striking scrotal hyperpigmentation at birth, despite an otherwise normal appearance and a normal newborn screening limited to classical congenital adrenal hyperplasia (CAH). Initial biochemical evaluations, including glucose and electrolytes, were unremarkable; however, due to severe hyperpi

Accurate calibration of biological dosimetry methods is crucial for reliable radiation dose assessment in both medical and research contexts. However, discrepancies in calibration coefficients (α, β, c) across various studies impede standardization and the comparability of dose-response curves. This study conducted a systematic analysis of calibration data from 62 studies covering different radiation types (60Co gamma rays, 137Cs gamma rays, and X-rays) with varying energies and dose rates (0.00196-23.85 Gy/min), to explore the sources of variability and their effects on dicentric chromosome y

Chromosomal triplications are rare structural variations often associated with complex phenotypes. We report the molecular characterization of a novel intrachromosomal triplication at 18q12.1q21.2 identified in a fetus with ultrasound abnormalities. Conventional karyotyping and array-CGH revealed a partial tetrasomy and a 26 Mb region of loss of homozygosity (LOH), extending from the triplication to the telomere. Long-read sequencing (LRS) identified breakpoint junctions revealing a direct-inverted-direct triplication structure. Breakpoint analysis suggested that this rearrangement arose throu

TFE3-rearranged renal cell carcinoma (TFE3-RCC) is an aggressive kidney cancer driven by oncogenic TFE3 fusion transcription factors, yet the molecular machinery that enables these fusions to reprogram transcription and drive tumor growth remains poorly defined. Here, we identify the Cyclin C-CDK8/19 Mediator kinase module as an essential co-regulator of TFE3 fusion driven transcriptional programs and tumorigenesis. Inducible expression of PRCC-TFE3 in HK-2 cells, immortalized from normal renal epithelial cells, triggered a robust oncogene-induced senescence (OIS) phenotype. Using OIS as a fun

Pelizaeus-Merzbacher-like disease type 1 (PMLD1), also known as hypomyelinating dystrophy 2, is a typical slowly progressive hypomyelinating leukodystrophy (HLD) that manifests in the neonatal period or early infancy. In humans, PMLD1 is inherited as an autosomal recessive trait, with genetic variants identified in the gap junction protein gamma 2 (GJC2) gene. Six juvenile-age Toy Poodle dogs exhibited coarse, vertical head and trunk tremors from immediately after birth. Conventional magnetic resonance imaging revealed uniform signal hyperintensities throughout the subcortical white matter in

OBJECTIVE: To investigate the causes of false-negative results in noninvasive prenatal testing (NIPT) and provide insights for technical optimization and genetic counseling in clinical practice.
METHODS: We retrospectively analyzed clinical and genetic data from three cases with false-negative NIPT results. In case 1, amniocentesis was performed due to fetal growth restriction (FGR) and a single umbilical artery (SUA). In case 2, a female infant was delivered at 35 weeks' gestation for FGR and vasa previa, with postnatal manifestations including neonatal respiratory distress syndrome, low bir

Facioscapulohumeral dystrophy (FSHD) is primarily associated with contraction of the D4Z4 macrosatellite array at the 4q35 locus. Unaffected individuals carry 11-150 D4Z4 repeats, whereas ∼95% of FSHD patients (FSHD1) exhibit a contraction to 1-10 units, along with reduced DNA methylation. In another ∼3% of patients (FSHD2), the disease results from a digenic mechanism associated with the presence of a pathogenic variant in the SMCHD1 gene, leading to the epigenetic deregulation of the 4q35 locus. However, 1%-2% of clinically diagnosed patients lack a defined genetic cause, highlighting diagno

PURPOSE: Neurofibromatosis type 2-related schwannomatosis (NF2-SWN) is an autosomal dominant tumor syndrome with complete penetrance and variable expressivity. Underlying patterns of disease burden and severity are largely unexplained.
METHODS: We comprehensively phenotyped 168 NF2-SWN patients over a mean duration of 4.5 years. We used a custom sequencing panel of NF2 and schwannomatosis genes to identify germline (n=166) and somatic variants in tumors (n=37). An optimized composite severity (CSS) score based on clinical and radiological data was created to analyze the effect of genetic vari

Cytogenetic studies in populations chronically exposed to mutagens are challenging due to a number of methodological constraints. The aim of the study was to demonstrate the applicability of dicentric assay, combined with advanced data interpretation, for radiation biodosimetry in chronically exposed medical staff. Dicentric frequencies were measured in cultured blood lymphocytes from 12 interventional radiologists with 5-42 years of professional exposure and 14 unexposed controls. The data processing methodology included a mathematical "unfolding" of observed aberration yields, a robust linea

Nuclear pore complexes (NPCs) mediate selective transport between the nucleus and the cytoplasm but also contribute to maintaining genome stability. Mutations in NPC genes cause genome instability and sensitivity to DNA-damaging agents, and DNA that is difficult to repair or replicate relocates to NPCs, including expanded CAG repeats, which are associated with several neurological diseases. Here, we show that other disease-related short tandem repeats also relocate to NPCs. Relocation depends on the NPC basket protein Nup1, but is independent of the rest of the basket. Abrogating relocation to

BACKGROUND: Inversions involving intron 22 (Inv22) of the F8 gene are detected in approximately 45% of all severe hemophilia A (HA) patients. Digital droplet polymerase chain reaction using milepost assays and multiplex ligation-dependent probe amplification (MLPA) allows robust diagnosis of inversions, copy number variations, and more complex rearrangements in F8.
OBJECTIVES: A total of 350 HA patients and 280 of their mothers from the Hemophilia Inhibitor Genetics Study cohort were analyzed to identify additional F8 mutations.
METHODS: Digital droplet polymerase chain reaction and MLPA wer

Sotos syndrome is a rare autosomal dominant disorder characterized by childhood overgrowth, including tall stature and macrocephaly, distinctive facial appearance, and neuropsychiatric manifestations. These features include intellectual disability and a broad spectrum of behavioral disturbances. Traumatic brain injury (TBI) is also known to cause various neuropsychiatric symptoms, including behavioral disturbances, even in individuals without underlying medical conditions. However, the influence of pre-existing neurodevelopmental disorders such as Sotos syndrome on behavioral outcomes followin

Ewing sarcoma (EwS) is characterized by chimeric fusions such as EWSR1::FLI1. Chromosome gain or loss is prevalent in EwS, and this chromosome instability (CIN) phenotype correlates with poor treatment outcomes. We previously showed that CENP-A, a centromere-specific histone H3 variant, interacts with EWSR1 via its SYGQ2 motif, an interaction extending to the EWSR1::FLI1 fusion protein. CENP-A is highly expressed in EwS cells, driven by EWSR1::FLI1. EwS cells exhibit significant CIN, mitigated by depleting EWSR1::FLI1 or CENP-A, and frequently assemble neocentromeres. Using CENP-A ChIP-seq, we

Complex genome rearrangements are intriguing because it remains unclear whether they occurred gradually or suddenly, and what promoted or impeded their development. We started by asking whether double loss of heterozygosity (dLOH) involving two different chromosomes is simply the result of two independent single LOH events. We examined hundreds of vegetatively growing diploid yeast strains with gene deletions known to stimulate LOH through different mechanisms. In dozens of these strains, dLOH mutants were overrepresented by tens or even hundreds of times. Interestingly, the (deleted) genes th

BACKGROUND: Dual molecular diagnoses, defined as the coexistence of pathogenic variants in two distinct disease-causing genes, challenge the traditional single-gene model of Mendelian inheritance. With the advent of whole-exome sequencing (WES), such complex genotypes are increasingly recognized.
OBJECTIVE: To investigate the clinical and genetic spectrum of dual rare genetic diseases in pediatric patients and to assess the diagnostic utility of WES in detecting diverse variant types.
METHODS: A retrospective analysis was conducted on 872 children with suspected rare genetic disorders who un

The RAD51 recombinase is central to repair of DNA damage arising from stalled or collapsed replication forks and DNA double strand breaks. Its essential role is revealed by the fact that this function evolved in bacteria but was retained in eukaryotes. In humans some of the RAD51 functions have been relegated to several paralogues which evolved by gene duplication. In addition to mutations, most cancers are also characterized by increased chromosomal instability manifesting as translocations, deletions, insertions, and other more complex forms of chromosomal re-arrangements. Given the central

Occupational exposure to ionizing radiation remains an important radiation protection concern among radiology workers. This study assessed cytogenetic damage among radiology workers in Ghana using the dicentric chromosome assay and cytokinesis-block micronucleus assay alongside routine thermoluminescent dosimeter (TLD) records. Eleven occupationally exposed workers and thirteen controls were recruited. No dicentric chromosomes were observed in either group. The mean annual occupational TLD dose was 3.89 ± 2.50 mSv, below the International Commission on Radiological Protection recommended occup
