Systemic Loss of Chromosome Y Associates With Age-Related Ophthalmic Diseases in Middle-Aged Men: A Longitudinal Multi-Omic Study.

Maintaining health during global population aging represents a critical challenge, where mosaic loss of chromosome Y (LOY) in the hematopoietic system surfaces as a prevalent somatic mutation in aging men. However, its longitudinal impact on sensory organ degeneration remains insufficiently characterized. Analyzing 223,051 male participants from the UK Biobank, we demonstrated that LOY uniquely and independently predicted an increased risk of incident age-related macular degeneration (AMD) (Hazard Ratio = 0.611, 95% Confidence Interval: 0.397-0.942). Restricted cubic splines identified a criti

The Diverse Clinical Spectrum of Noonan Syndrome in Pediatric Endocrinology: From Classical to Atypical Phenotypes.

Background/Objectives: Noonan syndrome (NS) is characterized by marked phenotypic variability. Although the classical phenotype includes short stature, craniofacial dysmorphism, skeletal abnormalities, and congenital heart defects, some patients present with subtle or atypical manifestations, making clinical recognition challenging. Methods: This retrospective case series included six pediatric patients with molecularly confirmed NS managed at the Department of Endocrinology and Metabolic Diseases, Polish Mother's Memorial Hospital-Research Institute, Lodz, Poland. Medical records were reviewe

Waist circumference outperforms cardiometabolic indices for identifying hypogonadism in Klinefelter Syndrome.

CONTEXT: Klinefelter syndrome (KS) is characterized by primary hypogonadism and increased cardiometabolic risk. The utility of cardiometabolic indices remains unclear. OBJECTIVE: To evaluate whether anthropometric and cardiometabolic indices provide incremental value beyond age for identifying hypogonadism in men with KS. DESIGN AND SETTING: Retrospective cross-sectional cohort study at a tertiary referral center. PATIENTS: 580 patients with KS were evaluated. Analyses assessing biochemical hypogonadism were restricted to untreated patients with available calculated free testosterone (cFT)

Case Report: Relapse of SLE associated with EBV infection in a patient after CAR-T cell therapy: a clinical case.

We present the case of a 13-year-old boy with refractory systemic lupus erythematosus (SLE) who achieved drug-free remission after anti-CD19 CAR-T cell therapy, but subsequently experienced Epstein-Barr virus (EBV) reactivation on day 142 after CAR T-cell infusion and peak EBV load was 24,400 IU/mL with a severe drop in platelets to 8×109/L, which leading to relapse of SLE and manifestations of chronic active EBV disease (CAEBVD). The whole exogenous gene testing found X chromosome Xp22.33q28 region (chrX: 200855-155239820) a duplicate of about 155.04Mbp,which is associated with the developmen

A rare case report: atypical presentation of Klinefelter syndrome in a 24-year-old male at suburban tertiary care hospital in Nepal.

Klinefelter syndrome (KS) is the most common chromosomal condition affecting males, usually caused by the presence of an extra X chromosome (47, XXY). However, it often remains undiagnosed until adulthood. We report a unique case of a 24-year-old man whose primary concern was persistent nausea and vomiting, an atypical presenting complaint of Klinefelter syndrome. Further evaluation revealed typical features of KS, including infertility despite normal sexual desire, gradual enlargement of the breasts, tall stature, sparse facial and body hair, small testes, and learning difficulties. An uncomm

When an Extra X Means Extra Risk: Venous Thromboembolism in Klinefelter Syndrome.

Klinefelter syndrome (KS) is a common but frequently underdiagnosed sex chromosome aneuploidy that confers a markedly elevated risk of venous thromboembolism (VTE), yet it remains absent from major anticoagulation guidelines. We present a 41-year-old man with known KS on testosterone replacement who developed massive bilateral pulmonary embolism with concurrent deep vein thrombosis, presenting with syncope, dyspnea, and right heart strain on imaging. Hematocrit was 48.8% at admission, below the erythrocytosis threshold despite ongoing testosterone therapy. Serial laboratory data over 13 months

Multidimensional Determinants of Quality of Life in Men With Klinefelter Syndrome: A Narrative Review of Workshop Evidence.

BACKGROUND: Klinefelter syndrome (KS), typically characterized by a 47, XXY karyotype, is the most common sex chromosomal aneuploidy in males, with a prevalence of approximately one in 600-660 individuals. Despite its frequency, the syndrome remains significantly underdiagnosed, with 50%-75% of affected individuals never receiving a clinical diagnosis. While the classic phenotype includes hypergonadotropic hypogonadism and infertility, recent evidence highlights a lifelong burden of neurocognitive, psychological, and socioeconomic challenges. OBJECTIVES: This study synthesizes current evidenc

Growing up with Klinefelter syndrome: challenges of care in children and adolescents and patients advocacy.

Klinefelter syndrome (KS) is a common, yet underdiagnosed sex chromosome aneuploidy characterized by considerable phenotypic variability and multisystem involvement beginning in early life. The increasing use of noninvasive prenatal testing has led to earlier diagnosis, highlighting the need to redefine clinical management across the pediatric age. In this narrative review, pediatric endocrinologists, neuropsychiatrists, psychologists, and genetic experts critically examine current evidence on genetic mechanisms, growth, pubertal development, cardiometabolic risk, and neuropsychological featur

WFS1 Deficiency Impairs PIAS4-Associated SUMOylation and Increases Ubiquitin-Mediated Spermatogenesis-Related Protein Degradation Leading to Testicular Male Infertility.

Although hundreds of genes have been shown to impair fertility in animal models, the identification of corresponding genetic causes in humans remains largely unknown. Here, we identified biallelic pathogenic variants in WFS1 in three unrelated infertile men presenting with oligoasthenoteratozoospermia and fertilization failure. Remarkably, using the establishment of a Wfs1 knockout (KO) mouse model, we found that WFS1 deficiency leads to disorganized sperm mitochondrial sheath assembly, structural abnormalities in the sperm head-neck junction, and defects in the sperm acrosome, which are consi

A dynamic "hormonal tug-of-war" driven by a functioning ovarian remnant after testis-preserving partial gonadectomy in ovotesticular disorders of sex development: a case report.

The endocrine behavior of residual ovarian tissue following testis‑preserving surgery in ovotesticular disorders of sex development (DSD) during adolescence remains poorly characterized. We report an adolescent male who presented with Tanner stage B4 gynecomastia despite male‑typical external genitalia. Baseline evaluation revealed elevated gonadotropins, relative hyperestrogenism (estradiol 58.7 pg/mL), and SRY‑negative chromosome mosaicism (47,XX,+der(8)/46,XX). Pelvic imaging demonstrated a small prostate and bilateral intratesticular cysts, and laparoscopy and cystoscopy confirmed a male i

Mosaic Loss of Chromosome Y (LOY): From Genomic Instability to Disease and Single-Cell Detection.

Mosaic loss of chromosome Y (LOY) is the acquired absence of the Y chromosome in a fraction of cells, is most readily observed in the haematopoietic compartment, and represents the most common form of clonal mosaicism in older men. Once dismissed as a benign correlate of aging, LOY is now recognized as a clinically meaningful biomarker associated with reduced survival and an expanding range of age-related disorders, including multiple solid and haematological cancers, cardiovascular disease, Alzheimer's disease, and type 2 diabetes mellitus. Large-scale genome-wide analyses have shown that sus

Testis Molecular Pathways in CAIS Unveil Testosterone/Estradiol on Germ Cell Tumor Risk in Non-Obstructive Azoospermia.

CONTEXT: Non-obstructive azoospermia (NOA) is the most severe form of male infertility, affecting 1% of all men, with a clinical picture characterized by no sperm production, hyalinization of the basal membrane of the seminiferous tubules, primary hypogonadism, and earlier onset of age-related comorbidities compared with fertile men. NOA is also characterized by etiologic heterogeneity and the non-genetic form has higher incidence of testicular germ cell cancer (TGCC) compared to the forms with genetic abnormalities. OBJECTIVE: We aimed to establish molecular pathways in the testicular somati

Metabolism and Type 2 Diabetes Across Life Stages in Klinefelter Syndrome: A Population-Based Cohort Study.

CONTEXT: Klinefelter syndrome (KS) is underdiagnosed, undertreated, and associated with metabolic dysfunction. OBJECTIVE: We compared incidences of metabolic disorders among men with KS, either undiagnosed (U-KS), diagnosed and untreated (D-KS), or treated with testosterone replacement therapy (T-KS). METHODS: This was a national Danish registry-based study from January 1994 to December 2022. We computed hazard ratios (HR) for incidence and severity of metabolic disorders between risk set matched strata of U-KS, D-KS, and T-KS and male control individuals. We evaluated the effect of parenter
