Pigmentary glaucoma in a patient with 48,XXYY syndrome.

OBJECTIVE: We report the case of a patient with 48,XXYY genetic syndrome and pigmentary glaucoma. BACKGROUND: 48,XXYY syndrome is a very rare sporadic genetic condition with a variety of clinical signs and symptoms. Similarly to Klinefelter syndrome, the principal symptom is hypergonadotrophic hypogonadism, leading to infertility. Several other common features of this syndrome, including developmental delay, behavioral difficulties and facial dysmorphism, have been described. Nevertheless, associations between 48,XXYY syndrome and ocular manifestations are rare or at least not clearly describ

Evaluation of the predictors of successful sperm retrieval of micro-TESE in cases with mosaic Klinefelter versus cases with non-mosaic Klinefelter: a prospective case series study.

BACKGROUND: We evaluated the predictors of eventful microsurgical testicular sperm extraction (micro-TESE) from infertile men with Klinefelter syndrome (KS). RESULTS: The mean age of the patients was 32.4 ± 6.3 years. The mean serum levels of follicle stimulating hormone (FSH), luteinizing hormone (LH), total testosterone (TT), estradiol (E2) and prolactin were 34.38 ± 14.66, 18.92 ± 6.54, 3.18 ± 2.08, 28.2 ± 10, 11.56 ± 5.09, respectively. The mean right (Rt) testicular and left (Lt) testicular volumes were 2.17 ± 0.83 ml, 2.2 ± 0.89 ml, respectively. Mosaic KS patients showed highly signifi

Cardiometabolic outcomes of early onset hypogonadism in males.

Testosterone is an important vascular hormone, with multiple effects reported on the vasculature. As such, boys and men with early onset hypogonadism may have altered cardiovascular function, with the potential to result in adverse cardiometabolic outcomes in adulthood. Given the fact that cardiovascular changes in the young can affect future cardiovascular health, there is a need to better understand the influence of androgens on the vasculature in those with conditions such as 46, XY Disorders of Sex Development and Klinefelter Syndrome. This review summarises what is known about hypogonadis

Chromosomal Microarray Analysis in Spina Bifida: Genetic Heterogeneity and Its Clinical Implications.

BACKGROUND: The etiology of spina bifida is multifactorial; the phenotype is the end result of both genetic and environmental influences. While whole exome sequencing has identified several pathogenic variants in Indian cohorts, the role of chromosomal imbalances and long contiguous stretches of homozygosity (LCSHs) remains largely unexplored in this population. Chromosomal microarray analysis (CMA) is an important tool that provides insights into such genetic aberrations, making it significant for evaluating patients with spina bifida. OBJECTIVE: To identify LCSHs and chromosomal imbalances

Age-related differences in psychopathology within sex chromosome trisomies.

Sex chromosome trisomies (SCTs) are a group of genetic disorders characterized by presence of a supernumerary sex chromosome, resulting in karyotypes other than XX or XY. These include XXX (Trisomy X), XXY (Klinefelter syndrome), and XYY (Jacobs syndrome). SCTs have been linked to increased risk for psychopathology; however, this relationship warrants additional research. Specifically, little is known regarding potential age-related variation in risk for psychopathology and how this may differ across karyotypes and subdomains of psychopathology. This has important implications for psychoeducat

An Overview of Oxidative Stress in Sex Chromosome Aneuploidies in Pediatric Populations.

BACKGROUND: Oxidative stress, defined as an imbalance between reactive oxygen species and antioxidant defenses, plays a pivotal role in the pathogenesis of sex chromosome aneuploidies (SCAs), such as Turner syndrome (TS) and Klinefelter syndrome (KS). Pediatric patients with SCAs are particularly susceptible due to hormonal deficiencies, metabolic disturbances, and systemic complications. METHODS: A comprehensive literature search was conducted in November 2024 using PubMed, Scopus, and Web of Science. Keywords included "antioxidants", "oxidative stress", "pediatrics", "Turner syndrome", "Kli

Predictors of sperm retrieval success in first-time and repeated Micro-TESE for nonobstructive azoospermia.

INTRODUCTION: Microdissection testicular sperm extraction (micro-TESE) is the preferred sperm retrieval method, although success rates vary based on factors such as age and hormone levels. We evaluate sperm retrieval outcomes in first-time and repeated micro-TESE, identifying predictors of success to guide clinical management. MATERIAL AND METHODS: Retrospective study analyzed 152 men with nonobstructive azoospermia (NOA) who underwent micro-TESE between 2020 and 2022. Patients were grouped based on sperm retrieval outcomes, and clinical, demographic and hormonal factors were assessed. Compar

Male Infertility and Reduced Life Expectancy: Epidemiology, Mechanisms, and Clinical Implications.

Male infertility is a prevalent condition affecting approximately 15% of couples worldwide. Recent evidence indicates that, beyond its immediate reproductive implications, male infertility may reflect broader health concerns. Large-scale cohort studies consistently show that men with poorer semen parameters have elevated all-cause mortality compared to fertile counterparts, with a dose-dependent pattern whereby more severe abnormalities correlate with a higher risk of early death. Proposed mechanisms linking infertility to reduced life expectancy encompass genetic, hormonal, and lifestyle fact

Characterization of somatic testicular cells during human development: fetal, peripubertal, adolescent and adult human testis from healthy and infertility related disease.

The transcription factor GATA4 is found in Sertoli and Leydig cells, whereas SOX9 is exclusive to Sertoli cells, being both factors essential for the normal development of murine and human fetal testis. In turn, the steroidogenic acute regulatory protein (STAR) is specifically expressed in Leydig cells. Nevertheless, the function of STAR, GATA4 and SOX9 in peripubertal, adolescent and adult testes in Klinefelter syndrome and azoospermic patients remains poorly understood. To characterize the developmental expression of STAR, GATA4 and SOX9 in human testicular somatic cells, we performed immuno

Evaluation of Short and Tall Stature in Children.

Short stature is defined as height that is more than 2 standard deviations below the mean height for age; tall stature is defined as height that is more than 2 standard deviations above the mean height for age. The initial evaluation of short and tall stature should include obtaining a comprehensive history, performing a physical examination, plotting serial height measurements on the Centers for Disease Control and Prevention or World Health Organization growth charts, calculating estimated adult height and comparing it with midparental height, determining bone age, and laboratory testing in

Recommendations for diagnosis and treatment of the aging male with Klinefelter syndrome.

This is the inaugural recommendation for the management of the Aging Male with Klinefelter Syndrome (KS), a demographic characterized by primary hypogonadism in the presence of one or more supernumerary X-chromosomes with irregular gene-inactivation, and metabolic dysregulation, contributing a component of functional hypogonadism as aging in these men progresses. The unique constellation of symptoms spanning multiple organ systems and health domains necessitates specialized diagnostic and therapeutic strategies. Neglecting the specific needs of this population can precipitate a considerable de

Finding acceptance: How adolescents and young adults with Klinefelter Syndrome navigate identity, disclosure, and support.

Klinefelter syndrome (KS; 47, XXY) is the most common chromosomal aneuploidy, affecting ~1:600 male births. Due to recent technological advances, KS is diagnosed more often in young individuals. This emerging population of adolescent and young adult (AYA) patients with KS seeking care makes it increasingly important to understand the unique experiences and perspectives of these individuals. This qualitative study explored (1) the psychosocial well-being of AYA with KS and (2) resources and support systems AYA with KS utilize to cope with challenges related to their diagnosis. This study includ

Novel tools for comparing the architecture of psychopathology between neurogenetic disorders: An application to X- vs. Y-chromosome aneuploidy effects in males.

BACKGROUND: Psychiatric symptoms are typically highly inter-correlated at the group level. Collectively, these correlations define the architecture of psychopathology-informing taxonomic and mechanistic models in psychiatry. However, to date, it remains unclear if this architecture differs between etiologically distinct subgroups, despite the core relevance of this understanding for personalized medicine. Here, we introduce a new analytic pipeline to probe group differences in the psychopathology architecture-demonstrated through comparison of two distinct neurogenetic disorders. METHODS: We

Klinefelter syndrome: A neurodevelopmental disease of the synapse.

Klinefelter syndrome (KS; 47,XXY) is the most common sex chromosome disorder, affecting approximately 1 in every 500 to 650 newborn males. Children with KS display a spectrum of phenotypic manifestations, including abnormal neurocognitive phenotypes. However, due to the limited research focusing on the central nervous system, our understanding of the neurobiology of KS at the cellular and molecular levels remains largely unclear. In this study, we utilized cortical organoids derived from pluripotent stem cells and transcriptomic analysis to explore the mechanisms underlying early brain develop

Demographic Composition of Participants in Sex Chromosome Aneuploidy Studies across the Globe: a 20-year Systematic Review.

BACKGROUND: Sex chromosome aneuploidies (SCAs), including Klinefelter syndrome (47,XXY), Turner syndrome (45,X), XYY syndrome, trisomy X (47,XXX), and rarer tetrasomies and pentasomies, affect approximately 1 in 400 births and are associated with a wide range of developmental, cognitive, and physical health outcomes. While clinical research on SCAs has expanded over the past two decades, it is unclear whether the populations included in these studies reflect the demographic diversity of those affected. Assessing representation is critical to ensuring research findings are generalizable and app

Diagnostic contribution of conventional and molecular karyotyping in congenital diaphragmatic hernia related copy number variations.

OBJECTIVE: Congenital Diaphragmatic Hernia (CDH) results from defects in the developing diaphragm and is characterized by herniation of abdominal contents into the thoracic cavity. Notably, CDH is linked to elevated morbidity and mortality rates due to its association with pulmonary hypoplasia. Copy number variations (CNVs) are significant contributors to the etiology of CDH. We aimed to investigate the involvement of new candidate CNVs in CDH etiology and the effectiveness of karyotyping and array-based Comparative Genomic Hybridization (a-CGH) in CDH diagnosis. MATERIALS AND METHODS: Among

Male infertility and its ties to next generation sequencing as a new forward path to definite diagnoses.

Based on the WHO infertility definition, couples who failed to conceive a baby after one year of unprotected sexual intercourse are infertile. About 50 million couples suffer infertility around the word and men are equally involved. Male infertility is a heterogeneous and multi-factorial complex disorder. Like most diseases, genetic factors play a role in this issue too. Genetic defects such as Klinefelter syndrome (XXY), Y Chromosome Microdeletions (YCMDs), and monogenic mutations contribute to male infertility. Currently, 4% of infertile males have a genetic reason, while 60-70% remain unexp
