Novel Splice Site Variant in ARFGEF2: Expanding the Phenotypic Spectrum of Periventricular Heterotopia.

Periventricular heterotopia (PH) is a rare developmental cause of intellectual disability (ID). One form is autosomal recessive PH with microcephaly (ARPHM), characterized by developmental delay and learning difficulties, with onset in infancy, plus recurrent seizures that onset in mid-adolescence. It is caused by biallelic variants in ARFGEF2. We report a consanguineous Pakistani family with three sibs afflicted with moderate/severe ID, developmental delay, muscle hypotonia, dystonia and contracture of distal limb joints, flat feet, and variable behavioral abnormalities. Single-nucleotide pol

Integrated Approaches in Drug Repositioning Highlight Ouabain and Helenalin as Potential Drug Candidates for Pulmonary Fibrosis.

BACKGROUND: Acute intermittent porphyria (AIP), an autosomal dominant inborn error, results from the half-normal activity of the heme biosynthetic enzyme hydroxymethylbilane synthase (EC 4.3.1.8; HMB-synthase). This disease is characterized by acute, life-threatening neurologic attacks that are precipitated by various drugs, hormones, and other factors. The enzymatic and/or biochemical diagnosis of AIP heterozygotes is problematic; therefore, efforts have focused on the identification of HMB-synthase mutations so that heterozygotes can be identified and educated to avoid the precipitating fact

All-optical microwave signal generation based on a DFB laser self-injection-locked to a fiber Fabry-Perot resonator.

Klein-Waardenburg syndrome or Waardenburg syndrome type 3 (WS-III; MIM 148820) is characterized by the presence of musculoskeletal abnormalities in association with clinical features of Waardenburg syndrome type 1 (WS-I). Since the description of the first patient in 1947 (D. Klein, Arch Klaus Stift Vererb Forsch 1947: 22: 336-342), a few cases have been reported. Only occasional families have demonstrated autosomal-dominant inheritance of WS-III. In a previous report, a missense mutation in the paired domain of the PAX3 gene has been described in a family with dominant segregation of WS-III.

Preimplantation genetic testing for neurofibromatosis type 1: molecular genetic aspects and impact on reproductive counseling.

STUDY QUESTION: How do the genetic complexities of neurofibromatosis type 1 (NF1) impact reproductive counseling, preimplantation genetic testing (PGT) design, and PGT treatment? SUMMARY ANSWER: We established association between both incidence and tissue mosaicism with multiple exon deletions and specific single-nucleotide variants (SNVs) in neurofibromin 1 (NF1), a clinical actionable finding that we structured as a flowchart outlining challenges in and an approach for reproductive counseling, PGT design, and PGT treatment for NF1. WHAT IS KNOWN ALREADY: NF1 has a prevalence of 1 in 2500-3

Genetic differences in familial adenomatous polyposis syndrome in a Hungarian population: A prospective single center study.

BACKGROUND: Familial adenomatous polyposis (FAP) is a disorder of autosomal dominant inheritance that is responsible for around 1% of colorectal cancer (CRC) cases. AIM: To determine the mutation profile of FAP-specific to the Hungarian population. METHODS: This prospective single-center study enrolled patients with clinically suspected FAP or attenuated FAP (aFAP). Whole-exome next-generation sequencing was performed to detect variants of 50 FAP priority genes and 173 CRC predisposing genes or other CRC disease-associated genes. To identify larger deletions and insertions, a multiplex ampli

Identification of a Novel Pathogenic ABCC6 Mutation Through Familial Genetic Analysis in Pseudoxanthoma Elasticum: A Case Report.

This study conducted genetic analysis on a patient clinically diagnosed with pseudoxanthoma elasticum (PXE) and their family to identify pathogenic mutations in the ABCC6 gene and analyze its inheritance pattern. We performed whole-exome sequencing of the ABCC6 gene on peripheral blood samples from the proband, his parents, and his sister, with validation via Sanger sequencing. Results revealed two heterozygous mutations in the proband: a previously reported missense mutation c.3412C>T (p. Arg1138Trp) and an unreported frameshift deletion mutation c.3160_3161del (p. Thr1054Glyfs*2). Both varia

Clinical and molecular characteristics of constitutional mismatch repair deficiency syndrome: a case series of five children and appraisal of diagnostic guidelines.

DNA mismatch repair (MMR) is critical for maintaining genome integrity through correction of single-base mismatches and insertion-deletion loops arising from DNA replication. Heterozygous germline alteration of MMR genes (MSH2, MSH6, MLH1, PMS2) cause autosomal dominant Lynch syndrome (LS), most commonly manifesting as colonic or endometrial cancers, although brain, ovarian, and other organ systems may be involved. Neoplasia in LS usually arises after the age of 30 years. Constitutional mismatch repair deficiency (CMMRD) is inherited in an autosomal recessive manner due to biallelic germline a

Cystinuria in an Australian Cattle Dog Family-A Seemingly Androgen-Associated Autosomal Dominant Trait.

In Australian Cattle Dogs (AUCDs), cystinuria was reported to be an autosomal dominant trait caused by a 6 bp deletion in the SLC3A1 gene (type II-A). Here we report an androgen association in this breed. A family of 11 adult AUCDs (five intact and one neutered male and five females) was genotyped for the SLC3A1 c.1095_1100del variant, and urine was examined for concentrations of cystine, ornithine, lysine, and arginine (COLA). Among this family, three males and five females tested heterozygous for the mutation, while all other AUCDs were homozygous for the wild-type allele. The two heterozygo

Is 7p14.1 an orofacial cleft risk locus? Genome-wide study of copy number variation in multiple populations provides both a replication of previous studies and an alternative explanation.

OBJECTIVE: Our understanding of the genetic causes of non-syndromic orofacial clefts (OFCs) is based largely upon genetic studies of common and rare nucleotide variants. Less is known about the role of copy number variations (CNVs) and the studies published to date have been limited to either small samples or targeted genomic regions. The objective of our study is to investigate the contribution of CNVs spread across the entire genome to OFC risk in a large multi-ancestry cohort. METHODS: We utilized PennCNV on microarray genotyping data to detect CNVs in 10,240 participants (2,484 with cleft

Identification of a novel mutation in metabotropic glutamate receptor 1 causing autosomal recessive spinocerebellar ataxia-13 in a Pakistani family.

BACKGROUND: Autosomal recessive spinocerebellar ataxia-13 is a rare multifactorial disorder characterized by physical disability, cerebellar ataxia, adaptive behavior, intellectual disability, response to sudden noise and light, sensation, skeletal, and oculomotor abnormalities. It affects both males and females equally. So far, only seven variants have been identified in the glutamate metabotropic receptor 1 ( GRM1 ) gene in 18 familial cases of Roma, Iranian, Pakistani, and Tunisian origins. METHODS: In the present study, we have investigated a large consanguineous family of Pakistani origi

A novel EVC2 splice-site variant expands the mutational and phenotypic spectrum of Weyers acrofacial dysostosis.

BACKGROUND: Weyers acrofacial dysostosis (WAD) is a rare autosomal dominant ciliopathy caused by heterozygous pathogenic variants in the EVC2 gene. The classic phenotype includes short stature, dental anomalies, and nail dysplasia. To date, all reported causative variants are truncating mutations located within the last exon (exon 22). In contrast, pathogenic variants in other regions, particularly splice-site variants, remain poorly characterized. The co-occurrence of WAD and epilepsy has rarely been documented. METHODS: We performed exome sequencing in a proband with comorbid developmental a

Neurofibromin 1 (NF1) Splicing Mutation c.61-2A>G: From Aberrant mRNA Processing to Therapeutic Implications In Silico.

The neurofibromin 1 (NF1) splice-site mutation c.61-2A>G (rs1131691100) is a rare, pathogenic, autosomal dominant variant that disrupts NF1 tumor-suppressor function, causing neurofibromatosis type 1 (NF1). Its pathogenic mechanism is poorly understood, and the potential for personalized therapeutic genome editing remains unknown due to the absence of a standard framework for investigating splicing disorders. Here, we performed a comprehensive multi-omics analysis of a de novo c.61-2A>G case from South Korea, integrating short- and long-read whole genome sequencing, whole transcriptome sequenc

Ageing Through the Looking-Glass: The Different Flavours of Clonal Haematopoiesis.

Clonal haematopoiesis (CH) is the presence of acquired mutations in blood cells and is a consequence of ageing that is linked to malignancy, cardiovascular disease and other diseases of ageing. CH is a reflection of genomic instability with ageing; however, there is evidence that CH may exacerbate features of normal ageing, including inflammageing and immunosenescence, and more directly contribute to disease causation. CH can manifest as mosaic loss of X or Y, autosomal mosaic chromosomal rearrangements, or point mutations or small insertions or deletions. Until recently, little has been known

Alternative Translation Initiation in PRKN Delays the Onset of Parkinson's Disease and Offers a Therapeutic Target.

OBJECTIVE: Biallelic variants in PRKN cause autosomal recessive Parkinson's disease (PD) with a median age at onset of 31 years. When evaluating the 16 previously published carriers of a homozygous deletion of Exon 2 from the International Parkinson's Disease and Movement Disorder Society Gene Database (MDSGene) database, the median age at onset is later (39.5 years) than in carriers of other PRKN pathogenic variants. We investigated whether these carriers show delayed disease onset compared with carriers of other pathogenic PRKN variants and explored the underlying molecular mechanism. METHO

Successful Surgical Sperm Extraction in a Patient With Cystinosis.

Cystinosis is a genetic lysosomal storage disease with an autosomal recessive inheritance pattern. The mutation causing this disease occurs in the CTNS gene, which is located on chromosome 17 and codes for the lysosomal cystine transporter cystinosin. Cystinosis is characterized by intracellular accumulation of cystine in various organs and tissues, which may lead to impairment of organ function. Kidney injury is characterized by a progressive decline in glomerular filtration rate, leading to end-stage kidney disease if not treated. Other manifestations of cystinosis include ocular abnormaliti

Monogenic Etiologies of Kidney Cysts in the Pediatric Population: An Observational Cohort Study.

KEY POINTS: In a cohort of children, 79% of families with kidney cysts had a monogenic diagnosis. Resolved cases were more likely to have a family history of kidney cysts and unresolved cases more often had unilateral cysts. Broad genetic testing revealed genetic diversity and informed prognosis and clinical management in childhood kidney cysts. BACKGROUND: Pediatric kidney cysts may indicate an underlying genetic disorder, yet the full spectrum of causes remains incompletely defined. With advances in genetic testing enabling broader evaluation, this study assessed the diagnostic utility of c

A Partial UMOD Deletion Results in Altered Uromodulin Synthesis and Autosomal-Dominant Tubulointerstitial Kidney Disease-Uromodulin.

Autosomal dominant tubulointerstitial kidney disease-uromodulin (ADTKD-UMOD) is classically due to missense variants in UMOD that display a dominant-negative effect. Here, we describe a family with high clinical suspicion for ADTKD-UMOD for which no molecular diagnosis was obtained through standard techniques including extensive panel sequencing. Whole genome and long-read sequencing in multiple affected family members uncovered a large deletion (1,313 bp) encompassing part of exon 3, the entire intron 3 and exon 4, and part of intron 4, conserved regions of UMOD in which most known pathogenic

Development of a quantitative real-time PCR-based newborn screening system for citrin deficiency using dried blood spots.

BACKGROUND: Citrin deficiency is an autosomal recessive metabolic disorder caused by pathogenic variants in SLC25A13, which manifests as an age-dependent spectrum ranging from neonatal intrahepatic cholestasis (NICCD) to adolescent and adult citrin deficiency (AACD, formerly CTLN2). Early detection through newborn screening (NBS) would be a prerequisite for early treatment, but conventional NBS based on citrulline and other metabolite levels has only limited sensitivity. This study aimed to develop a simple and accurate quantitative real-time PCR (qPCR)-based newborn screening (NBS) method usi

High cancer-associated mutational burden in normal blood of xeroderma pigmentosum group C, but not groups A, D, or F.

Xeroderma pigmentosum (XP) is a rare autosomal recessive congenital syndrome characterized by defective nucleotide excision repair (NER), leading to extreme photosensitivity and a strong predisposition to skin cancer. One of the 8 complementation groups, XP complementation group C (XP-C), is defective specifically in the global genome component of NER and presents not only with increased skin cancer but also with hematologic cancers. Using error-corrected single-molecule sequencing, we show a uniquely high spontaneous somatic mutational load in peripheral blood mononuclear cells (PBMCs) of pat
