Diazoxide Choline Extended-release Tablets in Prader-Willi Syndrome: A Randomized, Double-blind, Withdrawal Period Study.

CONTEXT: The hallmark condition of Prader-Willi syndrome, a rare, genetic neurobehavioral/metabolic disorder, is life-threatening hyperphagia. OBJECTIVE: We assessed the efficacy and safety of diazoxide choline extended-release (DCCR) tablets for the treatment of hyperphagia in adults and children four years of age and older with Prader-Willi syndrome. METHODS: We conducted a 16-week, randomized withdrawal study in children and adults with Prader-Willi syndrome and hyperphagia. Participants who previously completed randomized (13-week DCCR or placebo) and open-label (2.5-4.5 years DCCR) stud

A chromosome region linked to neurodevelopmental disorders influences locomotor behavior through sex-specific neural circuits.

Biological sex shapes the manifestation and progression of neurodevelopmental disorders (NDDs), however, the underlying mechanisms remains unclear. Hemideletion of the 16p11.2 region (16p11.2 del/+) is associated with NDDs, and 16p11.2 del/+ mice exhibit sex-specific, striatum-related phenotypes relevant to NDDs. In this study, using snRNA-seq, we identify cell type- and sex-specific transcriptomic changes in D1- and D2-spiny projection neurons (SPNs), with greater impact in males. Fiber photometry recordings reveal reduced neuronal activity in the dorsal striatum of 16p11.2 del/+ males, but n

Survival in infants with trisomy 18, palliative care and ethical reflections: a single center considerations.

BACKGROUND: Trisomy 18 was once considered a fatal diagnosis due to the presence of cardiac and extracardiac lesions. However, with the increasing use of therapeutic management, 3% to 25% of infants with trisomy 18 may survive beyond their first year, depending on the interventions provided. Currently, there are no clear and widely accepted criteria to guide medical decisions for children with trisomy 18. This means that patients could often be at risk of either over-treatment or therapeutic abandonment. We aimed to explore the effectiveness of intensive and non-intensive treatments in enhanci

Developing Del2Phen: A Novel Phenotype Description Tool for Chromosome Deletions.

Information on the health-related consequences of rare chromosome disorders is often limited, posing challenges for both patients and their families. The Chromosome 6 Project aims to bridge this knowledge gap for structural aberrations involving chromosome 6 by providing parents of affected children with information on the expected phenotypes of their child. To achieve this, detailed phenotype and genotype data are collected directly from parents worldwide and supplemented with data from literature reports, resulting thus far in a dataset of over 500 individuals. This comprehensive data pool w

Mothers' and Fathers' Recode Use With Young Children With Down Syndrome.

PURPOSE: Parental recodes, a form of responsivity, help facilitate language development in children with Down syndrome (DS). However, little is known about the complexity and referents of recodes or differences between mothers and fathers and how this relates to child language. METHOD: This study included 15 children with DS (Mage = 39.67 months, range: 24-61 months) and their mothers and fathers. Mother-child and father-child play interactions were recorded in the home. RESULTS: We found no differences between mothers' and fathers' use of recodes. A moderate positive association existed bet

Alterations in electroencephalography signals in female fragile X syndrome mouse model on a C57BL/6J background.

Fragile X Syndrome (FXS), the most common monogenic cause of autism spectrum disorder, exhibits sex differences in prevalence and phenotypic severity. Electroencephalography provides translational insights into its pathophysiology, yet prior research focuses predominantly on males. In C57BL/6J mice, male Fmr1 knockout models show increased absolute gamma power across developmental stages, while female phenotypes, particularly in juveniles, remain uncharacterized. This study addresses this gap by comparing juvenile female Fmr1 knockout and wild-type mice. Frontal-parietal differential electroen

Elevated serotonin receptor 2A signaling restores learning and memory in a Fragile X syndrome model.

Serotonin (5-hydroxytryptamine, 5-HT) has central roles enabling learning and memory, particularly via serotonin receptor 2A (5-HT2AR) signaling. Drosophila Fragile X syndrome model (dfmr1 null mutant) studies reveal impaired learning and memory, which may reflect serotonergic signaling deficits. Here, we use classical olfactory T-maze conditioning to assess behavior, combined with imaging to assess 5-HT and 5-HT2AR levels within the underlying Mushroom Body (MB) brain circuitry. Null dfmr1 mutants exhibit learning and memory deficits that are corrected by elevating 5-HT signaling via 1) overe

T2/FLAIR Mismatch Subregion as an Independent Prognostic Biomarker in IDH-Mutant and 1p/19q Noncodeletion Astrocytoma.

BACKGROUND AND PURPOSE: While the global T2/FLAIR mismatch sign is a diagnostic hallmark of IDH-mutant and 1p/19q noncodeletion astrocytoma, the prognostic significance of partial mismatch patterns remains unclear. This study investigated the differences in MRI between subregions with a T2/FLAIR mismatch and a match, and the impact of these differences on astrocytoma clinical outcomes. MATERIALS AND METHODS: This retrospective study analyzed 221 patients with histopathologically confirmed IDH-mutant and 1p/19q noncodeletion astrocytoma, stratified by the presence (n=131) or absence (n=90) of

Omics analyses decoding mechanisms underlying the self-flocculating phenotype of yeast cells and stress tolerance for robust production.

A unique self-flocculating yeast strain SPSC01 was developed through protoplast fusion for fuel ethanol production with high product titers. In this study, we conducted comparative multi-omics analyses on SPSC01 to elucidate mechanisms underlying its self-flocculating phenotype and associated stress tolerance, the most desirable merit for robust production in industry. Leveraging two cutting-edge third-generation sequencing technologies, we achieved a gapless high-quality and chromosome-level assembly for the genomes of SPSC01 and its parental strains. Through comprehensive genome analyses, we

Myeloid Fmr1 deficiency in mice results in reduced serum cholesterol and altered bile pathway gene expression.

Fragile X Syndrome (FXS) is a genetic disorder caused by increased CGG repeats in the Fragile X Messenger Ribonucleoprotein 1 (FMR1) gene which encodes an RNA-binding protein that can alter mRNA processing, translation and stability. Among the effects of FMRP deficiency is the modulation of metabolic pathway gene expression resulting in reduced cholesterol. In this report, the role of Fmr1 in modulating serum cholesterol of mice fed Western diet was investigated. Fmr1-KO mice had reduced serum cholesterol that occurred even as LDLR expression was reduced, suggesting a non-LDLR pathway of chole

Identification of RALY as a novel regulator of DSCR1 transcription.

Down syndrome critical region 1 (DSCR1) is located on chromosome 21, whose N-terminal coding sequence lacks recognizable motifs, and its precise transcriptional and translational regulation remains incompletely understood. Utilizing a bicistronic reporter system, we identified the core promoter region of DSCR1, which enables investigation of its gene regulatory mechanisms. Proteomic analyses via LC-MS/MS revealed that RALY directly binds to the DSCR1 promoter, influencing its transcriptional activity. Notably, RALY overexpression markedly suppresses DSCR1 expression, whereas siRNA-mediated kno

The Italian Angelman Syndrome Registry (IReAS): a tool for standardized data collection and genotype-phenotype analysis.

BACKGROUND: Angelman syndrome (AS) is a rare and heterogeneous genetic disorder characterized by intellectual and psychomotor delay, speech deficits, seizures and behavioural issues. To evaluate the feasibility of collecting data by many Italian centers involved in pathology management, and to investigate the relationship between various symptoms and genotypes, a dedicated AS registry was developed. This study aims to present preliminary findings from the Italian AS registry (IReAS), with a specific focus on exploring genotype-phenotype correlations. MATERIALS AND METHODS: The IReAS, establis

Fine-mapping a genome-wide meta-analysis of 98,374 migraine cases identifies 181 sets of candidate causal variants.

Migraine is a highly prevalent neurovascular disorder for which genome-wide association studies (GWAS) have identified over one hundred risk loci, yet the causal variants and genes remain mostly unknown. Here, we meta-analyze three migraine GWAS including 98,374 cases and 869,160 controls and identify 122 independent risk loci of which 35 were new. Fine-mapping of a meta-analysis is challenging because some variants may be missing from some participating studies and accurate linkage disequilibrium (LD) information of the variants is often not available. Here, using the exact in-sample LD, we f

INSIG1 parallel substitution drives lipid/sterol metabolic plasticity mediating desert adaptation in ungulates.

Desert ungulates, such as Camelus bactrianus and Hippotraginae antelopes, exhibit extraordinary adaptation to extreme environment. Deciphering these genetic adaptations is critical for understanding evolutionary resilience under climate change. Here, we generate a chromosome-level genome for domestic Bactrian camel and integrate comparative genomics analyses to uncover genomic adaptation in arid-desert ungulates. We find elevated molecular evolution rates with intensified positive selection among desert-adapted lineages. Convergent positively selected genes are mainly involved in energy metabo

Development of a patient-centered conceptual disease model in Ring 14 syndrome: a patient-centered model of lived experience.

PURPOSE: Ring 14 Syndrome is a rare genetic disorder caused by an anomaly in the 14th chromosome that often leads to intractable epilepsy, moderate to severe intellectual disabilities, slow growth, ocular abnormalities, and more. The presentation and severity of symptoms can greatly vary, making Ring 14 syndrome a complex condition to understand, manage, and treat. Conceptual disease models, also known as disease concept models, provide a formal framework based on the lived experience of patients and families that helps to describe the functional and quality of life impacts of a disease across

Understanding of trisomy 21 prenatal screening among pregnant women in France: A cross-sectional study in light of 2019 guidelines.

OBJECTIVE: Cell-free DNA (cfDNA) screening test has been reimbursed in France since 2019 as part of the national trisomy 21 (T21) screening program. The expansion of cfDNA sequencing in September 2024 to include additional chromosomal anomalies raises new challenges for information delivery and informed consent. This study aimed to assess pregnant women's understanding of trisomy 21 prenatal screening information, distinct from satisfaction with or access to information, in light of these developments. STUDY DESIGN: A cross-sectional online survey was conducted in August-September 2024 among
